보육교사 대상의 영아 심폐소생술 현장교정교육의 효과 (The CPR Educational Program Effect of Infant CPR Immediate Remediation for Child Care Teachers)

  • Purpose: The purpose of this study was to evaluate the effectiveness of infant CPR (Cardiopulmonary Resuscitation) immediate remediation[LAB2]. Methods: This is a quasi-experimental study with a non-equivalent comparison group pre-post test design. Participants were 73 child care teachers in J district in Seoul. The experimental group (35) received immediate remediation with video learning [LAB3]for infant CPR education and the control group (38) received video learning only. We investigated confidence of infant CPR and tested single-rescuer infant CPR performance depending on a checklist (8 items). Data were collected before and right after to evaluate the confidence of infant CPR and before, right after and 2 weeks after the education to evaluate the skill performance and final acceptance. Results: There was a significant increase in confidence of infant CPR in the experimental group and control group. There was a statistically significant difference in the degree of skill performance between groups according to the amount time (F=10.58, p<.001). The degree of skill performance was tested two weeks after the education and immediate remediation was 5.35 times more effective compared to the video image demonstration (B=1.677, p<.001). Conclusion: The findings provide the necessity of immediate remediation education of infant CPR to increase confidence and skill performance ability.

간호학생에게 실시한 심폐소생술 교육의 효과 (The Effectiveness of Cardiopulmonary Resuscitation Training Targeted for Nursing Students)

  • The purpose of the study is to evaluate the effectiveness and competence level of trainees of Cardiopulmonary resuscitation training targeted for nursing students. 70 nursing students of Y nursing college are recruited as subjects from Dec. 1st, 1998 through Dec. 8th. 1998. For the pre-test. demographic data related to CPR and knowledge of CPR were evaluated. For the post-test, the next week of pre-test, three difference groups of subjects were tested their knowledge of CPR. CPR training was designed by two components which were 90 mins lecture and demonstration by one professor and individual practice using two educational models with two professors. As the tool of measurement estimating pre or post knowledge of CPR. questionnaires were developed based on self-diagnosis questionnaires of American Heart Association(AHA). The questionnaires were multiple choices (50 questions) and open end questions regarding CPR process. Each multiple choices questions valued 2 points (Score varied min. 0 point to max 100 points.). Collected data were computerized and analyzed by SPSS-WIN. Frequency and percentage of each questions analyzed. The differences of the knowledge and competency level of subjects between pre and post test was analyses by paired t-test. The followings are research outcome. 1. In the pre-test, 95% of subjects answered that they already knew what CPR was. but only 82% described correctly what CPR was. 49% learned CPR before, and 80s5 of them learned at high school. 2. 37 questions scores increases, and 10 questions scores decreased. 3 questions didnt change their score. After getting training, ratio of 80% correct score significantly increased 4 times. 3. In post-test. knowledge level of trainees increased compared to that of pre-test. (t=-15.075, p=.000) 4. Competence level also increased (t=-14.86, p=.00). In result, after getting CPR training, most CPR knowledge increased except open the air tract, toddler CPR, and alternative behavior when the air tract is blocked. CPR training needs to extend the educational scope not only CPR lecture but also psychomotor skill practice. CPR trainees are in need of appropriate feedback as well as enough opportunities of skill practice.

대학생의 기본 심폐소생술 교육 평가 - 간호과와 비간호과를 대상으로- (The Evaluation of Basic Cardiopulmonary Resuscitation Training Targeted for College Students)

  • The purpose of this study was to evaluate the accuracy of CPR skills and the satisfaction of CPR Training targeted for college students. Also, this study made comparative study of nursing students and non-nursing students. The sample consisted of 248 students(132 nursing students, 116 non-nursing students). CPR Training was designed by two components which were a lecture and demonstration by one professor and individual practice by instructors. As tool of measurement estimation satisfaction of CPR Training questionnaires were developed based on educated contents. The accuracy of CPR skills were checked by SkillReporter CPR training manikin. Collected data were computerized and analyzed by SPSS-WIN program. The results were as follows : 1. The skills of CPR were cardiac compression 92.79 times/min, correct cardiac compression rate 90.85%, ventilation 9.93 times/min, correct ventilation rate 79.34%. 2. The mean of the accuracy of CPR skills were 2.94(SD .87). 3. When errors were analyzed, the highest item was stomach distension(51.2%) of ventilation skills and too little(70.6%) of cardiac compression skills. 4. The mean of the satisfaction of CPR training were 2.87(SD .17), the highest item of the satisfaction of CPR Training was practice. 5. We found significant statistical differences based on the accuracy and the error, non-nursing students were high in correct cardiac compression per minute(t=3.615, p=.000) and ventilation too much(t=4.292, p=.000), nursing students were high in correct ventilation rate(t=-3.885, p=.000) and cardiac compression too shallow) t=-2.842, p=.005).

  • Unsatisfied results of return of spontaneous circulation (ROSC) estimates were caused by the fact that the predictability of the predictors was insufficient. This unmet estimate of the predictors may be affected by transitional events due to behaviors which occur during cardiopulmonary resuscitation (CPR). We thus hypothesized that the discrepancy of ROSC estimates found in statistical characteristics due to transitional CPR events, may affect the performance of the predictors, and that the performance of the classifier dichotomizing between ROSC and No-ROSC might be different during CPR. In a canine model (n=18) of prolonged ventricular fibrillation (VF), standard CPR was provided with administration of two doses of epinephrine 0 min or 3 min later of the onset of CPR. For the analysis of the likelihood of a successful defibrillation during CPR, Support Vector Classification was adopted to evaluate statistical peculiarity combining time and frequency based predictors: median frequency, frequency band-limited power spectrum, mean segment amplitude, and zero crossing rates. The worst predictable period showed below about 1 min after the onset of CPR, and the best predictable period could be observed from about 1.5 min later of the administering epinephrine through 2.0-2.2 min. As hypothesized, the discrepancy of statistical characteristics of the predictors was reflected in the differences of the classification performance during CPR. These results represent a major improvement in defibrillation prediction can be achieved by a specific timing of the analysis, as a change in CPR transition.

노래를 이용한 심폐소생술의 교육 효과 (The effect of teaching cardiopulmonary resuscitation using a song)

  • Purpose: This study proposed an improved method of teaching cardiopulmonary resuscitation (CPR). Methods: A teaching method for CPR using a song was evaluated. To validate the effect of this teaching method, we performed a comparative reference test against a traditional CPR lecture-oriented education method. A total of 202 study participants were enrolled in this project. Half were taught by the traditional CPR education method, and the other half were taught by the alternative educational method using a song. Results: The results for those educated by the traditional lecture-oriented method showed scores of 4.84 / 6.00 (Mean of knowledge) and 70.22 / 100 (Mean of practice ability for CPR). The results for those educated by the alternative method showed scores of 5.26 / 6.00 (Mean of knowledge) and 74.13 / 100 (Mean of practice ability for CPR). The alternative educational method utilizing a song improved the results on written and practical examinations. This improvement could be attributed to the effect of music on memory circuits in the brain. Conclusion: We believe that the results of this study will help to improve the success rates of CPR.

대학생의 심폐소생술에 대한 교육경험에 따른 지식 - 일 광역시를 중심으로 - (Knowledge According to Learning Experiences of CPR for Health Occupation College Students)

  • Purpose: The first responder's role during a cardiac arrest scene is to initiate CPR. The AHA has recognized and included the first responder's role for improving the survival rate of cardiac arrest patients. Health personnel working in nursing, emergency care, dental hygiene, radiology, and ocular optics frequently confront sudden cardiac arrest while working. This study was to identify the relationship between the educational experience and recognition with the level of knowledge about CPR for college students. Method: Five hundred forty college students enrolled in the department of nursing science, radiological technology, ocular optics, emergency medical technician, or dental hygiene in Daejeon city were surveyed. The tool used was CPR knowledge developed by the authors based on a literature review including 2005 AHA's CPR guideline. Result: The higher educational experience of CPR was, the higher the level of knowledge. The knowledge of the students in nursing or emergency medical technician was higher than students in dental hygiene, radiology, and ocular optics. Conclusion: CPR class should be included in the curriculum for college students in order to improve their accuracy as a first responder to cardiac arresting patients.

심폐소생술 교육이 일반인의 심폐소생술에 대한 지식과 태도에 미치는 효과 (Effect of CPR Training for Lay Trainees on their Knowledge and Attitudes)

  • Purpose: This study was done to determine the effect of CPR training for lay trainees on their knowledge and attitudes. Method: This was a nonequivalent control group nonsynchronized design. The participants were 60 lay trainees who were conveniently assigned to an experimental or control group. The data were collected from July 1 to September 8, 2004 and analyzed using $x^2$-test, t-test and ANCOVA. Results: The 1st hypothesis, 'Posttest CPR knowledge scores for the experimental group will be higher than scores for the control group' was supported (F=59.44, p=0.001). The 2nd hypothesis, "Posttest CPR attitude scores for the experimental group will be higher than scores for the control group" was supported (F=29.94, p=0.001). Conclusion: This CPR training was effective in increasing the levels of knowledge and attitude for the lay CPR trainees.

장애인활동보조인의 심폐소생술 실습교육 효과 (The Effect of CPR Practice Education of Personal Assistants for the Disabled)

  • Purpose: This study was conducted to identify the effects of cardiopulmonary resuscitation (CPR) practice education on CPR knowledge, attitude, confidence, learning satisfaction and performance of the personal assistants for the disabled. Methods: This is a quasi-experimental study with a non-equivalent control group. The participants were 62 personal assistants for the disabled. Thirty-one subjects were assigned to an experimental group and the remaining subjects were assigned to a control group. The date collected were analyzed using the IBM SPSS Statistics 20.0 program by descriptive statistics, $x^2$-test, Fisher's exact test, and t-test. Results: CPR practice education was found to have a meaningful effect on CPR knowledge (p<.001), learning satisfaction (p<.001), and performance (p<.001) of the personal assistants for the disabled. Conclusion: Considering these results, it would be useful to develop CPR practice curriculum to enhance CPR knowledge, attitude, confidence, learning satisfaction and performance of the personal assistants for the disabled.

In-Hospital Utstein Style에 의한 응급의료센터의 심폐소생술 결과 분석 - 심정지 상태로 내원한 환자를 대상으로 - (Results Analysis of CPR in Emergency Center by In-Hospital Utstein Style)

  • Purpose: Investigates the results of CPR operation in visited patients with arrest state, and element to affect the results, and it is checks a problem, and it is made to promote. Methods: As for this study, analysis studied the results that operation CPR with the object arrest patients of 69 visited Jecheon Seoul hospital emergency center for during the period from January 2002 to February 2005. It was played writing partly changed In-hospital Utstein Style, and to stick a record in it. Paramedic which participated in CPR directly did the record of a variable, As for the data processing, use SPSS Version 12.0 for Windows. Results: It was male 68.1% female 31.9% for 69 people, and the sex ratio didn't affect return of spontaneous circulation, with female 8 people to male 16 people for ROSC(Return of spontaneous circulation) 24 people. with female 14 people to male 31 people for NROSC(Non-return of spontaneous circulation) 45 people(p>0.05). The initial EKG rhythm was asystole 34.8%, VF 31.9%, The case that initial EKG was VF compared it to a patient of asystole, and a survivor had a lot of VF and there were a lot of survivors(p<0.05). The wasn't relativity between VF and PEA(p>0.05), The CPR lead time was short in ROSC with NROSC $25.0{\pm}15.0$ minutes, ROSC $11.9{\pm}10.7$ minutes(p<0.01). Epinephrine administer time was NROSC $3.0{\pm}4.1$ minutes, ROSC $2.1{\pm}1.9$ minutes(p>0.05). It was survivor 18 people(21.1%) than 24 hours and the PAM Index affected in ROSC. with $9.0{\pm}1.2$ points NROSC, with $1.6{\pm}1.7$ points ROSC(p<0.01). A correlation was high between a CPR lead time and PAM Index(p<0.02), a correlation was examined by being high between ROSC and a CPR lead time, PAM Index(p<0.01).

초등학교 고학년생의 기본심폐소생술 교육효과 및 지속성 (Efficiency and continuancy of basic CPR (Cardiopulmonary Resuscitation) education for the higher grade students of elementary schools)

  • Purpose : This study was designed to figure out the necessity of continuing basic CPR education for the higher grade students of elementary school. The assessment contents were knowledge, practice ability, precision level of CPR skills and continuation of the educational efficiency. Methods : Twenty two students of 4th and 5th grade of elementary school in K city in Chungcheongnam-do were recruited for this study. The study method was a control group of non-synchronized design. A preliminary study was done on October 27 in 2006. The main study was performed from February 14 to May 11 in 2007. The researcher adopted the method of Kyung-hui, Kang (1998) such as awareness, attitude and knowledge in control group, emergency medical technician test protocol, Anne/SkillReporter$^{(R)}$ in case of the basic CPR knowledge. Four times of measures were done in shortly after practicing CPR, 4 weeks after the education, 8 weeks after the education, and 12 weeks after the education. By using SPSS/PC+ (version 12.0), the researcher analyzed the collected data based on frequency, percentage, repeated measurement, ANOVA (analysis of variance), and sidak (multiple comparison - sidak). Results : 1) The confidence of people in the control group in terms of practicing CPR showed a statistically meaningful difference (t = 10.230, p = .000) before/after CPR education. Therefore, hypothesis No.1-1 was accepted. 2) The educational necessity of people in the control group showed no statistically meaningful difference (t = -1.695, p = 0.105) before/after CPR education. Therefore, hypothesis No.1-2 was rejected. 3) The knowledge points of people in the control group showed a statistically meaningful difference (t = -7.731, p = .000) before/after CPR education. Therefore, hypothesis No.2 was accepted. 4) The confidence of people in the control group in terms of practicing CRP showed no meaningful difference (F = 2.789, p = 0.072) as time passed. Therefore, hypothesis No.3 was rejected. 5) The knowledge of people in the control group showed a meaningful difference (F = 9.090, p = .000) as time passed. Therefore, hypothesis No.4 was accepted. 6) The capability of people in the control group in terms of practicing CPR showed a statistically meaningful difference (F = 42.795, p = .000) as time passed. Therefore, hypothesis No.5 was accepted. 7) The precision level of CPR skill of people in the control group showed a statistically meaningful difference (F = 25.198, p = .000) as time passed. Therefore, hypothesis No.6-1 was accepted. 8) The precision level of chest compression skill of people in the control group showed a statistically meaningful difference (F = 5.188, p = .003). Therefore, hypothesis No.6-2 was accepted. Conclusion : In a nutshell, CPR education for the 4th and 5th graders of elementary schools had an influence on their confidence in practicing CPR and on their knowledge. This study showed that as time passed. the educational effect declined in terms of knowledge point, capability of practicing CPR, and the precision level of CRP skill. The results of the study could be postulated into the fact that re-education within 8 weeks after the first education was essential to retaining the educational effect. Therefore, we need to vitalize the CPR education for elementary school students repeatedly on a regular basis in order to continue the educational effect after they were grown-ups and to make them play their roles as a first aider.
