An Examination of Theories of Health Behaviors for Guiding Research on Mammogram Screening Practices for Korean Immigrant Women (건강행위이론의 사회문화적 적합성에 대한 논의;한인여성의 유방암 방사선 검사 행위와 관련하여)

  • 연구 목적: 1980년대 이후 미국 여성들의 유방암 조기 진단을 위한 방사선 검진율은 급속히 증가하였음에도 불구하고 유색 인종의 여성들은 여전히 조기 검진의 혜택을 받지 못하고 있다. 유색인종 여성들의 낮은 검진율을 설명하기 위해 여러 건강행위이론을 이용한 관련 요인들이 연구되어 왔다. 이 논문은 미국 보건 의료관련 연구에서 가장 많이 쓰이는 다섯 건강행위 이론을 유색 인종 여성, 특히 한국 이민 여성들의 유방암 조기검진 이행에 적용하기 위해 사회문화적 적합성을 평가하기 위해 고안되었다. 연구 방법: 네 종류의 데이터 베이스(CINAHL, MEDLINE, PsycINFO, Sociological Abstracts)를 이용한 심층적 문헌 고찰을 통해 각각의 이론으로 유방암 조기 검진을 설명한 연구들을 모두 분석하였다. 연구 결과: 각 이론들의 배경, 주요 요인, 그리고 유색인종의 유방암 조기 검진에서의 적용 연구 등을 분석하였다. 결론: 서양 문화권 속에서 개발된 각 이론들이 한국적 정서와 행동을 설명하는데 명확한 한계가 있으며 이러한 한계를 극복하기 위해서는 기존의 이론들을 면밀하게 재분석하여 한국적 특성을 담아낼 수 있는 새로운 이론의 도출이 요구된다.

A Meta-analysis of the Timed Up and Go test for Predicting Falls (낙상 위험 선별검사 Timed Up and Go test의 예측 타당도 메타분석)

  • Purpose: Globally, falls are a major public health problem. The study aimed to evaluate the predictive validity of the Timed Up and Go test (TUGT) as a screening tool for fall risk. Methods: An electronic search was performed Medline, EMBASE, CINAHL, Cochran Library, KoreaMed and the National Digital Science Library and other databases, using the following keywords: 'fall', 'fall risk assessment', 'fall screening', 'mobility scale', and 'risk assessment tool'. The QUADAS-II was applied to assess the internal validity of the diagnostic studies. Thirteen studies were analyzed using meta-analysis with MetaDisc 1.4. Results: The selected 13 studies reporting predictive validity of TUGT of fall risks were meta-analyzed with a sample size of 1004 with high methodological quality. Overall predictive validity of TGUT was as follows. The pooled sensitivity 0.72 (95% confidence interval [CI]: 0.67-0.77), pooled specificity 0.58 (95% CI: 0.54-0.63) and sROC AUC was 0.75 respectively. Heterogeneity among studies was a moderate level in sensitivity. Conclusion: The TGUT's predictive validity for fall risk is at a moderate level. Although there is a limit to interpret the results for heterogeneity between the literature, TGUT is an appropriate tool to apply to all patients at a potential risk of accidental fall in a hospital or long-term care facility.

Arthroscopic Repair versus Non-operative Treatment of First-time Traumatic Anterior Shoulder Dislocations: A Numbers-needed-to-treat Analysis for Prevention of Recurrent Dislocations

  • Background: Arthroscopic surgical repair is a better intervention than non-operative (conservative) treatment for patients with shoulder dislocations. This systematic review determined the numbers-needed-to-treat (NNT) and relative risk reduction (RRR) associated with arthroscopic surgical repair versus non-operative treatment in reducing recurrence rates among patients with first-time traumatic anterior shoulder dislocations. Methods: We searched Google Scholar, MEDLINE, SPORTDiscus, and CINAHL from inception in 2015. All articles had to compare arthroscopic surgical repair and non-operative treatment and be written in English. We used the total number of subjects and the number of recurrent dislocations within each treatment to calculate the NNT and RRR for each study and the pooled data. Results: Six articles were selected and all clearly demonstrated that the arthroscopic surgical repair was more effective than non-operative treatment in reducing the recurrence episodes. The pooled NNT was 1.76 (95% confidence interval [CI]=NNT to benefit 1.50-2.13) and the pooled RRR was 86.0% (95% CI=77.0%-92.0%) among individuals who underwent arthroscopic repair. The average follow-up time was 56 months. Conclusions: A Strength of Recommendation Taxonomy level of evidence of 1 with a grade A recommendation supports the use of arthroscopic surgical repair over non-operative treatment in prevention of first-time traumatic anterior shoulder dislocations. We suggest that sports medicine practitioners consider the patients' age, occupation, and physical activity level when making a clinical decision.

An Integrative Review of Interventions for Internet/Smartphone Addiction among Adolescents (청소년 대상 인터넷/스마트폰 중독 중재연구에 대한 통합적 문헌고찰)

  • Purpose: The aim of this study was to investigate the characteristics of intervention programs for internet/smartphone addiction among adolescents through an integrative literature review. Methods: We searched MEDLINE(PubMed), CINAHL, Embase, DBpia, RISS, and KISS using a combination of "Internet or $phone^*$," "$addicti^*$," "adolescent," and "intervention or program or therapy." The searched studies were reviewed according to the criteria and an integrative literature review was conducted on the 14 selected papers published from January 1, 2013 to November 14, 2017. Results: Through the integrative review and analysis, a total of four attributes of interventions and their effects were derived. First, the number of group-focused interventions was significantly higher than that of individual-focused ones. Second, the interventions addressed various aspects of adolescents' problems while considering the complex nature of addiction problems. Third, there was a bias for region and sexual ratio. Lastly, most studies were actively conducted in community-based counseling or psychology settings. Conclusion: It is necessary to find more effective methods for addressing internet/smartphone addiction among adolescents through multidisciplinary approaches, including the consideration of various cultural and gender characteristics. This study provides a basis for developing future programs addressing addiction problems among adolescents.

A Concept Analysis of Compliance in Kidney Transplant Recipient Including Compliance with Immunosuppressive Medication (신장이식환자의 치료지시 이행에 대한 개념분석-면역억제제 복용이행을 포함하여)

  • Purpose: Compliance in kidney transplant recipients is critical for a positive prognosis. Especially compliance with medications after kidney transplantation is a major health care issue with implications for graft rejection and graft loss. But the definition of compliance in transplantation varies among centers. The purpose of this study was to clarify the concept of compliance in kidney transplant recipients. Methods: A literature search was conducted using RISS, MEDLINE, CINAHL. The concept analysis was guided by the methodology posited by Walker and Avant. Results: In this study, we found the attributes of the concept: 'compliance with immunosuppressive medication', 'compliance with follow-up', 'compliance with early detection of graft rejection and complication', and 'compliance with prevention of complication'. The antecedents of 'compliance in kidney transplant recipients' included 'having a kidney transplant surgery' and 'normal function of transplanted kidney'. The consequences of 'compliance in kidney transplant recipients' included 'affecting the function of the transplanted kidney' and 'affecting the health of kidney transplant recipients. Conclusion: This study may contribute to the development of tools for measuring compliance in kidney transplant recipients, as well as benefit nursing interventions research to increase compliance in kidney transplant recipients.

Meta-analysis on the Efficacy of Glutamate Receptor Antagonists for Acute Migraine Treatment (급성 편두통 치료를 위한 글루탐산 수용체 길항제의 임상적 유효성에 대한 메타분석)

  • Background: Glutamate is implicated in the pathophysiology of migraine, a common neurological disorder. Therefore, glutamate receptor antagonists (GluRAs) have been suggested as a novel migraine treatment that are able to overcome the limitations of triptans. Objective: The aim of this study was to perform a meta-analysis to assess the efficacy of GluRAs for patients with migraine. Method: The PubMed, Cochrane Library, CINAHL, and Clinical Trial.gov databases were searched for randomized placebo-controlled trials of the efficacy of GluRAs for patients with migraine conducted up to August 2019. Two independent reviewers screened the literature according to inclusion and exclusion criteria and performed quality assessment and data extraction. Review Manager 5.3 software was used for the meta-analysis. Results: Three studies involving a total of 206 patients were included in the final analysis. Compared with placebo, GluRAs significantly improved the pain-free response at 2 hours (odds ratio [OR]=3.85, 95% confidence intervals [CIs]=1.63-9.09) and the 24-hour sustained pain freedom (OR=7.40; 95% CIs=2.36-23.20). The use of rescue medications with GluRAs was lower compared to that with placebo, but the difference was not significant (OR=0.39, 95% CI=0.10-1.47). Conclusion: Our meta-analysis showed that GluRAs were more effective than placebo for patients with migraine.

Concept Analysis of Nursing Leadership (간호리더십에 대한 개념 분석)

  • Purpose: The purpose of this study was to provide a concept analysis of nursing leadership. Methods: The concept analysis process of Walker and Avant (2011) was used. A review of the literature was conducted using several databases, including PubMed, CINAHL, Ovid-Medline, DBpia, and KISS. The databases were searched using the following terms 'nursing leadership', 'nursing' & 'leadership', and 'nurse & 'leadership', and for studies published from January 2000 to March 2015. Strict inclusion and exclusion criteria were applied. Results: Analysis of these studies indicated nursing leadership attributes were individual growth, collaboration, nursing excellence, creative problem solving, and influence. The antecedents of nursing leadership were the individual maturity level, construction of mutual trust and confidential relationships, and a job requirement. The consequences were improvement in nursing productivity, development of a supportive work environment, and satisfaction with the nursing profession. Conclusion: The findings in this study provide a definite concept of nursing leadership. The findings also provide the theoretical evidence for the development of nursing leadership competencies in nursing students and professional nurses.

Brain-based Teaching Strategies for Nurse Educators: An Integrative Review (간호교육에서 뇌기반학습의 교수전략을 위한 통합적 고찰)

  • Purpose: Brain-based learning has become increasingly important in nursing education. The purpose of this study is to (a) synthesize the literature on brain-based learning in nursing education using Whittemore and Knafl's integrative review method, and (b) discuss teaching strategies for nursing educators. Method: Searches were made through the CINAHL, OVID, PubMED, and SCOPUS databases using the terms $nurs^*$ AND (brain based OR neuroscience) AND ($educa^*$ OR $learn^*$ OR $teach^*$). Included were original articles in the domain of undergraduate nursing education written in English in peer-reviewed journals between January 1984 and December 2013. Twenty-four papers met the criteria. Results: Three themes were retrieved: (a) activate whole-brain functions, (b) establish supportive educational environments, and (c) encourage learners to be more active. Conclusion: Brain-based learning enhances the learning capabilities of undergraduate nursing students.

Effectiveness of the Intervention Programs for Pressure Ulcer Prevention in Intensive Care Units : A Meta-analysis (중환자실의 욕창 예방 중재 프로그램의 효과 : 메타 분석)

  • Purpose : A meta-analysis was conducted to identify the effectiveness of strategies designed to prevent the incidence and prevalence of pressure ulcers in intensive care units (ICUs). Method : The search strategy was designed to retrieve studies both published and unpublished between 2007 and 2017 including studies in English across PubMed and CINAHL, as well as in Korean across RISS, DBPia, NDSL, KISS, and NAL. All adult ICU participants were 18 years or over. Inclusion criteria were randomized controlled trials, quasi-experimental and comparative studies. Two independent reviewers conducted quality assessments of the included studies by Scottish Intercollegiate Guidelines Network. A Review Manager 5 was used to analyze effect sizes and to identify possible sources of heterogeneity among the studies. Results : The odds ratio (OR) effect sizes were all statistically significant. The OR of total effect size was 0.30(95% CI: 0.19, 0.47), care bundle was 0.37(95% CI: 0.24, 0.57), position change was 0.45(95% CI: 0.10, 2.08), and a silicone border foam dressing was 0.14 (95% CI: 0.07, 0.29). Conclusion : The preventive interventions for patients in the ICUs have positive impacts on reducing the incidence of pressure ulcers.

A Systematic Review of Birth Experience Assessment Instrument (분만경험 측정도구의 체계적 고찰)

  • Purpose: This study aimed to conduct a systematic review and to describe characteristics of the birth experience assessment instrument. Methods: Literature related to the development of the birth experience assessment instrument was examined using a systematic review method. A literature search was conducted using the keywords as '[normal birth]; [$satisfac^*$ OR care quality]; [instrument OR scale] AND (development)' through PubMed, CINAHL, SCOPUS, PsycINFO, and RISS. The search used quality appraisal through QUADAS (Quality Assessment of studies of Diagnostic Accuracy included Systemic reviews) yielding 17 records. Results: The birth experience assessment instrument was categorized for instrumental characteristics: birth satisfaction (n=8), perception of labor experience (n=5), and birth care quality assessment in normal and operative birth experiences (n=4). Important key elements for content characteristics were as follows: nursing practice (n=10), pain control (n=5), environment (n=5), participation (n=4), and support (n=4). Conclusion: This study demonstrated that the birth experience instrument is appropriate for measuring quality of birth care in various clinical conditions. This review of the birth experience instrument reports that an appropriate psychometric tool for enhancing quality of birth care is important.
