The differentiation of pluripotent stem cells has been used to study disease mechanisms and development. We previously described a method for differentiating human pluripotent stem cells (hPSCs) into salivary gland epithelial progenitors (SGEPs). Here, cystic fibrosis transmembrane conductance regulator (CFTR) knockout hPSCs were differentiated into SGEPs derived from CFTR knockout hESCs (CF-SGEPs) using the same protocol to investigate whether the hPSC-derived SGEPs can model the characteristics of CF. CF-a disease that affects salivary gland (SG) function-is caused by mutations of the CFTR gene. Firstly, we successfully generated CFTR knockout hPSCs with reduced CFTR protein expression using the CRISPR-Cas9 system. After 16 days of differentiation, the protein expression of CFTR decreased in SGEPs derived from CFTR knockout hESCs (CF-SGEPs). RNA-Seq revealed that multiple genes modulating SG development and function were down-regulated, and positive regulators of inflammation were up-regulated in CF-SGEPs, correlating with the salivary phenotype of CF patients. These results demonstrated that CFTR suppression disrupted the differentiation of hPSC-derived SGEPs, which modeled the SG development of CF patients. In summary, this study not only proved that the hPSC-derived SGEPs could serve as manipulable and readily accessible cell models for the study of SG developmental diseases but also opened up new avenues for the study of the CF mechanism.
목적: 슬관절 주위 악성 골연부 종양 치료 시 종양의 도약전이나 슬관절 내 침범, 종양인공관절 치환술 후 반복적 감염 치료를 위한 인접골 절제, 국소재발 및 기계적 파괴가 발생한 경우 슬관절 상하부 전치환술은 하지와 슬관절 기능을 보존하는 한 방법이다. 이 중 반복된 감염 치료를 위해 슬관절면 반대측 골까지 절제 후 한시적 슬관절 고정술을 한 환자에서 가동관절로 재치환술 시 적응증, 합병증, 치환물의 생존율에 대하여 알아보고자 하였다. 대상 및 방법: 본 연구는 슬관절 상하부 전치환술 환자 34예 중 슬관절 주위 종양인공관절 치환술 후 반복적인 감염으로 슬관절면 반대측 골까지 절제한 후 한시적 슬관절 고정술이 불가피했던 13예를 대상으로 하였다. 진단, 원발병소의 위치, 슬관절 상하부 전치 환술을 받기 전까지 수술 횟수 및 기간, 재 재건술 후 치환물의 생존율, 합병증, 기능적 결과를 분석하였다. 결과: 슬관절 상하부 종양인공관절 치환물의 Kaplan-Meier 법에 의한 5, 10년 생존율은 각각 69.0%±12.8%, 46.0%±20.7%였다. 총 13예 중 6예(46.2%)에서 주 합병증이 발생하여 3예는 내고정물을 제거 후 슬관절 고정술을, 2예는 내고정물의 부분교체를, 나머지 1예는 감염된 육아조직만 제거하였다. 최종 추시상 가동관절을 유지한 10예의 Musculoskeletal Tumor Society 기능평가 점수는 평균 24.6점(21-27점)이었다. 슬관절 상하부 종양인공관절 치환술이 실패하여 슬관절 고정술로 재치환 한 3예의 기능평가 평균 점수는 12.3점(12-13점)이었다. 가동관절을 유지한 10예의 슬관절 가동 범위는 평균 67°였다(0°-100°). 슬관절 능동적 신전제한은 평균 48° (20°-80°)였다. 결론: 슬관절 주위 종양인공 치환술 후 반복적인 감염으로 슬관절면 반대측 골까지 절제 후 한시적 슬관절 고정술이 불가피했던 환자에서 슬관절 상하부 종양인공관절 치환술은 합병증의 위험성은 높으나 슬관절 고정술에 비해 기능적 결과가 월등하므로 시도할 가치가 있는 술식이다. 고정된 슬관절을 가동관절로 치환 시 반흔 조직을 철저히 제거하고 연부조직을 확보하여 종양인공관절을 삽입 후 굴곡 및 신전이 가능할 정도의 공간을 확보하는 것이 술식의 성공에 중요하다고 생각된다.
본 연구는 흰쥐의 담관 세포와 간세포에서 CFTR과 $AE1{\cdot}AE2{\cdot}AE3$ 유전자들의 발현 유무를 조사하고 흰쥐에서 담관 결찰 후 AE2 유전자의 발현의 변화를 관찰하고자 하였다. 200-250 g의 Sprague Dawley 계 흰쥐 24마리의 총담관을 결찰한 후 4 주 동안 1 주일에 6마리씩 희생하여 간세포와 담관 세포를 분리하였다. 6마리는 대조군으로 사용하여 간세포와 담관 세포에서 CFTR 유전자와 $AE1{\cdot}AE2$와 AE3 유전자 발현을 조사하고 담관 결찰 후 1 2 3 4주 간격으로 AE2 유전자 발현을 조사하였다. $AE1{\cdot}AE2$ 와 AE3는 간세포와 담관 세포에서 발현되었고 CFTR은 담관 세포에서만 발현되었다. 담관 결찰 담관세포군에서 AE2 유전자의 발현은 대조군인 정상 담관세포군에 비해서 낮았다. 결찰 담관세포군에서 AE2 유전자의 발현은 결찰 기간에 따라 차이가 없었다. 담관 결찰 간세포군에서 AE2 유전자의 발현은 대조군인 정상 간세포군에 비해서 경계적 유의성을 보이며 증가하는 경향이 있었다. 결찰 간세포군에서 AE2 유전자의 발현은 결찰 기간에 따라 차이는 없었다. 따라서 $CFTR{\cdot}AE1{\cdot}AE2$ 그리고AE3 는 간세포와 담관 세포에서 중탄산염이온과 수액을 매개하는 주된 이온 전달체라는 사실을 고려할 때 담도 담즙정체 간질환에서CFTR과 AE2 발현의 변화는 병리학적 기전에 중요한 역할을 할 수 있으리라고 생각된다.
온라인 화상회의, VOD 등의 멀티캐스트(multicast)특성을 갖는 서비스를 효율적으로 제공하기 위해서는 스위치 수준에서의 멀티캐스트 트래픽 처리를 위한 연구가 필요하다. ATM 스위치 증 입력 버퍼형은 하드웨어 구현 복잡도가 낮아 고속의 트래픽 처리와 대용량 스위치 구현에 적합한 반면, 높은 성능을 가지기 위해서는 임의접근(random access) 입력버퍼와 좋은 셀 스케쥴링 알고리즘이 필요하다. 본 논문에서는 입혁버퍼형 ATM 스위치에서의 멀티캐스트 셀 스케쥴링 알고리즘인 무충동 시간예약(CFTR)기법을 제안한다. CFTR 기법은 입력버퍼의 셀의 전송시점을 충돌이 없도록 예약함으로써 높은 처리율을 가질 수 있도록 하며, 이를 위해 입력단, 출력단 스케쥴러에 예약 테이블을 둔다. CFTR 기법은 각 출력 단 스케쥴러에서의 예약과정이 간단하고 독립적, 병렬적 수행이 가능하므로 고속 트래픽 처리에 적합하다. CFTR 기법의 성능평가를 위해 시뮬레이션을 통해 기존의 셀 스케쥴링 방식과 비교하며, 약간의 하드웨어 추가로 매우 좋은 성능을 보임을 알 수 있다.
Secretory proteins, including plasma membrane proteins, are generally known to be transported to the plasma membrane through the endoplasmic reticulum-to-Golgi pathway. However, recent studies have revealed that several plasma membrane proteins and cytosolic proteins lacking a signal peptide are released via an unconventional protein secretion (UcPS) route, bypassing the Golgi during their journey to the cell surface. For instance, transmembrane proteins such as the misfolded cystic fibrosis transmembrane conductance regulator (CFTR) protein and the Spike protein of coronaviruses have been observed to reach the cell surface through a UcPS pathway under cell stress conditions. Nevertheless, the precise mechanisms of the UcPS pathway, particularly the molecular machineries involving cytosolic motor proteins, remain largely unknown. In this study, we identified specific kinesins, namely KIF1A and KIF5A, along with cytoplasmic dynein, as critical players in the unconventional trafficking of CFTR and the SARS-CoV-2 Spike protein. Gene silencing results demonstrated that knockdown of KIF1A, KIF5A, and the KIF-associated adaptor protein SKIP, FYCO1 significantly reduced the UcPS of △F508-CFTR. Moreover, gene silencing of these motor proteins impeded the UcPS of the SARS-CoV-2 Spike protein. However, the same gene silencing did not affect the conventional Golgi-mediated cell surface trafficking of wild-type CFTR and Spike protein. These findings suggest that specific motor proteins, distinct from those involved in conventional trafficking, are implicated in the stress-induced UcPS of transmembrane proteins.

Background: Abnormal chloride (Cl-) transport has a detrimental impact on mucociliary clearance in both cystic fibrosis (CF) and non-CF chronic rhinosinusitis. Ginseng is a medicinal plant noted to have anti-inflammatory and antimicrobial properties. The present study aims to assess the capability of red ginseng aqueous extract (RGAE) to promote transepithelial Cl- secretion in nasal epithelium. Methods: Primary murine nasal septal epithelial (MNSE) [wild-type (WT) and transgenic CFTR-/-], fisher-rat-thyroid (FRT) cells expressing human WT CFTR, and TMEM16A-expressing human embryonic kidney cultures were utilized for the present experiments. Ciliary beat frequency (CBF) and airway surface liquid (ASL) depth measurements were performed using micro-optical coherence tomography (μOCT). Mechanisms underlying transepithelial Cl- transport were determined using pharmacologic manipulation in Ussing chambers and whole-cell patch clamp analysis. Results: RGAE (at 30㎍/mL of ginsenosides) significantly increased Cl- transport [measured as change in short-circuit current (ΔISC = ㎂/㎠)] when compared with control in WT and CFTR-/- MNSE (WT vs control = 49.8±2.6 vs 0.1+/-0.2, CFTR-/- = 33.5±1.5 vs 0.2±0.3, p < 0.0001). In FRT cells, the CFTR-mediated ΔISC attributed to RGAE was small (6.8 ± 2.5 vs control, 0.03 ± 0.01, p < 0.05). In patch clamp, TMEM16A-mediated currents were markedly improved with co-administration of RGAE and uridine 5-triphosphate (8406.3 +/- 807.7 pA) over uridine 5-triphosphate (3524.1 +/- 292.4 pA) or RGAE alone (465.2 +/- 90.7 pA) (p < 0.0001). ASL and CBF were significantly greater with RGAE (6.2+/-0.3 ㎛ vs control, 3.9+/-0.09 ㎛; 10.4+/-0.3 Hz vs control, 7.3 ± 0.2 Hz; p < 0.0001) in MNSE. Conclusion: RGAE augments ASL depth and CBF by stimulating Cl- secretion through CaCC, which suggests therapeutic potential in both CF and non-CF chronic rhinosinusitis.
Thanks to recent progress in availability of molecular and functional techniques it became possible to search for the basic molecular and cellular processes that mediate and control $HCO_3{^-}$ and fluid secretion by the pancreatic duct. The coordinated action of various transporters on the luminal and basolateral membranes of polarized epithelial cells mediates the transepithelial $HCO_3{^-}$ transport, which involves $HCO_3{^-}$ absorption in the resting state and $HCO_3{^-}$ secretion in the stimulated state. The overall process of HCO3 secretion can be divided into two steps. First, $HCO_3{^-}$ in the blood enters the ductal epithelial cells across the basolateral membrane either by simple diffusion in the forms of $CO_2$ and $H_2O$ or by the action of an $Na^+-coupled$ transporter, a $Na^+-HCO_3$ cotranporter (NBC) identified as pNBC1. Subsequently, the cells secrete $HCO_3{^-}$ to the luminal space using at least two $HCO_3{^-}$ exit mechanisms at the luminal membrane. One of the critical transporters needed for all forms of $HCO_3{^-}$ secretion across the luminal membrane is the cystic fibrosis transmembrane conductance regulator (CFTR). In the resting state the pancreatic duct, and probably other $HCO_3{^-}$ secretory epithelia, absorb $HCO_3{^-}.$ Interestingly, CFTR also control this mechanism. In this review, we discuss recent progress in understanding epithelial $HCO_3{^-}$ transport, in particular the nature of the luminal transporters and their regulation by CFTR.

In this report, we present the significance of protein interactions to HCO$_3$$^{-10}$ secretion in pancreatic duct cells. Antisense oligonucleotide (ASO) technology has become an attractive therapeutic modality for various diseases, including Mendelian disorders. ASOs can modulate the expression of a target gene by promoting mRNA degradation or changing pre-mRNA splicing, nonsense-mediated mRNA decay, or translation. Advances in medicinal chemistry and a deeper understanding of post-transcriptional mechanisms have led to the approval of several ASO drugs for diseases that had long lacked therapeutic options. For instance, an ASO drug called nusinersen became the first approved drug for spinal muscular atrophy, improving survival and the overall disease course. Mutations in the cystic fibrosis transmembrane conductance regulator (CFTR) gene cause cystic fibrosis (CF). Although Trikafta and other CFTR-modulation therapies benefit most CF patients, there is a significant unmet therapeutic need for a subset of CF patients. In this review, we introduce ASO therapies and their mechanisms of action, describe the opportunities and challenges for ASO therapeutics for CF, and discuss the current state and prospects of ASO therapies for CF.
