A Study on Environment-related Factors that Promote Wayfinding for People with Dementia - Focused on the Context of Long-term Care Facilities for People with Dementia (치매환자의 길찾기 향상을 위한 환경요소에 관한 연구 - 치매환자를 위한 노인전문병원 및 요양시설을 중심으로)

  • Purpose : The main objective of this paper is, to assess a body of research evidence that articulates the impact of environment-related factors on wayfinding for people with dementia who reside in Long-term care Facilities. The study also aims at identifying design implications that can be incorporated into design process and design decisions to improve wayfinding abilities for people with dementia. Methods : Comprehensive literature review has been conducted to identify empirical studies and carefully investigated peer-reviewed articles published in many different fields including architecture, psychology, nursing to gerontology. Results : The review found a growing body of studies that identified environment-related factors that improve wayfinding of people with Dementia living in long-term care facilities. Implications : The findings of the review can be considered as design guidelines to support the wayfinding abilities of people with dementia living in long-term care Facilities.

Knowledge of and Compliance with Standard Precautions by Nurses in Intensive Care Unit (중환자실 간호사의 표준주의에 대한 지식과 이행도)

  • Purpose: This study was done to identify the intensive care unit nurses' knowledge of and compliance with the standard precautions (universal precaution) as stated in infection control guidelines. Method: From September 14 to September 28, 2006, data were collected via a questionnaire survey from 189 Intensive Care Unit nurses working at three university branch hospitals and one general hospital in Gyeonggi province. Results: The mean knowledge score was 18.8/20.0 (93.9%). The mean compliance score was 3.4/4.0 (85.8%). Two factors influencing compliance were perception of the standard precautions and experience of needle stick injuries over the past year (p<.05). Two factors influencing knowledge were support of co-workers in the use of protective devices and the availability of hand-washing device or waterless alcohol gel (p<.05). Conclusion: In order to improve knowledge and compliance with standard precautions, all factors of importance for knowledge and compliance must be taken into consideration in the clinical work place and in education.

Comparison of Bacterial Contamination according to the Using Period of Feeding Bag and Disinfection Methods in Neurosurgery Intensive Care Unit Patients (신경외과중환자의 간헐적 비위관 경장영양 시 경장영양백 사용기간 및 소독방법에 따른 오염도 비교)

  • Purpose: The purpose of this study was to provide basic data for the guidelines for cleaning the feeding bags by comparing the level of contamination according to cleaning and disinfection methods when feeding tube of Neurosurgery Intensive Care Unit patients. Methods: This study was a true-experimental study, with 48 cases in total. The feeding bag of 48 patients were randomly assigned to detergent cleaning method or disinfectant decontamination method. The period of the experiment was January to July 2008, and enteral nutrition was given within 30 minutes for 2 hours, 4 times a day at 7AM, noon, 5PM, and 9PM for seven days. Feeding bags were cleaned after each feeding according to the assigned cleaning and disinfection method followed by microbial cultures on 4th and 8th day before the 7AM feeding. Results: After three and seven days of feeding and cleaning, the level of contamination was significantly lower when bags were cleaned with disinfectant than cleaning with detergent and tepid water. Conclusion: In cases where feeding bags are reused, for example, intensive care unit and home care settings, study findings indicate that cleaning bags with disinfectant is an effective way to prevent contamination of bags.

Improvement of Child Welfare Service Using Health Impact Assessment: A Case of the Cheongju Dream Start Center (건강영향평가를 통한 아동 복지서비스 개선: 청주시 드림스타트 센터 사례)

  • Objective: This study aimed to introduce Health Impact Assessment using the case of a HIA on 2008 Dream Start Project in Cheongju. Methods: We followed the typical procedure of HIA recommended by the Merseyside Guidelines on HIA. In scoping, the steering committee decided 5 key domains of child health to be assessed: prenatal care, vaccination, nutrition, access to health care, and child abuse and also the methods of collecting the evidence. The HIA appraisal team collected information from various sources including literature, community survey, and focus group interviews. The HIA appraisal team also synthesized the collected information in terms of the nature of health impacts and equity and made recommendations accordingly. Results: Positive impacts were expected in prenatal care, vaccination, and nutrition, while negative impacts were expected in access to health care. The impact of Dream Start on child abuse was uncertain. Several recommendations were made and submitted to the Dream Start team for their consideration. About 2 years later, we found many of them were implemented in 2009 Dream Start project. Conclusion: HIA was found to be applicable and effective to make decision makers in the welfare sector consider health in their work.

Evaluation of Menu Pattern and Nutritional Contents of Snack Menus Provided by Child Care Information Centers in Seoul (서울시 소재 육아종합지원센터에서 제공하는 어린이집 간식 식단의 유형 및 영양소 평가)

  • This study was conducted to evaluate menu pattern and nutritional contents of snack menus provided by child care information centers in Seoul. Snack menus during March 2013 (morning snack : 125 cases, afternoon snack : 100 cases), including information on portion size, were collected from five child care information centers, after which the number of menu items, menu patterns, and nutritional contents were analyzed. About two-thirds of total snack menus included two menu items. There were significant differences in service time (morning & afternoon snacks). 'Beverage+Food' pattern (66.7%) was the most commonly used; 'Beverage' pattern was significantly higher in morning snacks (10.4%) than in afternoon snacks (1.0%). Morning and afternoon snacks provided 124.5 and 170.6 kcal of energy and 116.4 and 90.9 mg of calcium, respectively, which are 8.9% and 12.2% as well as 19.4%, and 15.2% of children's daily energy and calcium requirements. To improve the quality of food and nutrition offered to children through snacks at child carre centers, a more detailed snack menu plan as well as nutritional guidelines for institutions should be developed.

Knowledge on Sepsis among Nurses in Intensive Care Units (중환자실 간호사의 패혈증에 대한 지식 조사)

  • Purpose: This study investigated some extent of nurses' knowledge level of sepsis in the intensive care units (ICUs). Methods: A total of 178 nurses from 5 ICUs at one hospital were asked to complete a structured questionnaire from September 10, 2012 to September 17, 2012. The questionnaire was composed of 30 items invented by Robson and colleagues and based on the guidelines published by Dellinger and colleagues. Independent t-test and ANOVA with post-hoc test were used for statistical analyses. Results: The mean score about sepsis of ICU nurses was $25.1{\pm}3.3$, and the average percentage who got correct answers was 83.8%. Of the participants, 25.3% thought they knew about understood sepsis well, and 89.1% wanted to have a sepsis screening tool. Conclusion: The ICU nurses' knowledge level on sepsis was low. Continuing education for ICU nurses is, therefore, required. For this, the development of educational programs and screening tools about sepsis should be preceded.

A Study on the Design Guidelines of the Private Zone for the Aged Care Residential Services in Australia (호주 노인간호 집합주거의 사적생활공간 계획 연구)

  • The purpose of the study is to propose the basic data for an aged residential building suitable for Korean environment and it is based on the outcomes of investigation on The Aged Care Residential Service in Australia including the scale and arrangement of private zone and specific characteristics of each compartment. The brief outcomes of the research are follows as. - The Aged Care Residential Services have been functionally running divided into Nursing home, Hostel and Retirement Village by the classification standard of the residential classification scale. - Single bed rooms have 75% of total residential area and the square of single bed room is about $20\textrm{m}^2$and that of two-bed room is about $31\textrm{m}^2$. - There are two types of ensuite such as one single-bed room and one ensuite and bed room for two or three persons and ensuite type. The square of ensuite varies as single is 50∼60%, single for public is 25∼30%, and double for public is 15∼20%. - Assisted bathrooms are generally located in the middle of building and it is suggested to be occupied about $1.25\textrm{m}^2$ per person. Sitting rooms are two types such as room type whose square has about $12\textrm{m}^2$ and a lounge type has $16\textrm{m}^2$.

The Effects of Oral Care with Difflam Spray 0.3% and Sodium Bicarbonate-Normal Saline Solution on Postoperative Oral Comfort, Sore Throat, and Halitosis (디프람 스프레이(Difflam spray 0.3%)와 중조 생리식염수 분무요법이 전신마취 하 수술 환자의 구강 안위감, 인후통 및 구취에 미치는 효과)

  • Purpose: This study aimed to compare the effects of the Difflam spray 0.3% with the sodium bicarbonate-normal saline spray on oral care for postoperative patients. Methods: The participants were randomly allocated to either the Difflam (n=22) group or the saline solution (n=23) group. The data were collected at the 1-, 4-, 7-, and 10-hour marks after returning to the ward post operation using the Beck's subjective oral discomfort scale, Visual Analogue Scale for sore throat, and portable halitosis detector. Results: The sore throat (F=7.25, p=.001) score significantly decreased after oral care using the Difflam spray 0.3%. However, the difference in the scores of oral comfort (F=0.34, p=.797) and halitosis (F=0.91, p=.443) between the two groups was not statistically significant. Conclusion: These findings suggest that the Difflam spray 0.3% is effective in improving postoperative sore throat. A further study that explores the effect of various oral solutions for postoperative patients is needed to present systemic and effective evidence-based oral care guidelines.

Experiences of School Health Teachers in Special Schools for Students with Disabilities (특수학교 보건교사의 돌봄 경험)

  • Purpose: The gradual increase in the number of children with disabilities in Korea highlights the necessity for further research on the role of school health teachers in their care. This study explored the specific experiences of school health teachers in special schools through individual in-depth interviews. Methods: In this study, 11 school health teachers participated, and data were collected through individual in-depth interviews. Thematic analysis was employed to analyze the collected data. Results: The experiences of health teachers at special schools were classified into 5 themes and 13 subthemes. These included "concerns about working in special schools and attachment after working," "management of diverse diseases and ongoing, careful observation," "ambivalence regarding students with disabilities," "competences needed to care for students with disabilities," and "enhancing care education for students with disabilities: demanding institutional support." Conclusion: Effective student health management depends on open communication channels and the establishment of strong connections among health teachers, school personnel, and parents of impaired students. To enhance the quality of care for children with disabilities, specialized education programs to improve teachers' healthcare competency should be developed. Additionally, specific guidelines for the range of medical treatments provided by health instructors, should be devised.

A Study on the Planning of Cohabitating Housing for the Elderly (고령자의 동거형 주택계획에 관한 연구)

  • The purpose of this study is to find out the effective planning guidelines of elderly housing fur the aged people living with their sons or daughters. To investigate the actual conditions and the problems of housing environments, two hundred and six old people in Gwangiu city, aged over sixty and living with their sons and daughters, were interviewed individually and they provided the useful answers to the questions. The houses for three generations need to have the proper degree of privacy fur each family member, by arranging rooms for their own independent living space while securing the sufficient common space. Therefore, independent and safe living environments are to be made for the elderly and the choice of various types be made according to their own demands. It is also necessary to guarantee the continuation of inhabitation by establishing health care systems and life support services.
