소세포성 폐암과 연관된 망막증 1예 (A Case of Cancer Associated Retinopathy with Small Cell Lung Carcinoma)

  • 저자들은 시야 결손 등의 시력 변화를 주 증상으로 내원한 환자에서 소세포 폐암과 이로 인한 부종양성증후군으로 동반된 암 관련 망막증을 진단하고 항암화학요법을 시행하여, 원발 병변의 부분 관해 보였으나, 시야 결손은 회복되지 않았던 1예를 경험하였기에 문헌 고찰과 함께 보고 하는 바이다.

  • Purpose: Brown's syndrome is characterized by the limited elevation in adduction from mechanical causes around the superior oblique tendon trochlea complex. In this particular case, post-traumatic facial deformity accompanied by Brown's syndrome was observed. We would like to report the satisfying cosmetic results obtained by reconstructing orbital roof and superior orbital rim and repositioning of zygoma. Methods: A 12-year-old patient was observed with facial deformity with strabismus in her right eye and orbital dystopia after the car accident and was eventually diagnosed with traumatic Brown's syndrome. Reconstructive surgeries could not be performed at the time of trauma due to the cerebral hemorrhage. At the second year after the trauma, a depressed fracture of the right orbital roof and superior orbital rim were reconstructed via the intracranial approach, and orbital dystopia was corrected via the zygoma triple ostectomy. In addition, a strabismus surgery was performed one year after her plastic surgery. Results: Facial deformity with orbital dystopia and strabismus was confirmed to be fully reconstructed after the surgery. Moreover, when the patient came in for a follow-up thirteen years after the operation, a developmental imbalance of the facial bones, diplopia, or any other surgical complications were not to be found. Conclusion: After the trauma, the patient with Brown's Syndrome accompanied by post-traumatic facial deformity, who went under the corrective surgeries after the meticulous examination and assessment pre-surgically, was able to acquire cosmetic satisfaction via those operations.

Solitary Xanthogranuloma of the Upper Cervical Spine in a Male Adult

  • We present the rare case of solitary xanthogranuloma in the upper cervical column mimicking a Brown-Sequard syndrome. A 29-year-old man complained with right hemiparesis and left hypoesthesia after a car accident. Computed tomography and magnetic resonance images revealed a lobulated homogenously well-enhancing mass in between posterior arch of the atlas (C1) and spinous process of the axis (C2) resulting in a marked spinal canal narrowing with cortical erosions. The patient was managed by complete resection of the tumor with partial laminectomy with lower half of C1 posterior arch and upper half of C2 spinous process. The authors advise complete removal of the xanthogranuloma and consideration as a differential diagnosis of lesions among upper cervical lesions.

수혈 관련 급성 폐손상이 동반된 외상환자에서 체외막 산화기의 적용 경험 (Application of Extracorporeal Membranous Oxygenation in Trauma Patient with Possible Transfusion Related Acute Lung Injury (TRALI))

  • The case of a patient with a transfusion-related acute lung injury (TRALI) to whom extracorporeal membrane oxygenation (ECMO) had been applied is reported. A 55-year-old male injured with liver laceration (grade 3) without chest injury after car accident. He received lots of blood transfusion and underwent damage control abdominal surgery. In the immediate postoperative period, he suffered from severe hypoxia and respiratory acidosis despite of vigorous management such as 100% oxygen with mechanical ventilation, high PEEP and muscle relaxant. Finally, ECMO was applied to the patients as a last resort. Aggressive treatment with ECMO improved the oxygenation and reduced the acidosis. Unfortunately, the patient died of liver failure and infection. TRALI is a part of acute respiratory distress syndrome (ARDS). The use of ECMO for TRALI induced severe hypoxemia might be a useful option for providing time to allow the injured lung to recover.

Bispecific Antibody-Bound T Cells as a Novel Anticancer Immunotherapy

  • Chimeric antigen receptor T (CAR-T) cell therapy is one of the promising anticancer treatments. It shows a high overall response rate with complete response to blood cancer. However, there is a limitation to solid tumor treatment. Additionally, this currently approved therapy exhibits side effects such as cytokine release syndrome and neurotoxicity. Alternatively, bispecific antibody is an innovative therapeutic tool that simultaneously engages specific immune cells to disease-related target cells. Since programmed death ligand 1 (PD-L1) is an immune checkpoint molecule highly expressed in some cancer cells, in the current study, we generated αCD3xαPD-L1 bispecific antibody (BiTE) which can engage T cells to PD-L1+ cancer cells. We observed that the BiTE-bound OT-1 T cells effectively killed cancer cells in vitro and in vivo. They substantially increased the recruitment of effector memory CD8+ T cells having CD8+CD44+CD62Llow phenotype in tumor. Interestingly, we also observed that BiTE-bound polyclonal T cells showed highly efficacious tumor killing activity in vivo in comparison with the direct intravenous treatment of bispecific antibody, suggesting that PD-L1-directed migration and engagement of activated T cells might increase cancer cell killing. Additionally, BiTE-bound CAR-T cells which targets human Her-2/neu exhibited enhanced killing effect on Her-2-expressing cancer cells in vivo, suggesting that this could be a novel therapeutic regimen. Collectively, our results suggested that engaging activated T cells with cancer cells using αCD3xαPD-L1 BiTE could be an innovative next generation anticancer therapy which exerts simultaneous inhibitory functions on PD-L1 as well as increasing the infiltration of activated T cells having effector memory phenotype in tumor site.

한국판 아스퍼거 증후군 진단척도(The Korean version of Asperger Syndrome Diagnostic Scale : K-ASDS) 의 신뢰도 및 타당도 연구 (A STUDY OF RELIABILITY & VALIDITY FOR THE KOREAN VERSION OF ASPERGER SYNDROME DIAGNOSTIC SCALE)

  • 연구 목적 : 국내에서는 아스퍼거 장애를 평가하고 진단하는 구조화된 검사 도구의 개발이 거의 이루어지지 않은 상태로, 아스퍼거 장애에 대한 진단적 타당도의 검증 및 향후 연구의 활성화를 위해 본 연구에서는 한국판 아스퍼거 증후군 진단 척도(the Korean version of Asperger's Syndrome Diagnostic Scale)의 신뢰도와 타당도를 검증하는 연구를 수행하였다. 방법 : 소아정신과 외래를 방문한 아동들 중 DSM-IV 진단기준과 소아정신과 의사에 의해 아스퍼거 장애, 전반적 발달 장애(PDD)와 비전형 전반적 발달 장애(PDD NOS), 의사소통 장애로 진단된 아동을 대상으로 그들의 부모들에게 한국어로 번역한 ASDS를 실시하였다. 총 55명이 연구에 포함되었으며(아스퍼거 장애 15명, PDD와 PDD NOS 20명, 의사소통 장애 20명), 연령의 범위는 5세 1개월부터 15세 6개월까지였다. 공존 타당도 및 변별 타당도를 알아보기 위하여 또 다른 아스퍼거 장애 질문지와 자폐 증상을 평가하는 K-CARS를 함께 실시하였다. 결과 : 자료 분석 결과, K-ASDS의 내적 합치도는 Chronbach's $\alpha=0.88$로 높게 나타났으며, 각 하위척도별 내적 일치도 또한 높았다. K-ASDS와 아스퍼거 장애 질문지 간의 상관도 통계적으로 유의미하였고, 하위 척도 상에서 Asperger disorder, PDD, Communication disorder 집단 간의 차이도 유의미하였다. 판별 분석 결과, K-ASDS가 자폐 장애와 의사소통 장애로부터 아스퍼거 장애를 유의미하게 구분해주는 판별 함수를 얻었으며, 이들 장애를 구분해주는 전체 정확 판별율은 $71\%$였다. 결론 : 본 연구 결과, 한국판 ASDS의 신뢰도와 타당도가 입증되었으며, 아스퍼거 장애 아동들을 평가하고 진단하는데 유용하게 사용될 수 있을 것이다.

흉벽 손상후 발생한 외상성 Hemobilia -2례 보고- (Traumatic Hemobilia Following Blunt Chest Trauma -Report of 2 Cases-)

  • Hemorrhage into the biliary system as a consequence of injury to the liver has been called "traumatic hemobilia," a term introduced by Sandblom in 1948. The source of gastrointestinal hemorrhage has been frequently misinterpreted, resulting in inadequate or inappropriate treatment, often with catastrophic results and needless fatalities. It is now being diagnosed with increasing frequency, due to more widespread knowledge of the syndrome and improved diagnostic means. we experienced 2 cases of hemobilia following blunt chest trauma, One patient had! multiple rib fractures on right chest by car traffic accident and 13 days later, suddenly massive melena was developed with nausea, vomiting, jaundice and severe pain on right upper quadrant. And so, he had operated on the ligation of Rt. hepatic artery and partial right hepatectomy for a traumatic hemobilia. The other one also revealed similar symptoms 20 days later following blunt chest injury by falling down accident. However, uneventful recovery was seen without any of surgical intervention in this case.

다발성 외상으로 유발된 급성호흡부전증후군 환자에서 정맥-정맥동맥 체외막 산화기(ECMO)의 적용 - 1예 보고 - (Application of Veno-venoarterial Extracorporeal Membrane Oxygenation in Multitrauma Patient with ARDS - A case report -)

  • 급성호흡부전증후군은 치료가 어렵고 치명율이 높다. 인공호흡기 치료에 반응하지 않는 중증의 경우 체외막 산화기(Extracorporeal membrane oxygenation, ECMO)가 적용되고 있다. 교통사고로 다발성 외상을 받은 21세 여자가 흉부 방사선상 양측폐에 심한 침윤 소견과 동맥혈가스 검사상 급성호흡부전증후군(Acute respiratory distress syndrome, ARDS)으로 판단되어 정맥동맥 체외산화기(Veno- arterial Extracorporeal Membrane Oxygenation, VA ECMO) 치료를 시작하였다. ECMO 적용 2일째 흉부 방사선상 폐 침윤 소견 악화되면서 동맥혈 산소 포화도(Systemic oxygen saturation, $SaO_2$)가 85% 미만으로 저하되었다. 이에 중추신경계와 폐의 산소화를 향상시키기 위해 ECMO의 유출로에 곁회로를 이용하여 우측 경정맥에 연결하여 정맥-정맥동맥 ECMO (Veno-venoarterial ECMO, V-VA ECMO)로 전환하였고 이후 동맥혈 산소 분압($PaO_2$) 65 mmHg, $SaO_2$ 94%로 향상되었다. V-VA ECMO로의 전환은 전신에 산소 공급을 늘리기 위해 고려해 볼 수 있는 방법으로 생각된다.

  • In this study, an air cleaner is considered to improve comfort, safety, and health of automobile passengers. The performance and installation conditions of the air cleaner have been studied to investigate their effects on the air quality in the cabin room using numerical analysis. A five-passenger sedan and a seven-passenger minivan that have comparatively large indoor volume have been considered. The distributions of the local mean age and the volume averaged age of indoor air are calculated according to the variation of the placement and the air flow of the air cleaner. In addition, a decrease of contamination concentration, especially VOCs(volatile organic compounds), by the air cleaner is numerically analyzed with time-accurate unsteady calculation to quantify the effect of the air cleaner on the indoor air quality. As a result, the effective installation and operation conditions of the air cleaner for the automobile cabin room could be suggested.
