Comparative fracture strength analysis of Lava and Digident CAD/CAM zirconia ceramic crowns

  • PURPOSE. All-ceramic crowns are subject to fracture during function. To minimize this common clinical complication, zirconium oxide has been used as the framework for all-ceramic crowns. The aim of this study was to compare the fracture strengths of two computer-aided design/computer-aided manufacturing (CAD/CAM) zirconia crown systems: Lava and Digident. MATERIALS AND METHODS. Twenty Lava CAD/CAM zirconia crowns and twenty Digident CAD/CAM zirconia crowns were fabricated. A metal die was also duplicated from the original prepared tooth for fracture testing. A universal testing machine was used to determine the fracture strength of the crowns. RESULTS. The mean fracture strengths were as follows: $54.9{\pm}15.6$ N for the Lava CAD/CAM zirconia crowns and $87.0{\pm}16.0$ N for the Digident CAD/CAM zirconia crowns. The difference between the mean fracture strengths of the Lava and Digident crowns was statistically significant (P<.001). Lava CAD/CAM zirconia crowns showed a complete fracture of both the veneering porcelain and the core whereas the Digident CAD/CAM zirconia crowns showed fracture only of the veneering porcelain. CONCLUSION. The fracture strengths of CAD/CAM zirconia crowns differ depending on the compatibility of the core material and the veneering porcelain.

  • CAD/CAM systems (computer-aided design / computer-aided manufacturing) used for decades in restorative dentistry have its application to implant dentistry. This study aimed to overview CAD/CAM systems used implant dentistry, especially emphasizing custom implant abutments manufacturing. CAD/CAM custom abutments present the advantages of being specific to each patient and providing a better fit than the stock and cast custom abutments. This cutting edge technology of virtual-designed and computer-milled implant abutments will likely replace traditional implant restorative protocols and become the standard for implant dentistry in the foreseeable future.

  • CAD/CAM 기술은 이미 치과 보철학 영역에서 전반적으로 사용되고 있으며, 1994년 총의치 제작을 위한 CAD/CAM 기술이 처음 소개된 이후 의치 제작과정의 CAD/CAM 적용범위가 증가되고 있는 추세이다. 본 논문에서는 무치악 환자의 양악 총의치 제작을 위한 전통적인 다섯 단계 중 납의치 제작과정에 CAD/CAM기술을 도입하였다. 최종 인상채득하여 악간 관계를 기록한 주모형을 스캔한 후 Amann Girrbach사의 Ceramill full denture system (fds)을 이용해 납의치를 제작하였다. Ceramill fds는 스캔된 상하악 모형에 전용 소프트웨어를 이용해 인공치 배열 및 의치상을 제작하여 이를 밀링하는 방식으로 기존의 기공과정을 디지털로 전환하는 특징을 가진다. 이를 통해 총의치 제작시간의 단축 및 심미적, 기능적으로 만족스러운 의치제작이 가능했기에 본 증례를 보고하고자 하는 바이다.

  • Currently with the continuous development of ceramic and cementation materials, CAD-CAM(Computer-aided design/Computer-aided manufacture) restorations are becoming increasingly popular in esthetic dentistry. Preparation design is influenced by the selected restorative material, the fabrication method, and the ability to bond the restoration. For long-lasting CAD/CAM inlay/onlay restoration, clinicians should understand the basic knowledge of CAD/CAM restoration's cavity design to obtain the fracture resistance and proper fitting margin. This article gives an overview of preparation guidelines for CAD/CAM inlay/onlay restorations.

  • 본 증례는 다운 증후군이 있는 환자의 구강 재건 시 CAD/CAM 시스템 사용의 이점을 모색해보았다. 다운 증후군 환자들의 경우, 보철물 파절의 위험성이 크며 불가피하게 보철물을 재제작할 경우, CAD/CAM 시스템의 기존 데이터를 이용하여 최소한의 기공 과정만 진행하여 보철물을 빠른 시일 내에 재제작할 수 있다. 결과적으로 이는 심미적 및 기능적 개선을 통해 삶의 질을 높일 수 있는 보철물 제작 시 환자의 불편감과 치과의사의 노력을 최소화하는 방법이다.

  • Novel methods producing supplementary and prosthetic material by cutting or discharge processing via computer design have been proposed as alternatives for traditional casting methods and are being utilized for commercial purposes. The CAD/CAM system used in dentistry can be classified into three-dimensional input of target values, restoration design, and material processing. The marginal fidelity in production of In-Ceram core has important clinical implications and is a key consideration issue in CAD/CAM production as well. Through this research, the author arrived at the following conclusion aaer conducting comparison analysis of marginal fidelities between the In-Ceram core produced via CAD/CAM and that produced through the traditional method ; 1. In the cases of mesial, distal, and lingual margins, the core margins via CAD/CAM produced lower values than those via the traditional method, but the differences were found to be statistically insignificant. 2. In the case of labial flange, the core margins via CAD/CAM produced lower values than those via the traditional method and the differences were found to be statistically significant. (p<0.05) 3. In comparision with overall marginal fidelity, the core margins via CAD/CAM produced lower values than those via the traditional method, but the differences were found to be statistically insignificant. 4. Among the core margins produced via the traditional method did not have statistically significant differences but fir those produced via CAD/CAM had statistically significant differences between labial and lingual sides and between labial and mesial sides. (p <0.05).

Marginal and internal fit of nano-composite CAD/CAM restorations

  • Objectives: The purpose of this study was to compare the marginal and internal fit of nano-composite CAD-CAM restorations. Materials and Methods: A full veneer crown and an mesio-occluso-distal (MOD) inlay cavity, which were prepared on extracted human molars, were used as templates of epoxy resin replicas. The prepared teeth were scanned and CAD-CAM restorations were milled using Lava Ultimate (LU) and experimental nano-composite CAD/CAM blocks (EB) under the same milling parameters. To assess the marginal and internal fit, the restorations were cemented to replicas and were embedded in an acrylic mold for sectioning at 0.5 mm intervals. The measured gap data were pooled according to the block types and measuring points for statistical analysis. Results: Both the block type and measuring point significantly affected gap values, and their interaction was significant (p = 0.000). In crowns and inlays made from the two blocks, gap values were significantly larger in the occlusal area than in the axial area, while gap values in the marginal area were smallest (p < 0.001). Among the blocks, the restorations milled from EB had a significantly larger gap at all measuring points than those milled from LU (p = 0.000). Conclusions: The marginal and internal gaps of the two nano-composite CAD/CAM blocks differed according to the measuring points. Among the internal area of the two nano-composite CAD/CAM restorations, occlusal gap data were significantly larger than axial gap data. The EB crowns and inlays had significantly larger gaps than LU restorations.

성공적인 캐드캠 수복을 위한 치아형성과 구강스캔 (Tooth preparation and Intraoral scanning for successful CAD/CAM restorations)

  • In recent years, with the introduction of various restorative materials, restorations using CAD/CAM equipment have been increasing in the esthetic dentistry. The critical steps in the fabrication of indirect restorations with CAD/CAM equipment are proper cavity preparation and making accurate impressions. The process of tooth preparation for CAD/CAM restoration should include a mechanical understanding of milling. In addition, during tooth preparation, the clinician should be familiar with additional equipment and techniques for obtaining the convenience. In order to obtain an accurate oral scan, the clinician should understand the limitations of the oral scan and be skilled at techniques for obtaining a successful image when making oral scans. This article focused clinical guidelines for the preparation of CAD/CAM restorations and introduced clinical methods for making successful impression of oral scans in narrow and deep tooth cavity areas.
