농촌 주민을 위한 심폐소생술 교육 효과 (The Effect of CPR Education in a Rural Community)

  • Purpose: Bystander CPR has demonstrated improved survival of out-of-hospital cardiac arrest. The purpose of this study was to evaluate the effects of CPR education in the rural community. Method: One hundred eighty-nine rural laypersons (105 in the experimental group and 84 in the control group) participated in this study. In the experimental group, questionnaires were completed after participating in a small group BLS (Basic Life Support) course. In the control group questionnaires were collected from two rural health service centers. Data were analysed using $X^2$test, independent t-test and paired t-test. Result: There were significant differences in barriers to performing CPR, attitude and intention toward CPR and necessity recognition between the two groups. Conclusion: The proportion of positive attitudes toward CPR and willingness to perform CPR was higher in the experimental group than the control group. CPR education increased anxiety of being sued because of a bad outcome. For future CPR education for laypersons, the reluctance of bystanders to perform CPR should be reconsidered and CPR education should be extended to the rural community.

폐암에서의 Herpes Simplex Virus Thymidine Kinase 유전자 치료와 Cytokine 유전자 치료의 복합요법 (Combination Gene Therapy of Herpes Simplex Virus Thymidine Kinase and Cytokines in Lung Cancer)

  • 배 경 : 암세포는 정상 세포와 다르므로 면역 기전에 의해 제거되어야 함에도 불구하고 암이 발생하는 것은 암세포가 면역 감시 체계를 회피하기 때문이다. 약제감수성 증강 유전자인 herpes simplex thymidine kinase(HSTK) 유전자를 이용하여 암세포를 파괴하여 종양 특이항원이 더 잘 유리되도록 하고 사이토카인 유전자를 이용하여 면역세포를 유도하여 이 장애를 극복할 수 있는지 보고자 이 실험을 하였다. 방 법 : Lewis 폐암 세포주(LLC)에 adenovirus를 이용하여 HSTK를 형질도입하고 이에 의해 ganciclovir에 대한 LLC의 감수성을 증강시키는지를 관찰하고 mixed population assay를 이용하여 bystander 효과를 관찰하였다. Ad-HSTK, Ad-IL-2, Ad-GMCSF의 형질도입이 LLC의 종양 형성 능력에 영향을 미치는지 관찰하였다. 또한 그러한 형질 도입이 기존의 종양에 대한 항암 효과를 가져오는지 관찰하였다. 항암 효과의 기전을 확인하기 위해 쥐의 비장을 관찰하였다. 결 과 : Ad-HSTK의 형질도입은 ganciclovir에 대한 LLC의 감수성을 현저하게 증강시켰다. Ad-HSTK, Ad-IL-2, Ad-GM-CSF를 형질도입한 LLC를 쥐에 주사하고 ganciclovir로 처리하였을 때 종양 형성 능력이 감소하였다. Ad-HSTK, Ad-IL-2, Ad-GM-CSF를 형질도입한 LLC를 종양백신으로 사용하였을때 종양 성장이 어느 정도 저해되는 것을 관찰하였다. 특히 HSTK와 GM-CSF를 복합 형질도입했을 때 더 강력한 항암효과가 일어나는 것을 관찰하였다. 그러나 HSTK와 IL-2을 복합 형질도입했을 때는 각각을 단독으로 형질도입했을 때보다 항암효과가 상승되지 않았다. HSTK와 GM-CSF의 복합 형질도입한 LLC를 종양백신으로 사용하였을 때 비장의 수상세포 침윤이 현저히 증가하였다. 결 론 : HSTK와 GM-CSF의 복합 형질도입으로 만든 종양백신은 수상세포를 활성화시키므로써 항암면역기능을 유의하게 증강시킬 수 있었다.

  • Microbeam is a new avenue of radiation research especially in radiation biology and radiation protection. Selective irradiation of an ionizing particle to a targeted cell organelle may disclose such mechanisms as signal transaction among cell organelles and cell-to-cell communication in the processes toward an endpoint observed. Bystander effect, existence of which is clearly evidenced by application of the particle microbeam to biological experiments, suggests potential underestimation in the conventional risk estimation at low particle fluence rates, such as environment of space radiations in ISS (International Space Station). To promote these studies we started the construction of our microbeam facility (named as SPICE) to our HVEE Tandem accelerator (3.4 MeV proton and 5.1 MeV $^4$He$\^$2+/). For our primary goal, "irradiation of single particle to cell organelle within a position resolution of 2 micrometer in a reasonable irradiation time", special features are considered. Usage of a triplet Q magnet for focussing the beam to submicron of size is an outstanding feature compared to facilities of other institutes. Followings are other features: precise position control of cell dish holder, design of the cell dish, data acquisition of microscopic image of a cell organelle (cell nucleus) and data processing, a reliable particle detection, soft and hard wares to integrate all these related data, to control and irradiate exactly determined number of particles to a targeted spot.

Effects of Tumor Microenvironmental Factors on DNA Methylation and Radiation Sensitivity in A549 Human Lung Adenocarcinoma

  • Background: Tumor response to anticancer therapies can much be influenced by microenvironmental factors. In this study, we determined the effect of these microenvironmental factors on DNA methylation using A549 human lung adenocarcinoma cell line. Materials and Methods: We subjected A549 cells to various conditions mimicking tumor microenvironment including hypoxia, acidosis (sodium lactate), oxidative stress ($H_2O_2$), bystander effect (supernatant from doxorubicin (Dox)-treated or irradiated cells), and immune cell infiltration (supernatant from THP-1 or Jurkat T cells). Genomic DNA was isolated from these cells and analyzed for DNA methylation. Clonogenic cell survival, gene expression, and metabolism were analyzed in cells treated with some of these conditions. Results and Discussion: We found that DNA methylation level was significantly decreased in A549 cells treated with conditioned media from Dox-treated cells or Jurkat T cells, or sodium lactate, indicating an active transcription. To determine whether the decreased DNA methylation affects radiation sensitivity, we exposed cells to these conditions followed by 6 Gy irradiation and found that cell survival was significantly increased by sodium lactate while it was decreased by conditioned media from Dox-treated cells. We further observed that cells treated with conditioned media from Dox-treated cells exhibited significant changes in expression of genes including BAX and FAS (involved in apoptosis), NADPH dehydrogenase (mitochondria), EGFR (cellular survival) and RAD51 (DNA damage repair) while sodium lactate increased cellular metabolism rather than changing the gene expression. Conclusion: Our results suggest that various tumor microenvironmental factors can differentially influence DNA methylation and hence radiosensitivity and gene expression in A549 cancer cells.
