Anthropometric analysis of maxillary anterior buccal bone of Korean adults using cone-beam CT

  • PURPOSE. The aim of this study was to evaluate the thickness of buccal and palatal alveolar bone and buccal bony curvature below root apex in maxillary anterior teeth of Korean adults using Cone-beam CT images. MATERIALS AND METHODS. The 3D image was reconstructed with dicom file obtained through CBCT from 20 - 39 year old Korean subjects (n = 20). The thickness of buccal and palatal plate, root diameter, the buccal bony curvature angle below root apex and the distance from root apex to the deepest point of buccal bony curvature were measured on maxillary anterior teeth area using OnDemand3D program. RESULTS. Mean thickness of buccal plate 3 mm below CEJ was $0.68{\pm}0.29\;mm$ at central incisor, $0.76{\pm}0.59\;mm$ at lateral incisor, and $1.07{\pm}0.80\;mm$ at canine. Mean thickness of palatal plate 3 mm below CEJ was $1.53{\pm}0.55\;mm$ of central incisor, $1.18{\pm}0.66\;mm$ of lateral incisor, $1.42{\pm}0.77\;mm$ of canine. Bucco-lingual diameter 3 mm below CEJ was $5.13{\pm}0.37\;mm$ of central incisor, $4.58{\pm}0.46\;mm$ of lateral incisor, and $5.93{\pm}0.47\;mm$ of canine. Buccal bony curvature angle below root apex was $134.7{\pm}17.5^{\circ}$ at central incisor, $151.0{\pm}13.9^{\circ}$ at lateral incisor, $153.0{\pm}9.5^{\circ}$ at canine. Distance between root apex and the deepest point of buccal bony curvature of central incisor was $3.67{\pm}1.28\;mm$ at central incisor, $3.90{\pm}1.51\;mm$ at lateral incisor, and $5.13{\pm}1.70\;mm$ at canine. CONCLUSION. Within the limitation of this study in Korean adults, the thickness of maxillary anterior buccal plate was very thin within 1mm and the thickness of palatal plate was thick, relatively. The buccal bony curvature below root apex of maxillary central incisor was higher than that of lateral incisor and canine and it seems that the buccal bony plate below root apex of central incisor is most curved.

  • 어린이의 공간유지와 교합유도의 치료를 위하여 가철성 장치는 소아치과 임상에서 일상적으로 사용되고 있다. 상악의 경우와는 달리 하악에서는 가철성 장치를 사용하는데 있어 유지력의 저하에 따른 치료효과의 감소, 환자의 불편감과 거부감으로 인한 미착용 등의 문제가 흔히 발생한다. 이것은 유치열기나 초기 혼합치열기 어린이의 경우, 하악구치부의 undercut양이 충분치 못함에도 불구하고, 여기에 협측 undercut으로부터 유지력을 얻는 각종 clasp를 적용하기 때문으로 사료된다. 또한 전통적인 가철성 장치들의 acrylic base plate가 설측에 위치하고 있어 설측 연조직이나 치조능 undercut과 많은 문제를 일으키는 현상을 볼 수 있다. 어린이들은 하악 장치의 작업모형을 위한 인상채득과정에서 술자의 지시대로 혀를 바르게 거상하지 못하는 예가 많은 것도 그 원인요소로 들 수 있다. 본 연구는 전통적인 하악의 Hawley acrylic plate로부터 clasp와 acrylic baseplate의 위치를 바꾸어 협측 base plate와 설측 clasp을 이용한 buccal acrylic appliance를 유치열기 및 초기 혼합치열기 어린이의 구강에 적용하여 그 장단점을 조사하고 환아들의 수용도와 효율성을 기존의 lingual acrylic appliance와 비교, 검토할 목적으로 시행되었다. 공간유지 및 치아이동을 필요로 하는 5개 증례를 대상으로 각 증례에 buccal acrylic appliance와 기존의 lingual acrylic appliance를 모두 적용하여, 유지력, 환자 수용도, 혀의 기능과의 조화, 치료 효과 등의 측면을 상호 비교해 보았다. 조사 결과, 조사 대상의 모든 증례에서 유지력과 환아의 수용도 측면에서 협측 장치가 설측 장치에 비해 월등히 우수한 것으로 나타났다. 또한 치료 효과의 측면에서도 협측 장치는 설측 장치와 유사한 것으로 나타났다. 본 연구를 통하여 하악에 가철성 장치를 사용할 경우, 전통적인 설측 장치와 관련된 각종 부작용을 해결하는 데에 buccal acrylic appliance가 훌륭한 대안으로서 임상적용을 추천할 만 하다고 사료되었다.

성견의 발치와 협측골 외측에 적용한 교원질 차단막과 재조합 골형성단백질의 치조제 보존 효과 (Application of rhBMP-2 loaded collagen membrane on the buccal plate for ridge preservation: a pilot study in dogs)

  • Objective: The aim of this study was to determine the effect of multi-layer of a collagen membrane alone or loaded rhBMP-2 on the buccal plate for ridge preservation after tooth extraction. Material and methods: Following bilateral extraction of the maxillary 1st and 3rd premolars in five dogs, rhBMP-2 loaded collagen membrane was applied to the buccal plates at the 1st premolar and collagen membrane only was applied to the buccal plates at the 3rd premolar unilaterally. The collagen membranes applied in the experimental groups were piled into four layers. The corresponding sites of the contralateral side healed naturally. After 3months of healing, the animals were sacrificed. Radiographic and histologic analyses were performed. Results: There was no significant difference in the healing of extraction socket at both 1st and 3rd premolars. In microcomputed tomography, the widths of the residual ridge of the experimental groups were similar with the controls. Histologically, the experimental groups did not exhibit different pattern compared to the controls regardless of the addition of rhBMP-2. Conclusion: Layering of the collagen membrane with or without rhBMP-2 on the buccal plate failed to show the effectiveness in dimensional preservation of the extraction socket.

하악 협측골 시상분절술 및 장골 이식술을 이용한 거대치아종의 치험례 (A CASE REPORT OF THE HUGE COMPLEX ODONTOMA TREATED WITH THE SAGITTAL SPLITTING OF BUCCAL BONE PLATE AND ILIAC BONE GRAFT IN LEFT MANDIBLE ANGLE)

  • This is the case report of huge complex odontoma treated with sagittal splitting of buccal bone plate and iliac bone graft in left mandible angle. The 22 years old patient was admitted to the department of Oral and Maxillofacial Surgery of Kyungpook National University Hospital with the chief complaint of swelling on the left mandible angle area. We used extra oral Risdon incision and splitted the buccal cortical bone after making the horizontal bone cut buccally. The tumor mass was removed with cutting into the pieces with surgical bur to prevent mandibular fracture. The dead space was grafted with autogenous iliac bone graft and the splitted buccal cortical bone was fixed with two L-type miniplate. After 12months follow up check, we noticed good process of bone healing and satisfactory aesthetic result. In this case, my operative approach provided the excellent surgical access to the hard tissue mass and minimized post operative complication comparing with the conventional surgical approaches.

Analysis of the root position of the maxillary incisors in the alveolar bone using cone-beam computed tomography

  • Purpose: The purpose of this study was to measure the buccal bone thickness and angulation of the maxillary incisors and to analyze the correlation between these parameters and the root position in the alveolar bone using cone-beam computed tomography (CBCT). Materials and Methods: CBCT images of 398 maxillary central and lateral incisors from 199 patients were retrospectively reviewed. The root position in the alveolar bone was classified as buccal, middle, or palatal, and the buccal type was further classified into subtypes I, II, and III. In addition, the buccolingual inclination of the tooth and buccal bone thickness were evaluated. Results: A majority of the maxillary incisors were positioned more buccally within the alveolar bone, and only 2 lateral incisors(0.5%) were positioned more palatally. The angulation of buccal subtype III was the greatest and that of the middle type was the lowest. Most of the maxillary incisors exhibited a thin facial bone wall, and the lateral incisors had a significantly thinner buccal bone than the central incisors. The buccal bone of buccal subtypes II and III was significantly thinner than that of buccal subtype I. Conclusion: A majority of the maxillary incisor roots were positioned close to the buccal cortical plate and had a thin buccal bone wall. Significant relationships were observed between the root position in the alveolar bone, the angulation of the tooth in the alveolar bone, and buccal bone thickness. CBCT analyses of the buccal bone and sagittal root position are recommended for the selection of the appropriate treatment approach.

A CASE REPORT OF THE MANDIBULAR INFECTED BUCCAL CYST

  • The author observed a cystic lesion which had new radiographic entities in the 6 years old male patient and obtained the following features. 1. The main clinical feature is the swelling. 2. The lesion is situated on the buccal surface of a erupting mandibular first molar. 3. The affected molar is tilted, so that the apices are adjacent to the lingual plate of the mandible. 4. The affected molar is of normal morphology, caries-free and vital. 5. The radiographic feature is the cystic radiolucency which is extended over the buccal surface of the roots of the affected molar and slightly inferior to the root apices. 6. The dental papilla of the affected molar shows the lamina dura of relatively normal density and definitition. 7. The buccal surface shows the laminated new bone formation. 8. The inferior concave border of the lesion is delineated by a thick and calcified layer of cortical bone. 9. The lesion causes displacement of the developing second molar of which anterior cortex is partially destroyed.

전산화단층사진상을 이용한 하악관의 위치 및 하악골의 피질골 두께에 관한 연구 (A Computerized Tomographic Study on the Location of the Mandibular Canal and the Cortical Thickness of the Mandible)

  • The location of the mandibular canal and the cortical thickness of the mandible is important in the practice of dentistry. This study was performed on twenty chosen dry mandibles, which were of adults and included fully erupted premolars and molars. The purpose of this study was to evaluate the location of the mandibular canal and the cortical thickness of the mandible on computed tomograms and to aid in the surgical treatment plans. The obtained results were as follows; 1. The horizontal distance between the mandibular canal and the buccal external border was 6.6±0.9mm on Somesial root of the first molar), and it was increased posteriorly. The horiwntal distance between the mandibular canal and the lingual external border was 4.1±1.lmm on S/sub 0/, and it was decreased posteriorly. 2. The vertical distance between the alveolar crest and the mandibular canal was 16.9±1.6mm on S/sub 0/, and it was decreased posteriorly. The vertical distance between the inferior border of mandible and the mandibular canal was 8.8±1.3mm on S/sub 0/, and it was increased anteriorly and posteriorly. 3. The thickness of the buccal cortical plate was 2.2±0.4mm on S/sub 0/. and it was increased posteriorly. But, that of the lingual cortical plate was 2.0±0.6mm on S/sub 0/ and it was decreased posteriorly. 4. The area of the buccal cortical plate was 66.5±1.0mm² on S/sub 0/. and it was increased posteriorly. But, that of the lingual cortical plate was 65.8±0.9mm² on S/sub 0/ and it was decreased posteriorly.

Assessment of the relationship between the maxillary molars and adjacent structures using cone beam computed tomography

  • Purpose: This study investigated the relationship between the roots of the maxillary molars and the maxillary sinus using cone beam computed tomography (CBCT), and measured the distances between the roots of the maxillary molars and the sinus floor as well as the thickness of the bone between the root and the alveolar cortical plate. Materials and Methods: The study sample consisted of 83 patients with normally erupted bilateral maxillary first and second molars. A total of 332 maxillary molars were examined using CBCT images. The vertical relationship of each root with the maxillary sinus was classified into four types on CBCT cross-sectional images. The distance between the sinus floor and root and the bone thickness between the root and alveolar cortical plate were measured. Results: In the buccal roots of the maxillary molars, a root protruding into the sinus occurred most frequently. A root projecting laterally along the sinus cavity was most common in the palatal roots of the maxillary first molars. The mesiobuccal roots of the maxillary second molar were closest to the sinus. The mesiobuccal roots of the first molars were closest to the cortical plate. Conclusion: The relationship between the roots of the maxillary molars and the sinus differed between the buccal and palatal roots. A root protruding into the sinus occurred more frequent in the buccal roots of the maxillary molars. The mesiobuccal root of the maxillary second molar was closest to the maxillary sinus floor and farthest from the alveolar cortical plate.

Proximity of maxillary molar apexes to the cortical bone surface and the maxillary sinus

  • Objectives: This study aimed to analyze the proximity of maxillary molar roots to their overlying cortical bone surfaces and the maxillary sinus. Materials and Methods: Cone-beam computed tomographic images of 151 patients with completely erupted upper molars that had 3 separate roots were studied. The following distances were measured: from the root apex to the cortical plate and maxillary sinus floor, and from the apical 3-mm level of the root to the cortical plate. Differences between groups were analyzed with 1-way analysis of variance and the Scheffé post hoc test, the significance of differences between cone-beam computed tomography views with the paired t-test, and the significance of differences among age groups with linear regression analysis. The significance level was set at p < 0.05. Results: The mesiobuccal and distobuccal root apexes of maxillary second molars were more distant from the buccal cortical plate than the maxillary first molars (p < 0.05). The apical 3-mm level of the mesiobuccal root of the first molar was closer to the buccal cortical bone than the second molar (p < 0.05). In the maxillary first molars, the thickness of the buccal cortical bone decreased in all roots with age (p < 0.05). In all root apexes of both molars, the difference in the vertical level between the maxillary sinus floor and the root apex increased with age (p < 0.05). Conclusions: Awareness of the anatomical profile of maxillary molar apices in relation to the cortical bones and maxillary sinus will be beneficial for apical surgery.

압축하중시 RPI clasp의 3가지 다른 proximal plate 형태에 따른 지대치 주위조직의 광탄성 응력 분석 (Photoelastic Stress Analysis of the Abutment Surrounding Tissue According to Shape of the Proximal Plate of the RPI Clasp)

  • Purpose: The purpose of this study was to investigate the stress distribution of the surrounding tissues according to 3 proximal plate types of the RPI clasp. Methods: The removeable partial denture which mandibular right and left second premolars and mandibular molars were lost was attached to a three dimensional photo elastic epoxy resin model. Then 120N of vertical load was applied. After 3-dimensional photoelastic stress analysis was used to record the isochromatic fringe patterns. Results: Kratochvil type guiding plane exhibited little uniform stress distribution on load center and alveolar ridge, but higher stress concentration on buccal surface of second premolar. Krol type guiding plane exhibited the stress concentration on the front of load center and relatively higher stress concentration on buccal surface of first premolar. However, this type had no effect on canine. Researcher type guiding plane showed the stress concentration on second premolar and molar, but the little stress distribution on first premolar. Conclusion: In all types, excessive stress concentration was appeared and three types were not significant different.
