질병지각에 대한 체계적 문헌고찰 - 국내 학술지 논문을 중심으로 - (A Systematic Review of Illness Perceptions Using the IPQ-R or BIPQ in South Korea)

  • 본 연구는 국내 질병지각 연구의 동향을 알아보기 위한 체계적 문헌고찰로서 질병지각 연구의 특성을 확인하고 이와 관련된 요인 간의 관계를 파악하였다. 2019년 5월까지 국내 학술지에 발표된 논문 10편을 최종 분석에 사용하였다. 포함된 논문 모두 상관관계 조사연구로, 평균 대상자 수는 175명이었으며, 혈액투석, 폐결핵, 암 등의 질병을 대상으로 하였다. 질병지각 측정 도구로는 Illness Perception Questionnaire-Revised (IPQ-R)를 사용한 연구가 3편(30%), Brief Illness Perception Questionnaire (BIPQ)를 사용한 연구가 7편(70%)이었고, BIPQ를 사용한 연구는 모두 총점을 이용하여 대상자의 질병지각을 분석하였다. 질병지각과 관련된 요인으로는 우울, 삶의 질, 자가관리, 사회적 지지 등이 연구되었으며, 그 중 사회적 지지가 가장 높은 빈도로 대상자의 질병지각과 관련이 있는 것으로 나타났다. 추후 다양한 질병군을 대상으로 질병지각 개별 영역에 대한 연구가 수행되어야 할 것으로 보이며, 질병지각을 고려한 중재프로그램을 개발해야 할 필요성이 있다.

혈액투석 환자의 우울과 삶의 질에서 질병인식의 통제효과 (Control Effect of Illness Perception on Depression and Quality of Life in Patients with Hemodialysis: Using Structural Equation Modeling)

  • Purpose: This study aimed to evaluate the effect of illness perception on depression and quality of life in patients with hemodialysis. Methods: This is a cross-sectional study using structural equation modeling. Depression, quality of life and cognitive and emotional illness perceptions were measured by the Center for Epidemiologic Studies Depression-10 (CESD-10), Short Form-36 version 2 (SF-36v2), and Brief Illness Perception Questionnaire (BIPQ) in 272 hemodialysis patients. Results: Prevalence of depression was 76.1%, and higher in female patients on hemodialysis. Cognitive illness perception had effect on depression and quality of life, while emotional illness perception had effect only on depression. Quality of life was explained 69.6% through depression (${\beta}=-.74$, t= -8.17) and cognitive illness perceptions (${\beta}=.21$, t= 3.06), while depression was explained 40.5% through cognitive (${\beta}=-.34$, t= -5.99) and emotional (${\beta}=.59$, t= 9.37) illness perceptions. Conclusion: In the inevitable choice of dialysis, illness perception could decrease depression and improve quality of life in hemodialysis patients.

폐결핵 환자의 질병지각, 자기효능감과 자가간호 행위와의 관계연구 (Relationship of Illness Perception, Self-efficacy, and Self-care among Pulmonary Tuberculosis Patients)

  • Purpose: A descriptive study was conducted to examine the relationship of illness perception, self-efficacy, and self-care of patients who were diagnosed pulmonary tuberculosis. Methods: The study was conducted using a convenient sample from two hospitals in Incheon and Seoul. One hundred forty four patients completed a questionnaire which included questions on general characteristics, a Brief Illness Perception Scale, a Self-efficacy Scale and a Self-care Scale. Data were analyzed by using descriptive statistics, t-test, ANOVA, Pearson Moment Correlations and stepwise multiple regression analysis. Results: There was a statistically significant positive correlation between illness perception and self-care (r=.335, p=.000) and between self-efficacy and self-care (r=.687, p=.000). Stepwise multiple regression analysis revealed that among the three variables the most powerful predictor was self-efficacy. Self-efficacy and illness perception were significant factors and accounted for 54.2% of the variance of self care in pulmonary tuberculosis patients. Conclusion: Self-efficacy and illness perception were influencing factors in self care among patients with pulmonary tuberculosis. Hence further interventional researches are needed that consider these two variables.

퇴원을 앞둔 노인 환자의 사회적 지지와 질병지각이 우울에 미치는 영향 (Influence of Social Support and Illness Perception on Depression among Hospitalized Older Adults Prior to Discharge from an Acute Care Hospital)

  • Purpose: This study was designed to identify factors affecting depression among hospitalized older adults prior to discharge from an acute care hospital. Methods: This descriptive study included adults aged 65 or older who were hospitalized in a general hospital in Seoul, Korea. Depression was measured by the Short Form Geriatric Depression Scale and illness perception was evaluated by the Brief Illness Perception Questionnaire. Social support was examined using the Perceived Geriatric Social Support Scale. Data were collected from August 25 to October 12, 2015. Data analysis included descriptive statistics, independent t-test, Kruskal-Wallis test, Pearson's correlation coefficient, and hierarchical multiple regression. Results: Among a total of 120 participants, 57 patients (47.5%) experienced depression. Mean depression score was $7.37{\pm}3.67$. Depression was associated with illness perception (r=.53, p<.001), social support (r=-.19, p=.043), number of admission due to the recurrence (r=.31, p=.001), and time to recognize discharge plan (r=.25, p=.044). In hierarchical multiple regression, illness perception (${\beta}=.45$, p<.001) and time to recognize discharge plan (${\beta}=.21$, p=.039) were predictors of depression (F=7.68, p<.001, Adjusted $R^2=.38$). Conclusion: The findings of this study suggest that management of illness perception and timely notice of discharge are important to reduce depression in hospitalized elderly patients.

허혈성 뇌졸중 환자의 디스트레스 측정도구 개발 및 평가 (Development and Psychometric Evaluation of the Ischemic Stroke Distress Scale (ISDS))

  • Purpose: This study aimed to develop a scale to measure distress in patients with ischemic stroke and verify its validity and reliability. Methods: Preliminary items were developed from literature review and in-depth interviews. The final preliminary scale was confirmed through a content validity test of eight experts and a preliminary survey of 10 stroke patients. The participants for psychometric testing were 305 stroke patients in the outpatient clinic. Validity and reliability analyses included item analysis, exploratory and confirmatory factor analysis, convergent validity, known-group validity, and internal consistency of the scale. Results: The final scale consisted of 17 items and 3 factors. The three distinct factors were 'self-deprecation, worry about future health, and withdrawal from society' and this structure was validated using a confirmatory factor analysis. Convergent validity was supported by comparison with the Center for Epidemiologic Studies Depression Scale (r = .54, p < .001) and Brief Illness Perception Questionnaire (r = .67, p < .001). Known-groups validity was verified by dividing groups according to 'duration since diagnosis' (t = 2.65, p = .009), 'presence of sequela' (t = 10.16, p < .001), and 'awareness of distress' (t = 12.09, p < .001). The internal consistency of the scale using Cronbach's α for the total items was .93. Conclusion: The Ischemic Stroke Distress Scale is a valid and reliable tool that reflects stroke distress effectively. It is expected to be used as a basic tool to develop various intervention strategies to reduce distress in ischemic stroke patients.
