A Study on Effect of the Image Applying to Breast Implants in Breast Specific Gamma Imaging (유방전용감마카메라에서 유방 보형물이 영상에 미치는 영향에 관한 고찰)

  • There are limits to check the lesion as inserting a breast implant patients. So the application of BSGI based on Nuclear Medicine examination has increased. In this study, therefore we confirmed the effect of the image applying to breast implants in Breast Specific Gamma Imaging. We utilized Dilon 6800 BSGI scanner and developed the phantom. The self-development phantom was a rectangular shape of $230{\times}190{\times}80mm$ size and had 5 spheres which consisted of diameters of 10, 13, 17, 22, 28 mm in central part. We injected $^{99m}TcO_4$ into the self-development phantom in the proportion of four to one and made each additional phantom filled with 0.9 % sodium chloride, silicon and paraffin. Each additional phantom was placed between detector and self-development phantom. Each image was acquired five times depending on the type and thickness of the additional phantom. Statistical analysis with SPSS ver.18 was applied. In the test of variation according to the thickness of all additional phantoms, as the phantoms which 0.9% sodium chloride, silicon and paraffin increased, the attenuation variation was higher(P<0.005). There was no significant difference in the attenuation variation and the quality of image for type of the additional phantom. Therefore, if the effect of the image applying to breast implants in Breast Specific Gamma Imaging is confirmed, the higher diagnostic value can be achieved.

  • Breast cancer is growing rapidly year by year and has the highest incidence since 2001. As a result, the interest in mammography for early detection of breast cancer is increasing. However, mammography is accompanied by radiation exposure and therefore it is necessary to reduce exposure dose through appropriate test conditions. The significance of this study is that breast dose studies, which were limited to ordinary women, were applied to breast implant patient. Using MCNP simulation, the phantom with prosthesis inserted was developed to compare dose by tube voltage by pressure thickness. In addition phantom without prostheses has higher dose than phantom with prostheses. If these results were used as basic data, it would be possible to recommend test condition guideline only for breast implant patients.

A Numerical Voxel Model for 3D-printed Uncompressed Breast Phantoms (3D 프린팅 비압박 유방 팬텀 제작을 위한 복셀 기반 수치 모델에 관한 연구)

  • Physical breast phantoms would be useful for the development of a dedicated breast computed tomography (BCT) system and its optimization. While the conventional breast phantoms are available in compressed forms, which are appropriate for the mammography and digital tomosynthesis, however, the BCT requires phantoms in uncompressed forms. Although simple cylindrical plastic phantoms can be used for the development of the BCT system, they will not replace the roles of uncompressed phantoms describing breast anatomies for a better study of the BCT. In this study, we have designed a numerical voxel breast phantom accounting for the random nature of breast anatomies and applied it to the 3D printer to fabricate the uncompressed anthropomorphic breast phantom. The numerical voxel phantom mainly consists of the external skin and internal anatomies, including the ductal networks, the glandular tissues, the Cooper's ligaments, and the adipose tissues. The voxel phantom is then converted into a surface data in the STL file format by using the marching cube algorithm. Using the STL file, we obtain the skin and the glandular tissue from the 3D printer, and then assemble them. The uncompressed breast phantom is completed by filling the remaining space with oil, which mimics the adipose tissues. Since the breast phantom developed in this study is completely software-generated, we can create readily anthropomorphic phantoms accounting for diverse human breast anatomies.

  • In every cancer early detection and early treatment is the best way to decrease mortality of patients. Moreover early detection of breast cancer increases the possibility of breast conservation treatment. Although mammography is the most powerful modality for early detection, it is hazardous to be used for young women due to X-ray exposure. Another modality of image diagnosis is ultrasound echo technique. But it is not so powerful to detect breast cancer compared to mammography. Palpation is another modality, but is largely dependent on the skill and experience of medical doctors. A new technique is tested its validity in phantom experiments with good results.

  • This research was study the accuracy of three-dimensional stereotactic breast biopsy, using a core Needle Biopsy and to assess the accuracy of Stereotactic biopsy and Sono guided biopsy. Using Stereotactic QC phantom to measure the accuracy of the 3D sterotactic machine. CT Scan and equipment obtained in the measured X, Y, Z and compares the accuracy of the length. Using Agar power phantom compare the accuracy of the 3D sterotactic machine and 2D ultrasound machine. Z axis measured by the equipment to compare the accuracy and reliability. Check the accuracy by using visual inspection and Specimen Medical application phantom. The accuracy of the 3D sterotactic machine measured by Stereotactic QC phantom was 100%. Accuracy as compared to CT, all of X, Y, Z axis is p > 0.05. The accuracy of the two devices was 100% as measured by Agar powder phantom. There was no difference between t he t wo d evices as C T and p > 0.05. 3D sterotactic machine of the ICC was 0.954, 2D ultrasound machine was 0.785. 2D ultrasound machine was different according to the inspector. Medical application phantom experiments in 3D sterotactic machine could not find the Sliced boneless ham. 2D ultrasound machine has not been able to find a small chalk powder group. The reproducibility of the three-dimensional stereotactic breast biopsy was better than effect of Sono guided biopsy.

Observation with Calcifications of Breast Tissue Phantoms Using Acoustic Resonance (공명현상을 이용한 유방조직 팬텀의 석회화 관찰)

  • Diagnosis of breast ultrasound is better than mammography in the early detection of breast cancer, but, it is difficult to detect microcalcification. We studied on detection for calcification of breast tissue using acoustic resonance and power doppler with 7.5 MHz linear probe in breast ultrasound. We first constructed breast tissue phantom made of gelatin and saw breast, and then observed calcification by the change of external vibration. Calcification injected breast tissue phantom visualized the difference for brightness and region of color in ROI regions of power doppler. Acoustic resonance almost never visualized in low frequency regions, plateau constituted in about 300-400 Hz and colors vanished according to the increase of frequency.

  • This study was fulfilled to evaluate the absorbed dose of breast and adjacent organs using MIRD type phantom in brachytherapy of breast cancer. The absorbed dose was analyzed assuming left or right breast is source organ which is $^{103}Pd$ or $^{192}Ir$. As a result, $^{192}Ir$ dose is higher than $^{103}Pd$ in source organ and also in contralateral breast. Particularly, significant adjacent organs are lung, liver, heart and contralateral breast in brachytherapy of breast cancer.

Contralateral Breast Doses Depending on Treatment Set-up Positions for Left-sided Breast Tangential Irradiation (좌측 유방암 환자의 방사선 치료 시 환자자세에 따른 반대편 유방의 산란선량 측정)

  • Purpose : To evaluate Contralateral Breast Doses with Supine and Prone Positions for tangential Irradiation techniques for left-sided breast Cancer Materials and Methods : We performed measurements for contralateral doses using Human Phantom at each other three plans (conventional technique, Field-in-Field, IMRT, with prescription of 50 Gy/25fx). For the measurement of contralateral doses we used Glass dosimeters on the 4 points of Human Phantom surface (0 mm, 10 mm, 30 mm, 50 mm). For the position check at every measurements, we had taken portal images using EPID and denoted the incident points on the human phantom for checking the constancy of incident points. Results : The contralateral doses in supine position showed a little higher doses than those in prone position. In the planning study, contralateral doses in the prone position increased mean doses of 1.2% to 1.8% at each positions while those in the supine positions showed mean dose decreases of 0.8% to 0.9%. The measurements using glass dosimeters resulted in dose increases (mean: 2.7%, maximum: 4% of the prescribed dose) in the prone position. In addition, the delivery techniques of Field-in-field and IMRT showed mean doses of 3% higher than conventional technique. Conclusion : We evaluated contralateral breast doses depending on different positions of supine and prone for tangential irradiations. For the phantom simulation of set-up variation effects on contralateral dose evaluation, although we used humanoid phantom for planning and measurements comparisons, it would be more or less worse set-up constancy in a real patient. Therefore, more careful selection of determination of patient set-up for the breast tangential irradiation, especially in the left-sided breast, should be considered for unwanted dose increases to left lung and heart. In conclusion, intensive patient monitoring and improved patient set-up verification efforts should be necessary for the application of prone position for tangential irradiation of left-sided breast cancer.

A study of dose and image quality with Convergence FFDM and DBT using tissue-equivalent phantom in digital mammography (유방조직등가 팬텀을 이용한 디지털유방촬영장치의 FFDM과 DBT의 선량과 영상품질에 대한 융합 연구)

  • In this study, we measure dose against various density and thickness using phantom to compare FFDM to DBT of Digital mammography equipment and evaluate usefulness of DBT through compare the image quality of FFDM and DBT. We use mammography equipment, Selenia Dimensions ; this is able to examine breast by both FFDM and DBT, The results are that when the thickness of phantom is 6cm or more and density is 70% or more and the thickness of phantom is 7cm or more and density is 50% or more, AGD of DBT is lower than that of FFDM. The evaluation results of image quality are that in the tumor and small calcification group that composed by mammary tissue and fat, FFDM is great and in fibrin, DBT is great. But in the all thicknesses of BR3D phantom that reflected overlapped tissue of breasts, DBT is great in calcification group, fibrin and tumor. DBT is greater image quality and lower dose more than FFDM in Thick and high density breast, Therefore, DBT is more useful in Korean women's breast that is characterized dense breast than FFDM.

Evaluation of Image Quality according to Insert Position and Thickness Change by Fabricating Modified ACR Phantom in Mammography (유방엑스선검사에서의 변형된 ACR 팬텀 제작을 통한 모조병소의 위치와 두께 변화에 따른 영상의 품질 평가)

  • To maintain improved image quality in mammography, the quality control process is performed using the ACR (American college of radiology) phantom. In addition, many studied were performed by fabricating the customized breast phantom to provide more information in mammography. Thus, the purpose of this study was to evaluate the image quality by designing the modified ACR phantoms. The five modified acrlylic ACR phantoms were designed by considering insert position and phantom thickness. The phantoms were consisted of 4.5, 3.0, and 1.5 cm in terms of phantom thickness, and 3.0, 2.0, and 0.5 cm in terms of insert position, respectively. The acquired images were evaluated by PSNR (peak signal to noise ratio), RMSE (root mean square error), CC (correlation coefficient), CNR (contrast to noise ratio), and COV (coefficient of variation). Based on the similarity analysis, the result is suitable between conventional and new designed phantoms. In addition, the CNR and COV results in terms of insert position showed that image quality for 0.5 cm was 2.3 and 27.4% improved compared with 2 and 3 cm, respectively. According to phantom thickness results, the CNR result for 1.5 cm and COV result for 4.5 cm were 50.1 and 62.7% improved compared with that those conditions. In conclusion, we confirmed that the image quality depends on the breast size and thickness through modified ACR phantom study.
