Background: To investigate the impact of the breast size, shape, maximum heart depth (MDH), and chest wall hypotenuse (the distance connecting middle point of the sternum and the length of lung draw on the selected transverse CT slice) on the volumetric dose to heart with whole breast irradiation (WBI) of left-sided breast cancer patients. Materials and Methods: Fifty-three patients with left-sided breast cancer undergoing adjuvant intensity-modulated radiotherapy (IMRT) were enrolled in the study. The primary breast size and shape, MHD and DCWH (chest wall hypotenuse) were contoured on radiotherapy (RT) planning CT slices. The dose data of hearts were obtained from the dose-volume histograms (DVHs). Data were analyzed by one-way analysis of variance (ANOVA), Student's t-test and linear regression analysis. Results: Breast size was independent of heart dose, whereas breast shape, MHD and DCWH were correlated with heart dose. The shapes of breasts were divided into four types, as the flap type, hemisphere type, cone type and pendulous type with heart mean dose being $491.8{\pm}234.6cGy$, $752.7{\pm}219.0cGy$, $620.2{\pm}275.7cGy$, and $666.1{\pm}238.0cGy$, respectively. The flap type of breasts shows a strong statistically reduction in heart dose, compared to others (p=0.008 for V30 of heart). DCWH and MHD were found to be the most important parameters correlating with heart dose in WBI. Conclusions: More attention should be paid to the heart dose of non-flap type patients. The MHD was found to be the most important parameter to correlate with heart dose in tangential WBI, closely followed by the DCWH, which could help radiation oncologists and physicsts evaluate heart dose and design RT plan in advance.
Although the screening with a mammography has been shown to be economical, simple and effective in detecting breast cancer, it is accompanied by the risk from radiation. Therefore, this study analyzed the glandular dose and organ dose according to the target-filter combination and the presence and absence of implants using Monte Carlo simulation. The results indicate that at a tube voltage of 30 kV and a tube current of 50 mAs, the dose increased in the order of Mo/Mo. Mo/Rh, Rh/Rh and W/Rh in proportion to the atomic number of the target-filter. In addition, in phantom without implant a reduction in dose was seen when compared to the phantom with implant. The organ dose was highest in the lens except for the breast on the examination side regardless of the presence or absence of the implant. These results may contribute to use basic data for the diagnostic reference level of breast plastic surgery patients.

The use of primary breast irraditation with advantage of improved cosmesis in breast cancer may be the potential risks of radiation for a change in the number of normal breast cancers and lung fibrosis. The magnitude of the scattered dose for a variety of radiation treatment techniques from patient of breast cancer and phantom was measured by adequate dosimeters. We can reduce the dose of the normal breast to treated with radiation by understanding the factors contributing to the unwanted dose and by determining ways to decrease this dose.
This study was undertaken to determine the effects of fish oil supplementation with low dose on the lipid content and fatty acid composition of breast milk. Among 18 lactating women who were exclusively breast-fed their babies, 6 were in control group and 12 were in fish lil groups. The subjects in fish oil groups were supplemented with 1.96g/d or 3.92g/d of fish oil for 2 weeks from 10 to 12 weeks of postpartum. All subjects consumed their usual diet at home. Breast milk samples were collected at the final day of experiment. By fish oil supplementation, the concentrations of triglyceride, cholesterol, free fatty acid and phospholipid as well as total lipid in breast milk tended to increase, but not significant. There was no dose-dependent response. The fatty acid composition of breast milk was not changed by fish oil supplementation. These results suggest that low dose of fish oil supplementation may increase of lipid content, but does not affect on the fatty acid composition of breast milk.
Background: Radiotherapy is an important treatment of choice for breast cancer patients after breast-conserving surgery, and we compare the feasibility of using dual arc volumetric modulated arc therapy (VMAT2), single arc volumetric modulated arc therapy (VMAT1) and Multi-beam Intensity Modulated Radiotherapy (M-IMRT) on patients after breast-conserving surgery. Materials and Methods: Thirty patients with breast cancer (half right-sided and half left-sided) treated by conservative lumpectomy and requiring whole breast radiotherapy with tumor bed boost were planned with three different radiotherapy techniques: 1) VMAT1; 2) VMAT2; 3) M-IMRT. The distributions for the planning target volume (PTV) and organs at risk (OARs) were compared. Dosimetries for all the techniques were compared. Results: All three techniques satisfied the dose constraint well. VMAT2 showed no obvious difference in the homogeneity index (HI) and conformity index (CI) of the PTV with respect to M-IMRT and VMAT1. VMAT2 clearly improved the treatment efficiency and can also decrease the mean dose and V5Gy of the contralateral lung. The mean dose and maximum dose of the spinal cord and contralateral breast were lower for VMAT2 than the other two techniques. The very low dose distribution (V1Gy) of the contralateral breast also showed great reduction in VMAT2 compared with the other two techniques. For the ipsilateral lung of right-sided breast cancer, the mean dose was decreased significantly in VMAT2 compared with VMAT1 and M-IMRT. The V20Gy and V30Gy of the ipsilateral lung of the left-sided breast cancer for VMAT2 showed obvious reduction compared with the other two techniques. The heart statistics of VMAT2 also decreased considerably compared to VMAT1 and M-IMRT. Conclusions: Compared to the other two techniques, the dual arc volumetric modulated arc therapy technique reduced radiation dose exposure to the organs at risk and maintained a reasonable target dose distribution.

Background: Breast cancers are becoming more frequently diagnosed at early stages with improved long term outcomes. Late normal tissue complications induced by radiotherapy must be avoided with new breast radiotherapy techniques being developed. The aim of the study was to compare dosimetric parameters of planning target volume (PTV) and organs at risk between conformal (CRT) and intensity-modulated radiation therapy (IMRT) after breast-conserving surgery. Materials and Methods: A total of 20 patients with early stage left breast cancer received adjuvant radiotherapy after conservative surgery, 10 by 3D-CRT and 10 by IMRT, with a dose of 50 Gy in 25 sessions. Plans were compared according to dose-volume histogram analyses in terms of PTV homogeneity and conformity indices as well as organs at risk dose and volume parameters. Results: The HI and CI of PTV showed no difference between 3D-CRT and IMRT, V95 gave 9.8% coverage for 3D-CRT versus 99% for IMRT, V107 volumes were recorded 11% and 1.3%, respectively. Tangential beam IMRT increased volume of ipsilateral lung V5 average of 90%, ipsilateral V20 lung volume was 13%, 19% with IMRT and 3D-CRT respectively. Patients treated with IMRT, heart volume encompassed by 60% isodose (30 Gy) reduced by average 42% (4% versus 7% with 3D-CRT), mean heart dose by average 35% (495cGy versus 1400 cGy with 3D-CRT). In IMRT minimal heart dose average is 356 cGy versus 90cGy in 3D-CRT. Conclusions: IMRT reduces irradiated volumes of heart and ipsilateral lung in high-dose areas but increases irradiated volumes in low-dose areas in breast cancer patients treated on the left side.
The purpose of this study was to evaluate the efficacy of brachytherapy of breast cancer by dose assessment which a steady increased in Korea women. The dose assessment was performed using the MCNPX program, a MonteCarlo simulation technique. The sources used for brachytherapy was 192Ir. And nanoparticle which used for dose enhancement was gold. The density of nanoparticle was 7, 18 and 30 mg. Evaluation of absorbed dose according to distance is measured at a distance of 30, 50, 100 and 200 cm from the patient. As a result, The breast absorbed dose results increased in proportion to the density of nanoparticle. And the surrounding organs were not significantly different according to the density. But, in some organs, the absorbed dose decreased as the density of nanoparticles increased. Absorbed dose according to the distance was in inverse proportion to distance.

The typical physical wedge (FW) was compared with 4he virtual wedge (VW) to do)ermine the difference In the dose distribution affecting on the treated breast and the contralateral breast, lung, heart and surrounding perlpheral soft tissue. Methods and Materials: The data collected consisted of a measurement taken with solid water, a Humanoid Alderson Rando phantom and patients. The radiation doses at the ipsllateral breast and skin, contralateral breast and skin, surrounding peripheral soft tissue, and Ipsllateral lung and heart were compared using the physical wedge and virtual wedge and the radiation dose distribution and DVH of the treated breast were compared. The beam-on time of each treatment technique was also compared Furthermore, the doses at treated breast skin, contralateral breast skin and skin 1.5 cm away from 4he field margin were also measured using TLD in 7 patients of tangential breast Irradiation and compared the results with phantom measurements. Results: The virtual wedge showed a decreased peripheral dose than those of a typical physical wedge at 15$^{\circ}$, 30$^{\circ}$, 45$^{\circ}$, and 60$^{\circ}$. According to the TLD measurements with 15$^{\circ}$ and 30$^{\circ}$ virtual wedge, the Irradiation dose decreased by 1.35$\%$ and 2.55$\%$ In the contralateral breast and by 0.87$\%$ and 1.9$\%$ In the skin of the contralateral breast respectively. Furthermore, the Irradiation dose decreased by 2.7$\%$ and 6.0$\%$ in the Ipsllateral lung and by 0.96$\%$ and 2.5$\%$ in the heart. The VW fields had lower peripheral doses than those of the PW fields by 1.8$\%$ and 2.33$\%$. However the skin dose Increased by 2.4$\%$ and 4.58$\%$ In the Ipsliateral breast. VW fields, In general, use less monitor units than PW fields and shoriened beam-on time about half of PW. The DVH analysis showed that each delivery technique results In comparable dose distribution in treated breast. Conclusion: A modest dose reduction to the surrounding normal tissue and uniform target homogeneity were observed using the VW technique compare to the PW beam in tangential breast Irradiation The VW field is dosmetrically superlor to the PW beam and can be an efficient method for minimizing acute, late radiation morbidity and reduce 4he linear accelerator loading bV decreasing the radiation delivery time.

Purpose: Try to compare dose distribution and lung dose of radiation treatment plan of the breast cancer that used Irregular Surface Compensator (ISC) and treatment plan that used a wedge filter. Materials and Methods: Established a treatment plan to be distributed over 95% of prescription dose (5,040 cGy) of the two tangent-half fields that used a wedge filter and ISC at a breast organization as made to breast cancer patient having an irregular surfaces after surgery. Compared high dose area and DVH, and verified a treatment plan as used film with rectangular phantom. Results: Maximum dose point in breast tissue appeared to 107.5% in case of tangent-half fields Tx plan that used a wedge filter, and lung volumes exposed above 20 Gy by 7.63%. In case of ISC, maximum dose point in breast tissue appeared to 106.4%, and lung volumes exposed above 20 Gy by 6.5%. The film measurement results that used phantom, 105$\sim$110% high dose region was distributed to the upper part and both edges of phantom. However in case of ISC, appeared by 100$\sim$105% dose conformity distribution. Conclusion: In general, the Irregular Surface Compensator (ISC) can improve the dose conformity of breast tissues, as well as reduced hot spots in the lung and in the breast. Such an advantage by using ISC technique is more beneficial for patients who have more irregular surfaces after surgery.
The research was about the relation between the dorsal side dose measured by using the phantom body (Alderson Rando Phantom) and factors like contacted material of the patients, the size of the field, angle of incidence. Compared with mylar (tennis racket), the dose on $10{\times}10\;cm^2$ field size of cotton was increased by 2% and by 8% in the case of breast board. In the case of $15{\times}15\;cm^2$ field size, the dose was increased by 6% compared with $10{\times}10\;cm^2$ size. The field size of $20{\times}20\;cm^2$ resulted in 10% increase of dose, while $5{\times}5\;cm^2$ produced 13% decrease. Compared with incident angle $0^{\circ}$, the cases for the incident angle $5^{\circ}$ had 0.4% less dose for breast board, 0.5% for tennis racket, 1.1% for cotton. The cases for the incident angle $10^{\circ}$ had 1.5% less dose for breast board, 1.9% for tennis racket, 2.6% for cotton. For the incident angle $15^{\circ}$, breast board, tennis racket, cotton caused decrease of dose by 3.9%, 2.6%, 3.86% respectively. Resultantly carbon material can cause more skin dose in treatment field. By the results of this study, we recommend that one should avoid the contact between the carbon material and skin.
