A System for Concurrent TMS-fMRI and Evaluation of Imaging Effects (동시 뇌경두개자기자극-기능자기공명영상 시행을 위한 홀더 제작과 시뮬레이션 및 영상 데이터 평가)

  • Purpose : The purpose of this study was to setup a concuurent transcranial magnetic stimulation (TMS)-functional MRI (fMRI) system for understanding causality of the functional brain network. Materials and Methods: We manufactured a TMS coil holder using nonmagnetic polyether ether ketone (PEEK). We simulated magnetic field distributions in the MR scanner according to TMS coil positions and angles. To minimize image distortions caused by TMS application, we controlled fMRI acquisition and TMS sequences to trigger TMS during inter-volume intervals. Results: Simulation showed that the magnetic field below the center of the coil was dramatically decreased with distance. Through the MR phantom study, we confirmed that TMS application around inter-volume acquisition time = 100 miliseconds reduced imaging distortion. Finally, the applicability of the concurrent TMS-fMRI was tested in preliminary studies with a healthy subject conducting a motor task within TMS-fMRI and passive motor movement induced by TMS in fMRI. Conclusion: In this study, we confirmed that the developed system allows use of TMS inside an fMRI system, which would contribute to the research of brain activation changes and causality in brain connectivity.

Usefulness of 18F-Florbetaben in Alzheimer's Disease Diagnosis (알츠하이머병 진단에서 18F-Florbetaben의 유용성)

  • Alzheimer's disease is the most common degenerative brain diseases that causes dementia. ${\beta}$-amyloid neuritic plaque density that accumulates in the brain is difficult to perform daily living, such as memory loss, language ability deterioration. It is used to estimate ${\beta}$-amyloid neuritic plaque density in adult patients with cognitive impairment who are being evaluated for Alzheimer's disease and other causes of cognitive impairment. Using the $^{18}F$-Florbetaben with high sensitivity and specificity for the ${\beta}$-amyloid neuritic plaque density to evaluate the usefulness for the early diagnosis of Alzheimer's disease. In $^{18}F$-FDG Brain imaging shows no specific findings. And it appeared on the MR-Brain imaging without atrophy of the hippocampus. However, the intake of ${\beta}$-amyloid neuritic plaque density in $^{18}F$-Florbetaben informs that it is the progress of Alzheimer's disease. Therefore, $^{18}F$-Florobetaben is very useful for early diagnosis of Alzheimer's disease.

A rare case of dysembryoplastic neuroepithelial tumor combined with encephalocraniocutaneous lipomatosis and intractable seizures

  • Encephalocraniocutaneous lipomatosis (ECCL) is a rare neurocutaneous syndrome that affects ectomesodermal tissues (skin, eyes, adipose tissue, and brain). The neurologic manifestations associated with ECCL are various including seizures. However, ECCL patients very rarely develop brain tumors that originate from the neuroepithelium. This is the first described case of ECCL in combination with dysembryoplastic neuroepithelial tumor (DNET) that presented with intractable seizures. A 7-year-old girl was admitted to our center because of ECCL and associated uncontrolled seizures. She was born with right anophthalmia and lipomatosis in the right temporal area and endured right temporal lipoma excision at 3 years of age. Seizures began when she was 3 years old, but did not respond to multiple antiepileptic drugs. Brain magnetic resonance (MR) imaging performed at 8 and 10 years of age revealed an interval increase of multifocal hyperintense lesions in the basal ganglia, thalamus, cerebellum, periventricular white matter, and, especially, the right temporal area. A nodular mass near the right hippocampus demonstrated the absence of N-acetylaspartate decrease on brain MR spectroscopy and mildly increased methionine uptake on brain positron emission tomography, suggesting low-grade tumor. Twenty-four-hour video electroencephalographic monitoring also indicated seizures originating from the right temporal area. Right temporal lobectomy was performed without complications, and the nodular lesion was pathologically identified as DNET. The patient has been seizure-free for 14 months since surgery. Although ECCL-associated brain tumors are very rare, careful follow-up imaging and surgical resection is recommended for patients with intractable seizures.

A Study on the Metabolite Changes in Brain Diseases: 3 Teslar 1H Magnetic Resonance Spectroscopy (뇌질환 대사물질 변화의 고찰; 3 Teslar 수소 자기공명분광법)

  • The purpose of this study is to know the differences of metabolism in abnormal brain disease using a single-voxel proton MR spectroscopy(1H MRS) Together with five normal volunteers and each five patients with brain diseases, pathologically proved, underwent MRI and 1H MRS. The quantitative results of 1H MRS in adrenoleukodystrophy(ALD), hepatic encephalopathy(HE), and infarction gave unique information on the metabolite changes related with the white matter: the concentration of NAA decreased in all diseases; Cho, mI and Lac increased in ALD; Cho decreased in HE; and ${\beta}{\cdot}{\gamma}$-Glx and Lac increased in infarction. It is concluded that 1H MRS is capable of diagnosing brain diseases by monitoring metabolite changes in vivo that subsequently develope into abnormalities. 1H MRS may be a useful clinical tool for in both diagnosis and prognosis of brain diseases.

Primary Osteolytic Intraosseous Atypical Meningioma with Soft Tissue and Dural Invasion : Report of a Case and Review of Literatures

  • Primary intraosseous meningioma is a rare tumor, and atypical pathologic components both osteolytic lesion and dura and soft tissue invasion is extremely rare. A 65-year-old woman presented with a 5-month history of a soft mass on the right frontal area. MR imaging revealed a 4 cm sized, multilobulated, strongly-enhancing lesion on the right frontal bone, and CT showed a destructive skull lesion. The mass was adhered tightly to the scalp and dura mater, and it extended to some part of the outer and inner dural layers without brain invasion. The extradural mass and soft tissue mass were totally removed simultaneously and we reconstructed the calvarial defect with artificial bone material. The pathological study revealed an atypical meningioma as World Health Organization grade II. Six months after the operation, brain MR imaging showed that not found recurrence in both cranial and spinal lesion. Here, we report a case of primary osteolytic intraosseous atypical meningioma with soft tissue and dural invasion.

Isolation and Characterization of cDNA Encoding Pyridoxal Kinase from Ovine Liver

  • cDNA fragments of ovine liver pyridoxal kinase were amplified by PCR using degenerate oligonucleotide primers derived from partial amino acids sequences of the enzyme. Using PCR products as probes, several overlapping cDNA clones were isolated independently from an ovine liver and a human brain cDNA library. The largest cDNA clone for each was selected for sequence analysis. The ovine liver cDNA encodes a polypeptide of 297 amino acid residues with Mr of 32,925, whereas the human clone is comprised of an open reading frame encoding 312 amino acid residues with Mr of 35,102. The deduced sequence of the human brain enzyme is completely identical to that of human testes cDNA recently reported (Hanna et al., 1997). The ovine enzymes have approximately 77% sequence identity with the human enzyme although the two sequences are completely different in the N-terminus comprising 32 residues. This result suggests that pyridoxal kinase is highly homologous in mammalian species.

  • Cortical laminar necrosis appears as hyperinense lesions with a laminar pattern on T1 weighted magnetic resonance (MR) imaging, without signs of hemorrhage or calcification on T2 weighted MR imaging or computed tomography. It has been reported to be associated with hypoxia, metabolic disturbances, drugs, and infections. We present a 12 month-old male infant who suffered diffuse brain injuries following car accident and showed laminar necrosis of cortex.

Algorithm for automatic recognition of corpus callosum from saggital brain MR images (두뇌 자기공명영상에서의 corpus callosum의 자동인식 알고리즘)

  • In this paper, a new method to find the corpus callosum from sagittal brain MR images is proposed, which uses the statistical characteristics and shape information of corpus callosum. First, we extract regions satisfying the statistical characteristics of the corpus callosum and then find a region matching the shape information. In order to match the shape information, a new directed window region growing algorithm is proposed instead of using conventional contour matching algorithms. Using the proposed algorithm, we adaptively relax the statistical requirement until we find a region matching the shape information. Experiments show very promising results.

Noisy Image Segmentation via Swarm-based Possibilistic C-means

  • In this paper, we propose a swarm-based possibilistic c-means(PCM) algorithm in order to overcome the problems of PCM, which are sensitiveness of clustering performance due to initial cluster center's values and producing coincident or close clusters. To settle the former problem of PCM, we adopt a swam-based global optimization method which can be provided the optimal initial cluster centers. Furthermore, to settle the latter problem of PCM, we design an adaptive thresholding model based on the optimized cluster centers that yields preliminary clustered and un-clustered dataset. The preliminary clustered dataset plays a role of preventing coincident or close clusters and the un-clustered dataset is lastly clustered by PCM. From the experiment, the proposed method obtains a better performance than other PCM algorithms on a simulated magnetic resonance(MR) brain image dataset which is corrupted by various noises and bias-fields.

The Segmentation of white matter and gray matter from brain MR Image (뇌의 자기공명(MR) 영상에서 백질과 회백질의 추출)

  • 본 논문에서는 뇌의 자기공명(이하 MR로 줄임) 영상에서 양측 대뇌반구의 뇌백질과 뇌회백질의 추출에 관하여 연구하였다. MR 영상은 특정 장기에서 일정한 gray level 값을 유지하는 전산화단층촬영(이하 CT로 줄임) 영상과는 달리 사람마다 gray level 값이 다르며 한 사람에 대해서도 각 슬라이스에 따라 gray level 값이 다르므로 각 슬라이스별로 조직의 특성을 파악하여 백질과 회백질의 추출에 이용하였다. 먼저 뇌를 둘러싸고 있는 두피, 근육, 두개골과 함께 안구를 제거한 후 두 개강 내에 위치한 뇌간과 소뇌의 특성을 차례로 인식하여 대뇌반구로부터 분리한 후 제거하였다. 또한 추출된 대뇌의 영상으로부터 백질과 회백질의 체적을 구하고, 뇌신경게 진단방사선과 전문의의 manual 작업과 비교하여 본 논문에서 제시한 방법의 정확도를 검증하였다.
