공황장애의 임상적 특성과 Brain-Derived Neurotrophic Factor 농도와의 관계 (Associations between Clinical Characteristics and Plasma BDNF Levels of Panic Disorder)

  • Objective : Brain-derived neurotrophic factor (BDNF) is implicated in the pathophysiology of several neuropsychiatric disorders. However, there are few studies on BDNF of panic disorder. In this study, we investigated plasma BDNF levels in patients with panic disorder, and evaluated whether there are associations between clinical characteristics of panic disorder and plasma BDNF levels. Methods : We included 110 patients with panic disorder and 110 health controls in the current study. Plasma BDNF levels were measured by the enzyme-linked immunosorbent assay (ELISA). Plasma BDNF level differences were evaluated according to the clinical characteristics, such as duration of illness, recent stressful life event, agoraphobia, and insomnia. Results : The mean plasma BDNF levels of patients with panic disorder were significantly lower, as compared with those of controls (192.50 pg/mL vs. 693.75 pg/mL, t=8.838, p<0.001). The mean plasma BDNF levels of patients who had recent stressful life events were significantly higher, as compared with those who did not ($269.79{\pm}358.96pg/mL$ vs. $136.94{\pm}187.06pg/mL$, t=-2.525, p=0.013). Conclusion : These results suggested that BDNF plays a potential role in the pathophysiology of panic disorder.

  • Investigation of molecular and cellular mechanisms of synaptic plasticity is the major focus of many neuroscientists. There are two major reasons for searching new genes and molecules contributing to central plasticity: first, it provides basic neural mechanism for learning and memory, a key function of the brain; second, it provides new targets for treating brain-related disease. Long-term potentiation (LTP), mostly intensely studies in the hippocampus and amygdala, is proposed to be a cellular model for learning and memory. Although it remains difficult to understand the roles of LTP in hippocampus-related memory, a role of LTP in fear, a simplified form of memory, has been established. Here, I will review recent cellular studies of LTP in the anterior cingulate cortex (ACC) and then compare studies in vivo and in vitro LTP by genetic/pharmacological approaches. I propose that ACC LTP may serve as a cellular model for studying central sensitization that related to chronic pain, as well as pain-related cognitive emotional disorders. Understanding signaling pathways related to ACC LTP may help us to identify novel drug target for various mental disorders.

Sex differences in QEEG in adolescents with conduct disorder and psychopathic traits

  • Background: Sex influences is important to understand behavioral manifestations in a large number of neuropsychiatric disorders. We found electrophysiological differences specifically related to the influence of sex on psychopathic traits. Methods: The resting electroencephalography (EEG) activity and low-resolution brain electromagnetic tomography (LORETA) for the EEG spectral bands were evaluated in 38 teenagers with conduct disorder (CD). The 25 male and 13 female subjects had psychopathic traits as diagnosed using the Antisocial Process Screening Device. All of the included adolescents were assessed using the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, Text Revision (DSM-IV-TR) criteria. The visually inspected EEG characteristics and the use of frequency-domain quantitative analysis techniques are described. Results: Quantitative EEG (QEEG) analysis showed that the slow-wave activities in the right frontal and left central regions were higher and the alpha-band powers in the left central and bitemporal regions were lower in the male than the female psychopathic traits group. The current source density showed increases in paralimbic areas at 2.73 Hz and decreases in the frontoparietal area at 9.37 Hz in male psychopathics relative to female psychopathics. Conclusions: These findings indicate that QEEG analysis and techniques of source localization can reveal sex differences in brain electrical activity between teenagers with CD and psychopathic traits that are not obvious in visual inspections.

  • Purpose: The aim of this study was to investigate the factors influencing eating attitudes in female nursing students. Methods: A cross-sectional study was conducted on 121 university students in 2016. Data was collected using self reported questionnaires asking about circadian rhythm (Morningness-Eveningness Questionnaire), daytime sleepiness (Epworth Sleepiness Scale), sleep quality (Pittsburgh Sleep Quality Index), and eating attitudes (Eating Attitudes Test-26). Stress resistance was measured with a brain quotient test. The data were analyzed with a t-test, ANOVA, Pearson's correlation analysis and linear regression using SPSS 22.0. Results: Eating attitudes were associated with daytime sleepiness, sleep quality, and stress resistance of the right hemisphere. Among these, daytime sleepiness and sleep quality had a significant effect on eating attitudes. Conclusion: The results of this study suggest that sleep should be taken into account when developing interventions for preventing eating disorders in female nursing students.

Association of Type 2 Diabetes Mellitus With Perivascular Spaces and Cerebral Amyloid Angiopathy in Alzheimer's Disease: Insights From MRI Imaging

  • Background and Purpose: According to the amyloid cascade hypothesis, fibrillary amyloid-beta load in the brain causes Alzheimer's disease (AD) with toxic effects. Recently, perivascular spaces (PVSs), fluid-filled cavities around small penetrating arterioles and venules in the brain, and the glymphatic system relationship with type 2 diabetes mellitus (DM2) and AD has been an important research topic from a physiopathological point of view. There are two types of PVSs that are associated with sporadic atherosclerosis and cerebral amyloid angiopathy. In this study, we evaluated the relationship between the number and localization of enlarged PVSs in AD. Methods: A total of 254 patients with AD and 125 healthy controls were included in this study All the patients were evaluated with neurological and cognitive examinations and magnetic resonance imaging (MRI). PVSs on MRI were graded by recording their number and location. The study was a retrospective study. Results: In our study, the number of white matter convexity-central semiovale localized PVSs was higher in patients than in the control group. In addition, the number of PVSs in this localization score was higher in patients with DM2. Cerebral PVS counts were higher in patients with AD than in the control group. Conclusions: These results suggest the important role of cerebral amyloid angiopathy, one of the vascular risk factors, and the glymphatic system in the pathogenesis of AD. In addition, the results of our study suggest that the evaluation of PVSs levels, especially at the (centrum semiovale), using imaging studies in AD is a potential diagnostic option.

아급성기 뇌경색 환자의 좌측 편마비에 대한 한의 치료 1례 (The Effect of Korean Medicine Treatment on Cerebral Infarction with Hemiplegia: A Case Report)

  • Introduction: Stroke refers to a sudden brain disease that results in disorders in the anatomy of the brain. The cause is a sudden circulatory disorder of the cerebrovascular system that creates a consciousness disorder and hemiplegia. Despite aggressive treatment after the onset, stroke is a social problem because the patient has difficulty in recovering from sequelae that can include limb movement disorders, language disorders, and emotional disorders. In this study, we describe the effect of traditional Korean medicine treatment on the sequelae in a subacute cerebral infarction patient admitted to a Korean medical hospital. Case presentation: A 67-year-old male patient was diagnosed with cerebral infarction circa 2005, and his condition had not improved. Around March 15, 2018, he experienced the sudden onset of a cerebral infarction in his daily life. This was confirmed by a brain MRI, and he was hospitalized at other hospitals, but he showed no improvement. We conducted a manual muscle test (MMT) to evaluate the patient's exercise and strength. His gait level was measured to evaluate his degree of walking. He was treated with Bojungikgi-tang and acupuncture twice a day. After 34 days of inpatient treatment, the patient's exercise strength improved from Grade 2+~Grade 3 to Grade 3+ determined by the MMT, and his walking ability improved from Gait Level 3 to Gait Level 4. Conclusion: The findings of this study indicate that acupuncture and herbal medicine treatment can help treat patients with hemiplegia due to cerebral infarction.

궁치화담전 투여가 PTZ 경련 유발시 뇌조직 중 경련 및 산화 관련물질에 미치는 영향 (Anticonvulsant, Antioxidant Effect of Gungchihwadam-jeon)

  • This study was performed to evaluate the anticonvulsant, antioxidant effect of modified formulas Korean traditional medicine Gungchihwadam-Jeon(GCHDJ). The extract of GCHDJ was administered (p.o.) to mice for 14 days in anticonvulsant and antioxidant tests. The pretreatment of GCHDJ extract prohibited the pentylenetrazol(PTZ)-induced convulsion in PTZ-induced convulsion, lowered level of brain r-aminobutyric acid(GABA) was restored by the pretreatment of GCHDJ. Increased level of brain glutamic acid was lowered to normal state by GCHDJ, and increased activity of brain r-aminobutyric acid transaminase(GABA-T) was reduced by GCHDJ. In PTZ-induced convulsion, increased level of brain lipid peroxide was lowered to normal state by the pretreatment of GCHDJ. Increased activity of brain xanthine oxidase(XOD) was lowered to normal state by GCHDJ, and increased activity of brain aldehyde oxidase lowered to normal state by GCHDJ. In PTZ-induced convulsion, increased activities of superoxide dismutase(SOD) and catalase in brain were lowered by the pretreatment of GCHDJ, whereas increased level of glutathione and increased activity of gluthathione peroxidase in brain were not changed significantly. Above results suggest that GCHDJ have anticonvulsant. antioxidant effect. That seems to be strongly related with the levels of GABA, glutamate, lipid peroxide and the activities of GABA-T, XOD, aldehyde oxidase, SOD, catalase in brain tissue. From these results, GCHDJ could be applied to various convulsive disorders.

뇌 신경조절에서의 식이 폴리페놀 화합물의 역할 (The Roles of Dietary Polyphenols in Brain Neuromodulation)

  • 최근의 연구결과를 통해서 중추 신경계와 위장관은 장-뇌 축을 따라서 양방향의 상호작용이 일어나고 있다는 것이 분명해지고 있다. 전임상 연구로부터 장내 마이크로비오타가 다양한 생리적 기능을 통해서 중추 신경계의 기능을 조절할 수 있음이 밝혀지고 있다. 폴리페놀 화합물은 과일, 채소, 차, 커피, 와인과 같은 식품에 존재하는 식물 유래의 물질로, 항산화, 항염증, 항균, 면역 조절, 항암, 혈관 확장 및 프리바이오틱스와 유사한 효과를 보유하고 있어 식이를 통해 섭취할 경우 건강에 직접적인 효과를 나타낸다. 최근 들어 폴리페놀 화합물이 인지 기능뿐만 아니라 산화적 스트레스 및 염증성 손상에 대해 작용하는 신경 보호에 유익한 효과를 줄 수 있다는 증거가 보고되고 있다. 본 총설에서는 신경 세포 신호 전달 경로의 자극, 신경 염증, 혈관 기능 및 장내 마이크로비옴과의 상호작용에 따른 폴리페놀 화합물의 신경 보호 효과와 관련된 작용 메커니즘에 대한 일반적인 개요를 제시한다. 폴리페놀 화합물의 대사 산물은 혈액-뇌 장벽을 가로 지르는 신경 전달 물질을 이용하고 뇌 혈관 시스템을 조절하여 작용하거나, 간접적으로 장내 마이크로비오타에 작용한다. 또한, 폴리페놀 화합물은 노화 관련 인지 기능 저하 및 신경 퇴행과 같은 신경계 질환을 다양한 생리 기능을 통해 효과적으로 관리할수 있다는 사실이 제시되고 있다. 폴리페놀 화합물은 신경 염증을 감소시키고 기억과 인지 기능을 향상 시키며 장내 마이크로비오타를 조절하는 능력을 지니고 있기 때문에 신경계 질환의 예방 및 치료에 있어 잠재적인 기능성 식품으로 주목 받을 것으로 기대된다.

Enrichment of Short-Chain Ceramides and Free Fatty Acids in the Skin Epidermis, Liver, and Kidneys of db/db Mice, a Type 2 Diabetes Mellitus Model

  • Patients with diabetes mellitus (DM) often suffer from diverse skin disorders, which might be attributable to skin barrier dysfunction. To explore the role of lipid alterations in the epidermis in DM skin disorders, we quantitated 49 lipids (34 ceramides, 14 free fatty acids (FFAs), and cholesterol) in the skin epidermis, liver, and kidneys of db/db mice, a Type 2 DM model, using UPLC-MS/MS. The expression of genes involved in lipid synthesis was also evaluated. With the full establishment of hyperglycemia at the age of 20 weeks, remarkable lipid enrichment was noted in the skin of the db/db mice, especially at the epidermis and subcutaneous fat bed. Prominent increases in the ceramides and FFAs (>3 fold) with short or medium chains ($LXR{\alpha}/{\beta}$ and $PPAR{\gamma}$, nuclear receptors promoting lipid synthesis, lipid synthesis enzymes such as elongases 1, 4, and 6, and fatty acid synthase and stearoyl-CoA desaturase were highly expressed in the skin and livers of the db/db mice. Collectively, our study demonstrates an extensive alteration in the skin and systemic lipid profiles of db/db mice, which could contribute to the development of skin disorders in DM.

The Effect of Adding Kami-guibi-tang to Acetylcholinesterase Inhibitor Treatment on the Cognitive Function of Mild Alzheimer's Disease Patients: Study Protocol of a Randomized, Placebo-Controlled, Double-Blind Pilot Trial

  • Background: Alzheimer's disease (AD) is a chronic neurodegenerative disease that causes disorientation, mood swings, problems with language, and difficulty remembering recent events. Acetylcholinesterase inhibitors (AchEIs) and memantine have been used to slow the course of the disease, but they can neither modify its progression nor prevent disease onset. Previous studies have suggested that Kami-guibi-tang (KGT) could be beneficial for supporting cognitive function in AD patients, but few clinical trials have been published. This pilot study aimed to evaluate the effect of KGT in improving cognitive function in AD patients. Methods: The study will be a randomized, placebo-controlled, double-blind, single-center trial conducted using subjects diagnosed with mild AD by neurologists. Study subjects will be randomly assigned to either a treatment or control group. The treatment group will receive KGT granules for 24 weeks, while the control group will receive placebo granules. AchEI administration will be maintained in both groups during the entirety of the study. Subjects will be assessed using the following exams: the Seoul Neuropsychologic Screening Battery (SNSB) for cognitive function; brain magnetic resonance imaging (MRI) for brain metabolite, neurotransmitter, and cerebral blood flow (CBF) measurements; the Korean version of Quality of Life-Alzheimer's Disease (KQol-AD) for quality of life; the Caregiver-Administered Neuropsychiatric Inventory (CGA-NPI) for neurobehavioral symptoms; blood tests for amyloid and tau proteins and general blood parameters; and electrocardiography (ECG) before and after taking the medication. Discussion: Our findings will provide insight into the feasibility of large-scale trials to consolidate evidence for the efficacy of KGT for dementia treatment.
