Organ dose conversion coefficients in CT scans for Korean adult males and females

  • Dose monitoring in CT patients requires accurate dose estimation but most of the CT dose calculation tools are based on Caucasian computational phantoms. We established a library of organ dose conversion coefficients for Korean adults by using four Korean adult male and two female voxel phantoms combined with Monte Carlo simulation techniques. We calculated organ dose conversion coefficients for head, chest, abdomen and pelvis, and chest-abdomen-pelvis scans, and compared the results with the existing data calculated from Caucasian phantoms. We derived representative organ doses for Korean adults using Korean CT dose surveys combined with the dose conversion coefficients. The organ dose conversion coefficients from the Korean adult phantoms were slightly greater than those of the ICRP reference phantoms: up to 13% for the brain doses in head scans and up to 10% for the dose to the small intestine wall in abdominal scans. We derived Korean representative doses to major organs in head, chest, and AP scans using mean CTDIvol values extracted from the Korean nationwide surveys conducted in 2008 and 2017. The Korean-specific organ dose conversion coefficients should be useful to readily estimate organ absorbed doses for Korean adult male and female patients undergoing CT scans.

Expression Profiles of Loneliness-associated Genes for Survival Prediction in Cancer Patients

  • Influence of loneliness on human survival has been established epidemiologically, but genomic research remains undeveloped. We identified 34 loneliness-associated genes which were statistically significant for high-lonely and low-lonely individuals. With the univariate Cox proportional hazards regression model, we obtained corresponding regression coefficients for loneliness-associated genes fo individual cancer patients. Furthermore, risk scores could be generated with the combination of gene expression level multiplied by corresponding regression coefficients of loneliness-associated genes. We verified that high-risk score cancer patients had shorter mean survival time than their low-risk score counterparts. Then we validated the loneliness-associated gene signature in three independent brain cancer cohorts with Kaplan-Meier survival curves (n=77, 85 and 191), significantly separable by log-rank test with hazard ratios (HR) >1 and p-values <0.0001 (HR=2.94, 3.82, and 1.78). Moreover, we validated the loneliness-associated gene signature in bone cancer (HR=5.10, p-value=4.69e-3), lung cancer (HR=2.86, p-value=4.71e-5), ovarian cancer (HR=1.97, p-value=3.11e-5), and leukemia (HR=2.06, p-value=1.79e-4) cohorts. The last lymphoma cohort proved to have an HR=3.50, p-value=1.15e-7. Loneliness-associated genes had good survival prediction for cancer patients, especially bone cancer patients. Our study provided the first indication that expression of loneliness-associated genes are related to survival time of cancer patients.

소적정원산(消積正元散) 및 옻나무 추출물 투여로 체중증가 및 일반활동도의 개선을 보인 진행성 위암환자 1례 (A Case of Advanced Gastric Cancer Patient Treated with Sojeukjungwon-san and Allergen Removed Rhus Verniciflua Stokes(aRVS))

  • Advanced gastric cancer is the most common type of all cancers in Korea, which account for approximately 18.1% of all incident cancers, and mortality from advanced gastric cancer is estimated at nearly 15.6% of death that caused by all cancer. In the conventional medicine, treatments of advanced gastric cancer include chemotherapy such like FOLFOX. In this case report, we introduce a case of advanced gastric cancer with greater omentum, thyroid, supraclavicular lymph node metastasis who had received chemotherapy. Chemotherapy was stopped because of weight loss and neurologic symptom such like cognitive disorder. The patient visited $M{\cdot\mu}$ Integrative cancer center in 2008, and was treated by Sojeukjungwon-san and allergen removed Rhus verniciflua stokes(aRVS). There was no evidence of brain metastasis. The patient showed improvement of cognitive disorder and gained weight. Further case study will be needed in order to determine the effects of Sojeukjungwon-san and allergen removed Rhus verniciflua stokes(aRVS) for the quality of life of advanced gastric cancer patients after chemotherapy.

Anticancer activity of ginsenosides Rh2 on various cancer cells

  • Background: This study has mainly focused on finding pharmacological effects of ginsenosides that can reduce the unwanted side effects of the cytotoxic anticancer drugs and are highly effective on prostate cancer, colorectal cancer, liver cancer, hormone-dependent breast cancer, triple-negative breast cancer, and brain cancer (neuroblastoma). Methods: Minor and rare ginsenosides (GS) of Rh2 which have a high absorption ability and excellent pharmacological actions were treated with the 6 different types of cancer cell lines and their anticancer activities were investigated by analyzing gene expressions associated with various cancers through qPCR and other relevant methods. Results: In cancer cells exposed to Rh2, cell viability and cell migration were reduced, and apoptosis was induced. Each cancer cell was divided into three groups according to the cell proliferation response by Rh2; 1) A group in which the cell viability decreases inversely to an increase in Rh2 treatment concentration; 2) A group in which the cell viability rapidly decreases in Rh2 treatment above a certain level of concentration; 3) A group in which the cell viability was not suppressed below 20-30% even with 100 μL of Rh2, the highest concentration used in this study. Conclusions: It was shown that Rh2 has a significant effect on inhibiting the proliferation of prostate cancer cells and hormone-dependent breast cancer cells.

Gamma Knife Radiosurgery for Brain Metastases from Breast Cancer

  • Objective : The authors conducted a retrospective cohort study to determine prognostic factors and treatment outcomes of brain metastases (BM) from breast cancer (BC) after Gamma Knife radiosurgery (GKS). Methods : Pathologic and clinical features, and outcomes were analyzed in a cohort of 62 patients with BM from BC treated by GKS. The Kaplan- Meier method, the log-rank test, and Cox's proportional hazards model were used to assess prognostic factors. Results : Median survival after GKS was 73.0 weeks (95% confidence interval, 46.0-100.1). HER2+ [hazard ratio (HR) 0.441; p=0.045], Karnofsky performance scale (KPS) ${\geq}70$ (RR 0.416; p=0.050) and systemic chemotherapy after GKS (RR 0.282; p=0.001) were found to be a favorable prognostic factor of overall survival. Actuarial local control (LC) rate were $89.5{\pm}4.5%$ and $70.5{\pm}6.9%$ at 6 and 12 months after GKS, respectively. No prognostic factors were found to affect LC rate. Uni- and multivariate analysis revealed that the distant control (DC) rate was higher in patients with; a small number (${\leq}3$) of metastasis (HR 0.300; p=0.045), no known extracranial metastasis (p=0.013, log-rank test), or the HER2+ subtype (HR 0.267; p=0.027). Additional whole brain radiation therapy and metastasis volume were not found to be significantly associated with LC, DC, or overall survival. Conclusion : The treatment outcomes of patients with newly diagnosed BM from BC treated with GKS could be affected primarily by intrinsic subtype, KPS, and systemic chemotherapy. Therapeutic strategy and prognosis scoring system should be individualized based on considerations of intrinsic subtype in addition to traditionally known parameters related to stereotactic radiosurgery.

Targeted Nanomedicine that Interacts with Host Biology

  • Nanotechnology is of great importance to molecular biology and medicine because life processes are maintained by the action of a series of molecular nanomachines in the cell machinery. Recent advances in nanoscale materials that possess emergent physical properties and molecular organization hold great promise to impact human health in the diagnostic and therapeutic arenas. In order to be effective, nanomaterials need to navigate the host biology and traffic to relevant biological structures, such as diseased or pathogenic cells. Moreover, nanoparticles intended for human administration must be designed to interact with, and ideally leverage, a living host environment. Inspired by nature, we use peptides to transfer biological trafficking properties to synthetic nanoparticles to achieve targeted delivery of payloads. In this talk, development of nanoscale materials will be presented with a particular focus on applications to three outstanding health problems: bacterial infection, cancer detection, and traumatic brain injury. A biodegradable nanoparticle carrying a peptide toxin trafficked to the bacterial surface has antimicrobial activity in a pneumonia model. Trafficking of a tumor-homing nanoprobes sensitively detects cancer via a high-contrast time-gated imaging system. A neuron-targeted nanoparticle carrying siRNA traffics to neuronal populations and silences genes in a model of traumatic brain injury. Unique combinations of material properties that can be achieved with nanomaterials provide new opportunities in translational nanomedicine. This framework for constructing nanomaterials that leverage bio-inspired molecules to traffic diagnostic and therapeutic payloads can contribute on better understanding of living systems to solve problems in human health.

Predictive Factors of Survival Time of Breast Cancer in Kurdistan Province of Iran between 2006-2014: A Cox Regression Approach

  • Background: Breast cancer is the most common cancer and the second most common cause of cancer-induced mortalities in Iranian women, following gastric carcinoma. The survival of these patients depends on several factors, which are very important to identify in order to understand the natural history of the disease. Materials and Methods: In this retrospective study, 313 consecutive women with pathologically-proven diagnosis of breast cancer who had been treated during a seven-year period (January 2006 until March 2014) at Towhid hospital, Sanandaj city, Kurdistan province of Iran, were recruited. The Kaplan-Meier method was used for data analysis, and finally those factors that showed significant association on univariate analysis were entered in a Cox regression model. Results: the mean age of patients was $46.10{\pm}10.81$ years. Based on Kaplan-Meier method median of survival time was 81 months and 5 year survival rate was $75%{\pm}0.43$. Tumor metastasis (HR=9.06, p=0.0001), relapse (HR=3.20, p=0.001), clinical stage of cancer (HR=2.30, p=0.03) and place of metastasis (p=0.0001) had significant associations with the survival rate variation. Patients with tumor metastasis had the lowest five-year survival rate (37%)and among them patients who had brain metastasis were in the worst condition (5 year survival rate= $11%{\pm}0.10$). Conclusions: Our findings support the observation that those women with higher stages of breast malignancies (especially with metastatic cancer) have less chance of surviving the disease. Furthermore, screening programs and early detection of breast cancer may help to increase the survival of those women who are at risk of breast cancer.

서울을 제외한 7개 지역암등록본부 자료를 활용한 국가 암통계 추정의 타당성 (A Validation of Estimating the National Cancer Incidence in Korea using the Databases of 7 Population-based Regional Cancer Registries except Seoul)

  • Objectives : A method of estimation using 8 populationbased cancer registries databases in Korea(KRCR DB) has been introduced as another strategy for validly estimating the national cancer incidence(NCI) in Korea. The purpose of this study was to evaluate the validity of this method with using the 7 KRCR DBs, excluding Seoul covering 21% of the total population of Korea. Methods : We designed the study method(NCSE_7) as same as the estimating method with using 8 KRCR DBs (NCSE_8) in order to ensure maximal comparability. We defined the expected number of cancer cases in each registry as the number of observed cases and then we added the weighted observed cases according to gender, age and the proportion of the population covered by each registry for the population of the seven regions and the population of all areas, with excluding these seven regions. From the expected number of total cancer incidents, the estimated NCI was calculated by dividing the expected number of cancer cases by the number of the total population. The standard error(SE) of the estimated incidence was also taken from the expected number of total cancer incidents. Results : Compared with the results of the NCSE_8, the overall age-standardized rates(ASR) in men and women became over-estimated and under-estimated, respectively. Primary sites that showed statistically significant differences were the colo-rectum, prostate, breast and thyroid. The index of death certificate only(DCO)and microscopically verified(MV)% indicating levels of data quality were decreased, especially for the brain in DCO% and kidney in the MV%. Conclusions : The database of Seoul regional cancer registry has a key role for the method to estimate the valid nationwide cancer statistics in Korea with using the population-based cancer registries databases.

각종 인체 암조직의 당의 산화경로 분석 (Oxidative Pathway of $C^{14}-glucose$ in Various Human Cancer Tissues)

  • Tissue homogenates of 12 kinds of human cancer tissues were incubated separately in medium containing $C^{14}-1-glucose$ and $C^{14}-6-glucose$ as a substrate in order to observe the oxidative pathway of glucose in the tumor tissues. At the end of 3 hours incubation in the Dubnuff metabolic shaking incubator, respiratory $CO_2$ samples trapped by alkaling which was placed in the center well of incubation flask were analysed for total $CO_2$ production rates and their radioactivities. The tissue homogenate samples after incubation were analyzed for their concentrations of glucose, lactate and pyruvate. Calculations were made on the glucose consumption rate and accumulation rates of lactate and pyruvate. Fractionation of oxidative pathway of glucose was carried out by calculating $C^{14}O_2 yields from C-1 and C-6 carbon of glucose. The following results were obtained. 1. In 12 kinds of human cancer, total $CO_2$ production rates were less than $8{\mu}M/gm$ except 2 cases. These lower values impressed that oxidative metabolism in the tumor tissues generally inhibited as compared with that in normal tissues. On the other hand, fractions of $CO_2$ derived from glucose to total $CO_2$ production rates (RSA) were less than 10% in every case. These facts showed that oxidation of glucose into $CO_2$ was remarkably inhibited in the tumor tissues. 2. Factions of glucose disappeared into $CO_2\;(RGD_{CO_2})$, lactate $(RGD_L)$, pyruvate $(RGD_P)$ to glucose consumption rates were as follows. $RGD_{CO_2}$ were less than 2% in cases of in this experiment and $RGD_L$ showed more than 5% except in 2 cases. These facts showed that anaerobic degradation of glucose into 3 carbon compounds was easily proceeded but further degradation into $CO_2$ via the TCA cycle was greatly inhibited resulting in accumulation of lactate. There are large variation in values of $RGD_P$ in different kinds of tumor tissue but relatively higher values in $RGD_{CO_2}$ were obtained in the tumor tissues as compared with those of normal tissues. 3. The oxidative pathway of glucose in tumor tissues were analyzed from the values of RSA which were obtained in $C^{14}-1\;and\;C^{14}-6-glucose$ incubation experiments. It was found that 3% of $CO_2$ derived from glucose were oxidized via the principal EMP-TCA cycle and the remainder were via alternate pathway such as HMP in the liver cancer and values in other cancer tissues were as follows; 4% in the tongue cancer, 6% in the colon cancer, 6% in the lung cancer, 9% in the stomach cancer, 11% in the ovarian cancer, 12% in the neck tumor, 22% in the uterine cancer, 22% in the bladder tumor, 32% in the spindle cell sarcoma and 65% in the brain tumor. These values except later 2 cases showed less than 30% which is the lowest value among the normal tissues. Even in the brain tumor in which showed highest value in the tumor group. It is reasonable to suppose that this fraction was remarkably decreased because values in normal brain tissue was more than 90%. From the above data, it was concluded that in tumor tissues, oxidation of glucose via TCA cycle was greatly inhibited but correlation between degree of inhibited oxidation of glucose via TCA cycle and malignancy of tumor were not clarified in this experiments.

소아 종양 환아의 중추 신경계 합병증 (CNS Complications in Childhood Cancer)

  • 목 적 : 소아의 악성 종양에 대한 새로운 치료법의 개발로 환아의 생존 기간이 증가하는 추세에 있으나 이로 인한 신경계 합병증의 발생도 증가하고 있다. 저자들은 항암 치료를 시행한 소아 종양 환아에서 나타난 중추 신경계 합병증에 대한 임상 양상을 알아보고자 하였다. 방 법 : 1995년 1월부터 2002년 11월까지 동산병원 소아과에서 항암치료를 시행한 환아 중 신경계 합병증이 동반되었던 15례의 환아를 대상으로 그 임상상을 후향적으로 검토하였다. 결 과 : 전체 15례 중에서 방사선학적 검사로 원인 질환을 밝힌 경우는 모두 13례로 백질뇌병증이 7례, 무기질화 미세혈관병증이 4례, 뇌경색이 3례, 뇌출혈이 1례, 저산소성 허혈성 뇌증이 1례, 2가지 이상의 합병증을 함께 나타낸 경우는 2례였으며, 혈액학적 방사선학적 검사상에서 이상 소견이 없이 MTX의 부작용으로 생각되는 경련성 질환이 2례였다. 뇌출혈과 뇌경색이 나타낸 경우는 예후가 나빴으며 전해질 불균형으로 인한 저산소성 허혈성 뇌증에서는 원인 교정 후 후유증 없이 회복되었다. 결 론 : 소아 종양 환아에서 치료 중 발생한 신경계 합병증은 질환의 예후에 큰 영향을 미치고 신경인지 발달에 장애를 남기므로 이들 합병증에 대한 예방, 진단 및 즉각적인 치료와 추적관찰이 필요할 것이다.
