외상성 쇄골하 동맥 파열 (Traumatic Subclavian Artery Rupture)

  • We have experienced two cases of traumatic subclavian artery rupture at the department of thoracic and cardiovascular surgery, Youngdong Severance hospital, Yonsei University college of medicine. One was combined with brachial plexus injury and the other was combined with brachial plexus injury and subclavian vein rupture. They were treated with graft interposition after segmental resection of ruptured subclavian artery and neurorrhaphy for brachial plexus injury. Post operative courses were not eventful.

  • Iatrogenic vascular injuries may occur during venipuncture, arterial cannulation, or catheterization procedures. Brachial arteriovenous fistula (AVF) resulting from antecubital vascular access is rare and develops slowly. We report the case of an 18-year-old man who had developed iatrogenic brachial AVF. He had a history of several venipunctures in the left arm at the age of 10 months. Doppler ultrasonography and computed tomographic angiography were used to establish a diagnosis of brachial AVF, and surgical correction of the AVF was performed. As our case indicates, delayed surgery can be considered as a treatment option and may be associated with a decreased risk of vascular complications in the management of iatrogenic brachial AVF in infants.

진구성 상완 신경총 마비에 대한 유리박근이식술 (Gracilis Muscle Transplantation in Neglected Brachial Plexus Palsy)

  • Complete denervation after severe brachial plexus injury make significant muscle atrophy with loss of proper function. It is much helpful to reconstruct the essential function of the elbow flexion movement in patient with total loss of elbow flexion motion after brachial plexus lesion which was not recovered with nerve surgery or long term conservative treatment from onset. In whole arm type brachial plexus injury, if there were no response to neurotization or neglected from injury, the volume of the denervated muscle is significantely reduced month by month. About 18 months most of the muscle fibers change to fibrous tissues and markedly atrophied irreversibly, further waiting is no more meaningful from that period. Authors performed 14 cases of functioning gracilis muscle transfer from 1981 to 1995 with microneurovascular technique, neuromusculocutaneous free flaps were performed for reconstruction of lost elbow flexion function. Average follow-up period was 5 years and 6 months. We used couple of intercostal nerves as a recipient nerve which were anastomosed to muscular nerve from obturator nerve in all cases. Recipient vessels were three deep brachial artery and eleven brachial artery which were anastomosed to medial femoral circumflex artery with end to end or end to side fashion. Average resting length of the transplanted gracilis were 24 cm. We can get average 54 degree flexion range of elbow with fair muscle power from flail elbow. There were one case of muscle necrosis with lately developed thrombosis of microvascular anastomosed site which comes from insufficient recipient arterial condition, 3 cases of partial marginal necrosis of distal skin of the transplanted part which were not significant problem with spontaneously solved with time goes by gracilis muscle has constant neurovascular pattern with relatively easy harvesting donor with minimal donor morbidity. Especially it has similar length and shape with biceps brachii muscle of upper arm and longer nerve pedicle which can neurorrhaphy with intercostal nerve without nerve graft if sufficient mobilization of the nerves from both sides of gracilis and intercostal region. Authors can propose gracilis muscle transplantation with intercostal nerves neurotization is helpful method with minimal donor morbidity for neglected brachial plexus palsy patients.

  • Subclavian injuries in blunt trauma are reported in less than 1% of all arterial injuries or chest related injuries. We report a female 68 yr-old patient whom has visited our emergency center due to a motorcycle traffic accident with complaints of right chest wall and shoulder pain. Her injury severity score was 22 and she was found with a comminuted clavicle fracture and subclavian artery injury. She developed delayed symptoms of pallor, pain and motor weakness with loss of pulse in her right arm. Attempts at intervention failed and thus, she underwent emergency artificial graft bypass from her subclavian artery to her brachial artery. Her postoperative course was uneventful and she is happy with the results. Although rare, a high index of suspicion for the injury must be noted and the inevitable surgical option must always be considered.

말초동맥손상 11례 보고 (Eleven Cases on Peripheral Arterial Injuries)

  • Eleven cases of peripheral arterial injuries treated at Korea University Woo Sok Hospital during these 3 years and 3 months from Feb. 1971Eleven cases of peripheral arterial injuries treated at Korea University Woo Sok Hospital during these 3 years and 3 months from Feb. 1971 to May 1974 were reviewed. Common causes of injuries were stab wound, automobile accidents and iatrogenic injuries during operation. Of the 11 arterial injuries 3 were femoral artery which` was the commonest in frequency and the next was 2 cases of brachial artery. The most frequent type of injury was transection. Laceration, contusion and spasm was also recognized. The need to operate immediately following an injury was emphasized. Operative procedures were end to end anastomosis and saphenous vein graft in 5 and 2 cases, respectively. Other cases were undergone multiple suture ligature due to staphyllococcal infection, insertion of polyethylene catheter, and lateral suture after thromboendarterectomy. Fractures and extensive soft tissue damage associated with arterial injuries with widespread destruction of the collateral circulation aggravated the situation and complicated the amputation of lower extremity in 4 cases. The factors influencing the amputation were time lag, presence of associated injuries and complications.[KTCS 1974;1:109-116] Common causes of injuries were stab wound, automobile accidents and iatrogenic injuries during operation. Experience with the emergency vascular repair of upper limb arterial transection with concurrent acute compartment syndrome: two case reports

  • Upper extremity vascular injuries occurring with acute compartment syndrome are very challenging to manage in an emergency context in resource-poor settings. The need to always recognize the likelihood of coexisting compartment syndrome guides surgeons to perform concomitant fasciotomy to ensure a better outcome. We managed three vascular injuries in the upper extremities in two patients with concomitant imminent compartment syndrome observed intraoperatively. The first injury was complete brachial artery disruption following blunt trauma, while the second and third injuries were radial and ulnar artery transection caused by sharp glass cuts. Both patients were treated with vascular repair and fasciotomy. Secondary wound coverage was applied with split-thickness skin grafting, and the outcomes were satisfactory. Concomitant fasciotomy potentially improves the outcomes of vascular repair in emergency vascular surgery and should be considered for all injuries with the potential for acute compartment syndrome.

교통 사고 10일 후 발생한 쇄골하 동맥 가성동맥류 1례 (Subclavian artery pseudoaneurysm of 10 days after a traffic accident: A Case Report)

  • 둔상에 의한 쇄골하 동맥 가성동맥류의 발생은 드물며, 관통상 이후에 이차적으로 발생하는 경우가 있다. 대게 쇄골하 동맥은 주변의 인대, 근막뿐만 아니라 쇄골, 첫 번째 갈비뼈, 심부 경부 조직들에 의해 보호받고 있어 둔상으로 의한 합병증으로 쇄골하 동맥의 손상이 발생하는 경우는 드물다. 쇄골하 동맥의 손상은 외상 초기에 나타나며, 동맥 파열은 생명을 위협할 수 있는 출혈, 가성동맥류 형성, 상완신경총 압박 등을 유발할 수 있다. 쇄골하 동맥 손상은 쇄골골절, 총상, 관통상이나 중심정맥삽관 같은 술기의 합병증으로 발생하는 것이 대부분이다. 쇄골 주변의 큰 혈종이나 맥박이 느껴지는 종괴가 있다면 심각한 혈관 손상 가능성이 높아지므로 이러한 소견이 있는지 이학적 검사를 통해 확인해야만 한다. 1993년에 외상성 혈관손상의 치료에 있어 혈관 내 스텐트 삽입 시술이 처음 발표된 이후 혈관내 스텐트 삽입 시술을 통해 외상성 혈관 손상을 치료하는 사례가 점차적으로 많아지고 있다. 이 연구는 교통사고 10일 후에 발생한 쇄골하 동맥 가성동맥류 환자에서 혈관내 스텐트 삽입을 통한 성공적 치료와 관련된 임상양상과 추정되는 병태생리에 대해 보고한 사례연구이다.

Direct Axillary Arterial Cannulation Using Seldinger's Technique in Aortic Dissection

  • Background: The axillary artery is frequently used for cardiopulmonary bypass, especially in acute aortic dissection. We have cannulated the axillary artery using a side graft or by directly using Seldinger's technique. The purpose of this study was to assess the technical problems and complications of both cannulation techniques. Materials and Methods: From January 2003 to December 2009, 53 patients underwent operations using the axillary artery for arterial cannulation. The axillary artery was cannulated with a side graft in 35 patients (side graft group) and directly using Seldinger's technique in 18 patients (direct group). Results: The results were compared between two groups, focusing on cannulation-related morbidities including neurologic morbidity. Arterial damage or dissection of the axillary artery occurred in 1 (2.9%) patient in the side graft group and in 1 (5.6%) patient in the direct group. Malperfusion and insufficient flow did not occur in either group. There were no postoperative complications related to axillary cannulation, such as brachial plexus injury, compartment syndrome, or local wound infection, in either group. Conclusion: Technical problems and complications of the axillary arterial cannulation in both techniques were rare. Direct arterial cannulation using Seldinger's technique was done safely and more simply than the previous technique. It was concluded that both axillary arterial cannulation techniques are acceptable and it remains the surgeon's preference which technique should be used.

상완골 근위부 분쇄 골절에서의 상완골 두 치환술의 단기 추시 결과 (The Short Term Clinical Follow-up Study for Hemiarthroplasty in Proximal Humeral Fracture)

  • 목적: 근위 상완골 골절의 치료 방법은 일반적으로 Neer 분류법에 의하고, 심하게 전위된 3분 골절 및 4분 골절의 경우 상완골두 치환술의 적응증이 된다. 저자들은 근위 상완골의 분쇄 골절로 인하여 상완골두 치환술을 시행 받고, 최소 1년 이상 추시 가능하였던 10례의 단기적 임상적 결과를 보고 하고자 한다. 대상 및 방법: 저자들은 1999년 7월부터 2005년 3월까지 상완골 근위부 골절로 상완골 두 치환술을 시행 받은 환자 중 최소 1년 이상 추시 가능 하였던 10명(10 견관절)의 환자를 대상으로 하였다. Neer 분류법에 의하면 5명은 3분 골절이었고, 나머지 5명은 4분 골절이었으며 외상 후 수술까지 소요 시간은 평균 6.1일 이었다. 남자 4명, 여자 6명이었고, 평균 연령은 67.4세(최소 56세, 최고 76세)였다. 술 후 견관절 기능은 Constant score와 SST(Simple shoulder test) 및 UCLA 평가법(modified UCLA score for hemiarthroplasty)을 이용하여 평가하였다. 결과: 최종 방문 시 Constant 점수는 평균 51.4(최저 34점, 최고 60점)점이었고, 변형된 SST의 경우는 12개 문항 중 평균 7.8개 문항이 가능하다고 하였다. 수상 당시 액와동맥 파열 및 상완신경총 손상이 있었던 1예를 제외한 경우 Constant score는 평균 53.5(최저 44점, 최고 60점)점 이었고, SST의 경우는 12개 문항 중 평균 7.2($1{\sim}8$)개 문항이 가능하다고 하였다. 최종 추시 UCLA score는 동통, 기능성, 근력 및 운동 범위 항목 각각이 평균은 8.2($6{\sim}10$)점, 6.6($2{\sim}8$)점, 6.9($4{\sim}8$)점 이었고, 총점 21.7($12{\sim}26$)점 이었다. UCLA 평가법에 의한 결과는 우수 3예, 양호 6예, 불량 1예 이었고 불량의 경우는 혈관 및 신경 손상이 있었던 1예 이었다. 환자의 주관적 만족도는 10명의 환자 중 2명의 환자는 흡족, 7명의 환자는 양호한 결과라고 하였으나, 1명의 경우는 불만이었다. 결론: 단기 추시 결과이지만, 상완골 두 치환술은 관혈적 정복술이 어려운 상완골 근위부 골절에있어서 일차적 치료방법이라고 생각된다. 상완골 두 치환술은 고령의 환자에게 있어서 강직을 예방할 수 있고 일상적인 생활이 가능하도록 하는 수술 방법이나, 관절운동 범위 및 근력의 회복은 충분하지 않았다.
