슬관절(膝關節)의 퇴행성 관절염에 대한 고찰 (A Study on Osteoarthritis of the Knee)

  • Osteoarthritis(degenerative joint disease), the most common arthropathy affecting the aged people, is characterized by degeneration of articular cartilage with proliferation and remodeling of subchondral bone. Osteoarthritis of the knee, which probably has greater social cost and more associated disability than osteoarthritis of any other joint, prevalence is known to increase with age, and females have higher rates than males; radiographic abnormalities are present in more than 30% of persons more than 65 years old, with approximately 40% of these persons symptomatic. Though the etiology of osteoarthritis is not entirely understood, much information is available regarding risk factors for the development of knee osteoarthritis that permit some reasonable guideline for preventive strategies. Traumatic damage or occupational or recreational overuse of knee joint may result in osteoarthritis, and obesity also is related to osteoarthritis of the knee. A variety of other possible risk factors for the development of knee osteoarthritis have been proposed, including increased bone mass, smoking, diseases such as diabetes or elevated serum uric acid, and some metabolic factors, but the contribustions of these and other factors such as smoking or race and diseases such as diabetes are as yet undetermined. The usual clinical manifestations include pain, stiffnesss, crepitus and loss of function. In oriental medicine, osteoarthritis of the knee is very similar to diseases such as Bijung(痺症), Haksulpung(鶴膝風), Youkjeolpung(歷節風) in symptoms. The diseases such as Bijung(痺症), Haksuipung(鶴膝風), Youkjeolpung(歷節風) is related to the of function liver(肝) and kidney(腎) and risk factors are regarded as Pung(風), Han(寒), sub(濕). The diagnosis of osteoarthritis of the knee has often been based on radiographic appearance and clinical manifestations. The acupuncture therapy of osteoarthritis of the knee has often been based on Stomach meridian(ST), Spleen meridian(SP), Kidney meridian(KI), Liver meridian(LR).

Clinical evaluation of temporomandibular joint disorder after orthognathic surgery in skeletal class II malocclusion patients

  • This study was performed in order to evaluate the occurrence of temporomandibular joint disorder after surgical correction of skeletal class II malocclusion. Materials and Methods: This study included 21 patients who underwent orthognathic surgery for the correction of dentofacial deformities by a single surgeon at Mokdong Hospital, Ewha Womans University from 2000 to 2010. They underwent bilateral sagittal split ramus osteotomy for the treatment of undesirable mandibular advancement. The temporomandibular disorder (TMD) symptoms prior to surgery were recorded and the radiographic evaluation (panorama, bone scan, and magnetic resonance imaging [MRI]) of the post-surgery temporomandibular joint (TMJ) were assessed in order to evaluate condylar resorption, remodeling and disc displacement. The minimum follow-up period, including orthodontic treatment, was 12 months. Orthognathic procedures included 1-jaw surgery (n=8 patients) and 2-jaw surgery (n=13 patients). The monocortical plate was used for bilateral sagittal split ramus osteotomy fixation. Results: Among class II malocclusion patients with TMD symptom, clicking improved in 29.1%, and maximum mouth opening increased from $34.5{\pm}2.1$ mm to $37.2{\pm}3.5$ mm. The differences were not statistically significant, however. Radiographic changes in bone scan improved slightly based on the report by radiologist but not in TMJ dynamic MRI. Conclusion: No particular improvements were found in patients with joint sound only. Patients with limitation of mouth opening showed an increase in the degree of opening, but the difference was not statistically significant (P>0.05).

Transglutaminase-2 Is Involved in Expression of Osteoprotegerin in MG-63 Osteosarcoma Cells

  • Osteoprotegerin (OPG) is a secreted glycoprotein and a member of the tumor necrosis factor receptor superfamily. It usually functions in bone remodeling, by inhibiting osteoclastogenesis through interaction with a receptor activator of the nuclear factor ${\kappa}B$ (RANKL). Transglutaminases-2 (Tgase-2) is a group of multifunctional enzymes that plays a role in cancer cell metastasis and bone formation. However, relationship between OPG and Tgase-2 is not studied. Therefore, we investigated the involvement of 12-O-Tetradecanoylphorbol 13-acetate in the expression of OPG in MG-63 osteosarcoma cells. Interleukin-$1{\beta}$ time-dependently induced OPG and Tgase-2 expression in cell lysates and media of the MG-63 cells by a Western blot. Additional 110 kda band was found in the media of MG-63 cells. 12-O-Tetradecanoylphorbol 13-acetate also induced OPG and Tgase-2 expression. However, an 110 kda band was not found in TPA-treated media of MG-63 cells. Cystamine, a Tgase-2 inhibitor, dose-dependently suppressed the expression of OPG in MG-63 cells. Gene silencing of Tgase-2 also significantly suppressed the expression of OPG in MG-63 cells. Next, we examined whether a band of 110 kda of OPG contains an isopeptide bond, an indication of Tgase-2 action, by monoclonal antibody specific for the isopeptide bond. However, we could not find the isopeptide bond at 110 kda but 77 kda, which is believed to be the band position of Tgase-2. This suggested that 110 kda is not the direct product of Tgase-2's action. All together, OPG and Tgase-2 is induced by IL-$1{\beta}$ or TPA in MG-63 cells and Tgase-2 is involved in OPG expression in MG-63 cells.

  • Electrical stimulation among several factors that influence bone remodeling has been studied by many investigators with great enthusiasm in orthodontic field. The action mechanisms of Pulsed Electromagnetic Field (PEMF) are different from those of the conventional electrode application method in that PEMF induces endogenous current in the living tissues. PEMF is known to have the healing effect in nonunion of bone and osteoporosis. It is widely used in orthopaedic scopes and the possibility of using the method in clinical orthodontics Is also conceivable. But the exact mechanisms by which the PEMF exerts its effects are not clearly understood. Therefore, the author wanted to see the effect of PEMF on five groups of rat calvarial cells obtained by sequential enzyme digestion method, and observed the changes in enzyme activation, collagen synthesis and $^3H-thymidine$ incorporation. The results were as follows: 1. Under the effect of PEMF, there were no changes in the alkaline phosphatase activity in five groups of cell populations. 2. Both the PEMF group and the PTH with PEMF group shelved no changes in acid phosphatase activities and there were no differences between two experimental groups. 3. Under the effect of PEMF, there was significant increase of collagen synthesis in the group V cell population. 4. Under the effect of PEMF, there were significant increases of $^3H-thymidine$ incorporation in the group IV and V cell populations.

치주인대 신장에 의한 치아의 급속 견인 시 성견 치주조직의 변화 (Histological Periodontal Tissue Reaction to Rapid Tooth Movement by periodontal Distraction in Dogs)

  • 본 연구의 목적은 periodontal distraction을 통해 급속 견인된 치아의 치조골과 치주인대에서 일어나는 변화를 조직학적으로 관찰하고, 치주조직이 정상적으로 재생될 수 있는지를 규명하는 것이다. 4 마리의 성견을 대상으로 하여, 좌우 상악 제2소구치를 발치 후에 상악 제1소구치 원심측 치간골에 홈을 주는 치조골 수술을 통해 골 저항을 약화시키고 제1소구치와 제3소구치에 periodontal distraction 장치를 장착하였다. 장치는 0.225mm씩, 하루에 2회 활성화시키며 상악 제1소구치를 발치와 공간으로 급속 견인하였다. 치아의 급속 견인은 5 일, 10 일, 20 일간 시행하였으며, 20 일간 견인한 대상에서는 2 주, 4 주, 8 주 후까지 유지기간을 두었다. 20 일간의 periodontal distraction을 통해 상악 제1소구치는 평균 5.02mm 원심 이동하였고, 고정원인 제3소구치는 0.58mm 근심 이동하였다. 조직학적 검사에서 distraction된 치주인대 공간에서는 골 재생과 개조가 빠르게 일어나, 견인 10일에 유골조직이 치아의 견인방향에 평행하게 생성되었고, 새로운 골 조직의 형성과 골 개조를 통한 골 성숙은 periodontal distraction 방향을 따라 활발히 진행되었으며, 이는 다른 골의 distraction osteogenesis에서 나타나는 결과와 비슷하였다. 급속 견인된 치아의 치주인대는 상당히 넓게 나타났고, 교원섬유와 치아간의 정상 관계는 급속 견인이 끝난 후 2주에 나타나기 시작하여 8주에 거의 회복되었다. 그러나, 치주인대의 골 쪽에서는 치아 급속 견인 후 8주까지도 새로운 골이 계속 형성되고 있었고, 교원섬유속이나 Sharpey 섬유는 보이지 않았다. 고정원으로 이용된 치아 주위의 조직반응은 통상적인 교정치료에서와 같이 압박 측에서의 골 흡수와 신장 측에서의 골 형성 소견을 나타냈다. 이상의 결과로 볼 때, periodontal distraction을 통한 치아의 급속 견인 시, 치주인대 조직은 잘 반응하여 치주조직의 재생이 활발히 일어남을 알 수 있었다.

성견에서 표면처리된 교정용 마미크로 임플랜트의 골 접촉률 및 동요도 (Bone-implant contact and mobility of surface-fronted orthodontic micro-implants in dogs)

  • 본 연구는 비글견에 식립된 sandblasted, large grit and acid-etched (SLA) 표면처리된 교정용 마이크로임플랜트와 평활면 마이크로임플랜트에 교정력을 가한 후 시간 경과에 따른 동요도와 골접촉률의 차이를 규명하기 위해 시행되었다. 비글 성견 네 마리를 이용하여 상, 하악 협측과 구개측 골에 대해 SLA 표면처리된 표면처리군 48개, 평활면의 비처리군 48개의 마이크로임 플랜트 96개를 식립하고 2주의 치유기간 후 교정력(150 - 200 g)을 지속적 으로 가했으며 식립 4주 후에 두 마리를 희생시키고, 12주 후에 나머지 2마리를 희생시켰다. 표면처리군과 비처리군 간의 마이크로 임플랜트의 동요도와 골과 임플랜트 간 접촉률을 조직학적인 측면에서 측정 비교하여 다음과 같은 결과를 얻었다. 상악 협측과 구개측에서는 표면처리군과 미처리군의 동요도에서 유의성 있는 차이가 없었으나 하악협측에서는 표면 처리군이 유의하게 안정적인 동요도를 보였다. 마이크로임플랜트와 인접골 간 접촉률은 상악 협측에서는 4주와 12주 모두 표면처리군과 미처리군 간에 유의 한 차이가 없었으나 하악 협측과 구개측의 경우 4주와 12주 모두 표면처리군이 비처리군에 비해 유의하게 높은 접촉률을 보였다. 표면처리군은 비처리군에 비해 임플랜트 주변에서 활발한 골개조가 관찰되었으며 모든 군에서 이물반응은 관찰되지 않았다. 본 연구를 통해 SLA 표면처리된 마이크로임플랜트는 평활면 마이크로임플랜트에 비하여 식립 초기에는 식립 부위에 따라 유의하게 높은 인접골 간 접촉률과 동요도의 안정성을 보임으로써 다양한 크기와 방향의 교정력의 적용이 가능할 것이라 생각한다.

성견에서 수직적 골결손부에 골이식후 교정력 적용시기에 따른 치주조직의 반응 (Periodontal Response According to the Timing of Orthodontic Force Application alter Bone Graft into Angular Bony Defect in the Dog)

  • 본 연구는 수직적 골결손부에 골이식을 시행한 후 교정력 적용시기에 따른 치주조직의 반응을 알아보고자 시행하였다. 체중 15 kg내외의 성견 9마리에서 상악 좌우측 제 3절치의 원심 치근면에 수직적 골결손을 형성하고 치주질환을 유발시킨 2주 후 치주판막술을 시행하고 골결손 기저부 수준의 치근면에 기준 홈을 형성하였다. 골이식 후 치유기간에 따라 2주군, 4주군, 6주군으로 각각 3마리씩 분류하고 우측 제 3절치는 치주판막술만 하는 비이식측으로, 좌측 제 3절치는 치주판막술과 골이식을 병용한 이식측으로 설정하여 양측 모두 술후 2주, 4주, 또는 6주부터 120 gm의 교정력을 적용하여 8주 동안 치체이동 시킨 후 2주간 보정하였다. 이후 동물을 희생시키고 조직표본을 만든 후 치주조직의 반응을 비교 관찰하여 다음과 같은 결과를 얻었다. 1. 모든 실험동물에서 비이식측에 비해 이식측의 골재생양이 많이 나타났다. 2. 6주군에서는 홈에서 백악-법랑 경계부까지 거리의 1/2이상 치조골과 백악질이 재생되었고 상피의 하방증식없이 결합조직대가 치근면과 부착을 이루었다. 3. 4주군에서는 5주군과 유사한 치주조직 재생소견을 보였으나 그 양은 더 적게 나타났다. 4. 2주군의 경우 골재생이 양측 모두 홈에 한정되어 나타났으며 상피의 하방증식이 홈의 기저부까지 연장되고 백악질과 결합조직의 재생은 관찰되지 않았다. 이상의 결과는 치주질환에 의한 수직적 골결손 환자에서 치주판막술과 골이식을 시행하고 교정적 치아이동을 할 경우 교정력 적용시기에 따른 치주조직의 반응이 차이가 있음을 시사하였다.

Delayed intentional replantation of periodontally hopeless teeth: a retrospective study

  • Purpose: The purpose of this study was to retrospectively evaluate the survival of periodontally hopeless teeth that were intentionally extracted and replanted after a delay and to compare the radiographic characteristics of the survival group with those of the failure group. Methods: The clinical and radiographic data from patients who underwent delayed intentional replantation between March 2000 and July 2010 were reviewed. Twenty-seven periodontally hopeless teeth were extracted and preserved in medium supplemented with antibiotics for 10-14 days. The teeth were then repositioned in the partially healed extraction socket and followed for 3 to 21 months. The radiographic parameters were analyzed using a paired t test and the cumulative survival rate was analyzed using Kaplan-Meier analysis. Results: Seven replanted teeth failed and the overall cumulative survival rate was 66.4%. In the survival group, the amount of bone loss was reduced from 68.45% to 34.66% three months after replantation. There was radiologic and clinical evidence of ankylosis with 5 teeth. However, no root resorption was found throughout the follow-up period. In the failure group, bone formation occurred from the bottom of the socket. However, a remarkable radiolucent line along the root of a replanted tooth existed. The line lengthened and thickened as time passed. Finally, in each case of failure, the tooth was extracted due to signs of inflammation and increased mobility. Conclusions: Delayed intentional replantation has many advantages compared to immediate intentional replantation and could serve as an alternative treatment for periodontally involved hopeless teeth. However, techniques for maintaining the vitality of periodontal structures on the tooth surface should be developed for improved and predictable results.

Polydioxanone/pluronic F127 담체에 유입된 골막기원세포의 조골활성 (OSTEOGENIC ACTIVITY OF CULTURED HUMAN PERIOSTEAL-DERIVED CELLS IN A THREE DIMENSIONAL POLYDIOXANONE/PLURONIC F127 SCAFFOLD)

  • Three-dimensional porous scaffolds play an important role in tissue engineering strategies. They provide a void volume in which vascularization, new tissue formation, and remodeling can occur. Like any grafted materials, the ideal scaffold for bone tissue engineering should be biocompatible without causing an inflammatory response. It should also possess biodegradability, which provides a suitable three-dimensional environment for the cell function together with the capacity for gradual resorption and replacement by host bone tissue. Various scaffolds have already been developed for bone tissue engineering applications, including naturally derived materials, bioceramics, and synthetic polymers. The advantages of biodegradable synthetic polymers include the ability to tailor specific functions. The purpose of this study was to examine the osteogenic activity of periosteal-derived cells in a polydioxanone/pluronic F127 scaffold. Periosteal-derived cells were successfully differentiated into osteoblasts in the polydioxanone/pluronic F127 scaffold. ALP activity showed its peak level at 2 weeks of culture, followed by decreased activity during the culture period. Similar to biochemical data, the level of ALP mRNA in the periosteal-derived cells was also largely elevated at 2 weeks of culture. The level of osteocalcin mRNA was gradually increased during entire culture period. Calcium content was detactable at 1 week and increased in a time-dependent manner up to the entire duration of culture. Our results suggest that polydioxanone/pluronic F127 could be a suitable scaffold of periosteal-derived cells for bone tissue engineering.

치주인대섬유아세포가 파골세포분화에 미치는 영향 (Human Periodontal Ligament Fibroblasts Support the Osteoclastogenesis of RAW264.7 Cells)

  • The fibroblasts are the principal cells in the periodontal ligament of peridontium. As the periodontal ligament fibroblasts (PDLF) show similar phenotype with osteoblasts, the PDLF are thought to play an important role in alveolar bone remodeling. Cell-to-cell contacted signaling is crucial for osteoclast formation. Recently it has been reported that PDLJ enhance the bone resorbing activity of osteoclasts differentiated from hematopoietic preosteoclasts. The aims of this study were to $clarify\;^{1)}$ the mechanism of PDLF-induced osteoclastogenesis $and\;^{2)}$ whether we can use preosteoclast cell line instead of primary hematopoietic preosteoclast cells for studying the mechanism of PDLF-induced osteoclastogenesis. Osteoclastic differentiation of mouse macrophage cell line RAW264.7 was compared with that of mouse bone marrow-derived M-CSF dependent cell (MDBM), a well-known hematopoietic preosteoclast model, by examining, 1) osteoclast-specific gene expression such as calcitonin receptor, M-CSF receptor (c-fms), cathepsin K, receptoractivator nuclear factor kappa B (RANK) ,2) generation of TRAP(+) multinucleated cells (MNCs), and 3) generation of resorption pit on the $OAAS^{TM}$ plate. RAW264.7 cultured in the medium containing of soluble osteoclast differentiation Factor (sODF) showed similar phenotype with MDBM-derived osteoclasts, those are mRNA expression pattern of osteoclast-specific genes, TRAP(+) MNCs generation, and bone resorbing abivity. Formation of resorption pits by osteoclastic MNCs differentiated from sODF-treated RAW264.7, was completely blocked by the addition of osteoprotegerin (OPG), a soluble decoy receptor for ODF, to the sODF-containing culture me야um. The effects of PDLF on differentiation of RAW264.7 into the TRAP(+) multinucleated osteoclast-like cells were examined using coculture system. PDLF were fxed with paraformaldehyde, followed by coculture with RAW264.7, which induced formation of TRAP(+) MNCs in the absence of additional treatment of sODF. When compared with untreated and fixed PDLF (fPDLF), IL-1 ${\beta}$-treated, or lipopolysaccha-ride-treated and then fixed PDLF showed two-folld increase in the supporting activity of osteoclastogenesis from RAW264.7 coculture system. There were no TRAP(+) MNCs formation in coculture system of RAW264.7 with PDLF of no fixation. These findigs suggested that we can replace the primary hematopoietic preosteoclasts for RAW264. 7 cell line for studying the mechanism of PDLF-induced osteoclastogenesis, and we hypothesize that PDLF control osteoclastogenesis through ODF expression which might be enhanced by inflammatory signals.
