HISTOLOGIC STUDY OF VARIOUS DENTAL IMPLANT-TISSUE INTERFACE (수종의 치근형 골내매식체와 조직간의 반응에 관한 조직학적 연구)

  • The success or failure of endosseous dental implants is related to the cellular activity at the implant surface. Success seems to be associated with the enclosure of the implant in a non-inflammed connective tissue or the formation of a direct bone implant interface. The purpose of this study was to examine the tissue reactions to the various implants at the submergible state in dog mandible. The $Br\"{a}nemark$, Core-Vent, Intergral, Bone spiral were selected for evaluation and also the Kimplant, Nephrite were used for the experimental study. After 4 months the animals were sacrificed. The interface zone between bone and implant was investigated using x-rays, light microscope, scanning electron microscope, transmission electron microscope. The following results were obtained from this study. 1. $Br\"{a}nemark$, Core-Vent, Kimplant, Integral showed no mobility and bone growth over the healing screws of the implants. Histologically most of the implant surface were covered by remodelled lamellar bone, and partly by a cellular layer or the thin fibrous tissue layer. 2. The Bone spiral showed no mobility and partially radiolucent line around the implant. The upper part of the implant was surrounded by a thin fibrous connective tissue and the middle, apical part of it were contacted with bone directly. 3. The Nephrite implant showed severe mobility and a radiolucent line around the implant. Histologically it showed mild inflammation and was surrounded by a fibrous connective tissue. 4. Scanning electron microscope showed that there was no amorphous ground substance in the Nephrite implant but the formation of ground substance over the collagen filaments in other implants. 5. Transmission electron microscope showed that collagen filaments were approached irregularly to the surface of all implants and in the $Br\"{a}nemark$, Core-Vent, Kimplant, Integral there was amorphous layer between the implant and the collagen filaments. It seemed to be ground substances.

Bone Formation by rhBMP-7 Transduced HEK 293 Cells in Nude Mouse (재조합 BMP-7 유전자가 전달된 HEK 293 세포에 의한 누드 마우스에서의 뼈형성)

  • To induce bone formation at ectopic site by tissue engineering and gene therapy, we transplanted collagen sponges containing rhBMP-7 transduced HEK 293 cells in the hypodermis of nude mice. Bone formation was investigated by histological and electron microscopic method at 3, 6, and 9 weeks after transplantation. At 9 weeks after transplantation, eosinophilic bony tissue was observed in the implanted collagen sponge and was confirmed as bone tissue by Von Kossa stain. In the transmission electron microscopic observation, the cells in newly formed bone tissue had eccentrically located nucleus and well developed rough endoplasmic reticulum (rER). Therefore, the cells were evaluated as osteoblasts. Those results suggest that it is possible to form a bone tissue in the ectopic site by transplantation of rhBMP-7 transduced HEK 293 cells. This will be contributed to push more advanced gene therapy for bone formation. However, the HEK 293 cell is unable to apply to the clinical gene therapy. Therefore it is worth to find more compatible cells for clinical application. In addition, collagen sponge is considered as an excellent scaffold and/or carrier for gene therapy and a good biomaterial for tissue engineering.

Fabrication and Cell Culturing on Carbon Nanofibers/Nanoparticles Reinforced Membranes for Bone-Tissue Regeneration

  • Poly-L-lactic acid (PLLA), PLLA/hydroxyapatite (HA), PLLA/multiwalled carbon nanotubes (MWNTs)/HA, PLLA/trifluoroethanol (TFE), PLLA/gelatin, and carbon nanofibers (CNFs)/${\beta}$-tricalcium phosphate (${\beta}$-TCP) composite membranes (scaffolds) were fabricated by electrospinning and their morphologies, and mechanical properties were characterized for use in bone tissue regeneration/guided tissue regeneration. MWNTs and HA nanoparticles were well distributed in the membranes and the degradation characteristics were improved. PLLA/MWNTs/HA membranes enhanced the adhesion and proliferation of periodontal ligament cells (PDLCs) by 30% and inhibited the adhesion of gingival epithelial cells by 30%. Osteoblast-like MG-63 cells on the randomly fiber oriented PLLA/TEF membrane showed irregular forms, while the cells exhibited shuttle-like shapes on the parallel fiber oriented membrane. Classical supersaturated simulated body fluids were modified by $CO_2$ bubbling and applied to promote the biomineralization of the PLLA/gelatin membrane; this resulted in predictions of bone bonding bioactivity of the substrates. The ${\beta}$-TCP membranes exhibit good biocompatibility, have an effect on PDLC growth comparable to that of pure CNF membrane, and can be applied as scaffolds for bone tissue regeneration.

  • Even if we can recover the function of mastication, pronunciation and esthetic using the fixed or removable prosthesis in the loss of teeth or hard tissue in the oral cavity, we use several kinds of implants in order to solve the problem which can be occured when we can't install the denture because of excessive bone resolution or psychlogical affairs. At present Titanium implant plays a major role in this field and has osseointegrated. And the study on the modern material is going on, that result in developing and using the implant which is coated with HA, bone induced material. In this, I studied histologically the change of the bone tissue which is occured when three kinds of HA coated implants. Such as Integral, Sustain, Biovent implanted into mandible of dog and got a 8-weeks healing period. I got the conclusion as follows: 1. Most of the implant which is covered with serveral kinds of HA coating implant in bone after 8 weeks being implanted and has osseointegrated, partially converted into the connective tissue. 2. Biovent formed the connective tissue in the perforated area of inferior alveolar canal and has osseointegrated.

A HISTOLOGIC STUDY OF THE OSSEOINTEGRATION PROCESS TO THE TITANIUM TOOTH ROOT IMPLANT (타이타늄 치근형 매식체에 대한 골유착 과정에 관한 조직학적 연구)

  • The severe atrophic edentulism with poor neuromuscular control militates against successful conventional denture therapy. To such situation, a prescribing of dental implant treatment has been considered with some probability. Implant materials used as a trial for dental implants includes metals, plastic polymers and ceramics. The purpose of this study was to observe histologic response in osseointegration process at titanium implant-tissue interface based on biocompatibility at specific period of sequential natures which were divided into a half month, one, month, two months, three months and immediate as a base line. In this study, unilateral lower left premolar and molar teeth were extraced in three dogs. After allowing to heal for 6 months, three kinds of osseointegrated implant, $Br{\aa}nemark$, Corevent and kimplant(a prototype of SNU implant study)were inserted in each dog respectively according to the above sequence from front to back. The specimens were taken from those dogs at the same time since implant were inserted quite reverse order of the specified periods, and decalcified and processed for histologic examination for the light microscopy and the electron microscopy. The microscopic histologic findings at the interface between titanium implants and tissue were interpretated as follows : A. Light microscopic findings : a. Immediate : Implant were surrounded by compact bone and spongy bone. Microcrak was observed in the superficial bone tissue. Osteocytes were disappeared and bone lacunae were observed as a vacant space in some parts. In the contacting with the spongy bone, bone trabeculae and bone marrow were in contact with the implant. b. A half Month : Osteoblasts exist as a monolayer in th inner bone trabeculae and do bone additiocn. Osteoblasts&inflammatory cells were observed in some parts. c. One Month : The presence of osteoclasts decreased. Osteoblasts did active bone fromation, and bone marrow was in contact with the implant in the many places. d. Two Months : Bone formation was advanced in comparison with the b and c. The presence of osteoclsts was not observed. e. Three Months : The superficial bone tissue contacted with the implants was entirely composed by the compact bone. B. Electron microscopic findings : a. A half month and one month group : In the parts of the active bone formation, osteoblasts with the well developed endoplasmic reticulum and Golgi apparatus were arranged in the monolayer. In the parts of the bone resorption, ruffled border was well developed and many osteoclasts with the well-developed golgi apparatus, mitochondria, vacuole, vesicle and lysosome were existed. b. Three months group : No osteoblasts were observed in the superficial bone tissue. Bone matrix with collaen fiber was observed. c. No significant dirrerence in the histologic findings was observed in $Br{\aa}nemark$, Core-vent and kimplant.

A magnetic resonance imaging study on changes in rat mandibular bone marrow and pulp tissue after high-dose irradiation

  • Purpose: This study was designed to evaluate whether magnetic resonance imaging (MRI) is appropriate for detecting early changes in the mandibular bone marrow and pulp tissue of rats after high-dose irradiation. Materials and Methods: The right mandibles of Sprague-Dawley rats were irradiated with 10 Gy (Group 1, n=5) and 20 Gy (Group 2, n=5). Five non-irradiated animals were used as controls. The MR images of rat mandibles were obtained before irradiation and once a week until week 4 after irradiation. From the MR images, the signal intensity (SI) of the mandibular bone marrow and pulp tissue of the incisor was interpreted. The MR images were compared with the histopathologic findings. Results: The SI of the mandibular bone marrow had decreased on T2-weighted MR images. There was little difference between Groups 1 and 2. The SI of the irradiated groups appeared to be lower than that of the control group. The histopathologic findings showed that the trabecular bone in the irradiated group had increased. The SI of the irradiated pulp tissue had decreased on T2-weighted MR images. However, the SI of the MR images in Group 2 was high in the atrophic pulp of the incisor apex at week 2 after irradiation. Conclusion: These patterns seen on MRI in rat bone marrow and pulp tissue were consistent with histopathologic findings. They may be useful to assess radiogenic sclerotic changes in rat mandibular bone marrow.

The Elastic Moduli and Fatigue Properties of Canine Trabecular Bone Tissue

  • The elastic modulus and fatigue properties of canine and human trabecular bone tissues (single trabecular) were experimentally determined on a microstructural level using four-point bending cyclic test, and they were compared based on microstructural characteristics and mineral density. The results showed that canine trabecular bone tissue had significantly lower modulus and lower fatigue strength than human tissue. The observed microstructural differences between the two tissues may be more responsible for the differences, although the lower mineral density in canine tissue might also have contributed to the lower modulus and fatigue strength.

Management of Failed Thumb Replantation (Early Soft Tissue Removal with Vascularized Flap Coverage of Amputated Phalangeal Bone) (무지 재접합 실패예에 대한 조기 치료로서 절단부의 수지골과 유리 피판술을 이용한 무지의 재건)

  • Failure of replantation is inevitable in finger replantation surgery, around 10% of failure rate are reported in many authors. Management of the failed finger replantation is one of big dilemma to microsurgeons. We report 5 cases of thumb reconstruction after failure of replantation. The reconstructive surgery composed with early debridement of soft tissue that are under gangrenous processing, extract the phalangeal bone without any soft tissues. Osteosynthesis of the extracted phalangeal bone with host phalangeal bone. The exposed bony portion covered with vascularized flaps such as revered radial forearm pedicled flap, free radial forearm flap and neurovascular island finger flap. This procedure underwent within a week after vascular insufficiency developed. All of the flaps are survived, bone union achieved within 3 months. The function and external appearance of the reconstructed thumb were encouraging; Pinch Power was average 1.2 Pounds. Early removal of necrotizing soft tissue followed by covering none vascular phalangeal bone which extracted from the dead phalanx with vascularized flap is one of the useful alterative solutions in failed replantation surgery in hand.

Histologic Study on the Effect of Two Types of Bovine Bone Powder in Extraction Socket of Beagle Dogs (성견 발치와에 매식한 2종의 Bovine Bone Powder가 치유에 미치는 영향에 관한 조직학적 연구)

  • Several extraction cases with advanced bone loss as a result of periodontal disease, root or labial bone fracture, extensive caries, and periapical lesions occur esthetic, functional problems and severe bone loss. Therefore, to treat these cases used several surgical methods and socket preservation among this therapies have been evaluated simple, effective and good prognosis in the implant placement. Socket preservation therapy have been used with barrier membranes or/and graft materials. Deproteinized bovine bone mineral have been evaluated ideal grafting materials. Recently, calcium-phosphate thin film coated bovine bone powders were developed in our country, but the study for these material wasn't reported. When two types of xenograft materials were implanted in extraction sockets of Beagle dogs, the effects of these were analyzed after 4 weeks and 8 weeks histological views. The results of this study were as follows. 1. In control groups, 4 weeks after implantation, the extraction sockets were filled with connective tissue which has dilated vessels and epithelial growth. And after 8 weeks, irregular connective bundles were observed. But new bone formation was not seen. 2. In Bio-Oss groups, epithelial growth was not seen and bone powder was covered with connective tissue fiber. New bone formation was found around the interproximal bone. There was no special change seen after 8 weeks, connective tissue fibers became more regular, and bone growth near bone powder was not made well. 3. In Ca-P BBP groups, epithelial cells didn't grow in the extraction sockets, there was a lot of new bone made around the bone powder after 8 weeks, new bone around bone powder was replaced with mature bone. It is thought that bone powder grafting into the extraction sockets is very useful for conservation of ridge, and Ca-P BBP is more effective in bone formation than Bio-Oss.

  • Aim of the study: An alternative source of adult stem cells that could be obtained in large quantities, under local anesthesia, with minimal discomfort would be advantageous. Adipose tissue could be processed to obtain a fibroblast-like population of cells or adipose tissue-derived stromal cells (ATSCs). This study was performed to confirm the availability of ATSCs in bone tissue engineering. Materials amp; Methods: In this study, adipose tissue-derived mesenchymal stem cell was extracted from the liposuctioned abdominal fat of 24-old human and cultivated, and the stem cell surface markers of CD 105 and SCF-R were confirmed by immunofluorescent staining. The proliferation of bone marrow mesenchymal stem cell and ATSCs were compared, and evaluated the osteogenic differentiation of ATSCs in a specific osteogenic induction medium. Osteogenic differentiation was assessed by von Kossa and alkaline phosphatase staining. Expression of osteocyte specific BMP-2, ALP, Cbfa-1, Osteopontin and osteocalcin were confirmed by RT-PCR. With differentiation of ATSCs, calcium concentration was assayed, and osteocalcin was evaluated by ELISA (Enzyme-linked immunosorbant assay). The bone formation by 5-week implantation of HA/TCP block loaded with bone marrow mesenchymal stem cells and ATSCs in the subcutaneous pocket of nude mouse was evaluated by histologic analysis. Results: ATSCs incubated in the osteogenic medium were stained positively for von Kossa and alkaline phosphatase staining. Expression of osteocyte specific genes was also detected. ATSCs could be easily identified through fluorescence microscopy, and bone formation in vivo was confirmed by using ATSC-loaded HA/TCP scaffold. Conclusions: The present results show that ATSCs have an ability to differentiate into osteoblasts and formed bone in vitro and in vivo. So ATSCs may be an ideal source for further experiments on stem cell biology and bone tissue engineering.
