개에서 요추부 척수를 압박하는 척추 골육종 발생례 (Vertebral Osteosarcoma Causing Compression of the Lumbar Spinal Cord in a Dog)

  • 15년 령의 중성화 수컷 요크셔 테리어가 양측 후지의 보행 장애로 내원하였다. 5개월 전부터 후구 부전마비가 시작되어 점점 진행되었고, 4일 전부터는 증상이 빠르게 악화되었다. 후구 부전마비를 일으킬 수 있는 신경 질환을 감별하기 위해 컴퓨터 단층 촬영과 자기 공명 영상이 실시되었다. 컴퓨터 단층 촬영을 통해 4번 요추의 골 결손과 인접 부위의 연부 조직 종괴를 확인하였다. 자기 공명 영상을 통해서는 4번 요추의 왼쪽에서 T1강조 영상에서의 등신호종괴와 T2강조 영상에서의 고신호 종괴가 척수를 압박하는 것을 확인하였다. T1강조 영상에서 조영제 주입 후 종괴의 신호가 균일하게 증가하였다. 종괴의 절제 후 통증은 완화되었다. 조직병리학적 검사를 통해 병변이 척추 골육종임을 확인하였다. 컴퓨터 단층 촬영과 자기 공명 영상은 개에서 척추 골육종을 진단하고 수술적 제거를 위한 계획을 세우는데 유용하게 이용될 수 있다.

전위성 관절내 종골 골절에서 최소 침습적 족근동 도달법 및 압박나사 내고정술을 이용한 수술적 치료 (The Operative Treatment using Mini-open Sinus Tarsi Approach for Displaced Intraarticular Calcaneal Fractures)

  • Purpose: This study was performed to evaluate the clinical outcomes of operative treatment using mini-open sinus tarsi approach for displaced intraarticular calcaneal fractures. Materials and Methods: We studied 18 cases (16 patients) of intraarticular calcaneal fractures who were treated with sinus tarsi approach by same surgeon. The mean age of patients was 44.8 years, and mean follow-up period was 17.2 months. The measurement of B$\ddot{o}$hler angle, Gissane angle, the degree of articular surface depression, and the period to union were performed through preoperative and postoperative radiographs. The clinical evaluation was performed according to hindfoot score of the American Orthopaedic Foot and Ankle Society (AOFAS) and scale of the Creighton-Nebraska health foundation (CNHF). Results: B$\ddot{o}$hler angle and Gissane angle had improved significantly from preoperative average $9.8^{\circ}$, $117.6^{\circ}$to average $22.4^{\circ}$, $113.4^{\circ}$ immediate postoperatively, and had maintained to average $21.8^{\circ}$and $114.2^{\circ}$ at the last follow-up. The degree of articular surface depression had improved significantly from preoperative average 5.2 mm to 1.2 mm at the last follow-up. All cases achieved bone union, and the period to union was average 10.5 weeks. AOFAS score was average 86.2 points at the last follow-up. There were 7 excellent, 9 good, and 2 fair results according to the CNHF scale. Therefore, 16 cases (88.8%) achieved satisfactory results. Conclusion: The minimally invasive sinus tarsi approach using headless compression screw seems to be an effective surgical method for displaced intraarticular calcaneal fractures, because of the possibility of accurate restoration of articular surface and the low risk of postoperative soft tissue complications.

비글견에서의 Calcium Polyphosphate의 조직반응에 관한 연구 (Tissue Response of Calcium Polyphosphate in Beagle Dog)

  • 치주조직 재생을 도모하기 위한 전통적인 시술방법으로 여러가지 골이식재를 이용한 골이식술이 이용되고 있다. 이번 실험의 목적은 골형성을 위한 재료로 Calcium Polyphosphate(CPP)를 사용하여 비글견에서의 조직 반응과 골유도성을 보는 것이고 또한 다른 이식재들간에 신생골의 형성에 어느정도 영향을 주는 가를 알아보는 것이다. 이번 실험에 사용된 CPP는 무수 $Ca(H_2PO_4)$를 condensation하여 무결정의 $Ca(PO_3)_2$를 얻고 이를 용율하고 냉각시킨 후 분쇄하여 얻은 것으로 3세된 비글견에 이식하여 관찰하였다. 조직학적으로 CPP granule의 경우는 키토산이나 $Na_2O$를 넣은 경우 모두 bone 의 ingrowth가 관찰되었고 다른 섬유조직의 개재를 볼수 없었다 동결탈회건조골을 넣은 경우에는 주위로 골형성 보이지 않았고 단지 섬유성 조직이 관찰 되었다. 아무것도 넣지 않은 경우에 비해서 동결탈회건조골이나 키토산, $Na_2O$를 넣은 CPP granules 경우에 더 많은 비율로 신생 골의 양이 나타나는 것을 볼수 있었다. 아무것도 넣지 않은 대조군과 이식재를 넣은 군간에는 유의성이 있는 것으로 나타났고(p<0.05). 또한 CPP granules with chitosan과 CPP granules with $Na_2O$ 사이에는 신생골의 형성에 유의성이 없었다. 이것으로 보아 CPP granules with chitosan과 CPP granules with $Na_2O$는 모두 골유도성이 있고 신생골의 형성을 촉진하므로 치주질환으로 인한 골결손 부위에 사용할 수 있는 재료로 우수한 특성을 지닌다고 사료된다.

TWO COLORIMETRIC ASSAYS VERIFY THAT CALCIUM SULFATE PROMOTES PROLIFERATING ACTIVITY OF HUMAN GINGIVAL FIBROBLASTS

  • Statement of problem. The role of calcium sulfate in stimulating the growth of gingival soft tissue has been reported in few studies. Such a unique property of calcium sulfate could serve as a trouble-solving broker in compensating for the lack of soft tissues in various oral surgeries. Purpose. The purpose of this study was to compare the proliferating activities of human gingival fibroblasts seeded on various bone graft barrier materials of calcium sulfate, collagen, and polytetrafluorethylene (PTFE). Material and methods. Two calcium sulfates ($CAPSET^{(R)}$. and $CalForma^{(R)}$, Lifecore Biomedical Inc., St. Paul, Minnesota, USA), a resorbable natural collagen ($Bio-Gide^{(R)}$, Geistlich Pharma Ag., Wolhusen, Switzerland), and a non-resorbable PTFE ($TefGen-FD^{(R)}$, Lifecore Biomedical Inc., St. Paul, Minnesota, USA) served as the human gingival fibroblasts' substrates and comprised the four experimental groups, whereas the untreated floors of culture plastics were used in the control group, in this study. Cells were trypsinized, seeded, and incubated for 48 h. The proliferating activities of fibroblasts were determined by XTT and SRB assay and absorbance (optical density, OD) was measured. One-way ANOVA was used to analyze the differences in the mean OD values between the groups of CAPSET, CalForma, Bio-Gide, TefGen, and the control (p<0.05). Results. From the XTT assay, the mean OD value of the control group, the highest, was significantly greater than that of any of the four experimental groups followed by CalForma, CAPSET, TefGen, and Bio-Gide. Further, the mean OD value of CalForma, was significantly greater compared to that of Bio-Gide. From the SRB assay, Calforma showed the highest mean OD value, which was significantly greater than that of any other groups, followed by the control, CAPSET, Bio-Gide, and TefGen. The mean OD values of both the control and CAPSET were significantly greater compared to that of TefGen (p<0.05). Conclusion. Assessment of the viability and proliferation of cultured fibroblasts seeded and incubated for 48 h on various barrier-material substrates using XTT and SRB assay showed that calcium sulfate $CalForma^{(R)}$ promotes the proliferating activity of human gingival fibroblasts.

  • ECD는 포말 대식세포의 전신 침범으로 염증 반응을 일으키는 드문 질환으로 다양한 임상양상과 불량한 예후를 보이는 질환이다. 본 증례는 골 동통과 폐막 침범이 된 자에서 흉막 생검을 통한 확진으로 ECD가 진단된 경우이며 기저질환으로 진성 적혈구 증다증이 있는 자에서 발현한 예를 경험하였기에 이를 문헌고찰과 함께 보고하는 바이다.

보온 재료에 따른 구명 조끼 별 단열성능의 비교 실험 및 해석 (Comparative Studies of Thermal Insulation Performance of Life Vests by Numerical Analysis and Experiment)

  • 구명 조끼는 익수자의 부력을 유지시키는 기능을 가지며, 낮은 해수 온도에서 저체온증에 빠지는 시간을 단축 시킬 수 있는 역할을 할 수 있다. 본 논문은 서멀 마네킹 실험과 수치적 방법을 적용하여 단열성능을 향상 시키기 위해서 개발된 팽창형 구명 조끼와 폼형 구명 조끼의 단열성능 및 저체온증 방지 효과를 평가하였다. 단열성능 평가를 위해서 서멀마네킨을 이용하여 열유속 및 열저항을 계측하였으며, 본 연구에서 제시된 구명 조끼의 단열성능을 기존의 제품과 비교하여 검토하였다. 또한 저체온증에 빠지는 정도를 상대적으로 파악하기 위해서 유한요소해석을 이용하여 구명 조끼의 종류에 따른 체온 저감 시간을 예측하고 이를 비교 평가하였다. 저체온증 예측모델은 Pennes의 신체 열전달 해석 모델을 기반으로 작성되었으며, 실험으로부터 계측한 열저항 값을 이용하여 대류 열전달 조건을 환산하여 계산되었다. 그 결과 본 연구에서 단열성능을 향상시키기 위해서 제시된 하는 구명 조끼가 기존 제품에 비해 단열성능이 우수하게 평가됨을 확인하였다.

하악 상행지부에 발생한 치성각화낭의 시상 분할 골절단술을 이용한 치험례 (TREATMENT OF OKC ON RAMUS OF MANDIBLE BY SAGITTAL SPLITTING TECHNIQUE)

  • Odontogenic keratocyst (OKC) is a epithelial developmental cyst which were first described by Phillipsen in 1956. The frequency of OKC has been reported to vary from 3% to 11% of odontogenic cysts. The most characteristic clinical aspect of OKC is the high frequency of recurrence. The mechanism of recurrence is thought to be related to residues of cyst epithelium and an intrinsic growth potential following excision. And since the lining of the OKC is thin and friable, removal of the cyst in one piece may sometimes be difficult. Complete removal of the cyst lining without leaving behind remnants attached to the soft tissue or bone is necessary to avoid recurrence. Therapeutic approaches vary in different studies from marsupialization and enucleation, which may be combined with adjuvant therapy such as cryotherapy or Carnoy's solution, to marginal or radical resection. The recurrent rate varies from approximately 20% to 62%. And OKC in the angle-ramus region of the mandible had a higher tendency to recur, because of the difficulty in accessing and removing OKC from the ramus. By employing a sagittal splitting of the mandible a good surgical access was provided and cyst could be removed completely. We present an illustrative case of a small, lobulated OKC that involved ramus on mandible, and a review of the contemporary literature.

II급 1류 부정교합 환자에서 Bionator의 적응증에 관한 연구 (AN EVALUATION ON THE INDICATIONS OF BIONATOR IN CLASS II DIVISION 1 MALOCCLUSION)

  • 본 연구의 목적은 bionator를 사용한 II급 1류 부정교합 환자의 치료후 결과를 토대로 치료전에 치료후 결과를 예측할 수 있는 항목들을 알아봄으로써 성장하는 II급 1류 부정교합을 가진 환자들의 치료에 도움을 얻는데 있다. bionator를 사용한 앵글 II급 1류 부정교합 환자 48명의 치료후 두부방사선계측사진을 통해 치료결과가 양호한 군(1군)과 치료결과가 불량한 군(2군)으로 나눈 후 양군의 치료전 측모두부방사선계측사진의 비교분석을 통해 두군 사이에 차이를 보이는 계측항목들을 알아보았고, 판별분석을 통해 다음의 결과를 얻을 수 있었다. 1. 치료전 골격계측 항목으로는 ANB, facial convexity angle, AB to facial plane angle 등이, 치성계측 항목으로는 L1 to A-Pog, U1 to facial plane, L1 to facial plane 등이, 연조직 계측항목으로는 Ricketts esthetic line 상에서 상, 하순의 돌출도가 양군 사이에 유의한 차를 보였다(SAS t-test, p<0.05). 2. 판별분석을 통해 유의성있게 나타나는 항목의 순위를 본 결과 L1 to facial plane, 하순의 돌출도, ANB과 FMIA 등이 양군의 치료결과의 예측에 도움을 주는 것으로 나타났다. 3. 증감판별분석을 통해 서로 독립적이며 상관계수가 높은 3개의 변수 - L1 to facial plane, articular angle, ANB- 를 선택하였으며, 이를 토대로 판별식을 도출하였다.

상아모세포의 조건배지를 이용한 백악모세포의 분화와 석회화 조절 (Regulation of cementoblast differentiation and mineralization using conditioned media of odontoblast)

  • For the regeneration of periodontal tissues, the microenvironment for new attachment of connective tissue fibers should be provided, At this point of view, cementum formation in root surface plays a key role for this new attachment. This study was performed to figure out which factor promotes differentiation of cementoblast Considering anatomical structure of tooth, we selected the cells which may affect the differentiation of cementoblast - Ameloblast, OD11&MDPC23 for odontoblasts, NIH3T3 for fibroblsts and MG63 for osteoblasts. And OCCM30 was selected for cementoblast cell line. Then, the cell lines were cultured respectively and transferred the conditioned media to OCCM30. To evaluate the result, Alizarin red S stain was proceeded for evaluation of mineralization. The subjected mRNA genes are bone sialoprotein(BSP), alkaline phosphate(ALP) , osteocalcin(OC), type I collagen(Col I), osteonectin(SPARC ; secreted protein acidic and rich in cysteine). Expression of the gene were analysed by RT-PCR, The results were as follows: 1. For alizarin red S staining, control OCCM30 didn't show any mineralized red nodules until 14 days. But red nodules started to appear from about 4 days in MDPC-OCCM30 & OD11-OCCM30. 2. For results of RT-PCR, ESP mRNAs of control-OCCM30 and others were expressed from 14 days, but in MDPC23-OCCM30 & OD11-OCCM30 from 4 days. Like this, the gene expression of MDPC23-OCCM30 & OD11-OCCM30 were detected much earlier than others. 3. For confirmation of odontoblast effect on cementoblast, conditioned media of osteoblasts(MG63) which is mineralized by producing matrix vesicles didn't affect on the mineralized nodule formation of cementoblasts(OCCM30). This suggest the possibility that cementoblast mineralization is regulated by specific factor in dentin matrix protein rather than matrix vesicles. Therefore, we proved that the dentin/odontoblast promotes differentiation/mineralization of cementoblasts. This new approach might hole promise as diverse possibilities for the regeneration of tissues after periodontal disease.

($H_2O_2$와 ascorbic acid가 사람 치주인대섬유모세포의 TIMP-2, Type 1 collagen, PDLs22 발현에 끼치는 효과 (Effects of $H_2O_2$ and ascorbic acid on TIMP-2, Type1 collagen, and PDLs22 levels in human periodontal ligament fibroblasts)

  • Reactive oxygen species (ROS) have been implicated in the pathogenesis of various diseases. And vitamin C has shown a protective effect for the tissues. The aim of this study was to evaluate the effects of $H_2O_2$ and ascorbic acid on matrix metalloproteinase-1 (MMP-1), tissue inhibitor of metalloproteinase (TIMP: TIMP-1, TIMP-2), Type 1 collagen, fibronectin, and PDLs22 level in human periodontal ligament fibroblasts (hPDLF) via reverse transcription-polymerase chain reaction (RT-PCR). hPDLF was obtained from a healthy periodontium and cultured in Dulbecco's modified Eagles's medium plus 10% fetal bone serum. The concentration of ascorbic acid in hPDLF was $50{\mu}g/ml$, and that of $H_2O_2$ in hPDLF was 0.03% and 0.00003%. Ascorbic acid only, $H_2O_2$ only and mixture of ascorbic acid and $H_2O_2$ were applied with hPDLF for 1-, 3-, and 30-min. respectively. The gene expression of MMP-1-, TIMP-1-, TIMP-2-, Type 1 collagen-, fibronectin-, and PDLs22-mRNA in hPDLF was analysed via RT-PCR. The results were as follows; 1. hPDLF in response to 30-min. incubation with 0.03% $H_2O_2$ did not show any gene expression. 2. In all the experimental groups, the gene expression of fibronectin mRNA showed the decreased tendency compared to control. 3. In all the experimental groups, the gene expression of TIMP-1 mRNA showed the tendency similar to control. 4. hPDLF in response to 30-min. incubation with 0.03% $H_2O_2$ and ascorbic acid increased mRNA induction for MMP-1. 5. In all the experimental groups, hPDLF increased mRNA induction for PDLs22, collagen type 1, and TIMP-2 compared to control. Within the limited experiments, $H_2O_2$ and ascorbic acid increased mRNA induction for PDLs22, collagen type 1, TIMP-2 in hPDLF. More research will be needed in order to confirm the relative importance of the different roles of ROS and antioxidants in hPDLF from a periodontal regeneration or repair standpoint.
