Retrospective Study of Bone Resorption after Maxillary Sinus Bone Graft

  • Purpose: This research sought to determine the resorption rate of bone grafted to the maxillary sinus according to the grafted material's type, patient's age, systemic disease, implant size, site of implant placement, and residual ridge height. Materials and Methods: This research targeted 24 patients who had immediate Osstem$^{(R)}$ implant (US Plus$^{(R)}$) placement after bone graft. The panorama was taken before the surgery, after the surgery, and 6 months after the surgery. Vertical height change and resorption rate of the grafted bone were measured with the same X-rays and compared. The influence of the following factors on the grafted bone material's resorption rate was evaluated: grafted material type, patient's age, systemic disease, implant size, site of implant placement, and residual ridge height. Results: Patients in their 40s had $34.0{\pm}21.1%$ resorption rate, which was significantly higher compared to the other age groups (P<0.05). There was no significant relationship between systemic disease and grafted bone resorption. There was no significant relationship between implant size (diameter, length) and grafted bone resorption. There was no significant relationship between the site of implant placement and grafted bone resorption. The ramal bone-grafted site was significantly more resorbed than the ramal bone/Bio-Oss$^{(R)}$-grafted site, maxillary tuberosity bone/Bio-Oss$^{(R)}$-grafted site, and ramal bone/maxillary tuberosity bone/Bio-Oss$^{(R)}$-grafted site (P<0.05). There was no significant difference in the grafted bone resorption rate in the sinus between more than 4 mm and less than 4 mm residual ridge heights. After an average of 6 months, a second surgery was done; given an average follow-up of 1.9 years, the success rate and survival rate of the implant were 96.9% and 98.4%, respectively. Conclusion: These results indicate that the bone resorption rate of grafted bone among patients in their 40s is higher compared to patients in their 50s and over, and that only autogenous bone (ramus) shows higher resorption rate than the mixed graft of autogenous bone and xenogenous graft (Bio-oss) after maxillary sinus graft.

  • Purpose: This study presents radiographic and laboratory analysis and comparison of bone resorption rate of grafted endochondral bone and intramembranous bone on the aspect of height and volumetric change. Patients and Methods: 18 partially edentulous patients who needed alveolar ridge augmentation for implant placement during the years 2002 to 2004 were selected for this study. Group A consisted of 5 males & 3 females who were treated with intraoral(intramembranous) bone and Group B consisted of 8 males & 2 females who were treated with iliac(endochondral) bone. Non-standard periapical X-ray was taken at day 1, 2 month, 4 months, 8 months after the surgery. Resorption rate of grafted bone were measured on these X-rays and compared. Also we calculated volume of grafted bone with models which was fabricated at 1.5 months, 6 months. Results: There was bone resorption in both groups. Group B showed more bone resorption than Group A. In Group A, the resorption rate according to the radiographic measurements was 9.81 %, and resorption rate according to volumetric measurement was 16.5 %. In group B, the resorption rate according to the radiographic measurements was 15.9 %, and resorption rate according to volumetric measurement was 30.6 %. Significant difference is on radiographic resorption of post-op 2, 4, 8 months on two groups (P < 0.05). Also significant difference is on volume resorption on two groups (P < 0.05). Conclusion: We found that more bone resorption occurred with iliac(endochondral) bone and when we use intraoral bone, that bone can maintain their vitality for alveolar ridge augmentation.

  • Purpose: The Purpose of this study is to show the total survival rate of implants with maxillary sinus grafting and the effects that reach the survival rate by classifying types of graft materials, implant type, operation method, residual bone height and evaluate graft material resorption rate after sinus grafting Patients and Methods: 61 dental implants placed with sinus bone grafting in 24 patients at Wonkwang University Sanbon Dental Hospital were installed simultaneously or after regular healing. Various bone grafts (autograft, xenograft, allograft, alloplast) and fourth implant type (GSII, Xive, Implantium, Novel biocare) were used. All implants were investigated clinically and radiographically, being with average 20 months follow-up period after installation. Results: 3 fixtures were lost, resulting in 95.1% cumulative survival rate of 61 osseointegrated dental implant. Survival rate according to bone material type, Implant type, operation method, residual bone height, have no statistically significant differencies. The mean preoperative residual alveolar bone height was 4.75 mm, average postoperative height of graft materials 10.8 mm, vertical bone resorption rate was 10% after 2 years. Resorption rate according to operation method was 7% (simultaneous) and 5% (delayed) after 1 year. Conclusion: It can be suggested that maxillary sinus grafting may have predictable result with various bone graft materials and implant type, residual bone height, operation method

SLA surface를 가진 taper straight designed implant의 방사선학적 평가와 생존율: 1년 추적 관찰을 통한 후향적 연구 (Radiologic evaluation and survival rate of taper-straight designed implant with SLA surface: A 1-year follow-up retrospective study)

  • 목적: 이번 연구는 최근 시판되고 있는 국내산 taper straight 타입의 SLA surface를 가진 임플란트의 변연골 흡수도와 생존율을 평가하여 임상적 유용성을 알아보기 위해 시행하였다. 대상 및 방법: 전남대학교 치과병원 치주과에서 2016년 8월부터 2019년 12월까지 KISPLANT®가 식립된 성인남녀 83명의 125개의 임플란트 중 포함 기준을 충족시키는 19명의 40개의 임플란트를 1년 동안의 관찰 대상으로 하였다. 처음과 1년 후의 치근단 방사선 사진에서 변연골 수준을 측정하고, 이를 이용하여 임플란트 생존율과 성공률을 분석하였다. 또한 치주염 심도, SPT, 식립 부위, 치아 발거 이유, 즉시 식립 여부, 전신질환에 따라 변연골 흡수도에 유의한 차이가 있는지를 확인하기 위해 t-test를 수행하였다. 결과: 근심측 변연골 흡수도는 0.74 ± 1.07 mm, 원심측 변연골 흡수도는 0.53 ± 1.04 mm로 나타났다. 1년 동안 40개의 임플란트 중 3개의 임플란트에서 2 mm 이상의 변연골 흡수가 일어나 임플란트 성공률은 92.5%이고 생존율은 100%이었다. t-test를 수행한 결과 근심측 변연골 흡수도와 식립 부위에서 유의한 차이가 있었고 나머지 요인에서는 유의한 차이가 없었다. 결론: 이번 연구에서 변연골 흡수도와 성공률, 생존율을 평가해 본 결과 임플란트 식립 1년 후 높은 생존율과 성공률을 보였고 낮은 변연골 흡수도를 보여 taper straight 타입의 SLA surface를 가진 임플란트는 우수한 임상 결과를 보였다.

Tilted implants for implant-supported fixed hybrid prostheses: retrospective review

  • Objectives: This review assessed the performance of implant-supported fixed hybrid prostheses in 21 patients who received a total of 137 implants between 2003 and 2010. The implants were evaluated for marginal bone resorption, complications, success rate, and survival rate based on their vertical angularity, type of bone graft, and measured implant stability. Materials and Methods: One-way ANOVA and chi-square tests were used to analyze the relationships among long-term evaluation factors and these variables. The mean initial bone resorption in the implant group with a vertical angle of more than 20° was 0.33 mm and mean final bone resorption was 0.76 mm. In contrast, the mean initial bone resorption in the implant group with a vertical angle of less than 10° was 1.19 mm and mean final bone resorption was 2.17 mm. Results: The results showed that mean bone resorption decreased with an increase in the vertical placement angle of the implants used in fixed hybrid prostheses, as well as in the group without additional bone grafts and those with high implant stability. The success rate of implants placed after bone grafting was found to be higher than those placed simultaneously. Conclusion: These results suggest that implant-supported fixed hybrid prostheses may be an effective treatment option for edentulous patients, and intentionally placing implants with high angularity may improve outcomes.

Evaluation of Survival Rate and Crestal Bone Loss of the Osstem GS II Implant System

  • The survival rate of the OSSTEM GS II Implant 1 year after serving the prosthetic function in 2 domestic and foreign medical institutes was 97.57%; the success rate was 95.7%, and the average alveolar bone resorption was 0.24mm(n=389). In particular, the alveolar bone resorption occurred differently according to the placement location as well as whether or not the patient underwent bone grafting operation, but the implant s length and diameter did not have significant impact on alveolar bone resorption.

이식된 자가골의 종류와 형태에 따른 임플란트 변연골 흡수량에 관한 후향적 연구 (The retrospective study of marginal bone loss around dental implants according to different autogenous bone grafts)

  • Introduction: This study examined the cumulative resorption of implants placed in a severely atrophic mandible and analyzed the radiologic bone resorption in the marginal bone, after an autogenous bone graft including both block and particulates that had been harvested from the ramus and iliac crest. Materials and Methods: A retrospective study was performed on patients who had bone grafts for augmentation followed by implant installation in the mandible area from 2003 to 2008. Twelve patients (6 men and 6 women) who received 34 implants in the augmented sites were evaluated. Cumulative radiologic resorption around the implants was measured immediately, 3 months, 6 months and 12 months after implant installation surgery. Results: The installed implant in grafted bone showed 0.84 mm marginal bone resorption after 3 months and 50% total cumulative resorption after 1 year. The mean marginal bone resorption around the implant installed in the grafted bone was 0.44 mm after 3 months, 0.52 mm after 1 year, after which it stabilized. The implant survival rate was 97% (failed implant was 1/34). Marginal bone resorption of the installed implant in the autogenous onlay block bone grafts was 0.98 mm after 3 months, which was significantly higher than that of a particulated bone graft (0.74 mm) (P <0.05). Conclusion: An autogenous graft including block type and particulate type is a predictable procedure for the use of dental implants in a severely atrophic mandible. Implant placement in augmented areas show a relatively high survival and minimal bone loss, as revealed by a radiologic evaluation.

Retrospective clinical study on sinus bone graft and tapered-body implant placement

  • Objectives: This study evaluated implant success rate, survival rate, marginal bone resorption of implants, and material resorption of sinus bone graft in cases wherein tapered body implants were installed. Materials and Methods: From September 2003 to January 2006, 20 patients from Seoul National University Bundong Hospital, with a mean age of 54.7 years, were considered. The mean follow-up period was 19 months. This study covered 50 implants; 14 implants were placed in the maxillary premolar area, and 36 in the maxillary molar area; 24 sinuses were included. Results: The success rate was 92%, and the survival rate was 96.0%. The mean amount of sinus augmentation was $12.35{\pm}3.27$ mm. The bone graft resorption rate one year after surgery was $0.97{\pm}0.84$ mm; that for the immediate implantation group was $0.91{\pm}0.86$ mm, and that for the delayed implantation group was $1.16{\pm}0.77$ mm. However, the difference was not statistically significant. The mean marginal bone resorption one year after restoration was $0.17{\pm}0.27$ mm (immediate group: $0.12{\pm}0.23$ mm; delayed group $0.40{\pm}0.33$ mm); statistically significant difference was observed between the two groups. Conclusion: Tapered body implant can be available in the maxillary posterior edentulous ridge which sinus bone graft is necessary.

하악골의 불연속 결손부 재건 시 비혈행화 장골이식술의 골흡수율에 관한 연구 (Evaluation of Bone Resorption Rate after Nonvascularized Iliac Bone Graft for Mandibular Discontinuity Defect)

  • Purpose: Mandible resection and discontinuity defect created lead to aesthetic and functional problems. The iliac crest bone graft exhibits relative ease for bone harvesting, possibility of two team approach, ability to close the wound primarily, large amount of corticocancellous bone and relatively few complications. Whereas the use of free vascularized flaps has donor site morbidity and worse-fitting bone contour, the use of nonvascularized iliac bone graft has advantages in the operation time and patients' recovery time. So, nonvascularized iliac bone graft could be an attractive option. Methods: Twenty-one patients (M:F=1:1.1) underwent iliac crest bone harvesting for reconstruction of mandibular discontinuity defect (mean length : $61.6{\pm}17.8$ mm), from May 2005 to October 2011 at the Department of Oral and Maxillofacial Surgery in Kyungpook National University. The average age was $44.1{\pm}16.4$ years and the mean follow up periods was $28.2{\pm}22.7$ months. Bone resorption rate, according to age, sex, primary lesion, location and distance of defect, type of fixation plate, time of graft and pre-operative radiation therapy, were measured in each patient. Results: The mean bone resorption rate was $16.1{\pm}9.0%$. Bone resorption rate was significantly increased in mandibular defect that is over 6 cm in size (P=0.015, P<0.05) and the cases treated pre-operative radiation therapy (P=0.017, P<0.05). All was successfully fixed and maintained for the long-term follow-up. There were a few donor site complications and almost all patients were shown favorable outcome without severe bone resorption in this study. Conclusion: The nonvascularized iliac bone graft seems to be a reasonably reliable treatment option for reconstruction of mandibular discontinuity defects.

Evaluation of Crestal Bone Resorption of the TiUnite(R) Anodized Implant System

  • Purpose : This study sought to examine the aspects of crestal bone resorption and to evaluate the clinical outcomes of the TiUnite$^{(R)}$ (Nobel Biocare, Sweden) anodized implant system. Materials and Methods : Among the 67 patients (211 fixtures) who were treated using TiUnite(r) implants at Seoul National University Bundang Hospital between March 2004 and January 2007, 26 (91 fixtures) were considered in this study. Initial and secondary stabilities were measured using Periotest$^{(R)}$ and Ostell(tm) Mentor. The radiographic evaluation of crestal bone resorption was carried out by measuring the change in crestal bone level at the time of surgery compared to that 1 year after loading. Panoramic radiograph and periapical radiograph were used. Based on the radiographic findings, the shapes of crestal bone resorption were classified. Results : The average amount of crestal bone resorption after 1 year of functional implant loading was 0.30 mm. There was no saucerization in 40 implant fixtures (43.9%), although more than 1 thread were exposed in 51 implant fixtures (56.6%). The success rate of the implants was 94.5%, and the survival rate was 100%. Conclusions : Good clinical outcomes and minor crestal bone resorption were noted in this study. Saucerization for the establishment of biological width was not a general finding in the TiUnite$^{(R)}$ anodized implant system.
