Sequential anti-inflammatory and osteogenic effects of a dual drug delivery scaffold loaded with parthenolide and naringin in periodontitis

  • Purpose: Our pilot study showed that a 3-dimensional dual drug delivery scaffold (DDDS) loaded with Chinese herbs significantly increased the regenerated bone volume fraction. This study aimed to confirm the synergistic anti-inflammatory and osteogenic preclinical effects of this system. Methods: The targets and pathways of parthenolide and naringin were predicted. Three cell models were used to assess the anti-inflammatory effects of parthenolide and the osteogenic effects of naringin. First, the distance between the cementoenamel junction and alveolar bone crest (CEJ-ABC) and the bone mineral density (BMD) of surgical defects were measured in a rat model of periodontitis with periodontal fenestration defects. Additionally, the mRNA expression levels of matrix metallopeptidase 9 (MMP9) and alkaline phosphatase (ALP) were measured. Furthermore, the number of inflammatory cells and osteoclasts, as well as the protein expression levels of tumor necrosis factor-alpha (TNF-α) and levels of ALP were determined. Results: Target prediction suggested prostaglandin peroxidase synthase (PTGS2) as a potential target of parthenolide, while cytochrome P450 family 19 subfamily A1 (CYP19A1) and taste 2 receptor member 31 (TAS2R31) were potential targets of naringin. Parthenolide mainly targeted inflammation-related pathways, while naringin participated in steroid hormone synthesis and taste transduction. In vitro experiments revealed significant antiinflammatory effects of parthenolide on RAW264.7 cells, and significant osteogenic effects of naringin on bone marrow mesenchymal stem cells and MC3T3-E1 cells. DDDS loaded with parthenolide and naringin decreased the CEJ-ABC distance and increased BMD and ALP levels in a time-dependent manner. Inflammation was significantly alleviated after 14 days of DDDS treatment. Additionally, after 56 days, the DDDS group exhibited the highest BMD and ALP levels. Conclusions: DDDS loaded with parthenolide and naringin in a rat model achieved significant synergistic anti-inflammatory and osteogenic effects, providing powerful preclinical evidence.

혈소판유래성장인자-BB가 골간질세포와 치주인대세포의 성상에 미치는 영향 (A study of the effects of PDGF-BB on the characteristics of bone stromal and periodontal ligament cells)

  • The main goal of periodontal therapy is to restore the lost periodontal tissue and establish the attachment appratus. Current acceptable therapeutic techniques are included : removal of diseased soft tissue, demineralization of exposed root surface, using the barrier membrane for preventing the downgrowth of gingival epithelial cell, insertion of graft materials as a scaffolding action, and biological mediators for promoting the cell activity. The latest concept one among them has been studied which based on the knowledge of cellular biology of destructed tissue. Platelet-derived growth factor(PDGF) is one of the polypeptide growth factor which have been reported as a biological mediator to regulate activities of wound healing progress including cell proliferation, migration, and metabolism. The purposes of this study is to evaluate the influences of the PDGF as biological mediator to periodontal ligament and bone marrow cell. Both right and left maxillary first molar were extracted from rat which had treated with 0.4% ${\beta}-Aminopropionitril$ for 5 days, and feeded until designed date to sacrifice under anesthesisa. Periodontal ligament were removed from the extracted socket of the rat, and cultured with Dulbecco's Modified Essential Medium(DMEM) contained with 10% Fetal Bovine Serum, 100U/ml penicillin, $100{\mu}g/ml$ streptomycin, $0.5{\mu}g/ml$ amphotericin-B. Bone marrow cell were culture from bone marrow suspension with which washed out from femur with same medium. The study was performed to evaluate the effect of PDGF to periodontal ligament and bone cell, cell proliferation rate, total protein synthesis, and alkaline phosphatase activity of rat periodontal ligament(PDL) cell and bone stromal(RBS) cell in vitro. The effects of growth factors on both cells were measured at 3, 5th day after cell culture with (control group) or without growth factors(experimental group). The results were as follows: 1. The tendency of cell proliferation under the influence of PDGF showed more rapid proliferation pattern than control at 3 and 5 days after inoculation. 2. The activity of Alkaline phosphatase revealed 14, 80% increased respectively at 3, 5 days culture than control group. Measurements of ALPase levels indicated that PDL cells had significantly higher activity when compared with that of co-culture groups and GF only(P<0.05). And, ALPase activity in 10 days was higher than that of 7 days(P<0.05). 3. The tendency of formation of the mineralized nodule were observed dose-depend pattern of PDL cells. There was statistically significant difference among group l(PDL 100%), 2(PDL 70%:GF 30%), and 3(PDL 50%:GF 50%)(P<0.01). But, there was no difference among group 3, 4(PDL 30%:GF 70%), and 5(GF 100%). 4. Also, the number of nodule was greater in co-culture of PDL 70% and GF 30% than in culture of PDL 70%(P<0.05). From the above results, it is assumed that the PDGF on PDL cells and RMB cell culture. GF stimulates the cell growth, which is not that of PDL cells but GF. And, the activity of ALPase depends on the ratio of PDL cells, and ALPase may relate to the initial phase of nodule formation. Also, it is thought that the calcified nodule formation principally depends on PDL cells, is inhibited by GF, and affected by cell density. In conclusion, platelet-derived growth factor can promote rapid osteogenesis during early stage of periodontal tissue regeneration.

칼슘과 비타민 D 섭취가 티타늄 임플랜트 주위의 골 형성에 미치는 영향 (EFFECT OF CALCIUM AND VITAMIN D SUPPLEMENTATION ON BONE FORMATION AROUND TITANIUM IMPLANT)

  • The purpose of this study was to observe the effect of calcium and vitamin D to the titanium implant osseointegration in animal model. 32 rats, 10 weeks of age, were divided into two group: additional calcium and vitamin D supplementation group and a control group. Titanium screw implant(diameter, 2.0mm; length, 3.5mm; pitch-height 0.4mm) were placed into tibia of 32 rats, 16 in the control group and 16 in the experimental group. The rats were sacrificed at different time interval(1, 2, 4, and 8 weeks after implantation) for histopathologic observation, histomorphometric analysis and immunohistochemistry with osteocalcin and osteopontin antibody. Histopathologically findings, newly formed bone was seen at 1 weeks and became lamellar bone at 2 weeks, and mature trabecullar bone was seen at 4 weeks experimental group. In control group, thickness of regenerated bone increased till 4 weeks gradually and trabecullar bone was seen at 8 weeks. By histomorphometric analysis, bone marrow density was increased significantly at 1 and 2 weeks in experimental group compared to control group. Osteocalcin immunoreactivity was strong at 1 week experimental group and reduced after 4 weeks gradually. But it was continuously weakly from 1 to 4 weeks in control group. Osteopontin immunoreactivity was very strong in newly formed bone from 2 to 8 weeks experimental group. And the amount of osteopontin expression was more abundant in experimental group. The results of this study suggest that calcium and vitamin D supplementation promotes bone healing around dental implants

AMD3100 improves ovariectomy-induced osteoporosis in mice by facilitating mobilization of hematopoietic stem/progenitor cells

  • Inhibition of an increase of osteoclasts has become the most important treatment for osteoporosis. The CXCR4 antagonist, AMD3100, plays an important role in the mobilization of osteoclast precursors within bone marrow (BM). However, the actual therapeutic impact of AMD3100 in osteoporosis has not yet been ascertained. Here we demonstrate the therapeutic effect of AMD3100 in the treatment of ovariectomy-induced osteoporosis in mice. We found that treatment with AMD3100 resulted in direct induction of release of SDF-1 from BM to blood and mobilization of hematopoietic stem/progenitor cells (HSPCs) in an osteoporosis model. AMD3100 prevented bone density loss after ovariectomy by mobilization of HSPCs, suggesting a therapeutic strategy to reduce the number of osteoclasts on bone surfaces. These findings support the hypothesis that treatment with AMD3100 can result in efficient mobilization of HSPCs into blood through direct blockade of the SDF-1/CXCR4 interaction in BM and can be considered as a potential new therapeutic intervention for osteoporosis.

In control group, thickness of regenerated bone increased till 4 weeks gradually and trabecullar bone was seen at 8 weeks. In histomorphometric analysis, marrow bone density increased significantly in experimental group compared to control group. Fibronectin immunoreactivity was strong at 2 weeks in experimental group and reduced after 4 weeks gradually. But it was maintained continuously from 2 to 8 weeks in control group. Collagen type I immunoreactivity was very strong from 2 to 4 week in experimental group. And the amount of Collagen type I expression was more abundant in experimental group. Conclusion: The results of this study suggest that calcium and vitamin D supplementation promote bone healing around titanium implants in osteoporosis induced animals.

만성 카드뮴 처치 쥐에서 골조직 장애에 미치는 녹차 Catechin의 영향 (Effects of Green Tea Catechin on Bone Disorder in Long-Term Cadmium Treated Rats)

  • 만성 카드뮴 중독 흰쥐에서의 골조직의 변화와 이에 대한 녹차 catechin의 해독기전을 관찰하고져 하였다. 실험동물은 100g 내외의 sprague-Dawly종 흰쥐를 대상으로 카드뮴을 투여하지 않은 정상군과 카드뮴 투여군으로 나누었다. 카드뮴 투여군은 다시 식이 내 catechin 공급 수준에 따라 Cd-0C 군(catechin 비공급군), Cd-0.25C군(catechin 2.5 g/kg of diet), Cd-0.5C군(catechin 5 g/kg of diet)으로 나누어 10 및 20주간씩 자유섭식시켰다. 카드뮴은 50 ppm C$d^{2+}$ 농도의 식수로 자유로이 공급하였다. 실험동물은 각기간별로 사육후 뼈조직의 형태학적 변화를 관찰하고 20주째 조직학적 변화 및 골밀도를 관찰하여 만성 카드뮴 중독으로 인한 골조직 장애에 미치는 녹차 catechin의 효과를 관찰하였다. 경골과 대퇴골에서의 카드뮴 함량은 10주에서는 catechin 비공급군인 Cd-0C군에 비하여 catechin 공급군에서 감소하였으며 catechin공급수준에 따른 차이는 없었다. 20주에는 Cd-0C군에 비하여 Cd-0.25C군과 Cd-0.5C군이 각각 유의적으로 감소하였다. 경골과 대퇴골 두 뼈조직의 무게변화를 관찰한 결과는 모두 10주, 20주에서 정상군에 비해 카드뮴 투여군 모두가 다소 감소되었으며 실험군간의 유의성은 없었다. 뼈조직의 길이를 관찰한 결과 대퇴골, 경골 모두에서 10주, 20주 모두 카드뮴 투여군에서 감소되었으나 catechin을 투여한 군에서는 길이 감소가 다소 완화되었다. Cd-0C군에서 정상군에 비해 유의성은 없었으나 다소 감소하였다. 경골에서의 골밀도를 관찰한 결과 골밀도는 정상군에 비하여 Cd-0C군은 유의적으로 낮았으나 catechin공급군은 정상군 수준의 골밀도를 관찰하였다. 대퇴골의 골밀도를 측정한 결과 정 상군에 비해 Cd-0C군은 낮았으나 Cd-0.25C군과 Cd-0.5C군은 Cd-0C군에 비해 유의적으로 높았다. 광학현미경으로 관찰한 결과 정상군에서 보여지는 소견은 카드뮴 투여군에서도 잘 유지되어 있었다. 전자현미경 관찰한 결과 정상군에서 보여지는 소견은 카드뮴 투여군에서 도 잘 유지되어 있었다. 결론적으로 녹차 catechin은 만성 카드뮴 중독 흰쥐에서 경골과 대퇴골에서의 카드뮴 축적을 감소시켰으며 골밀도를 정상화시킴으로서 골조직의 변화를 완화시킴을 알 수 있었다.

첫 번째 골다공성 골절에 영향을 미치는 요인 (Risk Factors Associated with the First Osteoporotic Fracture)

  • Purpose: The purposes of this study were to identify the factors associated with the first reported fracture in osteoporotic patients. Methods: One hundred and sixty-four patients who presented with osteoporosis at K hospital in G city, South Korea between July and September 2010 were participated in study. Modified Korea National Health and Nutrition Examination Survey tool were used to measure participants' living habits. Computer aided nutritional analysis program (CAN-PRO) designed by Nutrition Information Center in the Korean Nutrition Society (2002) was used to analyze foods intake and to calculate the individual nutrients intake. Results: The risk of developing the first osteoporotic fracture was ten times higher in the patients between 65 to 74 years (OR=10.06, p=.010), and 28.41 times greater in those with increasing falls (OR=28.41, p<.001). Patients with higher bone marrow density (BMD) and higher consumption of animal protein diet had lower risks of developing the first osteoporotic fracture (OR=0.37, p=.011; OR=0.88, p=.040, respectively). Conclusion: Risk factors for osteoporotic fracture included the age from 65 to 74 years, an increase in falls, low animal protein consumption, and reduced BMD. Systematic educational program is needed to prevent fracture in osteoporotic patients.

Multiple myeloma presenting with a maxillary lesion as the first sign

  • Multiple myeloma is a clonal neoplastic proliferation of terminally differentiated B-lymphocytes involving the skeletal system in a multifocal fashion. Its oral manifestations are less common in the maxilla than in the mandible due to the lower amount of hemopoietic bone marrow in the maxilla. We report the case of a 50-year-old man who presented with a mass in the left maxillary alveolar region with tooth mobility. The mass had become enlarged after the teeth were extracted 15 days previously. Radiographs demonstrated multiple punched-out radiolucent lesions in the skull and pelvic region. Computed tomography images showed a soft tissue density mass in the left maxilla, eroding the floor and walls of the maxillary sinus. Although several analytical techniques were used to characterize the lesion, it was finally confirmed as multiple myeloma through immunohistochemistry.

여성의 생활양식 변화를 통한 골다공증 예방 교육프로그램의 개발 및 효과 (Development of the Education Program and It Effect on Osteoporosis and Life Style among Women)

  • The purpose of this study was to develop the educational program based on the self-efficacy theory of Bandura(1986) and to identify the effect of the program among women. For this purpose a non-equivalent control group, and a pretest- posttest design was used between the experimental and the control group. The subjects in this study were female and were over the age 40, 37 in the experimental group and 46 in the control group. In this study, the educational program was developed to increase the level of osteoporosis self efficacy and to prevent osteoporosis. The program consisted of watching, videotapes, telephone contact, lectures, and small group discussions. This study was conducted to determine whether the 6 month educational program would increase osteoporosis self- efficacy, thus modifying life styles related to osteoporosis increas BMD. The instruments utilized in this study were the Lifestyle Questionnaire, and the Osteoporosis Self-Efficacy Scale. Also, bone marrow density (BMD) on the left wrist was measured by DTX-200. The findings are as follows: 1. A significant decrease in BMD was observed in the control group. By contrast, no significant change in BMD was observed in the experimental group. 2. The Osteoporosis Self-Efficacy was not significantly changed in both the experimental and control groups. 3. In the experimental group, the number of exercise participants and their exercise times were significantly increased. Also the amount of caffeine intake was significantly decreased.

초소형 로봇을 이용한 점도성 유체의 혼합 효율 향상 (Enhancement of Mixing Performance in Viscous Liquid Using an Electromagnetically Driven Microrobot)

  • This paper presents an electromagnetically driven microrobot for the enhancement of mixing performance in high viscous liquid media such as blood and bone marrow. First, an electromagnetic system was fabricated, and the magnetic flux density generated from the system was compared with the theoretical value. Second, the reciprocating motion of the microrobot was demonstrated in microchannel using electromagnetic system. As a proof of concept, the mixing performance by the electromagnetically driven microrobot in high viscous liquid was investigated using safranin solution. As a result, it was completely mixed within 140 s with the reciprocating motion of the microrobot while it took 1680 s for natural diffusion. In addition, the mixing efficiency was quantitatively evaluated through a mixing index obtained by an image analysis. The proposed method provides not only wireless actuation of a microrobot with a simple design but also high mixing performance in variety of high viscous liquid media.
