FEA model analysis of the effects of the stress distribution of saddle-type implants on the alveolar bone and the structural/physical stability of implants

  • Background: As dental implants receive masticatory stress, the distribution of stress is very important to peri-implant bone homeostasis and implant survival. In this report, we created a saddle-type implant and analyzed its stability and ability to distribute stress to the surrounding bone. Methods: The implants were designed as a saddle-type implant (SI) that wrapped around the alveolar bone, and the sizes of the saddles were 2.5, 3.5, 4.5, and 5.5 mm. The X and Y displacement were compared to clarify the effects of the saddle structures. The control group consisted of dental implants without the saddle design (CI). Using finite element modeling (FEM), the stress distribution around the dental implants was analyzed. Results: With saddle-type implants, saddles longer than 4.5 mm were more effective for stress distribution than CI. Regarding lateral displacement, a SI of 2.5 mm was effective for stress distribution compared to lateral displacement. ASI that was 5.6 mm in length was more effective for stress distribution than a CI that was 10 mm in length. Conclusions: The saddle-type implant could have a bone-gaining effect. Because it has stress-distributing effects, it might protect the newly formed bone under the implant.

방사선 조사 후 매식한 임프란트의 표면 종류와 스테로이드 관주에 따른 골 치유 효과에 대한 조직 형태학적 연구 (HISTOMORPHOMETRIC STUDY ON THE INFLUENCE OF STEROID TOPICAL IRRIGATION AND IMPLANT SURFACE ON BONE HEALING IN THE IRRADIATED RABBIT TIBIA)

  • The purpose of this study was to evaluate and compare the influence of Steroid topical irrigation and implant surface on bone healing in the irradiated rabbit tibia. Implant to bone contact surface ratio and the pattern of bone healing around hydroxyapatite(HA) coated implant and pure titanium (Ti) implant which were inserted into the irradiated rabbit tibia were compared. 16 Korean house mature male rabbits were used as experimental animal. Each rabbit received 15 Gy of irradiation. 4 weeks after the irradiation, two holes were prepared in the irradiated tibia of each rabbits, where two surface type of implants were inserted :1) HA coated type and 2) pure Ti type. Right before placing implants, one group of rabbit received steroid irrigation and the other group did saline. After the irrigation, two implants of HA coated type and pure Ti type were inserted into the tibia of each rabbits. Each rabbit were sacrificed at 2nd, 4th, and 8th week after the implantation and the specimens were observed by the light microscope. The pattern of bone healing and histomorphometric analysis of the implant-bone interface were done. The results were as follows. 1. All implants inserted into the irradiated tibia of rabbit did not show any sign of clinical mobility and the bone around implants inserted into the irradiated tibia of rabbit did not show any resorption. 2. The bone to implant contact surface ratio around HA coated implants that received steroid irrigation got more bone to implant contact surface ratio than that of the saline irrigation. This result showed statistically significant(p<0.05). There was no statistically significant difference in 8th week group. 3. Though there was no statistically significant difference HA coated implants had more bone to implant contact surface ratio than pure Ti implant in 2nd and 4th groups, and there was no difference in 8th week group. 4. All implants inserted into the irradiated tibia of rabbit had exhibited successful osseointegraion.

  • 저자들은 본 증례의 경우 치근이 흡수된 전치와 매복 견치의 발치 후 심한 치조골의 흡수와 연조직의 변화가 예상되는 것을 고려하여 발치 후 즉시 임플란트 수술을 결정하였다. 초기 안정성을 확보하기 위해 가능한 적은 골삭제, 보다 큰 직경과 치근형태의 임플란트를 선택하여 발치 후 즉시 임플란트를 식립하였고, 장골에서 채취한 망상골로 골결손부를 채우고 부가적으로 상순 지지를 위해 흡수성 차폐막과 순측 골면에 onlay형태의 골이식술을 시행하여 자연스럽고 심미적인 결과를 얻을 수 있었다.

APPLICATION OF FINITE ELEMENT ANALYSIS TO EVALUATE IMPLANT FRACTURES

  • Statement of problem. Higher fracture rates were reported for Branemark implants placed in the maxilla and for 3.75 mm diameter implants installed in the posterior region. Purpose. The purpose of this study was to investigate the fracture of a fixture by finite element analysis and to compare different diameter of fixtures according to the level of alveolar bone resorption. Material and Methods. The single implant and prosthesis was modeled in accordance with the geometric designs for the 3i implant systems. Models were processed by the software programs HyperMesh and ANSA. Three-dimensional finite element models were developed for; (1) a regular titanium implant 3.75 mm in diameter and 13 mm in length (2) a regular titanium implant 4.0 mm in diameter and 13 mm in length (3) a wide titanium implant 5.0 mm in diameter and 13 mm in length each with a cementation type abutment and titanium alloy screw. The abutment screws were subjected to a tightening torque of 30 Ncm. The amount of preload was hypothesized as 650 N, and round and flat type prostheses were 12 mm in diameter, 9 mm in height were loaded to 600 N. Four loading offset points (0, 2, 4, and 6 mm from the center of the implants) were evaluated. To evaluate fixture fracture by alveolar bone resorption, we investigated the stress distribution of the fixtures according to different alveola. bone loss levels (0, 1.5, 3.5, and 5.0 mm of alveolar bone loss). Using these 12 models (four degrees of bone loss and three implant diameters), the effects of load-ing offset, the effect of alveolar bone resorption and the size of fixtures were evaluated. The PAM-CRASH 2G simulation software was used for analysis of stress. The PAM-VIEW and HyperView programs were used for post processing. Results. The results from our experiment are as follows: 1. Preload maintains implant-abutment joint stability within a limited offset point against occlusal force. 2. Von Mises stress of the implant, abutment screw, abutment, and bone was decreased with in-creasing of the implant diameter. 3. With severe advancing of alveolar bone resorption, fracture of the 3.75 and the 4.0 mm diameter implant was possible. 4. With increasing of bending stress by loading offset, fracture of the abutment screw was possible.

Retrospective long-term analysis of bone level changes after horizontal alveolar crest reconstruction with autologous bone grafts harvested from the posterior region of the mandible

  • Purpose: The goal of this study was to evaluate the long-term success of horizontal alveolar crest augmentation of the retromolar region of the mandible with particulated bone, as well as factors affecting subsequent peri-implant bone loss. Methods: A total of 109 patients (68 female, 41 male) suffering from alveolar ridge deficiencies of the maxilla and mandible were included in this study. All patients were treated with particulated retromolar bone grafts from the mandible prior to the insertion of endosseous dental implants. Mesial and distal peri-implant crestal bone changes were assessed at six time points. Several parameters, including implant survival and the influence of age, gender, localisation of the implant, diameter, covering procedures, and time points of implant placement, were analysed to identify associations with bone level changes using the Mann-Whitney U-test, the Kruskal-Wallis test, and Spearman's rank-order correlation coefficient. Results: A total of 164 dental implants were placed in the maxilla (n=97) and in the mandible (n=67). The mean observation period was $105.26{\pm}21.58$ months after implantation. The overall survival rate was 97.6% after 10 years. Overall, peri-implant bone loss was highest during the first year, but decreased over time. The mean amount of bone loss after 10 years was 2.47 mm mesially and 2.50 mm distally. Bone loss was significantly influenced by implant type and primary stability. Conclusions: The use of particulated autologous retromolar bone grafts is a reliable technique for the horizontal reconstruction of local alveolar ridge deficiencies. Our results demonstrate that implants placed in augmented bone demonstrated similar bone level changes compared to implants inserted in non-augmented regions.

Functional Endoscopic Sinus Surgery for a Patient with Maxillary Sinusitis Occurring after Implant Placement

  • Maxillary sinus membrane elevation and bone graft have been performed routinely in alveolar bone with insufficient residual bone height. There are a number of causes for development of maxillary sinusitis after these procedures. When maxillary sinusitis is caused by sinus membrane elevation, bone graft, and implant placement, various treatment such as medication, incision and drainage (I&D), implant removal, and the Caldwell-Luc procedure can be considered. Removal of an implant or the Caldwell-Luc procedure can be harmful if inflammation is not present in the oral cavity and survival of grafted bone and implant osseointegration can be expected despite the presence of maxillary sinusitis. In this case, functional endoscopic sinus surgery, which was often used in the otorhinolaryngology department, was performed without removal of the implant for a patient with maxillary sinusitis after one month following implant placement. Thus, we report on this case with a review of the literature.

RBM surface 임플란트의 골형성에 대한 EPMA 분석 (EPMA analysis of bone formation around RBM surface implant)

  • Purpose: To evaluate bone formation on the implant surface between machined fixture and RBM surface fixture through analyzing of distribution and concentration of Ca and P by EPMA. Material and Methods: Fixtures had divided in 2 group: Machined(Group I), RBM (Group II). Total 4 fixtures were implanted on rabbit which sacrificed on 2 week and 4 week for the histological specimens. By these specimens EPMA value were measured, compared and analysed by each group to figure out the evidence of clinical use of RBM implant. Result: After 2 weeks and 4 weeks, it was analyzed that bone formation area, distribution and concentration of Ca and P by EPMA. In distribution and concentration of P Group II was higher than Group I, but there were no statistical significances. In new bone formation area, Group II was more higher than Group I with statistically significances. Both of group, after 4 weeks area is little bit higher than after 2 weeks area but there is no statistically significances. Conclusion: RBM implant was better than machined implant on the early bone formation.

자가치아 뼈 이식재의 유용성에 관한 임상적 연구 (Clinical Study on the Efficacy of the Autogenous Tooth Bone Graft Material (AutoBT))

  • Purpose: This study is to evaluate the efficacy of the autogenous tooth bone graft material, clinically and radiologically, as related to implant installation. Methods: In oral and maxillofacial surgery department of Ajou University Hospital, guided bone regeneration (GBR), implant placement combined with GBR, sinus graft, implant placement combined with sinus graft, and defect filling were performed in 46 patients, using autogenous tooth bone. Among these, 66 implants were inserted with autogenous tooth bone. Implant stability quotient (ISQ) was measured by Osstell Mentor (Integration Diagnostics, Goteborg, Sweden) on 39 implants on the operation date and 4 months later, and on 21 implants 9months on the average at the final setting of restoration. Twenty-eight implants with GBR and sinus graft (GBR group: n=14, sinus graft group: n=14) were evaluated radiologically to measure the resorption of grafted autogenous tooth bone after loading. Results: The average initial stabilization of the installed implants was 67 ISQ, and the average secondary stabilization at 4 months later was 76. The average bone loss of GBR group as measured 8.0 months after application of prosthesis loading was 0.29 mm and the average bone loss of the sinus graft group as measured 7.6 months after application of prosthesis loading was 0.66 mm, respectively. In the histological assessment, formation of the new bone and continuous trabecular bone pattern was identified around autogenous tooth bone. Conclusion: Based on these results, we concluded that autogenous tooth bone is an excellent bone graft material that can substitute the autogenous bone.

Clinical evaluation of mandibular implant overdentures via Locator implant attachment and Locator bar attachment

  • PURPOSE. The aim of this study was to evaluate the clinical findings and patient satisfaction on implant overdenture designed with Locator implant attachment or Locator bar attachment in mandibular edentulous patients. MATERIALS AND METHODS. Implant survival rate, marginal bone loss, probing depth, peri-implant inflammation, bleeding, plaque, calculus, complications, and satisfaction were evaluated on sixteen patients who were treated with mandibular overdenture and have used it for at least 1 year (Locator implant attachment: n=8, Locator bar attachment: n=8). RESULTS. Marginal bone loss, probing depth, plaque index of the Locator bar attachment group were significantly lower than the Locator implant attachment group (P<.05). There was no significant difference on bleeding, peri-implant inflammation, and patient satisfaction between the two denture types (P>.05). The replacement of the attachment components was the most common complication in both groups. Although there was no correlation between marginal bone loss and plaque index, a significant correlation was found between marginal bone loss and probing depth. CONCLUSION. The Locator bar attachment group indicates lesser marginal bone loss and need for maintenance, as compared with the Locator implant attachment group. This may be due to the splinting effect among implants rather than the types of Locator attachment.

Fin type 임플랜트 고정체의 유한요소법적 분석 (FINITE ELEMENT ANALYSIS OF FIN-TYPE IMPLANT FIXTURES)

  • The purpose of this study was to analyze the stress pattern in different bone densities surrounding fin-type implant fixtures under the vertical and inclined loads ($30^{\circ}) of 200N. Von-Mises stress, the pricipal stress, and the displacement on the implant fixtures under the loads were calculated by using the finite element method. Four different types of bicon implant fixture were used for this study. The geometries of implant fixtures to develop the model were used by a sales brochure and profile project. Three-dimensional finite element model of the mandible was developed with 6.0 mm implant in diameter wurrounded by approximately 2.5 mm of bone. Bone densities were classified according to the elastic modulus of the tree. The finite element program MSC PATRAN (MSC, Software Corp., USA) were used for analysis of stress distribution. The value of the Von-Mises stress, the pricipal stress, and the displacement on the implant fixtures under the vertical and inclined loads were decreased when the diameter of implant fixture was increased, and increased when the elastic modulus was decreased. The stress on implant fixture under the vertical and inclined loads was distributed through the length of implant fixtures in D3 and D4. The distribution of stress was influenced by the direction of loads. In the wide diameter of implants, the stress was developed at outer surface of bone. In conclusion, this study suggest that stress developing on the peri-implant tissues might be influenced by the dimension of implant, elastic modulus of bone, and direction of loads.
