어머니의 골밀도와 생활습관이 소아청소년의 골밀도에 미치는 영향 (Maternal and lifestyle effect on bone mineral density in Korean children and adolescents aged 8-19)

  • Higher bone mineral density (BMD) at a young age, calcium intake, and exercise are important for prevention of osteoporosis later in life. We examined familial effects of BMD between mothers and children and adolescents aged 8-19 in Cheonan, Korea and the relationships between BMD and lifestyle parameters, including: food and nutrient intake and exercise. For daughters and sons, significant differences in BMD were observed at the three bone sites (total femur, femur neck, and lumbar spine) according to age, gender, body mass index, exercise, and milk consumption, compared to the reference value for each classification category. Mean differences in children's BMD were observed according to maternal BMD. Energy and calcium intake were lower in both children and mothers in comparison to the estimated daily energy requirement; however, their protein intake was much greater than the daily recommended intake. After adjusting for age and gender and for mother's age, body mass index, and total calorie intake, results of the food frequency test showed an association of a higher intake of meat, meat products, milk and milk products with greater BMD of total femur, femur neck, and lumbar spine of children. In addition, exercise was positively associated with higher BMD. Regression analysis showed a positive association of BMD with age, male gender, exercise, and mother's BMD. In conclusion, after adjustment for environmental parameters, maternal BMD had a positive influence on BMD in daughters and sons. This finding suggests that parents need to check their BMD in order to determine whether their children are at increased risk of low BMD.

치조열 환자의 장골이식술 후 예후 평가 (A Prognosis Evaluation after Iliac Bone Graft in Cleft Alveolus Patients)

  • Alveolar cleft exists in 75% of cleft patients, In alveolar cleft patients, alar base is widening, palatal fistular formation, maxillary growth disturbance & tooth loss of adjacent area is raised, Alveolar bone grafting, especially iliac bone grafting, is a general treatment method. As operation timing, bone grafting is classified with primary, early secondary, secondary, & late secondary, Here we report cleft width, marginal bone height, bone resorption rate, grafted shape & bone densities after secondary iliac bone grafting was done in the Dept. of oral and maxillofacial surgery of chonbuk national university hospital. We compared cleft width to bone resorption rate and grafted shape. Also, alveolar bone densities of grafted and contralateral site was compared with Emago 3 package? (Oral Diagonostic System, The Netherlands), The data obtained were analyzed using Spearman's rho coefficients and sign test with SPSS for window, The results were obtained as follows. 1. As alveolar cleft width is increase, bone resorption rate is, too. This relation showed significant difference(P<.01). 2, In proximal & distal area, alvolar cleft width and bone graft contour after bone grafting had a reverse proportional difference. It was not significant difference(P>.05). 3. After 3 month, in bone density results by using Emago 3 package? with periapical standard view, occlusal view & panoramic view, differences between grafted bone and alveolar bone of contralateral site didn't show a significant difference(P>.05). Thus, differences of bone densities in the alveolar bones didn't exist.

TG Osseotite 임플란트의 성공률에 대한 임상적 연구 (CLINICAL STUDY ON SUCCESS RATE OF TG OSSEOTITE IMPLANT)

  • The purpose of this study is to review the prognosis of the TG Osseotite implant(3i Co, USA) placed in partial edentulous area of oral cavity and to suspect the possible causes leading to failure. 124 TG Osseotite implants that had been inserted between 2000 - 2002 were followed up for 2 years(avg : 9.5 months) in function. Medical records, and radiographs were evaluated and analyzed by the over all success rate, gender and age factor, general disease, implant fixture length and diameter, implant site, bone density, and various surgical methods. Chi square test was used statistically. Of the 124 TG Osseotite implants, 9 implants(7.3%) were removed in early phase and 3 implants(2.4%) were in late phase. The cumulative survival rate was 90.2%. The failure of the TG Osseotite implant was closely related with the use of bone graft techniques such as sinus elevation or immediate implantation and not with the age, sex, general disease, implant site, bone density of implanted site. The failure of the TG Osseotite implant was well developed when it was the wide type of implant and it was inserted for single tooth replacement. The developement of peri-implantitis was the most important factor in the failure of the TG Osseotite implant.

Effects of marine oligomeric polyphenols on body composition and physical ability of elderly individuals with sarcopenia: a pilot study

  • [Purpose] We aimed to identify the effects of marine oligomeric polyphenol (MOP) intake in elderly individuals with sarcopenia. [Methods] Older adults (aged 65 years or older) were recruited based on the diagnostic criterion for sarcopenia and were randomly assigned to the MOP intake group (n=10) or the placebo (PBO) intake group (n=10). To determine the effect of MOP intake received for four weeks, the pre- and post-intake body composition (weight, skeletal muscle mass, and bone density) and senior fitness tests were assessed. [Results] Our results showed there were significant differences in the skeletal muscle mass (p=0.039), bone density (p=0.020), fat-free mass index (p=0.026), and 2.4 m up and go test (p=0.001) between pretest and post-test. There was a significant difference between the pre-test and post-test and an interaction effect for the one-leg stand test (p=0.010 and p=0.049, respectively). However, there were no significant differences in body fat percentage, calf circumference, grip strength, or the chair rise test. [Conclusion] Some variables exhibited significant differences in the pre- and post-assessments, and there was an interaction effect for the one-leg stand. However, this was insufficient to prove the effectiveness of MOP intake in improving sarcopenia. Therefore, additional studies are essential to examine the effects of MOP intake and exercise intervention on the body composition and fitness of patients over a longer period.

농촌지역 중.노년 여성에서의 운동 프로그램의 효과 (The Effect of an Exercise Program on Middle-aged and Aged Women in Rural Areas)

  • Purpose: This study was conducted to test the effect of a 12-week exercise program on body composition. blood pressure, blood glucose, blood lipid and bone mineral density for middle-aged and aged women in rural areas. Methods: The subjects were 33 women at the age of 40-75. For statistical analysis, descriptive statistics and paired t-test were used with SPSSWIN 16.0. Results: The results were summarized as follows. First, body weight, BMI. % body fat. abdominal adipose, and waist circumference of the women decreased significantly after implementing the 12-week exercise program. Second, diastolic blood pressure, serum cholesterol, triglyceride, and LDL decreased significantly, and the HDL level increased significantly after the 12-week exercise program. Third, bone mineral density did not increase significantly after the program. Conclusion: These results suggest that the exercise program has an effect in decreasing body composition, and improving blood pressure, blood glucose, and blood lipid in middle-aged and aged women.

일 지역주민의 골감소증 영향요인 (Factors related to Osteopenia in Community People)

  • Purpose: The purpose of this study was to investigate bone mineral density (BMD) and risk factors related to osteopenia in community people. Methods: The subjects consisted of 145 community people. The variables of osteoporosis knowledge and preventive health promoting behavior were measured using questionnaires. BMD and BMI (body mass index) were measured. Dietary nutrients intake related to osteoporosis was measured using 24hours recall. Data analysis was performed using the SPSS 18.0 program, using mean, standard deviation, ${\chi}^2-test$, t-test, and logistic regression. Results: Eighty six people (59.3%) had an osteopenia. According to normal group and osteopenia group, there was significant difference in gender (${\chi}^2=6.72$, p=.010) and smoking (${\chi}^2=5.18$, p=.023). Predictors of osteopenia group were male (OR=2.65, CI=1.03-8.98), BMI(OR=0.86, CI=0.72-0.96). Conclusion: Health care providers must consider factors that affect osteopenia such as gender, BMI. These results suggest the need to instruct nursing activities and interventions which are beneficial for prevention of osteoporosis.

걷기운동 및 근육강화운동이 류마티스 관절염 여성환자의 대퇴골 및 요추골 골밀도에 미치는 효과 (Effect of Brisk Walking and Muscle Strengthening Exercise on Bone Mineral Density of the Lumbar and Femur in Rheumatoid Arthritis Women)

  • This study was conducted to test the effect of brisk walking & muscle strengthening exercise program on bone mineral density(BMD) of the lumbar & femur in rheumatoid arthritis women. Research design was a quasi-experimental study of non-equivalent control group pretest-posttest design(16 weeks). 14 for the experimental group and 14 for the control group were selected from the outpatients on rheumatoid arthritis clinic of Dong-A University Hospital. The experimental group underwent 16 weeks of brisk walking and muscle strengthening exercise. Bone mineral density was measured before and after 16 weeks of exercise by DXA at lumbar spine, femoral neck, Ward's triangle and trochanter. The results were summarized as follows : 1. BMD of the lumbar spine in experimental group who carried out the brisk walking and muscle strengthening exercise was not significantly increased after 16weeks and there was no significant difference between experimental and control group(U=70.00 p>.05). 2. BMD of the femoral neck in the experimental group who carried out the brisk walking and muscle strengthening exercises was significantly increased after 16 weeks(Z=-2.901 p<.01). But, there was no significant difference between experimental and control group(U=83.00 p>.05). 3. BMD of the femoral Ward's triangle in the experimental group who carried out the brisk walking and muscle strengthening exercises was significantly increased after 16 weeks (Z=-2.355 p<.05). But, there was no significant difference between experimental and control group(U=86.00 p>.05). 4. BMD of the femoral trochanter in experimental group who carried out the brisk walking and muscle strengthening exercise was not significantly increased after 16weeks and there was no significant difference between experimental and control group(U=75.00 p>.05). These results suggest that brisk walking and muscle strengthening exercise program has an effect on promoting bone mineral density of femoral neck and Ward's triangle in rheumatoid arthritis women.

정량적 초음파 측정에 의한 골밀도의 남녀 차이: 20대 대학생 대상으로 (Sex Differences in Bone Marrow Density Measured by Quantitative Ultrasonometry: For 20 year old college student)

  • 본 연구에서는 정량적 초음파 측정 방식으로 획득한 골밀도를 남녀 간에 차이를 비교 분석 하였다. D대학에 재학 중인 104명(여자 69명, 남자 35명)의 학생을 대상으로 일반적 특성 및 골다공증 위험요인에 대해 설문조사를 실시하였고, 골밀도는 정량적 초음파 측정(QUS, quantitative ultrasonometry)으로 모든 참여자의 양측 종골(calcaneus)을 각 1회씩 측정하였다. SPSS ver. 19.0 프로그램 사용하여 남녀 간에 골밀도 차이를 독립표본 T-검정(Independent T-test)으로 유의성을 검증하였다. 전체 대상자 104명 중 여자 69(66.3%)명, 남자 35(33.7%)명으로 여자가 많았다. 왼쪽 종골의 T-값은 여자 0.443으로 남자 0.031 보다 높았다(p=0.161). 오른쪽 종골의 T-값 또한 여자가 남자 보다 높았다(0.555 vs. 0.049, p=0.161). 남녀 모두 왼쪽 종골 보다 오른쪽 종골의 T-값이 높았지만, 통계적인 유의성은 없었다(p>0.05). 신체활동량이 많을수록 양쪽 종골 모두 T-값도 증가하는 양상을 보였지만, 통계학적인 유의성은 없었다(p>0.05). 양측 종골의 골밀도는 남자 보다 여자가 높았고, 신체활동량이 많을수록 증가하였다.

이중에너지 방사선흡수 골밀도 장치의 품질관리 현황 (A Status Report on Dual Energy X-ray Absorptiometry Quality Control in Korea)

  • DEXA는 골다공증 진단에 가장 많이 사용되는 표준 측정법이다. 현재 국내에서 운영되고 있는 DEXA 장치는 5,671대이다. 본 연구에서는 국내에서 설치 운영되고 있는 DEXA장비의 품질관리 실태를 조사하였다. 3,000개 기관을 대상으로 한 설문 조사에서 12.6%의 응답을 받아 그 결과를 분석하였다. 결과에서 77.5%가 일일 품질관리를 시행하고 있으나 Shewhart chart는 68.5%, CUSUM chart는 86.0%가 모르거나 사용하지 않는다고 답하여 정확한 품질관리가 되지 않는 것으로 조사되었다. 정확한 골밀도 검사는 환자의 골절의 위험도를 산정하기 위한 자료로 활용된다. 하지만 부적절한 검사는 오진을 유발할 가능성을 높여준다. 따라서 정확한 골밀도 검사를 위해서는 적절한 보조기구를 사용하고 장치의 품질관리를 반드시 시행하여 장치와 자세에 대한 오차를 줄이고 검사 의료진의 교육을 통해 오류 요인을 줄이는 것이 필수적이다.

가돌리늄조영제가 이중에너지 X-선 흡수법을 이용한 골밀도검사에 미치는 영향 (Effects of Gadolinium Contrast agent on Bone Mineral Density Measurement using Dual Energy X-ray Absorptiometry)

  • 골밀도검사에서 측정값에 영향을 주는 요인 중 하나인 방사선 조영제는 진단 목적으로 사용되어 조직 등에 흡수되어 골밀도 측정값을 높이는 효과가 있어, 골밀도검사 시행 전에는 될 수 있으면 피하고 있다. 이러한 방사선 조영제와는 다른 물리적 특성과 작용기전을 갖는 MRI 조영제는 금속원소인 가돌리늄 기반으로 방사선 불투과성 특징을 가지고 있어 일반적으로 방사선을 이용한 검사 전에 MRI 검사를 선 시행하고 있다. 본 연구에서는 MRI 조영제가 이중에너지 X-선 흡수법을 이용한 골밀도검사에 미치는 영향을 알아보고자 하였다. 가돌리늄 기반의 MRI 조영제 2종을 아크릴 수조에 용량별로 주입하여 인체모형 척추 팬텀을 넣고, 각 주입량 종류별로 5회씩 총 30회 scan 하여 L1-L4 번까지의 골밀도와 T-score를 분석하였다. 측정된 2종의 조영제별 Total(L1-L4) 골밀도의 평균값은 가도테레이트 메글루민성분 0mL, 7.5mL, 15mL일때, 각각 0.952±0.052, 0.957±0.050, 0.956±0.05 g/㎠로 나타났으며, 가도부트롤성분 0mL, 5mL, 10mL일 때, 각각 0.953±0.001, 0.954±0.001, 0.945±0.001g/㎠로 모든 부위에서 통계적으로 유의한 차이는 없었다(p>0.05). Total (L1-L4) T-score의 평균값은 가도테레이트 메글루민성분은 0mL, 7.5mL, 15mL일 때, 각각 -0.46±0.05, -0.4±0, -0.42±0.04로 나타났으며, 가도부트롤성분 0mL, 5mL, 10mL일 때, 각각 -0.46±0.05, -0.46±0.05, 0.5±0.00으로 모든 부위에서 통계적으로 유의한 차이는 없었다(p>0.05). 본 실험에서는 MRI 조영제는 이중에너지 X-선 흡수법을 이용한 골밀도검사에 영향이 없는 것으로 나타났으나, 임상 대상이 아닌 제한적 조건의 인체 모형 팬텀을 사용하여 조영제의 배출시간에 영향을 주는 신장 기능, 신체 상태 등이 고려되지 못한 제한점이 있어 추후 추가적인 임상연구 진행이 필요할 것으로 사료된다.
