한국여성의 골밀도와 운동과의 관계 (The Relationship of Exercise to Bone Mineral Density of Korean Women in Taegu)

  • The relationship between exercise and hone mineral density (BMD) was investigated in 153 healthy women. The BMD of lumbar spine, femur(neck, ward's triangle, trochanter) and total body was determined by dual energy X-ray absorptiometry in a group subjects(65) aged 19-59 years who had been exercising(swimming or aerobic dancing) regularly for at least 2 years as well as in a similar group of nonexercising control subjects(88). Weight, height, total lean body mass(=weight-total fat body mass-bone mineral content), animal and meat Ca, Ca index, energy expenditure, BMD, PYD/Cr were significantly higher in the exercisers than the controls. There were significantly negative correlations between age, ALP and osteocalcin and BMD, but significantly positive correlations between weight, BMI, total fat body mass and total lean body mass and BMD. Stepwise multiple regression analysis revealed that total lean body mass may be a better independent predictor to BMD than total fat body mass. The nutrient intakes were more closely related to BMD in the exercisers than the controls, but energy expenditure was more closely related to BMD in the controls than the exercisers Stepwise multiple regression analysis revealed that BMD was closely related to menopause, osteocalcin, age, weight in both groups but energy intake in the exercisers alone, energy expenditure in control alone. In premenopausal women, the exercisers had significantly greater BMD than the controls. But, in postmenopausal women, no significant difference between two groups was detected. When compared to BMD of the subjects with same age range to minimize the effect of age, aerobic dancing appears to be capable of exerting a positive effect on BMD in a group of subjects aged 19-44. However, no relationship of the swimming to BMD could be identified in a group of subjects aged 37-59. The results of this study suggest that the usefulness of exercisng appears to be significantly greater in preemenopausal women than postmenoparusal women and weight bearing activity, aerobic dancing is associated with increasing BMD at the weight bearing sites and could be beneficial in the prevention of bone loss. But the usefulness of swimming on bone should be further investgated.

  • Purpose : Bony strength is dependent on bone mass and bony structure. So this study was designed to investigate the relationship between the bone mass and bony trabecular characteristics. Subjects and Methods : Study subjects were 51 females (average age 68.6 years) and 20 males (average age 66.4 years). Bone mineral density (BMD, $grams/cm^2$) of proximal femur was measured by a dual energy X-ray absorptiometry (DEXA). Regions of interest (ROIs) were selected from the digitized radiographs of proximal femur. A customized computer program processed morphologic operations (MO) of ROIs. 44 skeletal variables of MO were calculated from ROIs on the Ward's triangle and greater trochanter of femur. WHO BMD classes were predicted by MO variables of the same ROI. Classification and Regression Tree analysis was used for calculating weighted kappa values, sensitivity and specificity of MO. Results : The discriminating factors of morphologic operation were branch point, branch point [per cm sq]. Age also played important role in distinguishing osteoporotic classes. The sensitivity of MO at Ward's triangle and Greater Trochanter was 91.8%, 65.6%, respectively. The specificity of MO was 100% at Ward's triangle and Greater Trochanter. Conclusion : Bony trabecular characteristics obtained using radiological bone morphometric analysis seem to be related to bone mass.

Measurement of Age-Related Changes in Bone Matrix Using 2H2O Labeling

  • Age-related changes in bone metabolism are well established by biochemical markers of bone matrix in serum and urine, but analysis of the residual bone matrix, which is still turning over, has not been investigated. In the present study, we measured in vivo rates of bone protein synthesis using a precursor-product method based on the exchange of ²H from ²H₂O into amino acids. Four percent ²H₂O was administered to mice in drinking water after intraperitonial (i.p) bolus injection of 99.9% ²H₂O. Mice were divided into the two groups: growing young mice were administered 4% ²H₂O for 12 weeks after an i.p bolus injection at 5 week of age, whereas weight stable adult mice started drinking 4% ²H₂O 8 weeks later than the growing group and continued 4% ²H₂O drinking for 8 weeks. Mass isotopomer abundance in alanine from bone protein was analyzed by gas chromatography/mass spectrometry. Body ²H₂O enrichments were in the range of 1.88-2.41% over the labeling period. The fractional synthesis rates (ks) of bone protein were 2.000±0.071%/d for growing mice and 0.243±0.014%/d for adult mice. These results demonstrate that the bone protein synthesis rate decreases with age and present direct evidence of age-related changes in bone protein synthesis.

Dairy Dietary Calcium and Osteoporosis - An Overview

  • The osteoporosis is a disease characterized by lower bone mineral content, deterioration of bone tissue and a reduction in the protein and mineral matrix of the bone. The bone becomes more porous leading to increased bone fragility and risk of fracture, particularly of the hip, spine and wrist. Osteoporosis can result in disfigurement, lowered self·esteem, reduction or loss of mobility, and decreased independence. Adequate calcium intake through milk and milk products in childhood and adolescence is a decisive marker for obtaining a maximum bone mass (peak adult bone mass) and f3r the prevention of osteoporosis. Calcium is one of the most critical nutrients associated with the osteoporosis. Dietary calcium is of great significance for healthy skeletal growth and development. The bone mineral content and bone mineral density of young adults is directly related to the calcium intake through milk and dairy products. Milk and milk products are the important sources of calcium as the richness and bioavailability of this nutrient is very high as compared to other food products. If enough calcium is not supplemented through diet, calcium from the bone will be depleted to maintain the blood plasma calcium level. The article focuses on the various issues related to osteoporosis manifestation and the role of dietary calcium especially calcium derived from dairy products.

Differential effects of jump versus running exercise on trabecular bone architecture and strength in rats

  • [Purpose] This study compared differences in trabecular bone architecture and strength caused by jump and running exercises in rats. [Methods] Ten-week-old male Wistar rats (n=45) were randomly assigned to three body weight-matched groups: a sedentary control group (CON, n=15); a treadmill running group (RUN, n=15); and a jump exercise group (JUM, n=15). Treadmill running was performed at 25 m/min without inclination, 1 h/day, 5 days/week for 8 weeks. The jump exercise protocol comprised 10 jumps/day, 5 days/week for 8 weeks, with a jump height of 40 cm. We used microcomputed tomography to assess microarchitecture, mineralization density, and fracture load as predicted by finite element analysis (FEA) at the distal femoral metaphysis. [Results] Both jump and running exercises produced significantly higher trabecular bone mass, thickness, number, and fracture load compared to the sedentary control group. The jump and running exercises, however, showed different results in terms of the structural characteristics of trabecular bone. Jump exercises enhanced trabecular bone mass by thickening the trabeculae, while running exercises did so by increasing the trabecular number. FEA-estimated fracture load did not differ significantly between the exercise groups. [Conclusion] This study elucidated the differential effects of jump and running exercise on trabecular bone architecture in rats. The different structural changes in the trabecular bone, however, had no significant impact on trabecular bone strength.

Posterior Atalntoaxial Fusion with C1 Lateral Mass Screw and C2 Pedicle Screw Supplemented with Miniplate Fixation for Interlaminar Fusion : A Preliminary Report

  • Objective : To investigate the feasibility of C1 lateral mass screw and C2 pedicle screw with polyaxial screw and rod system supplemented with miniplate for interlaminar fusion to treat various atlantoaxial instabilities. Methods : After posterior atlantoaxial fixation with lateral mass screw in the atlas and pedicle screw in the axis, we used 2 miniplates to fixate interlaminar iliac bone graft instead of sublaminar wiring. We performed this procedure in thirteen patients who had atlantoaxial instabilities and retrospectively evaluated the bone fusion rate and complications. Results : By using this method, we have achieved excellent bone fusion comparing with the result of other methods without any complications related to this procedure. Conclusion : C1 lateral mass screw and C2 pedicle screw with polyaxial screw and rod system supplemented with miniplate for interlaminar fusion may be an efficient alternative method to treat various atlantoaxial instabilities.

허약아와 성장지표의 상관성에 대한 연구 (A Study on Comparison between Growth Indices of Weak Children Groups via Analyzing Bone Age and Body Composition)

  • Objectives The purpose of this study is to investigate the relationship between weak children and growth indices. Methods One hundred twenty-six of the first and the second grade elementary school students who didn't have to develop yet their secondary sexual characteristics answered the questionnaire. Their bone age and body composition were measured. Results 1. According to the Weak Children questionnaire analysis, respiratory problem was one of the biggest problems in the weak children group (35.7%), followed by digestive problems (23.0%), psycho-neurological problems (22.2%), neuromotor problems (9.5%), and urogenital problems (8.7%). 2. From the comparison between growth indices of weak children and that of normal children are as follows: 1) The group of children who had problem in their digestive system had lower growth indices than the normal average children group. The growth indices includes mean height, weight, total body water, protein mass, mineral mass, body fat, skeletal muscle mass, body fat percentage, body mass index (BMI), and basal metabolic rate (BMR). 2) The children who had urogenital weakness had lower mean height, RH (Recent Height), RH - MPH {Recent Height(%) - Mid-Parental Height(%)}, RUS (Radius, Ulna and Short bone) score, weight, protein mass, body fat, BMI, and visceral fat than the normal children group. 3. The results of the multiple comparisons of growth indices and weak children groups are as follows: 1) Digestive weak children were the lowest in total body water, protein mass, mineral mass, skeletal muscle mass, and basal metabolic rate. 2) Urogenital weak children were the lowest in height, RH, RH - MPH, RUS score, and weight. Conclusion These results showed that children's growth is strongly correlated to their own growth problems, especially to those children who have digestive and urogenital problems. Therefore, this may be an effective way for children growth treatment in Korean medicine to treat weak symptoms.

대구지역 여대생의 골밀도에 영향을 미치는 관련인자와 영양소 섭취와의 상관성 분석 (An Analysis of Related Factors and Nutrients Intake Affecting Bone Mineral Density of College Women in Daegu Area)

  • This study was conducted to examine bone mineral density and factors which effect on bone mineral density such as daily nutrients intake, age, menarch age and physical condition among healthy female college students in Daegu area from April 20, to May 20, 2004. 1. Average age was 20.5$\pm$0.96 years old, average height was 160.9$\pm$4.30㎝, and average weight was 55.9$\pm$7.67㎏. Body mass index was 21.6$\pm$2.91㎏/㎡, body fat was 25.6$\pm$5.79%, menarche age was average 12.5$\pm$1.1 years old and WHR(waist/hip circumference ratio) was 0.8$\pm$0.01㎝/㎝. 2. Average level of bone mineral density(T-score -0.56$\pm$0.91) was in normal range. But, 11 persons(24.4%) are over T-score -1.0, 33 persons(73.4%) were within -1.0 - -2.5 and one person(2.2%) was under -2.5. It is very anxious level for Osteopenia-low bone mass, as research result shows 73.4% of the subject of examine on the level of Osteopenia. 3. Daily calorie intake was 2,550㎉ and each nutrient intake, compared to the seventh recommended dietary allowances for korean, was as follows ; Calorie 112%, protein 123%, calcium 78%, phosphorus 137%, iron 68%, vitamin A 101%, vitamin $B_1$ 141%, vitamin $B_2$ 95%, niacin 107%, vitamin C 128% and zinc 120%. The ratio of calcium to phosphorus(Ca/P) is 0.66, low compared to RDA, but phosphorus intake is so high compared to RDA that precautions should be taken. The ratio of calcium to protein(Ca/Protein) is 8.55. 4. Menarche age and bone mineral density of calcaneus showed positive correlation and body mass index(BMI) indicated positive correlation. Age, height, weight. WHR and physical activity coefficient all do not indicate any significant correlation with bone mineral density. 5. Intake of Calorie, Ca, Ca/p ratio, carbohydrate and fat intake were positively correlated and, protein was negatively correlated, and Fe, Na, P and cholesterol were negatively correlated with BMD. These results indicate that average bone mineral density of subjects was in normal range, but subjects in the stage of osteopenia-low bone mass are many and bad effects are expected to have on their bone mineral density after menopause. Therefore, in order reach maximal bone mineral density, they should improve the balance between calcium and phosphorus and reduce salt intake. And it is thought that education and profound studies on relevant factors affecting the genesis of bone mineral density should be made.

하악 협측골 시상분절술 및 장골 이식술을 이용한 거대치아종의 치험례 (A CASE REPORT OF THE HUGE COMPLEX ODONTOMA TREATED WITH THE SAGITTAL SPLITTING OF BUCCAL BONE PLATE AND ILIAC BONE GRAFT IN LEFT MANDIBLE ANGLE)

  • This is the case report of huge complex odontoma treated with sagittal splitting of buccal bone plate and iliac bone graft in left mandible angle. The 22 years old patient was admitted to the department of Oral and Maxillofacial Surgery of Kyungpook National University Hospital with the chief complaint of swelling on the left mandible angle area. We used extra oral Risdon incision and splitted the buccal cortical bone after making the horizontal bone cut buccally. The tumor mass was removed with cutting into the pieces with surgical bur to prevent mandibular fracture. The dead space was grafted with autogenous iliac bone graft and the splitted buccal cortical bone was fixed with two L-type miniplate. After 12months follow up check, we noticed good process of bone healing and satisfactory aesthetic result. In this case, my operative approach provided the excellent surgical access to the hard tissue mass and minimized post operative complication comparing with the conventional surgical approaches.

Relationship among Bone Mineral Density, Body Composition, and Metabolic Syndrome Risk Factors in Females

  • Osteoporosis is a disease that increases the fracture rates and a major cause of increased mortality and morbidity in the elderly people. This study is to determine which components of body composition and metabolic syndrome risk factors are important to bone health, we analysed the relationship among bone mineral density (BMD), body composition and metabolic syndrome risk factors in females. Totally 630 females participated in a medical check-up program (mean age 47 years) were selected for this study. Body composition analysis was performed by segmental bioelectrical impedance method, muscle mass, and percent body fat were measured. We also measured metabolic syndrome risk factors including abdominal obesity, HDL-cholesterol, triglyceride, blood pressure and fasting glucose level. Metabolic syndrome was defined by NCEP-ATP III criteria. The lumbar spine and femoral neck BMD were measured using the dual energy X-ray absorptiometry. Osteopenia and osteoporosis were observed in 180 and 51 persons, respectively. Muscle mass and HDL-cholesterol decreased in osteopenia and osteoporosis groups compared to the control group, and the grade was shown progressively by the symptoms. Significant positive correlation between BMD and muscle mass was observed. Multi variable regression analyses showed that % body fat and muscle mass were independent predictors of BMD after adjustment of age, height and weight. In conclusion, the BMD showed negative correlation with the metabolic and body composition was associated with BMD.
