Primary Osteosarcoma of the Sphenoid Bone - Case Report - (접형골에 발생한 원발성 골육종 1예 - 증례보고 -)

  • Osteosarcoma is the most frequently encountered primary malignant tumor of the bone. But primary osteosarcoma of the skull(POS) is rare. The author presents a case of skull neoplasm identified as osteogenic sarcoma. A twentyseven-years-old male patient was admitted because of painful swelling at left temporal and zygomatic area with impairment of extraocular movement. Chest film and long bone series showed no evidence of abnormality. Skull films revealed round irregular bony destructive area at the left pterional area. CT and MRI revealed expansile destruction of the left sphenoid bone, lateral orbital wall and temporal bone within the hemorrhagic mass lesions which showed wall enhancement. Histologic examination confirmed a rare variant of osteosarcoma of the telangiectatic type.

Diagnostic imaging of nasal malignant melanoma in a dog (개 비강에서 발생한 악성 흑색종의 영상 진단 증례)

  • A neutered male, 8-year-old, Pekingese, weighing 4.3 kg with a history of anorexia, sneezing, nasal discharge, and epistaxis for one month was referred. Soft tissue swelling around the nasal bone and small defects of the hard palate with a tiny round dark red mass were found on physical examination. The laboratory tests represented mild leukocytosis. On skull radiographs, soft tissue swelling and osteolytic change of the incisor bone, nasal bone, and maxilla were found. On computed tomography scan images, there was soft tissue attenuating opacity with calcified spots in the bilateral nasal cavities and frontal sinuses. Loss of nasal turbinate pattern and nasal septum was found. And destruction of the insicor bone, nasal bone, maxilla, hard palate, perpendicular palatine bone, and cribriform plate were identified. Nasal malignant melanoma was confirmed by nasal biopsy.

Proliferative and Differentiative Effects of Trachelogenin Isolated from Germinated Safflower (Carthamus tinctorius) Seeds on Calvarial Bone Cells

  • Germination is well-known to enhance the digestibility, functionality, and palatability of plant seeds. To examine the functionality of germinated-safflower seed (GSS), proliferative and differentiative effects of GSS extract on the mouse calvarial bone cells were determined by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolinbromide (MTT) assay and alkaline phosphatase activity, respectively. Water extract of GSS increased dose-dependently proliferative and differentiative effects on calvarial bone cell, and its effects were stronger than those of ungerminated-safflower seeds (UGSS) extract. One major component was isolated from GSS extract by a series of purification procedure of solvent fractionation, Diaion HP-20, and Sephadex LH-20 column chromatographies. Its chemical structure was identified as trachelogenin (TC) by nuclear magnetic resonance (NMR) and mass spectrometry (MS) spectral analysis. Trachelogenin showed significant proliferative (125.7%) and differentiative (132.1%) effects on calvarial bone cells at $10^{-8}M$, and its effects were significantly higher than those of $17{\beta}-estradiol\;(E_2)$. TC was found to be a major active compound responsible for high proliferative and differentative effects of the water extract of GSS. Therefore, these results suggest that TC in GSS may be useful as potential therapeutic agent for the prevention and treatment of bone loss.

  • A congenital cholesteatoma is a benign mass formed from the keratinizing stratified squamous epithelium. It usually occurs in young children's anterosuperior part of the middle ear. A congenital cholesteatoma which originates from mastoid temporal bone or expands to posterior cranial fossa is rare. Standard treatment of an intracranial cholesteatoma is surgical removal with craniotomy. A 69-year-old woman was diagnosed with a congenital cholesteatoma of mastoid temporal bone that expanded to the posterior cranial fossa, which was successfully treated with transmastoid marsupialization without craniotomy. This is a first documented case of a congenital cholesteatoma of mastoid temporal bone that expanded to posterior cranial fossa, which was successfully treated with transmastoid marsupialization without craniotomy.

The Study of Correlations between Body Compositions and Bone Mineral Density in Young Women and Postmenopausal Women with Low Bone Mineral Density (젊은 여성과 대퇴경부 저골밀도 폐경기 여성의 신체조성과 골밀도의 상관관계 연구)

  • Purpose : The aim of this study was to investigate the correlations between body compositions and bone mineral density in young women and postmenopausal women with low bone mineral density. Methods : Eleven young women (age, $25.85{\pm}1.96yrs$; height, $160.28{\pm}2.16cm$; weight, $56.89{\pm}9.66kg$) and ten postmenopausal women (age, $25.85{\pm}1.96yrs$; height, $160.28{\pm}2.16cm$; weight, $56.89{\pm}9.66kg$) with low bone density on femur neck were participated in this study. All subjects performed the measure of body composition and BMD on lumbar body and femur neck. Results : The results were as follows. Percent body fat, waist-hip ratio and body mass index(BMI) were significantly increased in postmenopausal women. BMD in lumbar body and femur neck were significantly decreased in postmenopausal women. There were negative strong correlation between BMD of the femur and age in young women. There were negative strong correlation between BMD of the femur and age or the years of menopause in postmenopausal women. There were positive strong correlation between BMD of the femur and BMD of lumbar body. Conclusion : Percent body fat and BMI were more increased in young women than in postmenopausal women. And there were negative strong correlation between BMD and age or the years of menopause in postmenopausal women.

BK Channel Deficiency in Osteoblasts Reduces Bone Formation via the Wnt/β-Catenin Pathway

  • Global knockout of the BK channel has been proven to affect bone formation; however, whether it directly affects osteoblast differentiation and the mechanism are elusive. In the current study, we further investigated the role of BK channels in bone development and explored whether BK channels impacted the differentiation and proliferation of osteoblasts via the canonical Wnt signaling pathway. Our findings demonstrated that knockout of Kcnma1 disrupted the osteogenesis of osteoblasts and inhibited the stabilization of β-catenin. Western blot analysis showed that the protein levels of Axin1 and USP7 increased when Kcnma1 was deficient. Together, this study confirmed that BK ablation decreased bone mass via the Wnt/β-catenin signaling pathway. Our findings also showed that USP7 might have the ability to stabilize the activity of Axin1, which would increase the degradation of β-catenin in osteoblasts.

Atypical proliferative nodule in congenital melanocytic nevus with dural invasion: a case report

  • Here we report a case of a focal atypical proliferative nodule (PN) arising from a congenital melanocytic nevus (CMN). Diagnosis was challenging because it had both benign and malignant clinical features. Unusual histopathology, immunohistochemistry, and intraoperative findings of this atypical PN are discussed. A 5-year-old girl was admitted for a congenital $5{\times}5cm$ sized scalp mass. This hemangioma-like soft mass showed biphasic characteristics such as a slow, gradual, and benign increase in size but worrisome dural invasion with cranial bone defect. We removed the scalp mass with clear resection margins. Interoperatively, we found that the cranial bone defect had already filled. Histopathologic examination showed CMN with focal atypical PN. The nodule showed sharp demarcation and cellular pleomorphism. However, in immunohistochemical study, Ki-67 proliferation index and expression levels of protein S-100 and Melan-A were very low. These were unusual findings of atypical PNs. Despite her worrisome preoperative radiologic features, she showed an indolent clinical course compatible with previously reported biologic behavior. The patient underwent follow-up inspection with magnetic resonance imaging every 6 months for up to 3 years. The nodule appeared to be stationary at the last visit.

Relationship between vitamin K status, bone mineral density, and hs-CRP in young Korean women

  • Vitamin K intake has been reported as an essential factor for bone formation. The current study was conducted under the hypothesis that insufficient vitamin K intake would affect inflammatory markers and bone mineral density in young adult women. The study was a cross-sectional design that included 75 women in their 20s. Physical assessments, bone mineral density measurements, 24-hr dietary recalls, and biochemical assessments for high sensitivity C-reactive protein (hs-CRP) and percentages of undercarboxylated osteocalcin (%ucOC) were performed. An analysis of vitamin K nutritional status was performed comparing first, second, and third tertiles of intake based on %ucOC in plasma. Vitamin K intake levels in the first, second, and third tertiles were $94.88{\pm}51.48\;{\mu}g$, $73.85{\pm}45.15\;{\mu}g$, and $62.58{\pm}39.92\;{\mu}g$, respectively (P < 0.05). The T-scores of the first and third tertiles were 1.06 and -0.03, respectively, indicating that bone mineral density was significantly lower in the group with lower vitamin K intake (P < 0.05). There was a tendency for different serum hs-CRP concentrations between the first ($0.04{\pm}0.02$) and third tertiles ($0.11{\pm}0.18$), however this was not statistically significant. Regression analysis was performed to identify the correlations between vitamin K nutritional status, inflammatory markers, and bone mineral density after adjusting for age and BMI. Serum hs-CRP concentrations were positively correlated with vitamin K deficiency status (P < 0.05). And bone mineral density, which was represented by speed, was negatively correlated with vitamin K deficiency status (P < 0.05). In conclusion, status of vitamin K affects inflammatory status and bone formation. Therefore, sufficient intake of vitamin K is required to secure peak bone mass in young adult women.

HISTOLOGICAL COMPARATIVE STUDY OF GUIDED BONE REGENERATION WITH GORE-TEX-TXMEMBRANE AND COLLACOTE® (Gore-TexTMmembrane과 COLLACOTE®를 이용한 골유도 재생술의 조직학적 비교 연구)

  • The principle of guided tissue regeneration (GTR), as applied to bone healing, is based on the prevention of connective tissue from entering the bony defect during the healing phase. This allows the slower bone producing cells to migrate into and reproduce bone within the defect. GTR has demonstrated a level of success in regenerating bone defect. Several types of membrane barrier have been utilized to apply this principle in bone regeneration. The purpose of this study was to evaluate whether improved bone regeneration can be achieved with different membrane barriers ($Gore-Tex^{TM}$membrane, $COLLACOTE^{(R)}$). In the 10 NewZealand white rabbits, full-thickness bone defects on three sites of each rabbit calvaria were made. Experimental group 1 was covered with $COLLACOTE^{(R)}$, and group 2 was covered with $Gore-Tex^{TM}$membrane. Macroscopic, microscopic examinations were made serially on 1, 2, 3, 6, 12 weeks after operation. The results were as follows : 1. Macroscopically, both of experimental group 1, 2 were filled with bone-like mass but the defects of experimental group 1 disclosed markedly thinner than the original bone. 2. Microscopically, the defect of experimental group 1, 2 was filled with bony trabeculae without infection and adverse reaction. But multinucleated giant cell infiltration around $COLLACOTE^{(R)}$ was seen till 6th week. 3. Resorption of $COLLACOTE^{(R)}$ started from 3rd week and it was completely resorped on the 12th week.
