EFFECT OF PROTEIN-ENERGY RESTRICTION, PREGNANCY AND LACTATION ON THE DEVELOPMENT OF LONG BONE IN GROWING SHEEP

  • The effect of dietary protein-energy restriction (PER) on the growth of long bone were examined in sheep during growing period and then following a cycle of reproduction. Three months-old female sheep were offered protein-energy restricted feed for 6 months (growing period), thereafter changed to normal nutritional conditions for 8 months (reproduction period). A half of animals in each group took pregnancy, parturition and lactation. The nutritional restriction reduced the growth in bone diameter more than that in bone length. Compensatory growth resulting from the removal of the nutritional restraint strongly occurred in bone diameter, especially the bone cortical width, as compared to bone longitudinal growth. A cycle of reproduction severely decreased the growth in the bone cortical width relative to that in the bone width, and little effect was found on the growth in bone length and bone mass. The depression of bone development by pregnancy and lactation tended to exhibit severer in animals having received normal nutrition than in those having received PER. Bone mineral density was not affected by the nutritional restriction. A cycle of reproduction had an adverse effect on the mineral density between in animals having received normal nutrition and in those having received PER.

가미좌귀음(加味左歸飮)이 난소적출(卵巢摘出)로 유도(誘導)된 백서(白鼠)의 골다공증(骨多孔症) 미치는 영향(影響) (The Effect of Kamijoaguiem on Osteoporosis Induced by Ovariectomy in Rats)

  • Purpose : This study was peformed to evaluate the effect of Kamijoaguiem(JGE) on the bone mass and its related factors. Methods : We used ovariectomized rat as an estrogen-deficient animal model. The model rats of osteoporosis showed a significant decrease in bone density, bone ash density, calcium content of femur bone. At the 7th day after operating ovariectomy, rats were administered with JGE per orally, and continued for 10 weeks. And osteoporosis related parameters were determined to investigate the effect of JGE. Results : Bone density, bone ash density, bone calcium, magnesium and phosphorus was decreased in osteoporotic rats. JGE improved the decreased bone density, bone ash density and the decreased bone magnesium, but JGE didn't improve the decreased bone calcium and phosphorus in osteoporotic rats. Osteocalcin in serum and hydroxy-proline excretion in urine were increased in osteoporotic rats. Their levels were decreased when JGE was administered. ALP activity in serum was increased in osteoporotic rats. JGE didn't induce any significant changes. JGE showed significant increase in serum calcium level, total protein level, albumin level, BUN level, serum LDH activity. JGE didn't show significant increase in serum T-cholesterol density, triglyceride density, HDL-cholesterol density. JGE didn't show significant increase in RBC number, hemoglobin level, platelet number, hematocrit level. JGE showed inhibitory effect on the degradation of bone-matrix in osteoporotic rats, in histological examination to Hematoxylin-eosin stain. Conclusion : JGE might improve bone density due to inhibition of bone resolution in osteoporotic rats. It suggest that JGE may be useful prescription in osteoporosis.

뼈의 통증이 있을 때 감별해야 할 악성종양 (Malignancies in children who present with bone pain)

  • 소아청소년기에 뼈의 통증은 일차진료에서 흔히 접할 수 있는 문제이다. 대부분은 성장통이나 스포츠 활동과 관련된 물리적인 원인에 의한 것이지만 급성 림프구성 백혈병, 신경모세포종, 원발성 골종양 등의 악성종양이 뼈의 통증으로 발현하는 경우도 있다. 뼈의 통증을 호소하는 소아청소년 중 악성종양으로 진단 받는 환자는 1% 미만으로 그 빈도가 매우 낮지만 진단과 적절한 치료가 지체되면 환아의 예후에 악영향을 미칠 수 있으므로 어떠한 경우에 악성종양을 의심해야 하는지 숙지해야 한다. 환아가 잠을 잘 수 없을 정도로 심한 뼈의 통증을 2주 이상 호소하며, 발열, 피로감, 야간발한 등의 전신증상이 동반되거나 림프절 비대, 간비장 비대, 사지 혹은 배가 부어 보이고 덩어리가 만져지면 CBC with differential count, 말초혈액 바른검사, CRP, ESR, 단순 방사선 촬영을 시행해야 한다. 위의 검사결과 이상소견이 발견되면 즉시 소아청소년 혈액종양 전문의에게 의뢰를 해야 할 것이다.

족무지 내측종자골 족저부에 발생한 이소성 골화증으로 인한 족저각화증: 증례 보고 (Plantar Keratosis Induced by Heterotopic Ossification under the Medial Sesamoid Bone: A Case Report)

  • Heterotopic ossification is the formation of extra-skeletal bone in the muscle and soft tissues, and an osteoma is a benign bone-forming tumor composed of compact or mature trabecular bone limited almost exclusively to the craniofacial bones. This paper reports an extremely rare case of heterotopic ossification mimicking an osteoma that occurred independently at the plantar side of the medial sesamoid bone. The patient was a 46-year-old male with a three-month history of pain and a hard mass on the plantar aspect of the left forefoot sole. After excising the lesion, the patient's symptoms were relieved, and no pain or complications occurred. This paper discusses this exceedingly rare case of heterotopic ossification around the medial sesamoid bone with a review of the relevant literature.

Secondary Aneurysmal Bone Cyst in a Craniofacial Fibrous Dysplasia: Case Report

  • Aneurysmal bone cyst (ABC) is a rare non-neoplastic bone lesion that involves mostly the long bones and vertebrae and may occur very rarely in the craniofacial bones. ABCs may occur as secondary bony pathologies in association with various benign and malignant bone tumors and with fibrous dysplasia (FD). FD is a common non-neoplastic bony pathology mostly affecting craniofacial bones. Secondary ABC occurring in craniofacial FD is extremely rare, with only approximately 20 cases reported in the literature to date. Here, we report on a case of secondary ABC in a 25-year-old woman who has had a craniofacial deformity for over 10 years and who presented to us with a rapidly growing painful pulsatile mass in the right frontal region that began over 2 months prior to admission. On thorough examination of computed tomography and magnetic resonance imaging brain scans taken at two-month interval, an aggressive, rapidly enlarging ABC, arising from the right frontal FD, was diagnosed. The patient underwent preoperative embolization followed by gross total resection of the ABC and cranioplasty. The 6-month follow up showed no recurrence of the ABC, nor was any progression of the FD noticed.

Aneurysmal bone cyst arising from the surgically removed craniofacial fibrous dysplasia in the long-term follow-up: a case report

  • Fibrous dysplasia (FD) is a rare skeletal disorder characterized by abnormal fibro-osseous connective tissue replacing normal bone. Despite its benign behavior, craniofacial FD can cause morphological disfigurement, headache, and even blindness as a result of the produced mass effect. Surgical resection is recommended when the patient shows apparent clinical symptoms or aggravating facial asymmetry. Postoperative complications have been reported, such as hematoma, surgical site infection, abscess formation, resorption of the bone graft used for reconstruction, and recurrence. An aneurysmal bone cyst (ABC) is a rare benign bony lesion that can occur secondary to preexisting bone tumor. Secondary ABCs in craniofacial FD are extremely rare in the literature, accounting for less than 30, all of which are either case reports or series. We report an extremely rare case of symptomatic secondary ABC arising from craniofacial FD that had been misdiagnosed with abscess formation or recurrence and was surgically removed. Notably, 17 years elapsed between the primary surgery and the complication of secondary ABC. The patient underwent total removal of secondary ABC. After surgery, symptoms were relieved, with no recurrence observed during a 6-month follow-up.

구강 편평상피세포암의 골전이 모델 (BONE METASTASIS MODEL OF ORAL SQUAMOUS CELL CARCINOMA)

  • Background and Purpose: Bone metastases rarely occur in patients with oral squamous cell carcinoma (OSCC), so the molecular mechanisms of bone metastasis of OSCC remains unclear. Studies with animal models allow progresses in understanding the molecular events for bone metastasis and provide new targets for therapy. So we tried to establish a murine model for bone metastasis of oral squamous cell carcinoma. Materials and Methods: Human OSCC cells (KB cell line) were xenografted to nude mice via direct inoculation into the tibial marrow. Mice with tibial tumors were sacrificed once a week, until seven weeks after the injection of human tumor cells. Growth of tibial tumors were observed by histology. Expression of TGF-$\beta$ and CXCR-4 in bone OSCC (experimental) and subcutaneous tumor (control) was also evaluated by immunohistochemical staining. Results: Bone OSCC was successfully induced by intra-tibial injection of KB cells. Tumor mass was developed in the marrow tissues of tibia and finally invade the endosteum of tibia. Immunohistochemical staining showed higher expression of TGF-$\beta$ in bone tumors than in subcutaneous tumors. Conclusion: A murine model of bone metastasis of OSCC was suggested that imitated the clinical findings of distant vascular metastasis. This bone tumor model should facilitate understanding of the molecular pathogenesis of OSCC bone metastasis, and aid in the developement of treatment strategies against OSCC bone metastasis.

The Change of Bone Metabolism in Ovariectomized Rats : Analyses of MicroCT Scan and Biochemical Markers of Bone Turnover

  • Objective : The purpose of this study was to verify the appropriateness of ovariectomized rats as the osteoporosis animal model. Methods : Twelve female Sprague-Dawley rats underwent a sham operation (the sham group) or bilateral ovariectomy [the ovariectomy (OVX) group]. Eight weeks after operations, serum biochemical markers of bone turnover were analyzed; osteocalcin and alkaline phosphatase, which are sensitive biochemical markers of bone formation, and C-terminal telopeptide fragment of type I collagen C-terminus (CTX), which is a sensitive biochemical marker of bone resorption. Bone histomorphometric parameters and microarchitectural properties of 4th lumbar vertebrae were determined by micro-computed tomographic (CT) scan. Results : The OVX group showed on average 75.4% higher osteocalcin and 72.5% higher CTX levels than the sham group, indicating increased bone turnover. Micro-CT analysis showed significantly lower bone mineral density (BMD) (p=0.005) and cortical BMD (p=0.021) in the OVX group. Furthermore, the OVX group was found to have a significantly lower trabecular bone volume fraction (p=0.002). Conclusion : Our results showed that bone turnover was significantly increased and bone mass was significantly decreased 8 weeks after ovariectomy in rats. Thus, we propose that the ovariectomized rat model be considered a reproducible and reliable model of osteoporosis.

성인남녀 480명에서의 골밀도와 비만의 상관관계에 관한 임상적 연구 (A Clinical Study on the correlation between Bone Mineral Density (BMD) and Obesity in 480 normal adults)

  • Osteoporosis is the most common metabolic disease of the bone, and constitutes one of the most important major pubulic health problems world wide. Therefore, in order to be helped early diagnosis, treatment and prevention of osteoporosis, measurement of Bone Mineral Density (BMD) is to be needed. Authors has analysed Bone Mineral Density (BMD) of 480 normal adults who visited woo suk unoversity Oriental Meclical Center from April 1998 to July 1998. The aims of this study is to investigate correlation between Bone Mineral Density (BMD) and age distribution, to examine the correlation between Bone Mineral Density (BMD) and Obesity. The results were as follows. 1. In distribution of age, the peak bone density of lumbar spine was noted around 30 years, and the peak bone density of the femoral neck was noted around 20 years. The age related loss of bone density follows soon after peak density. And the signifficant difference was revealed between lumbar spine and femoral neck bone density (p<0.001) 2. In distribution of sex, the bone density in male was signifficantly higher than in female (p<0.001). 3. In the correlation between Bone Mineral Density (BMD) and Obesity, Bone Mineral Density (BMD) in obese group was signifficantly higher than in non-obese group. Especially, in female from 50 up to 69 years, BMD had a positive correlation with Body mass index(BMI).

가토에서 러버댐을 이용한 골결손부의 골조직 유도 재생술에 관한 실험적 연구 (AN EXPERIMENTAL STUDY OF GUIDED BONE REGENERATION OF BONE DEFECTS IN RABBIT USING RUBBER DAM)

  • The principle of guided tissue regeneration (GTR), as applied to bone healing, is based on the prevention of connective tissue from entering the bony defect during the healing phase. This allows the slower bone producing cells to migrate into and reproduce bone within the defect. The principle of guided tissue regeneration has demonstrated a level of success in regenerating bone defect. Several types of membrane barrier, each one with distinct properties, have been utilized to apply this principle in bone regeneration. The purpose of this study is to introduce and discuss the attributes of rubber dam as a barrier membrane and evaluate whether improved bone regeneration can be achieved by GTR using rubber dam. In the 15 New Zealand white rabbits, full-thickness bone defects on three sites of each rabbit calvaria were made. Non membrane group served as a control and experimental group 1 was covered with rubber dam and group 2 covered with Gore-Tex$^{TM}$ membrane. Macroscopic, radiographic, microscopic examinations were made serially on 1, 2, 3, 6, 12 weeks after operation. The results were as follows: 1. Macroscopically, the control site was collapsed and filled with connective tissue throughout the experimental period. But the defects of experimental groups 1 and 2 were filled with bone-like mass and showed the hard consistency on palpation. 2. Radiographically, the early new bone formation appeared similarly from the host bone in groups 1 and 2. 3. Microscopically, there were much connective tissue at the central part of control site but the defect of group 1 and 2 was filled with the mature bony trabeculae on the 12th week. This results suggest that rubber dam can be effectively used as a barrier membrane for guided bone regeneration.
