Epidermoid cyst is a cystic form of teratoma and believed to be derived from trapped embryonic cells along the lines of embryonic closure. A 28-year-old woman presented with a painless swelling over the left mandibular area. On panoramic view, the mandible revealed a $5.5{\times}2.0\;cm^2$ multilocular radiolucent lesion of the left mandibular body and a computed tomography scan showed expansion of both the buccal and lingual plates in the same area. Microscopy found stratified squamous epithelium of the cystic wall and cystic contents of keratinized material. The histological diagnosis wasan epidermoid cyst. The most common location of epidermoid cyst at the head and neck is in the orbit (47%), followed by the mouth floor (23%) and the cervical area (9~24%), but in the jaw bone, it is considered very infrequent. We report the uncommon epidermoid cyst in the mandibular body that had a good healing outcome after treatment with a conservative marsupialization during the 40 months follow-up.
Thyroglossal duct cyst is a congenital anomaly generally appearing as an asymptomatic midline neck mass. Generally, the duct cyst is benign, but 1 percent of cases may be malignant. These present the same histologic types as thyroid carcinomas. The most common pathologic type is papillary carcinoma. Regional lymph node metastasis of papillary carcinomas in thyroglossal cyst occurs in 7.7 percent of cases. Prognosis of papillary carcinoma is excellent. The treatment has been quite variable, but the most common initial treatment is Sistrunk's operation. Adequate excision of cyst and its tract including the mid-portion of the hyoid bone is the treatment of choice. We experienced two cases of papillary carcinoma arising in thyroglossal duct cyst between 1986 and 2002.
베이커씨 낭종은 슬관절 주위에서 가장 흔하게 나타나는 낭종성 종양으로서, 슬와부의 내측에서 보통 반막양근(Semimembranosus m.)과 비복근(Gastrocnemius m.)의 내측두 사이에서 발견된다. 그리고 다른 곳에 위치하는 경우도 드물게 보고되고 있지만, 주변 근육내로 확장된 베이커씨 낭종은 그 증례를 찾아보기는 쉽지 않다. 저자들은 비복근의 외측두와 슬와건(popliteus tendon) 사이에 위치한 베이커씨 낭종이 주변 족척근(plantaris m.)내로 확장(extension)된 매우 드문 증례를 경험하였기에 문헌 고찰과 함께 보고하는 바이다.
목적: 성인의 단순성 골 낭종에서 스테로이드 주입법의 치료 효과를 알아보고자 한다. 대상 및 방법: 1998년 2월부터 2003년 2월까지 추시 가능한 11예를 대상으로 Needle aspiration 또는 천공술로 병변에 도달하여 낭종액을 배액후 Depo-medrol (methylprednisolone acetate, 신풍제약, Korea)을 1회 최소 80 mg에서 최대 200 mg까지 주입하였고, 추시 방사선 촬영은 투여 후 2주 간격으로 시행하였으며 약물 투여 후 6주에 시행한 단순 방사선 촬영에서 치유의 증거가 보이지 않을 경우, 8주에 추가적으로 스테로이드를 재 투여하였다. 최대 5회까지 시행하였고, 변형된 Neer의 분류법, Pentimalli 분류법, Scaglietti와 Sandra 분류법에 따라 결과를 판정하였다. 결과: 스테로이드의 한번 주입으로 치유된 예는 없었으며, 2회 주입 후 3예, 3회 주입 후 3예, 5회 주입 후 2예에서 치유되었고, 2예에서는 5회 주입 후에도 호전되지 않았으나, 증상의 변화가 없어 현재 관찰중이다. 1예에서는 2회의 스테로이드 주입 후에도 골 낭종의 크기가 증가되어 병적골절의 위험이 있어 수술적 치료를 시행하였다. Pentimalli 등의 단순 방사선 사진의 방사선학적 판정기준에 의해 결과를 평가하면, 1예에서 우수, 7예에서 보통이었고 3예에서 불량이었다. 결론: 스테로이드의 국소적 주입 요법은 성인에서도 효과적이며 일차적 치료법으로 고려할 수 있다.
동맥류성 골낭종(aneurysmal bone cyst)은 비종양성 팽륜성 골 병변으로 정확한 병인은 밝혀지지 않았지만 반응성 현상으로 생각되며, 다른 골 병변과 동반되거나 원발성으로 발생한다. 동맥류성 골낭종은 주로 장골과 척추골에 발생하며, 늑골에서 발생하는 경우는 드물다. 저자들은 좌측 첫번째 늑골에서 발생하였으며, 연부조직 악성 종양으로 오인되었던 동맥류성 골낭종 1예를 경험하였기에 보고하는 바이다. 19세 여자 환자가 좌측 견갑부에서 촉진되는 종괴를 주소로 내원하였다. 방사선학적 검사상, 광범위한 연부조직 병소이며 낭성구조로 이루어진 불균일한 양상이었고, 인접한 좌측 첫번째 늑골의 부분적 골소실이 있어, 연부조직에서 발생한 악성 종양 의진하에 절제술을 시행하였다. 절제된 종괴의 육안 소견상, 종괴 변연부를 달걀 껍질같이 얇은 한층의 골조직이 싸고 있는 것이 관찰되어 늑골 기원의 병변이라는 것을 알 수 있었다. 광학 현미경 검사상, 종괴는 큰 동맥류성 공간으로 이루어져 있었고 혈액으로 차 있었다. 동맥류성 공간사이를 피복세포가 없이 섬유성 격이 나누고 있으면서 여기에는 파골세포와 유사한 다핵 거대세포, 단핵세포, 유골과 섬유점액성조직이 관찰되었다. 병리 소견에 따라 재검한 MRI에서, 늑골에서 종괴의 변연부로 이어지는 얇은 골구조가 관찰되어 늑골에서 발생한 동맥류성 골낭종이 연부조직 쪽으로 광범위하게 돌출해 나온 것임을 알 수 있었다.

This study was to analyze the changes of levels of alkaline phosphatase before and after enucleation of jaw cysts combined with bone grafting, and to evaluate biochemically the effectiveness of the early detection of bone healing and infection as a prognostic marker. Eighteen patients (13 males, 5 females) with cystic lesions of the jaws were divided into two groups. The bone graft group underwent enucleation and bone graft. The control group underwent only enucleation. Both groups were measured levels of ALP before surgery, and plus-minus 4 weeks postoperatively. The more discriminating results were obtained in the bone graft group. The results were as follows : 1. Levels of ALP after enucleation of jaw cysts were decreased in all patients with and without bone graft. 2. The bone graft group showed more marked decrease in variation of levels of ALP than the control group.(p=0.008) This should be considered as a result of increased osteoblastic activity and new bone formation. 3. Such variation could be used as a prognostic marker for bone healing after cyst operation. In the cost/benefit ratio, measurement of ALP activity could be useful as a convenient procedure in routine clinical practice.
Unicameral bone cysts are benign solitary lesions that occur in the first two decades of life and with a greater frequency in the humerus and femur. The cyst is usually asymptomatic unless a fracture or impending fracture is present. The traditional method of treating unicameral cysts has been curettage with bone graft or steroid injection. Also, pathologic fractures in the calcaneus, unlike in other bones, are reported especially in the children to be nonexistent. In general, closed treatment was recommended for most of the calcaneal fractures in children, but suggested open reduction when joint displacement was severe. We report a case of displaced intraarticular fracture of the calcaneus with unicameral bone cyst in a child treated by classic curettage, allograft and open reduction with screw fixation.

Purpose: This study was done to know the usefulness of fractal analysis when evaluating the radiologic changes after decompression on jaw bone cystic lesions using fractal analysis. Materials and methods: 30cases of cystic lesions were followed up after decompression. Panoramic image was used to observe radiologic changes around the cystic lesion. The part of the panoramic image which showed radiologic change was defined as region of interest(ROI); The fractal dimension of the ROI was calculated using box-counting method. Results: Using sign-rank test, there was a statistically significant difference in fractal dimensions after decompression therapy(P<0.0001). The fractal dimensions statistically increased after decompression(the median of D:0.12). Conclusions: The ROI after decompression showed higher fractal dimensions which offer the objective proof of the bone healing around cystic lesions after decompression treatment.
Purpose: Dermoid cysts are benign neoplasms that are derived from both ectoderm and mesoderm. Approximately 7 percent of all dermoid cysts occur in the head and neck, as most common sites are the lateral ends of the eyebrows, the midline in the nasal root and neck. Rarely they can be found in the frontal sinus, temporal bone, maxilla and the floor of the mouth. Dermoid cysts in the temporal fossa are extremely rare. We experienced a characteristic dermoid cyst that occupied the temporal fossa. Methods: A 16-year-old man had a progressive enlarging mass on the left eyebrow. Computerized tomographic scan showed a bulging mass in the temporal fossa, and it had the density similar to that of fat. The size of the mass was $3{\times}3{\times}2cm$, and it was composed of high density of fat with clear margin. There was no bony invasion, but the mass was fixed on bone. Results: We performed the surgery through coronal incision under general anesthesia. Because the mass was closely connected with temporal fat pads, we removed this mass with some portion of temporal fat pads, avoiding damage to the facial nerve. The postoperative course was ordinary without complication. Conclusion: The reports about dermoid cyst on the temporal fossa is uncommon. However, if there is a mass in the temporal fossa which has the density similar to that of fat in CT scan, we should consider the possibility of dermoid cyst. We suggest that excision through coronal incision with bewaring temporal fat pad can induce good result.
We reported a unique case of posttraumatic giant infratentorial extradural intradiploic epidermoid cyst. A 54-year-old male, with a previous history of an open scalp injury and underlying linear skull fracture in the left occipital region in childhood, presented with a painful subcutaneous swelling, which had been developed gradually in the same region and moderate headache, nausea, vomiting and cerebellar ataxia. The duration of symptoms on admission was 3 months. Imaging studies revealed occipital bone destruction and giant extradural intradiploic lesion. The preoperative diagnosis was giant infratentorial extradural intradiploic epidermoid cyst. Surgery achieved total removal of the lesion, which was histologically confirmed and the postoperative course was uneventful. To our knowledge, this is the first case of giant infratentorial extradural intradiploic epidermoid cyst with a traumatic etiology described in the literature.
