Effects of Electroacupuncture at Zusanli (ST36) on Ultrasonographic Gastric Emptying: A Pilot Study in Healthy Adults (족삼리 전침 자극이 초음파로 측정된 위배출능에 미치는 영향 : 건강인 대상 예비연구)

  • Objective: This study aimed to compare the effects of Zusanli and nonacupoint electroacupuncture stimulation on ultrasonographic gastric emptying and vital signs in eight healthy participants. Gastric emptying and its rate of change were analyzed to search for correlation with physical characteristics such as body mass index (BMI), sternocostal angle, and abdominal wall thickness. Methods: Eight healthy participants with no gastrointestinal disorders were enrolled in this study. Each participant went through three abdominal sonographies for gastric emptying assessment. At the second and third visits, participants received Zusanli and nonacupoint electroacupuncture stimulation in a random order. During the study period, we examined the BMI, sternocostal angle, and abdominal wall thickness of all participants. Vital signs (blood pressure, heart rate, and temperature) were also examined before and after the electroacupuncture stimulation. Results: Electroacupuncture stimulation at Zusanli significantly improved gastric emptying when compared to nonacupoint stimulation. Gastric emptying showed a positive correlation with BMI, sternocostal angle, and abdominal wall thickness, but this correlation was statistically insignificant. The improvement rate of gastric emptying by Zusanli electroacupuncture stimulation showed a positive correlation with BMI and sternocostal angle and a negative correlation with abdominal wall thickness. However, such results were also statistically insignificant. Among vital signs, only heart rate showed a significant decrease according to Zusanli electroacupuncture stimulation. Conclusions: A significant effect of Zusanli electroacupuncture was confirmed through ultrasonographic gastric emptying in healthy participants.

The Relationships of Plasma Leptin, Backfat Thickness and TDN Intake across Finishing Stage of Holstein Steers

  • Six 16 months old Holstein steers were offered ad libitum feed for 7 months, to determine the (1) relationships of backfat thickness (BFT) to plasma leptin, and insulin; and (2) associations of TDN intake/kg body weight (BW) to plasma leptin, BFT and insulin. Feed intake, body weight and BFT were measured on selected monthly ages from day 1 to 8, day 1 and 8, and day 8, respectively. Blood was sampled on day 8 and the plasma was analyzed for leptin, insulin, glucose, NEFA, total cholesterol and triglyceride. Body weight and BFT increased, while TDN intake per kg BW decreased from 16 to 23 months old. Plasma leptin increased and mimicked the level of insulin, resulting to significant correlation (r=0.54; p<0.002). TDN intake was negatively related to plasma leptin (r=0.49; p<0.004), insulin (r=0.41; p<0.02) and BFT at 12 to 13th rib (r=0.48; p<0.005). Backfat thickness at 12 to 13th rib was positively related to plasma leptin (r=0.45; p<0.01). Negative associations of TDN intake with plasma leptin and BFT during finishing period suggest long-term involvement of adipose tissues in the feed intake regulation of steers fed high concentrate diet.

Reconstruction of Full Thickness Ala Defect with Nasolabial Fold and Septal Mucosal Hinge Flap

  • Reconstruction of a full-thickness alar defect requires independent blood supplies to the inner and outer surfaces. Because of this, secondary operations are commonly needed for the division of skin flap from its origin. Here, we report a single-stage reconstruction of full-thickness alar defect, which was made possible by the use of a nasolabial island flap and septal mucosal hinge flap. A 49-year-old female had presented with a squamous cell carcinoma of the right ala which was invading through the mucosa. The lesion was excised with a 5-mm free margin through the full-thickness of ala. The lining and cartilage was restored using a septal mucosa hinge flap and a conchal cartilage from the ipsilateral ear. The superficial surface was covered with a nasolabial island flap based on a perforator from the angular artery. The three separate tissue layers were reconstructed as a single subunit, and no secondary operations were necessary. Single-stage reconstruction of the alar subunit was made possible by the use of a nasolabial island flap and septal mucosal hinge flap. Further studies are needed to compare long-term outcomes following single-stage and multi-stage reconstructions.

Short-term changes of human acellular dermal matrix (Megaderm) in a mouse model

  • Background: Physicians tend to overcorrect when applying the acellular dermal matrix for reconstructive option because of volume decrement problem after absorption comparing with initial volume. However, there are no studies on the exact volume decrement and absorption rate with commercial products in South Korea. To figure out absorption rate of acellular dermal matrix product in South Korea (Megaderm), authors designed this experiment. Methods: Nine mice were used and randomly divided into three groups by the time with sacrificing. The implant (Megaderm) was tailored to fit a cuboid form ($1.0cm{\times}1.0cm$ in length and width and 2.0 mm in thickness). A skin incision was made at anterior chest with blade #15 scalpel with exposing the pectoralis major muscle. As hydrated Megaderm was located upon the pectoralis major muscle, the skin was sutured with Ethilon #5-0. After the surgical procedure, each animal group was sacrificed at 4, 8, and 12 weeks, respectively, for biopsies and histological analysis of the implants. All samples were stained with routine hematoxylin and eosin staining and Masson's trichrome staining and the thickness were measured. A measurements were analyzed using Friedman test. Statistically, the correlation between thicknesses of Megaderm before and after implantation was analyzed. Results: After sacrificing the animal groups at postoperative 4, 8, 12 weeks, the mean tissue thickness values were $2.10{\pm}1.03mm$, $2.17{\pm}0.21mm$, and $2.40{\pm}0.20mm$ (p= 0.368), respectively. The remaining ratios after absorption comparing with after initial hydrated Megaderm were 82.7%, 85.4%, and 94.5%, respectively. In histopathological findings, neovascularization and density of collagenous fiber was increased with time. Conclusion: Author's hypothesis was absorption rate of implant would be increased over time. But in this experiment, there is no statistical significance between mean absorption thickness of implant and the time (p= 0.368). Also it can be affected by graft site, blood supply, and animals that were used in the experiment.

Understanding on MR Perfusion Imaging Using First Pass Technique in Moyamoya Diseases (Moyamoya 질환에서 1차 통과기법을 이용한 자기공명관류영상의 이해)

  • The purpose of this study was to investigated the usefulness of MR perfusion image comparing with SPECT image. A total of pediatric 30 patients(average age : 7.8) with Moyamoya disease were performed MR Perfusion with 32 channel body coil at 3T from March 01, 2010 to June 10, 2010. The MRI sequences and parameters were as followed : gradient Echo-planar imaging(EPI), TR/TE : 2000ms/50ms, FA : $90^{\circ}$, FOV : $240{\times}240$, Matrix : $128{\times}128$, Thickness : 5mm, Gap : 1.5mm. Images were obtained contrast agent administrated at a rate of 1mL/sec after scan start 10s with a total of slice 1000 images(50 phase/1 slice). It was measured with visual color image and digitize data using MRDx software(IDL version 6.2) and also, it was compared of measurement with values of normal and abnormal ratio to analyze hemodynamic change, and a comparison between perfusion MR with technique using Warm Color at SPECT examination. On MR perfusion examination, the color images from abnormal region to the red collar with rCBV(relative cerebral blood volume) and rCBF(relative cerebral blood flow) caused by increase cerebral blood flow with brain vascular occlusion in surrounding collateral circulation advancement, the blood speed relatively was depicted slowly with blue in MTT(Mean Transit Time) and TTP(Time to Peak) images. The region which was visible abnormally from MR perfusion examination visually were detected as comparison with the same SPECT examination region, would be able to confirm the identical results in MMD(Moyamoya disease)judgments. Hymo-dynamic change in MR perfusion examination produced by increase and delay cerebral blood flow. This change with digitize data and being color imaging makes enable to distinguish between normal and abnormal area. Relatively, MR perfusion examination compared with SPECT examination could bring an excellent image with spatial resolution without radiation expose.

Computation of Nonlinear Elastic Strains Occurring in the Leaflet of the Edwards MIRA Mechanical Heart Valve by the Applied High Blood Pressure (혈압에 의해 Edwards MIRA 기계식인공심장판막에 발생하는 비선형 탄성변형률의 계산)

  • This paper presents a computation of nonlinear elastic strains that may occur in the leaflet of the Edwards MIRA mechanical heart valve by the applied high blood pressure using the finite element analysis methodology. By adopting numerical analysis techniques of the commercial finite element analysis code, NISA, structural analyses of the Edwards MIRA mechanical heart valve are performed for the slight variation of leaflet thickness to get the elastic strains occurring in the leaflet while the high blood fluid pressures are applied to the leaflet surface in order that the maximum stress occurring in the leaflet may be less than the yield stress of the leaflet material(Si-Alloyed PyC). And so, only the geometric non-linearity is assumed because large geometric nonlinear elastic strains are expected rather than material nonlinear strains due to the applied high blood pressure. Computed linear and nonlinear elastic strains are compared to make sure the non-linearity of the computed elastic strain. The comparison result shows that large elastic strains occur clearly in the very thin leaflets as high blood pressures are applied. However, only the linear elastic strains occur for low blood pressures, and also for thick leaflets even for the high blood pressures. Hence the nonlinear structural analysis is very required in the structural design of a mechanical heart valve.

Experimental Assessment of Hemostatic Agents: Comparison with New Developed Chitosan-Based Material (신개발 키토산 제재의 지혈 효과에 대한 비교)

  • Purpose: Many hemostatic agents and dressings have been tested with variable degree of success. Chitosan has a positive charge, it attracts red blood cells, which have a negative charge. Our goal is to test the efficacy of new developed chitosan-based hemostatic materials in providing durable hemostasis in a high-flow arterial wound model. Methods: We compared each group with SD rats motality tests and in vitro blood compatibility test by blood clotting index (BCI). We devided the SD rats into 6 groups (N =15) by type of hemostatic agents. A: 100% nonwoven chitosan (degree of the deacetylation: 90%). B: 50% N-acetylation on nonwoven of chitosan gel (degree of the deacetylation: 50%). C: 60% N-acetylation on nonwoven of chitosan ge (degree of the deacetylation: 40%)l. D: Cutanplast$^{(R)}$. E: HemCon$^{(R)}$ F: Gauze. In vivo test, a proximal arterial injury was created in unilateral femoral arteries of 90 anesthetized SD rats. Each materials was made same size and thickness then applied to the injury site for 3 minutes. In vitro test, we compared each group with BCI in human blood. Results: In vivo test, group A showed lower motality rate of 46% than any other groups, Group B and C showed lower motality rate of 60% than group D and E's motality rate of 66%. In vitro test, BCI of group A ($30.6{\pm}1.2$) and B ($29.3{\pm}1.0$) were showed nearly about group D ($29.1{\pm}1.8$) and E ($27.4{\pm}1.6$). Group C ($37.1{\pm}2.0$) showed higher BCI than group A and B, it means group C decreased blood clotting. Conclusion: In conclusion, this study suggests a newly developed chitosan-based hemostatic materials induced durable hemostasis and increased blood clotting, and are considered as effective biologic hemostatic agents.

Relationship between Carotid Intima Media Thickness and Appendicular Skeletal Muscle Index according to Gender (성별에 따른 경동맥 내중막두께와 사지근육량지수와의 관련성)

  • It is reported that social distancing caused by the corona pandemic has resulted in a decrease in physical activity, and a decrease in muscle mass due to this increases the risk of vascular disease. Therefore, in this study, the risk of carotid intima media thickness was evaluated by measuring the thickness of CIMT, and the differences in variables according to gender were analyzed. From January 2022 to May 2022, a total of 220 people (121 males, 99 females) who performed blood tests, carotid ultrasound, and body composition analysis among those who visited the health check-up center of Busan H Hospital were included in this study. Significant differences in risk factors according to gender were analyzed using the chi-square test. The cut-off values of variables that can predict the risk of carotid intima media thickness were calculated, and the accuracy was evaluated by calculating the area under the curve, sensitivity, and specificity. As a result of the difference analysis, the higher the total cholesterol in men and the appendicular skeletal muscle index in women, the higher the risk of carotid intima media thickness. The cut-off value for predicting the risk of carotid intima media thickness was calculated to be greater than 199 mg/dL of total cholesterol in the male group and less than 5.9 kg/m2 of the appendicular skeletal muscle index in the female group. As a result of this study, the higher the total cholesterol in men and the lower the appendicular skeletal muscle index in women, the higher the risk of increased CIMT. Therefore, in women, the appendicular skeletal muscle index is expected to serve as an indicator to predict and prevent vascular changes at an early stage.

The Effects of the Bojungikgi-tang added Magnoliae Flos on the Pulmonary Thromboembolism and Delayed Allergy Response (보중익기탕(補中益氣湯) 가(加) 신이(辛夷)가 폐혈전색전(肺血栓寒栓) 및 지연형(遲延型) allergy반응(反應)에 미치는 영향(影響))

  • Objective : This experimental studies were done to research the effects of Bojungikgi-tang added Magnoliae Flos on the pulmonary thromboembolism and delayed allergy response. Methods : Healthy rats were devided into each group. This test is to inject sodium arachidonic acid to one group, and ADP(adenocine diphophate) to the other one afterward to compare the survival rate between each other. Delayed allergy response test is to inject PC(picryl chloride) to one group and measure the thickness of their ears, to inject SRBC(sheep red blood cell) allergen to the other, and measure their foot edema, and compare the each other rates Results : The extract of the Bojungikgi-tang added Magnoliae Flos revealed significant effect on the pulmonary thromboembolism induced by sodium arachidonate, or adenocine diphophate(ADP). And the extract of the Bojungikgi-tang added Magnoliae Flos revealed significant effect on delayed type hypersensitivity induced by picryl chloride(PC), or sheep red blood cell(SRBC). Conclusions : According to the above findings it is suggested that the extract of the Bojungikgi-tang added Magnoliae Flos revealed significant effect on the delayed type hypersensitivity and effect on the pulmonary thromboembolism.

Quantitative analysis of hemothorax by computed tomography (흉부 전산화 단층촬영을 이용한 혈흉의 정량분석)

  • Computed tomography[CT is an effective technique for the evaluation of the thorax following blunt trauma. To evaluate multiply injured 30 patients who were diagnosed as hemothorax in emergency room, computed tomography of thorax was done. The thickness of slice was one centimeter and the entire pleural cavity from the apex to the costophrenic angle was included in the evaluation. Integration and addition of the hemothorax area for each CT slice was made and amount of blood in the pleural cavity was estimated. The slice which showed largest area of hemothorax was selected and the height and width of the hemothorax area were measured. The number of slices which showed radiographic evidence of hemothorax was counted. Regression analysis was done and measured amount of hemothorax, the height and width of the hemothorax area for each slice and number of slices were put as variables. And following equation was derived. V=108.3A-0.8B-7.4C+84.7 [R2=0.74 [ V: amount of hemothorax, A: height, B: width, C: number of slices Total amount of blood from thoracic drainage was compared to the measured amount by computed tomography and the relation between the two values was statistically significant.[p=0.001 In conclusion, quantitative estimation of size of hemothorax was possible by the above equation and the process was very helpful for determination policy of treatment of individual patient.
