응급실에서의 채혈로 인한 용혈 및 재채혈 관련요인 (Factors Affecting Hemolysis in Blood Sampling and Repeated Sampling in the Emergency Department)

  • Purpose: This study was conducted to investigate the incidence of blood hemolysis and repeated blood sampling and to identify factors contributing to hemolysis and repeated blood sampling in the emergency department. Methods: A cross-sectional descriptive design was used. Participants were the patients who came to emergency department and are required a blood sampling for electrolyte level. All blood samples were collected by emergency department nurses and determined for hemolysis by experienced laboratory technologists. Data were analyzed using $x^2$-test, Fisher's exact test, Mann-Whitney u test and Binary Logistic Regression to determine significant differences. Results: A total of 402 valid samples were collected. Of these, 30 blood samples (7.5%) were found to be hemolyzed and 9 (2.2%) to be recollected. Statistically significant factors affecting on hemolysis and repeated blood sampling included the time of bloods sampling (night), the time of tourniquet application, and too-fast blood draw into the test tube. Conclusion: We recommend that nurses who take the blood sampling to consider the findings of the study and take the related factors into account as they set up the standardized care protocol in order for nursing quality improvement.

An Examination of Blood Lead Levels in Thai Nielloware Workers

  • Objectives: The objectives of this study were to determine the lead levels in blood samples from nielloware workers, to determine airborne lead levels, to describe the workers' hygiene behaviors, and to ascertain and describe any correlations between lead levels in blood samples and lead levels in airborne samples. Methods: Blood samples and airborne samples from 45 nielloware workers were collected from nielloware workplaces in Nakhon Sri Thammarat Province, Thailand. Lead levels were determined by flame atomic absorption spectrometry (FAAS), at a wavelength of 283.3 nm. FAAS was used especially adequate for metals at relatively high concentration levels. Results: The geometric mean of the 45 airborne lead levels was 81.14 ${\mu}g/m^3$ (range 9.0-677.2 ${\mu}g/m^3$). The geometric mean blood lead level of the 45 workers was 16.25 ${\mu}g/dL$ (range 4.59-39.33 ${\mu}g/dL$). No worker had a blood lead level > 60 ${\mu}g/dL$. A statistically significantly positive correlation was found between airborne lead level and blood lead levels (r = 0.747, p < 0.01). It was observed that personal hygiene was poor; workers smoked and did not wash their hands before drinking or eating. It was concluded that these behaviors had a significant correlation with blood lead levels (p < 0.001). Conclusion: Improvements in working conditions and occupational health education are required due to the correlation found between blood leads and airborne lead levels.

음주운전자 275명 혈액 중 마약류 및 남용약물의 분석 (Distribution of Abused Drugs in 275 Alcohol-positive Blood Samples of Korean Driver)

  • Even though driving under the influence of drug (DUID) is a worldwide problem, we, Korea has no regulation system yet except for alcohol, and there are little cases reported related to DUID. In order to investigate the type of abused drugs for drivers in Korea, we tried to analyze controlled and non-controlled drugs in alcohol-positive blood samples. 275 whole bloods, which were positive for alcohol on the roadside test, were collected from the police for two months ($Nov.{\sim}Dec.$ 2006). The analytical strategy was constituted of three steps: First, alcohol in blood samples were confirmed and quantified by gas chromatography. Second, controlled drugs were screened by $Evidence_{investigator}\;^{TM}$ (Randox, U.K.) as preliminary test. It was based on immunoassay by biochip array analyzer. Nine groups of drug abuse were screened: amphetamines, methamphetamines, cannabis, cocaine, opiates, barbiturates, methadone, benzodiazepines I (oxazepam) & II (lorazepam). Finally, confirmation of these drugs was performed by GC-MS. Blood samples were extracted by solid-phase extraction by $RapidTrace^{TM}$ (Zymark, U.S.A.). After trimethylsilyl (TMS) derivatization, eluates were analyzed to GC-MS. Total 49 drugs were investigated in this study including controlled drugs, antidepressants, 1st generation antihistamines, dextromethorphan, nalbuphine, ketamine, etc. For rapid detection, we developed the automated identification system. It was made up a new software, "DrugMan", modified Chemstation data analysis menu and newly developed macro modules. A series of peak selection, identification and reporting of the results were performed automatically by this system. Concentrations of alcohol in 275 blood samples were ranged from 0.011 to 0.249% (average, 0.119%). Among 149 blood samples, just six samples (4.0%) were showed positive results to the immunoassay: one methamphetamine and five benzodiazepines group I. By GC-MS confirmation, only benzodiazepines were detected and methamphetamine was not detected from immunoassay positive blood sample. Besides these drugs, 5 chlorpheniramines, dextromethorphan, diazepam, doxylamine, ibuprofen, lidocaine and topiramate were also detected in whole bloods by GC-MS. Conclusively, the frequency of drug abuse for Korean drivers was relatively low. There was none case which illegal drug was detected. However these results were limited to alcohol positive blood samples, so it is necessary to analyze more samples including alcohol negative blood.

적혈구 농축제재에서 HBV DNA의 노출정도 (Prevalence of HBV DNA in Packed Red Blood Cells)

  • 수혈 후 B형 간염 바이러스의 전파를 감소시키기 위한 방법으로 현재 적십자 혈액원에서 공급되는 적혈구 농축제재를 대상으로 HBV DNA, HBsAg, anti-HBs 및 anti-HBc를 측정 하여 수혈자에서 HBV DNA의 노출정도와 HBV DNA와 B형 바이러스 간염의 혈청학적 표지자와의 관련성을 조사하여 다음과 같은 결과를 얻었다. 대구 적십자 혈액원에서 공급되는 적혈구 농축제재에서는 HBsAg이 검출되지 않았으며, HBV DNA는 0.6%에서 검출되었는데, 모두 anti-HBc만 양성인 경우였다. Anti-HBc양성률은 37%였으며, 이들중 65.8%에서 anti-HBs도 양성으로 나타나 anti-HBc만 양성인 경우는 공혈혈액의 13.0% 정도로 추정되었다. 따라서, 우리나라와 같이 anti-HBc양성률이 높은 지역에서는 anti-HBs를 추가하는 경우 13%의 혈액제제만을 폐기함으로써 HBV DNA의 전파를 차단시킬수 있으리라 생각되며 현재 실시하고 있는 검사보다 예민도와 특이도가 높은 선별검사의 개발이 필요한 것으로 생각된다.

HPLC 형광검출법에 의한 Glyphosate의 혈중농도 측정 (Determination of Glyphosate in Whole Blood by HPLC-fluorescence Detection)

  • A rapid and sensitive method for the determination of glyphosate, a phosphated amino acid herbicide, in whole blood is presented. After removal of protein, the whale blood was purified by using the anion exchange resin (Dowex 1), and derivatized with 9-fluorenylmethyl chloroformate (FMCL). Derivatized glyphosate from blood sample was injected onto a Whatman partisil 10SAX column and separated with 0.1M phosphate buffer (pH 2.5) and acetonitrile (ratio=3:1). The high performance liquid chromatography-fluorescence detection gave the detection limit of 86pg and linearity of 0.9999 in the range of 0.25 $\mu$g/ml and 25 $\mu$g/ml. The recoveries of glyphosate added to the blood samples were ranged from 75.3% to 100.4% compared to the samples prepared in water. The derivatized glyphosate was stable at various acidity and temperature. This method has been successfully applied to the blood samples of lethal intoxication with the herbicide glyphosate.

돼지 써코바이러스 2형 유전형 분류를 위한 nested-PCR 적용 (Application of a nested-polymerase chain reaction assay to differentiate the genotypes of porcine circovirus 2)

  • The purpose of this study was to apply a nested-polymerase chain reaction (nPCR) assay to detect and differentiate PCV 2a and PCV 2b. The compared with nPCR and one-step PCR and nPCR showed more sensitive in the detection of PCV-2 from tissue and blood samples. The total of 52 tissue samples was collected from postweanning pigs from 2006 to 2010. All tissue samples showed positive for PCV-2 in one-step PCR and nPCR, followed by the nPCR in order to identify the genotypes of PCV-2. 2 samples (3.8%) showed positive for PCV 2a, and 35 samples were positive for PCV 2b (67.3%), 15 samples (28.9%) were positive the dual genotypes. In addition, 42 blood samples which were collected from the 5 different swine farms were compared figure out the detection rates of nPCR and one-step PCR. The PCV 2 was positive by one-step PCR in 21 samples (50.0%) and nPCR was positive in 37 samples (88.1%). The PCV 2 genotypes in blood samples and 32 samples (76.2%) were positive for PCV 2b and none were positive for PCV 2a, 5 samples (11.9%) were positive for dual genotypes. These results suggest that the nPCR is very efficient for genotyping blood samples and differentiating the genotypes of PCV-2 from field samples.

IN-VITRO CHARACTERIZATION OF THE THROMBOTIC POTENTIAL OF WHOLE BLOOD USING AN IMPEDANCE METHOD

  • This study presents an impedance method of in-vitro characterization of the thrombotic potential of whole blood. Whole blood samples of 0.2 cc were put into a micro-cell with embedded three electrodes immediately after venepuncture at $37^{\circ}C$. Anti-coagulated blood samples were also collected for hematocrit and blood viscosity analyses. The rate of change of electron flow was measured, which indicates the inverse of the thrombotic potential. A sudden decrease in the rate of change of electron flow was observed at a time equal to approximately 110 seconds. This sudden decrease was significantly delayed in anti-coagulated samples. After the sudden decrease, the rate continued to decrease, reaching a minimum value in unadulterated samples while the change in the rate in the anti-coagulated ones was found rather moderate. Based on these preliminary findings, the present method may be of used as a new tool for the diagnosis of the thrombotic potential of whole blood.

혈액중 포도당과 젖산의 분석을 위한 광섬유 생물센서 (Fiber-optic biosensor for analysis of glucose and lactate in blood samples)

  • Optical-fiber sensors have been developed to determine the concentrations of glucose and lactic acid in blood samples. Fluorescence dye [tris(2,2'-biphenyridine)-ruthenium(II)-chloride (RuBPY)] was entrapped by using a silicon to the unclad tip of a glass optic fiber. Enzymes like glucose oxidase (GOD) and lactate oxidase (LOD) have been immobilized by acrylamide resin adhesive, adsorption with zeolite or covalent bonding with aminopropyl-triethoxysilan. The fiber-optic glucose/lactate sensor was then used to analyze the concentrations of glucose and lactate in blood samples. The results were compared with the results of HPLC analysis and their difference was in error by less then 5 %.

운동선수들의 혈액분석을 통한 Etrythropoietin 간접도핑검사 (Blood Analysis for Indirect Doping Control of Erythropoietin in Sports)

  • The use of recombinant human erythropoietin (rhEPO), a stimulator of erythropoiesis, banned in sports because of the medical risk associated with thrombosis. Due to analytical difficulties to differentiate between natural human EPO (hEPO) and rhEPO, blood parameters of erythropoiesis such as contents of hemoglobin (cut-off value <17.5 g/d l for man, and < 16.0 g/dl for women), hematocrit and reticulocytes (cut-off value <2.0%) were measured to focus the misuse of rhEPO. We conducted anti-doping test for 122 blood samples of the World Cup athletes. The mean values of key parameters are as follows; 14.5$\pm$1.0 g/dl for hemoglobin, 41.7$\pm$2.8% for hematocrit, and 1.3$\pm$0.4% for reticulocyte. Blood sample was found to be stable up to 8 hours for the reticulocyte measurement. In addition, the soluble transferrin receptor and ferritin levels were measured by immunoassay methods using plasma samples (n=28) in which the mean value was 0.8$\pm$0.5 $\mu\textrm{g}$/$m\ell$ and 54.6$\pm$33.7 ng/$m\ell$, respectively. The results indicate that all samples tested were negative for the blood parameters of indirect anti-doping test for hEPO misuse. The statistical evaluation suggest that several other parameters such as red blood cell, mean corpuscular hemoglobin concentration, mean corpuscular volume, mean corpuscular hemoglobin and white blood cell could be considered as factors influencing hEPO function in addition to five parameters mentioned.

The Effect of ABO Blood Groups, Hemoglobinopathy, and Heme Oxygenase-1 Polymorphisms on Malaria Susceptibility and Severity

  • Malaria is one of the most important public health problems in tropical areas on the globe. Several factors are associated with susceptibility to malaria and disease severity, including innate immunity such as blood group, hemoglobinopathy, and heme oxygenase-1 (HO-1) polymorphisms. This study was carried out to investigate association among ABO blood group, thalassemia types and HO-1 polymorphisms in malaria. The malarial blood samples were collected from patients along the Thai-Myanmar border. Determination of ABO blood group, thalassemia variants, and HO-1 polymorphisms were performed using agglutination test, low pressure liquid chromatography and polymerase chain reaction, respectively. Plasmodium vivax was the major infected malaria species in the study samples. Distribution of ABO blood type in the malaria-infected samples was similar to that in healthy subjects, of which blood type O being most prevalent. Association between blood group A and decreased risk of severe malaria was significant. Six thalassemia types (30%) were detected, i.e., hemoglobin E (HbE), ${\beta}$-thalassemia, ${\alpha}$-thalassemia 1, ${\alpha}$-thalassemia 2, HbE with ${\alpha}$-thalassemia 2, and ${\beta}$-thalassemia with ${\alpha}$-thalassemia 2. Malaria infected samples without thalassemia showed significantly higher risk to severe malaria. The prevalence of HO-1 polymorphisms, S/S, S/L and L/L were 25, 62, and 13%, respectively. Further study with larger sample size is required to confirm the impact of these 3 host genetic factors in malaria patients.
