진구와 "오독도기"근(根)의 약효(藥效) 비교연구(比較硏究) (Comparison of Pharmacological Effects of Gentianae macrophyllae Radix and that of Lycoctoni Radix)

  • It has been known that Gentianae macrophyllae Radix is reputed to be effective in removal of 'Pungseub' and 'Heoyeol', and to be analgesic. Since ancient times, however, there have been many examples of substituting Lycoctoni Radix for Gentianae macrophyllae Radix. For that reason, this experiment was conducted to compare the diuretic, analgesic, anti-inflammatory, anti-pyretic, anti-bacterial effect, and toxicity of Lycoctoni Radix with those of Gentianae macrophyllae Radix. The results obtained in this work were summarized as follows; In the test of acute toxicity, Lycoctoni Radix was found to be toxic in comparison to Gentianae macrophyllae Radix which was found to be non-toxic. Both herbs were found to be diuretic and Gentianae macrophyllae Radix was more effective than the other and to be ineffective for the blood pressure and respiration. Lycoctoni Radix was found to be more analgesic than the other. Gentianae macrophyllae Radix was found to be more anti-inflammatory of the two and both demonstrated anti-pyretic effect. Anti-bacterial effect was detected only in Lycoctoni Radix. It is concluded that Gentianae macrophyllae Radix has mild diuretic, analgesic, anti-inflammatory and anti-pyretic effect, but Lycoctoni Radix, due to its toxicity, can not be substituted for Gentianae macrophyllae Radix.

좌심방 점액종 2례 보고 (Left atrial myxoma: 2 cases report)

  • The primary cardiac tumor was considered as a form of disease with poor prognosis in the past and it was hardly diagnosed before post mortem autopsy. But recent development of diagnostic procedure and the cardiac surgery using extracorporeal circulation increased the accuracy of diagnosis and the opportunity of successful treatment. The authors present two cases of left atrial myxoma which experienced during recent 4 years between 1979 and 1982. A 33 year old woman admitted with severe shortness of breath, generalized edema, ascites and the evidence of hepatopathy resulted from right sided heart failure. Preoperatively, the patient was treated with conservative medication to improve general condition for a few days. The tumor mass was removed successfully under the cardiopulmonary bypass. She, however, died of myocardial dysfunction showing low blood pressure. The tumor mass weighed 22gm and measured 5.2x4.5x3.6 cm in size. A 60 year old man admitted with shortness of breath on exertion and an episode of fainting. Following the removal of tumor mass under the cardiovascular bypass, the clinical course was satisfactory with no complication. The tumor mass weighed 105gm and measured 9x4x5 cm in size.

  • Ludwig's angina has been defined as a potentially lethal, rapidly spreading cellulitis, involving the sublingual and submandibular spaces, and is manifesed by a brawny, suprahyoid induration, tender swelling in the floor of mouth, elevation and posterior displacement of tongue. This paper is of interest not only because of severity of infection but also because of associated diabetic mellitus. Diabetes mellitus is a complex syndrome of disordered metabolism and elevated blood glucose, it results from deficiency of insulin secretion of combination of insulin resistance and inadequate insulin secretion. The effects of diabetic mellitus include neuropathy, vascular insufficiency, decreased leukocytic function, hematologic change etc. Clinically this may be refelected by the increased severity of infections seen in diabetics. The treatment of infections in diabetics are reduction of number of microbes through the use of appropriated antimicrobial agents and proper surgical drainage and improvement of the host factors by tight control of insulin replacement and immediate intervention to correct abnormalities of the local factors by drainage, debriment, and removal of avoidance of foreign bodies. The authors present the report of the Ludwig's angina in patient with diabetic mellitus, with literature review and good clinical result.

한국인(韓國人)에 있어서의 Sulfamethizol의 Acetyl화(化)에 관한 생물약제학적(生物藥劑學的) 연구(硏究) (Studies on Biopharmaceutics for Acetylation of Sulfamethizole to Korean)

  • In the course of the investigations on drug metabolism and drug disposition in man, it has been noted that there are often rather striking individual differences in the response to drugs and in the ability to metabolize and dispose of drugs. Some of the person-to-person differences are due to environmental factors, but some have clearly demonstrated to be genetically determined. This report determined the rate of acetylation and some constants of sulfa-methizole to korean. The results are as follows: The overall removal rate of drug from the body is 0.572(k, $hr^{-1}$) the rate constant for acetylation, 0.222($k_2$, $hr^{-1}$) the rate constant for excretion of unchanged drug, 0.350($k_3$, $hr^{-1}$) and the faction of a dose ultimately excreted in the urine as unchanged drug is 0.677. And this report determined the optimal time to make a single blood level determination after an oral dose of the drug and obtain a reasonable index of the rate of sulfamethizole metabolism. The resulting data(sulfamethizole levels at 5 hours after drug administration) clearly established bimodality even though only about 106 subjects were tested.

A Proteome Reference Map for Porcine Plasma Proteins

  • To profile the proteome in porcine plasma, blood samples were collected from adult male barrows and those plasma were retrieved. For the depletion or pre-fractionation of high-abundance proteins, plasma samples were treated with commercial kits. Then, protein profiling was initiated using one and two-dimensional electrophoresis. Proteins were spotted and then identified by MALDI-TOF-TOF and LC-MS-MS. In the results, more than forty six proteins were identified and the reference map was constructed. The pre-treatment for the removal of high-abundance proteins caused the changes in 2-DE images and some of the proteins were newly uncovered after the most of high abundant proteins were removed. However, it is expected for further steps necessary to identify more low-abundance proteins that may contain potential bio-markers.

Coordinate Expression of Renin and Cyclooxygenase-2 in Rats with Two-kidney, One Clip and Deoxycorticosterone Acetate-Salt Hypertension

  • The present study was aimed to examine whether the expression of renin is associated with that of cyclooxygenase-2 (COX-2) in the kidney. Male Sprague-Dawley rats were made two-kidney, one clip (2K1C) or deoxycorticosterone acetate (DOCA)-salt hypertensive, to stimulate or to inhibit the endogenous renin-angiotensin system, respectively. The expression of renin and COX-2 mRNA was determined in the cortex of the kidney by reverse transcription-polymerase chain reaction. 2K1C hypertensive rats showed an increased expression of renin as well as of COX-2 in the clipped kidney. The expression of renin was decreased in parallel with that of COX-2 in the contralateral non-clipped kidney. Removal of the renal arterial clip reversed the expression of both genes, along with the blood pressure, to the control level. On the other hand, DOCA-salt hypertension was associated with parallel decreases of renin and COX-2 expression. These results indicate that renin and COX-2 genes are coordinately expressed in the kidney.

  • This study was performed to evaluate the new surgical technique for debarking. Of the 8 mongrel dogs which underwent devocalization, 4 heads (group 1) received complete surgical removal of the vocal cords and another 4 heads (group 2) received fixation of vocal cords and vocal processes onto the thyroid cartilage. Volume of each dogs' voices was measured by a sound level meter(Function A) over 7 months at intervals of 10 days. All experimental animals'hematological values were measured before operation and postoperative at 2i 5 and 10 days. The changes of volume of dogs'voices and hematological values were analyzed by paired t-test. The volume in the dogs'voices in each group after operation was significantly lower than that before operation. At this results we detected that devocalizing effect was the same in each group. Postoperative volume of dogs'voices in group 1 had a tendency to increaser but group 2 had a tendency to decrease. The number of erythrocytes at 2 days after operation was significantly fewer than those before operation in group 1, but the group 2 did not have significant changes. The number of leukocytes at 2 days after operation significantly increased from those before operation in group 11 but the group 2 did not have significant changes. It was concluded that the new surgical technique, fixation of vocal cords and vocal processes onto the thyroid cartilages could be available for the devocalization of dogs and for the prevention of postoperative inflammation and blood loss.

  • Urine production is vital for the removal of certain waste products produced by metabolism in the body and for the maintenance of homeostasis in the body. The kidneys produce urine by the following three precisely regulated processes: filtration, reabsorption, and secretion. Urine is composed of water, certain electrolytes, and various waste products that are filtered out of the blood through the glomeruli. The physical features of urine are evaluated carefully to detect any abnormal findings that may indicate underlying diseases in the genitourinary system. A change in urine color may indicate an underlying pathological condition, although many of the causes of abnormal urine color are benign effects of medications and foods. A characteristic and specific odor may be the result of a metabolic disease rather than a concentrated specimen or a simple urinary tract infection. Although transient changes in urine output and nocturia are usually benign conditions, persistent abnormal findings require further workup, with a thorough medical history taking. This article presents many of the conditions that physicians may encounter and will help them in the diagnosis and in establishing a treatment plan.

Preliminary Study of Gender-Based Brain Lateralization Using Multi-Channel Near-Infrared Spectroscopy

  • It has been thought that males tend to use their brain hemispheres more laterally than females. However, recent fMRI studies have shown that there may be no difference in brain lateralization between genders. Functional near-infrared spectroscopy (fNIRS) presents a unique opportunity to acquire real time measurements of blood oxygenation changes to observe neural activity specific to the brain's left and right hemispheres. Using an in-house built multichannel fNIRS system, brain lateralization was observed from seven males and four females according to specially designed tasks for left and right hemisphere activation. The Pearson correlation coefficient and a modified Lateralization Index metric for continuous wave fNIRS systems were calculated to quantify brain lateralization. The preliminary results point to no significant difference in lateral hemodynamic changes between the genders. However, the correlation of symmetrical channel pairs decreased as the experiments progressed. To further develop this study, the subject's performance and the removal of global interference must be implemented for an improved study of brain lateralization.

Successful management of absent sternum in an infant using porcine acellular dermal matrix

  • Congenital absent sternum is a rare birth defect that requires early intervention for optimal long-term outcomes. Descriptions of the repair of absent sternum are limited to case reports, and no preferred method for management has been described. Herein, we describe the use of porcine acellular dermal matrix to reconstruct the sternum of an infant with sternal infection following attempted repair using synthetic mesh. The patient was a full-term male with trisomy 21, agenesis of corpus callosum, ventricular septal defect, patent ductus arteriosus, right-sided aortic arch, and congenital absence of sternum with no sternal bars. Following removal of the infected synthetic mesh, negative pressure wound therapy with instillation was used to manage the open wound and provide direct antibiotic therapy. When blood C-reactive protein levels declined to ${\leq}2mg/L$, the sternum was reconstructed using porcine acellular dermal matrix. At 21 months postoperative, the patient demonstrated no respiratory issues. Physical examination and computed tomography imaging identified good approximation of the clavicular heads and sternal cleft and forward curvature of the ribs. This case illustrates the benefits of negative pressure wound therapy and acellular dermal matrix for the reconstruction of absent sternum in the context of infected sternal surgical site previously repaired with synthetic mesh.
