Cardiopulmonary resuscitation (CPR) is performed by thoracic compression and artificial ventilation for the patient under emergent situation to maintain at least the minimum level of respiration and blood circulation for life survival. Good quality CPR requires monitoring respiration, however, traditional respiratory air flow transducers cannot be used because the transducer elements are facing the whole area perpendicular to the flow axis. The present study developed a new air flow transducer conveniently applicable to CPR. Specially designed "sensing rod" samples the air velocity at 3 different locations of the flow cross-section, then transforms into average dynamic pressure by the Bernoulli's law. The symmetric structure of the sensing holes of the sensing rod enables bi-directional measurement simply by taking the difference in pressure by a commercial differential pressure transducer. Both inspiratory and expiratory flows were obtained with symmetric measurement characteristics. Quadratic curve fitting provided excellent calibration formula with a correlation coefficient>0.999 (P<0.0001) and the mean relative error<1%. The present results can be usefully applied to accurately monitor the air flow rate during CPR.
Objectives : The management of massive brain swelling remains an unsolved problem in neurosurgical field. Despite newly developed medical and pharmacological therapy, the mortality and morbidity due to massive brain swelling remains high. According to many recent reports, surgical decompression with dura expansion is superior to medical management in patients with massive brain swelling. We performed surgical treatment on the first line of treatment, and followed medical management in case with refractory increased intracranial pressure(ICP). To show the quantitative effect of decompressive surgery on the intracranial pressure, we performed ventricular puncture and checked the ventricular ICP continuously during the decompressive surgery and postoperative period. Materials and Methods : Fifty-one patients with massive brain swelling, undergoing bilateral decompressive craniectomy with dura expansion, were studied in this study. In all patients, ventricular puncture was performed at Kocher's point on the opposite side of massive brain swelling. The ventricular pressure was monitored continuously, during the bilateral decompression procedures and postoperative period. Results : The initial ventricular ICP were varied from 13mmHg to 112mmHg. Immediately after the bilateral craniectomy, mean ventricular ICP decreased to $53.1{\pm}15.8%$ of the initial ICP(ranges from 5mmHg to 87mmHg). Dura opening decreased mean ICP by additional 36.7% and made the ventricular pressure $16.4{\pm}10.5%$ of the initial pressure (ranges from 0mmHg to 28mmHg). Postoperatively, ventricular pressure was lowered to $20.2{\pm}22.6%$(ranged from 0mmHg to 62.3mmHg) of the initial ICP. The ventricular ICP value during the first 24 hours after decompressive surgery was found to be an important prognostic factor. If ICP was over 35mmHg, the mortality was 100% instead of additional medical(barbiturate coma therapy and hypothermia) treatments. Conclusion : Bilateral decompression with dura expansion is considered an effective therapeutic modality in ICP control. To obtain favorable clinical outcome in patients with massive brain swelling, early decision making on surgical management and proper patient selection are mandatory.
According to the recent trends in the growing elderly population, the chronically ill have increased. Thus the importance of the health care issues for them has emerged. In this paper, we want to implement a chronic disease management system using smart mobile devices. Proposed chronic disease management system is consisted of the biometric sensor, smart mobile devices, the patient management server, patient management DB, and patient symptoms analysis agent. The biometric sensor detects a biological information. Smart mobile devices receive the patient information from the sensor and transmit the information to the patient management server. The patient management server, patient management DB, and patient symptoms agent analysis agent analyze to process data delivered through a wireless communication network. Bio-signals includes modules of ECG, blood pressure, blood sugar and PPG. We are able to determine the current health status by monitoring measured biometric data through chronically ill health management system. We will focus on the individual service to be appropriate for a patient group in a mobile environment.
This study was conducted to identify a effect of the comprehensive self-regulation program for hypertensives as a nursing intervention on self-care performance and the physiologic parameters in rural areas. For this purpose, a nonequivalent control group and a pre and post-test quasi- experimental design was used. Fifty-four were evaluated subjects from two Community Health Practitioner Posts in the suburbs of Taejon City. The subjects of the control and experimental groups were matched for age and sex. The self-regulation program developed by author given to the experimental group. The program consisted of group education on hypertension and self-care, self-regulation including the blood pressure self-monitoring, recording of self-care activities, and encouraging and reinforcing self-efficacy. The whole program was carried out from September to November of 1999. The data were analyzed by repeated measure ANCOVA, t-test, and ANCOVA. The results were as follows ; There was significant improvement in the scores on knowledge (F=.68, P=.004), perceived self-efficacy (F=26.39, P=.000), self-care performance (F=26.11, P=.000) of the experimental group compared with those of the control group. There was no significant change on the score of locus of control, perceived benefits and perceived barrier, blood cholesterol level, body weight between two groups (P>.05). From these results, it can be concluded that the self-regulation program is an effective nursing strategy to promote self-care performance of hypertensives in rural area.

Objectives: This study set out to investigate the relationship among the factors of metabolic syndrome diagnosis criteria, their risk factors including general characteristics, and the distribution of the diagnosis criteria and risk among the adult residents of a rural community. Methods: Among 1,968 residents, those who had three or more of the risk factors of metabolic syndrome, which include blood pressure, blood glucose, triglyceride, abdominal obesity, and HDL-C, were categorized as the metabolic syndrome group. And their correlations were analyzed. Results: As for the risk ratio with five factors of the metabolic syndrome diagnosis criteria, it was high according to age and smoking. In addition, the results show that body fat percentage, hs-CRP, insulin, BMI, PP2, total cholesterol, and W/Ht also had much impact on increasing the risk ratio of the metabolic syndrome diagnosis criteria. It turned out that metabolic syndrome was affected by the body mass index(BMI), insulin, waist to height ratio(W/Ht), and hs-CRP. It was 2.51 times crude odds ratio that BMI over the 25kg/m2 in the ratio of the fact of metabolic syndrome and adjusted for sex odds ratio 2.50times and W/Ht was 3.31times, adjusted for sex odds ratio 3.25 times. Conclusion: BMI, W/Ht and smoking of the general characteristics seem to have close relationships with high correlations between the metabolic syndrome diagnosis criteria and the risk factors. Thus there is an urgent need to evaluate them and take interventions and monitoring measures for the clustering of risk factors.
No definitive etiology or risk factors have been identified that predispose individuals to developing complex regional pain syndrome (CRPS). We experienced two cases of CRPS developed after arterial and venous puncture which were done in regular medical work. A 35-years old female patient was suffered from pain and allodynia with swelling at right hand and wrist after radial artery puncture for monitoring of blood pressure during general anesthesia. A 24-years old male patient had pain and swelling with allodynia at the right fingers and arm after median cubital vein puncture for blood sampling. They did not have proper pain management as CRPS patients in the past weeks and months after their pain occurred. They were diagnosed as CRPS and started undergoing various interventional procedures, which led to improve their pain condition. Our cases suggest that CRPS could develop without any proved tissue damage in routine medical practice. In conclusion, health care workers should be educated in knowledge about the uncommon medical condition and proper consultation to pain specialist when it happens.
To protect the patient's internal organs when a patient with cardiovascular disease occurs, it is important to reduce the elapsed time by providing emergency medical services. Decisions for conducting cardiopulmonary resuscitation are mainly made using the carotid palpation method, which directs the pulse of the carotid artery, which can diagnose the patient's condition according to one's own subject and cause cerebral blood flow to be blocked by excessive pressure in the carotid due to the weaken cardiopulmonary function. In this study, we developed a multimodal patch-type probe based on multi-channel ultrasound Doppler pairs and oxygen saturation measurement modules which can monitor cardiopulmonary functions. From the in-vivo experiments, the developed probe can be utilized as a novel tool that can increase the survival rate of cardiovascular disease patients by objectively monitoring the cardiopulmonary function of the patient quantitatively and promptly in an emergency situation.

Objectives: We examined the association of hearing with cardio-metabolic diseases, dyslipidemia, hypertension and diabetes mellitus according to the personal and occupational characteristics of workers exposed to noise. Methods: The subjects of the study were 237,028 workers who underwent 2, 3, and 4 kHz airway pure tone audiometry in 2015 and who underwent clinical tests to diagnose cardiovascular-metabolic diseases. Cardiovascular-metabolic diseases were defined using reference values for respective items including blood pressure (systolic/diastolic), fasting blood glucose, cholesterol, and triglycerides. The airway pure tone hearing threshold of 2, 3, and 4 kHz, the average threshold of 2-3-4 kHz, and the hearing loss by the average threshold of the primary examination were distinguished. Results: Workers with cardiovascular-metabolic disease had significantly higher average hearing thresholds and higher rates of hearing loss. Logistic regression analysis, which adjusted for demographic variables of gender and age and occupational variables such as workplace size, industry, and type of work, and cardiovascular-metabolic disease as independent variables, showed that the odds ratio of hypertension to hearing loss in the mid-frequency was 1.239 (95% confidence interval: 1.118-1.374). For hypertension was 1.159 (1.107-1.214) and for diabetes it was 1.166 (1.104-1.230) for hearing loss in the high-frequency. Hearing loss measured by mean hearing was 1.178 (1.105-1.256) for hypertension and 1.181 (1.097-1.271) for diabetes. Conclusions: Cardiovascular-metabolic diseases in noise-exposed workers are associated with an increased risk of hearing loss and should be accompanied by bio-monitoring of cardiovascular-metabolic diseases in addition to auditory surveillance.

Background: The purpose of this study was to compare the availability of propofol and fentanyl (P + F) with diazepam and morphine (D + M) for intravenous conscious sedation during third molar surgery. Methods: Forty patients without systemic disease were operated under IV conscious sedation administered by either of the two techniques. Monitoring consisted of continuous observation of pulse rate, blood pressure, oxygen saturation, and the respiratory rate and were recorded every 15 minutes. Cooperation score was measured 5 and 15 minutes after induction of IV sedation. Following the operation, the surgeon and patients completed questionnaires including pain visual analog scale, amnesia, and side effects. Results: The P + F group was significantly more cooperative than the D + M group. The side effects of D + M group included pain on injection, nausea/vomiting and abdominal pain. The side effects of P + F group included talkativeness, nausea/vomiting, temporary apnea, pain on injection and vertigo. Conclusions: In this study, there were following benefits in the P + F group; more cooperative status and less pain perception. But respiratory depression developed in some patients.
