Application of the Pulsatile Cardiopulmonary Bypass in Animal Model (이중 박동성 인공심폐기의 동물 실험)

  • Background: Currently, the cardiopulmonary machine with non-pulsatile pumps, which are low in internal circuit pressure and cause little damage to blood cells, is widely used. However, a great number of experimental studies shows that pulsatile perfusions are more useful than non-pulsatile counterparts in many areas, such as homodynamic, metabolism, organ functions, and micro-circulation. Yet, many concerns relating to pulsatile cardiopulmonary machines, such as high internal circuit pressure and blood cell damage, have long hindered the development of pulsatile cardiopulmonary machines. Against this backdrop, this study focuses on the safety and effectiveness of the pulsatile cardiopulmonary machines developed by a domestic research lab. Material and Method: The dual-pulsatile cardiopulmonary bypass experiment with total extracorporeal circulation was conducted on six calves, Extracorporeal circulation was provided between superior/inferior vena cava and aorta. The membrane oxygenator, which was placed between the left and right pumps, was used for blood oxygenation. Circulation took four hours. Arterial blood gas analysis and blood tests were also conducted. Plasma hemoglobin levels were calculated, while pulse pressure and internal circuit pressure were carefully observed. Measurement was taken five times; once before the operation of the cardiopulmonary bypass, and after its operation it was taken every hour for four hours. Result: Through the arterial blood gas analysis, PCO2 and pH remained within normal levels. PO2 in arterial blood showed enough oxygenation of over 100 mmHg. The level of plasma hemoglobin, which had total cardiopulmonary circulation, steadily increased to 15.87 $\pm$ 5.63 after four hours passed, but remained below 20 mg/㎗. There was no obvious abnormal findings in blood test. Systolic blood pressure which was at 97.5$\pm$5.7 mmHg during the pre-circulation contraction period, was maintained over 100 mmHg as time passed. Moreover, diastolic blood pressure was 72.2 $\pm$ 7.7 mmHg during the expansion period and well kept at the appropriate level with time passing by. Average blood pressure which was 83$\pm$9.2 mmHg before circulation, increased as time passed, while pump flow was maintained over 3.3 L/min. Blood pressure fluctuation during total extracorporeal circulation showed a similar level of arterial blood pressure of pre-circulation heart. Conclusion: In the experiment mentioned above, pulsatile cardiopulmonary machines using the doual-pulsatile structure provided effective pulsatile blood flow with little damage in blood cells, showing excellence in the aspects of hematology and hemodynamic. Therefore, it is expected that the pulsatile cardiopulmonary machine, if it becomes a standard cardiopulmonary machine in all heart operations, will provide stable blood flow to end-organs.

Radioprotective Effects of Aronia on Radiation Irradiated Rats (방사선에 조사된 쥐에서 아로니아의 방사선 방호효과)

  • The present study was intended to orally administer aronia to rats, irradiate radiation once to the whole bodies of the rats, and conduct blood tests to observe, compare, and analyze changes in blood cells, such as leukocytes, erythrocytes, and platelets, in order to examine the radioprotective effects of aronia. As experimental animals, 15 male Sprague-Dawley (SD) rats aged six weeks weighing 200~250 g were taken and divided into the normal group (A) of five rats, the 5 Gy control group (B) of five rats, and the 5 Gy experimental group (C) of five rats. The normal group (A) was not~irradiated at all, the control group (B) was administered with general diets and irradiated, and the experimental group(C) was orally administered with 50 mg/kg/day of aronia two times per day to achieve a distilled water oral dose of 100 mg/kg/day and irradiated thereafter (5 Gy at 500 cGy/min) for 14 days. After the experiment, differences in leukocytes, erythrocytes, and platelets among the normal group (A), the control group (B), and the experimental group (C) were examined by comparing the counts of the blood cells and the results showed no statistically significant differences. However, on a detailed review, the normal group (A) showed statistically higher mean values for all of lymphocytes, hemoglobin, and mean corpuscular hemoglobin as compared to the control group (B) and the experimental group (C). Statistically significant differences in the counts of lymphocytes were shown between the normal group (A) and the control group (B), and between the normal group (A) and the experimental group (C); furthermore, statistically significant differences in mean corpuscular hemoglobin were shown between the normal group (A) and the experimental group (C). Given the results of the present study, in irradiated rats, aronia was generally considered as having no radioprotective effect on leukocyte, erythrocyte, and platelet while having statistically significant radioprotective effects on lymphocytes, hemoglobin, and mean corpuscular hemoglobin. Based on the present experiment, diverse studies should be conducted hereafter.

A Case of Juvenile Glomerulonephropathy in a Cocker Spaniel Dog (어린 cocker spaniel 종에서 발생한 신부전증)

  • In the present study, we address systemically a case of renal disease developed in a 1 year-old male cocker spaniel dog in terms of clinical signs, clinical pathology and pathological examinations. The animal has been suffered from renal dysfunction signs such as polyuria, anorexia, vomiting, diarrhea and weight loss. The dog was very weak and emaciated and had foamy contents with foul-smell in oral cavity. The animals showed notable decrease in the number of red blood cells and severe decreases of hemoglobin and hematocrit with or without changes of mean corpuscular volume and mean corpuscular hemoglobin concentration values, indicating microcytic or normocytic hypochromatic anemia. In serum chemistry, blood urea nitrogen, creatinine, phosphorous, Na and Cl, which are associated with renal function, were dramatically increased. In addition, alanine aminotransferase, aspartate transferase, alkaline phosphatase, cholesterol, lipase and amylase were also significantly elevated, while K concentration was notably decreased. Urinalysis indicated prominent proteinuria with increase of bilirubin. Despite of symptomatic treatments, the dog was getting worse in healthy condition and dead in the end. At necropsy, both kidneys were brownish, pale, slightly small, and have diffuse, firm and subcapsular pits. Histologically, the kidneys indicated prominent segmental or diffuse interstitial fibrosis in cortex and medulla as well as glomerulonephritis. The clinical signs, clinical pathology and histopathological abnormalities of the young dog presented were consistent with chronic glomerulonephropathy, which was suspected to be a case of familial renal disease in the juvenile cocker spaniel dog.

Effect of Vitamin E Supplementation on Hematological and Plasma Biochemical Parameters during Long Term Exposure of Arsenic in Goats

  • The present investigation was designed to determine whether supplementation of different level of vitamin E for 12 months to arsenic exposed goats (50 ppm as sodium arsenite) affords protection against the blood hemato-biochemical parameters caused by the metalloid. A total of 24 crossbred (Alpine${\times}$Beetal) lactating goats were assigned randomly into 4 equal groups (control, $T_1$, $T_2$ and $T_3$) of 6 in each, on the basis of average body weight ($36.10{\pm}0.11$ kg) and milk yield ($1.61{\pm}0.04$ kg/d). The animals in $T_1$, $T_2$ and $T_3$ were given 50 ppm arsenic, while in $T_2$ and $T_3$, additionally; vitamin E at the rate of 100 IU and 150 IU/kg dry matter (DM) respectively was additionally supplemented for the period of 12 months. Hemoglobin (Hb), total leukocyte (TLC) and blood lymphocyte % were decreased (p<0.05) in arsenic fed groups and vitamin E supplementation in the experimental group showed a protective potential. Significant increases (p<0.05) in aspertate transaminase (AST) and alanine transaminase (ALT) activities among arsenic supplemented groups were recorded, however vitamin E supplementation at higher doses showed a protective effect (p<0.05) against AST but in the case of ALT no ameliorating effect was found in either of the doses. Plasma total protein was decreased (p>0.05) but creatinine level was periodically increased in all As supplemented groups and vitamin E supplementation did not produce any protective effect. It can be concluded that arsenic exposure resulted in varying degree of changes in hemato-biochemical parameters and activities of antioxidant enzymes in goats but concomitant treatment with Vitamin E is partially helpful in reducing the burden of arsenic induced effect.

Clinical experience with single-port access laparoscopic cystectomy and myomectomy

  • Objective: This study was performed to assess our clinical experience with single-port access (SPA) laparoscopic cystectomy and myomectomy and the surgical outcomes of those procedures at our institution. Methods: The authors evaluated the surgical outcomes of SPA laparoscopic cystectomy in 293 patients and SPA laparoscopic myomectomy in 246 patients. The surgical outcomes comprised operation time, the amount of blood loss during the operation, the change in hemoglobin (before and after the operation), the change in hematocrit (before and after the operation), switching to the multi-port access method, complications, transfusions, and the duration of the postoperative hospital stay. Results: The Pearson correlation coefficient and the Spearman correlation coefficient between the operation time and the amount of blood loss were 0.312 and 0.321 for SPA laparoscopic cystectomy, respectively, and 0.706 and 0.674 for SPA laparoscopic myomectomy, respectively. The drops in hemoglobin and hematocrit were $1.33{\pm}0.78g/dL$ and $4.14%{\pm}2.45%$, respectively, in SPA laparoscopic cystectomy, while the corresponding figures were $1.34{\pm}1.13g/dL$ and $4.17%{\pm}3.24%$ in SPA laparoscopic myomectomy, respectively. Conclusion: This study reported the surgical outcomes of SPA laparoscopic cystectomy and myomectomy and compared them to previously published findings on traditional laparoscopic cystectomy and myomectomy. No significant differences were found in the surgical outcomes between SPA and traditional laparoscopic cystectomy and myomectomy.

Effects of Aspirin on Nitrite Toxicity in Cultured Eel (Anguilla japonica) (양식 뱀장어 (Anguilla japonica)의 아질산 독성에 미치는 aspirin의 투여 효과)

  • The effect of aspirin on the recovery of nitrite-induced methemoglobinemia in cultured eels (Anguilla japonica) was studied. Methemoglobinemia was induced by exposing eels to nitrite (120 ㎎ NO2-N/ℓ) for 24 hr. The nitrite exposed eels were bathed in 20 ppm aspirin solution (Aspirin), 0.8 % NaCl solution (NaCl), 20 ppm aspirin plus 0.8% NaCl solution (NaCl + Aspirin) and 50% nitrite free water(control) for 24 hr to recover from nitrite toxicity. Peripheral blood was taken from the arterious bulb from all groups to analyse hematocrit value, hemoglobin concentration, and nitrite concentration of the blood. Histopathological features of gill were also observed. Aspirin and control groups were more effective than NaCl and NaCl + Aspirin groups in recovery of hematocrit value and hemoglobin concentration, methemoglobin rate and nitrite concentration. The histopathological features on the gill of aspirin group were similar to those of normal eels, but other groups showed focal hyperemia in the lamellar carpillaries, epithelial hyperplasia. These results suggested that aspirin was very effective to recover from methemoglobinemia in nitrite-induced cultured eels.

Effects of testosterone on the orchidectomized rats

  • The aim of this study was to determine the changes of body weight, organ weight, hematological values and biochemical parameters by testosterone (Testos) on the orchidectomized (Orch) rats. The animals were divided into 4 groups. Intact group (n=10) received no treatment and operation. Sham group (n=10) received only sham operation and no treatment. Orch group received operation and no treatment. Orch+Testos received operation and testosterone. The body weights of each group increased, but that of Orch+Testos group was significantly lower in Orch+Testos group than in all the other groups. There were significant differences (P<0.05, P<0.001) of body weights between Orch+Testos group and all the other groups. Also, organ weights such as heart, liver, spleen and kidney were measured. The heart weights were significantly lower (P<0.001) in the Orch+Testos group than in all the other groups. The liver weights in the Orch+Testos group were significantly differences in comparison with those in the Sham (P<0.001) and Orch group (P<0.05). On the other hand, there were no significantly differences in the organ weights of spleen and kidney between the Orch+Testos group and the any other groups. The hematological values of white blood cell (WBC), red blood cell (RBC), mean corpuscular volume (MCV), mean corpuscular hemoglobin (MCH) and mean corpuscular hemoglobin concentration (MCHC) were no significant differences in any other groups. The concentrations of serum total protein and albumin increased significantly (P<0.05) in the Orch+Testos group as compared to that in the Orch group. However, there were no significant differences in Ca, IP and Mg in any other groups. We conclude that testosterone was significantly decreased the body weight in the orchidectomized rats. Our findings suggest that testosterone may influence the process of lipid packaging and absorption in the orchidectomized rats.

A Study on the Relationships of the Menstrual Blood Loss, Iron Nutriture, and the Pregnancy History in Married Korean Women (한국인(韓國人) 기혼여성(旣婚女性)의 출산역(出産歷)에 따른 월경혈손실양(月經血損失量) 변화(變化)와 철분영양상태(鐵分營養狀態)에 관한 연구(硏究))

  • The aim of this study was to observe the relationships between the amount of menstrual blood loss (MBL) and age, parity, number of pregnancy and induced abortion, and the iron nutriture in married Korean women. Fifty-one healthy women aged 26 to 48 years were tested for their MBL, hemoglobin(Hb) concentration, hematocrit (Hct) value, mean corpuscular hemoglobin concentration (MCHC), serum iron and ferritin concentrations. The results of this study are summarized as follows : 1) The mean value and standard error of mean of the MBL was $32.0{\pm}3.94ml$, and the range of MBL was 3.8-127.2 ml for total subjects. Maximum number (23.5%) of the subjects fell in the group with MBL of 10-l9ml, while 17.7% showed MBL above 50m1. 2) There were no statistically significant differences on the mean amount of MBL between the age groups 25-48 years. 3) The mean amount of MBL in 1-2 and 3-4 para groups were 28.5 and 36.1 ml, respectively, but the difference between two groups was not significant 4) It was observed that the mean amount of MBL was gradually decreased as the number of pregnancy and induced abortion were increased, but the significance was not observed. 5) The mean values of Hb concentration, MCHC and serum iron concentration were slightly decreased in subjects with MBL of more than 50ml but the statistical significance was not observed. The mean of Hct value was not influenced by the amount of MBL. While serum ferritin concentrations were markedly decreased and the prevalence rate of anemia was markedly increased as the amount of MBL was increased. There was significant difference (P<0.05) between the mean amount of MBL with respect to the serum ferritin concentration.

Improvement of Lipid Metabolism and Antihyperglycemic by Lentinus edodes in High Fat-fed and Streptozotocin-treated Rats (고지방과 streptozotocin으로 유도한 제 2형 당뇨에서 표고버섯이 지질대사와 항당뇨 효능에 미치는 영향)

  • Abnormal regulation of glucose and impaired lipid metabolism that result from a defective or deficient insulin are the key etiological factor in type 2 diabetes mellitus (T2DM). The our study evaluated the beneficial effect of diet supplementation with Lentinus edodes on hyperglycemia and lipid metabolism in normal and type 2 diabetic rats. The animals were divided into 4 groups: group I(control) rats were fed standard diet (12% of calories as fat); group II (T2DM) rats were fed HFD (40% of calories as fat) for 2 weeks and then injected with STZ (50 mg/kg); group III and group IV rats were continually fed a diet containing 1% and 10% Lentinus edodes for 4 weeks after T2DM induction, respectively. After 4 weeks we determined biochemical parameters such as glucose, insulin concentration, serum total cholesterol (TC), triglycerides (TG), high-density lipoprotein (HDL), and glycosylated hemoglobin (HbA1c) concentration were also measured. There was a significant reduction in serum TC and TG in the Lentinus edodes supplement groups. The Lentinus edodes diet supplementation were found to have a potent lipid metabolism improvement as well as LDL concentration decreased and HDL concentration was increased. Concentrations of blood glucose and HbA1c in the experimental groups II were significantly decreased after 4 weeks compared with the control group. The Lentinus edodes diet supplementation is useful in regulating the glucose level, improves the insulin, HbA1c, serum lipid metabolism in experimental diabetic rats. We suggest that Lentinus edodes supplementation may have the control effects of diabetes mellitus by improving blood glucose control and lipid metabolism.

The Effect of Enteral Nutrition Formula Supplement on Body Weight of Nonobese NIDDM Patients (비비만형 인슐린 비의존형 당뇨병 환자에서 영양음료를 사용한 체중 증가의 효과)

  • Nonobese NIDDM patients were studied were studied with respect to changes in visceral protein status, serum glucose and lipids and insulin secretion capacity before and after intake of enteral formula. Patients with renal or hepatic disease, gastrectomy, malabsorption, weight gain over past 6 months and poorly controlled blood glucose level were excluded. Eighteen patients served as case and administered, in addition of their usual diet, 400ml of enteral formula for 8 weeks. Another 18 patients participated in controls and had usual food intake for 8 weeks. In the begining, the levels of fasting and postprandial glucose, glycated hemoglobin, triglyceride, HDL, LDL, total cholesterol, albumin, total protein and transferrin and glucose response area on oral glucose tolerance test were not different between two groups. The response areas of insulin, C-peptide and free fatty acid and serum IGF-1 level were higher in the case than in the control group. Energy intake of patients given enteral formula exceeded their estimated energy requirements(108%) and they consumed a mean of 112g protein per day. Patients given enteral formula showed an increase in body weight(4.4%), serum transferrin(10%), IGF-1(13%) and triglyceride(34%) while controls showed no changes in those parameters at 8 weeks compared to initial values. There were no significant changes after 8 weeks in the levels of glucose, glycated hemoglobin, HDL, LDL, total cholesterol, total protein and albumin and response areas of glucose, insulin, C-peptide and free fatty acid in both groups compared to initial values. This study suggests that nutrition supplement with enteral formula can increase body weight and visceral protein status in nonobese NIDDM patients without changes in blood glucose. However, excessive calorie intake could temporarily increase serum triglyceride. In addition, this study indicates that serum transferrin and IGF-1 are more sensitive indicators to changes of protein intake than serum albumin and total protein.
