Effects of SaengRyo-SaMul-tang on Atopic Dermatitis Induced by DNCB in Mice (생료사물탕(生料四物湯)이 DNCB로 유발된 생쥐의 아토피 피부염에 미치는 영향)

  • Objectives : Atopic Dermatitis (AD) is chronic skin disease characterized by allergic hypersensitivity reactions. Saeng-ryo-Samul-tang (SRSM) can treat skin disease by cooling down blood heat, cealering away congenital fever and detoxicating. This study was conducted to investigate the effects of SRSM on AD induced by 2,4-dinitrochlorobenzene (DNCB) in mice Methods : In this study, the effects of SRSM on changes in body weights, thicknesses of dorsum skin, thicknesses and weights of ear, changes of symptoms on the dorsum skin, histopathological degree of ear and dorsum skin, IL-10 and $TNF-{\alpha}$ in serum were observed. And the effects on the proliferation rates of splenocytes were also investigated in vivo and in vitro study. Results : In SRSM topical application (Topical) group, SRSM oral application (Oral) group and SRSM Combination (Combi) group thickness of dorsum skin decreased significantly. But in TPC, ORL and CBN group, weight of ear didn't show any changes, but thickness of ear decreased significantly. And TPC, ORL and CBN group showed meaningful effectiveness symptoms like desquamation and erythema on AD's clinical espect. In histopathological observation, spongiosis, edema and inflammatory cell infiltration of epidermal were remarkably diminished in TPC, ORL and CBN group. And SRSM diminished the proliferation rates of splenocytes in vivo and vitro study. Conclusions : The present study suggests that SRSM can significantly reduced symptoms of AD, therefore SRSM is effective to treatment of AD.

  • To develop the Korean Standard Differentiation of the symptoms and signs for the Stroke(KSDS), the committee on Stroke Diagnosis Standardization of Korean Traditional Medicine was organized by nineteen experts in College of Korean Medicine, The Consensus of the second Consultation Meeting was as follows : First is the definition of the stroke on the Korean Traditional Medicine. Second is the five categories to the Differentiation of the symptoms and signs for the Stroke - fire and heat, dampness and phlegm, blood stasis, qi deficiency, yin deficiency. Third is the indices of the Differentiation of the symptoms and signs for the Stroke respectively. KSDS-1 will be applied to the clinical practice and revised. The Consensus of the third Consultation Meeting had agreed 81 symptoms indexes of KSDS.

Gastrointestinal Carriage of Escherichia coli with Hemagglutination Activity and Colonization Factor Antigen I and its Relation to Diarrhea (Mannose-resistant Hemagglutination(MRHA) 및 Colonization Factor Antigen I(CFA I)을 표현하는 대장균의 장관내 존재와 설사증 발현과의 관계)

  • Colonization factor antigen I(CFA I) has been shown to be one of several virulence factors that promote attachment of enterotoxigenic E. coli(ETEC) to small intestinal epithelial cells of humans. The ability of ETEC to produce mannose-resistant hemagglutination(MRHA) of human blood group A has been used to detect CFA I. To determine gastrointestinal carriage in Korean children of E. coli with MRHA and CFA I, 116 strains of E. coli from diarrheal children admitted to Hanyang University Hospital were examined for MRHA of human erythrocytes and the presence of CFA I. Of 45 ETEC strains, 18(40%) gave a positive MRHA($MRHA^+$) and eight(18%) were positive for CFA I(CFA $I^+$). ETEC with CFA I were all heat-stable enterotoxin(ST) producers and two of these strains were of serogroups $O_{25}$. Of 17 classic enteropathogenic E. coli(EPEC), 7(41%) were $MRHA^+$ but all were negative for CFA I(CFA $I^-$). Of 30 enteroadherent E. coli(EAEC) strains, 11(37%) were $MRHA^+$ and one was CFA $I^+$. Of 24 nonpathogenic E. coli, 4(17%) were $MRHA^+$ but all were CFA $I^-$. It was shown that MRHA was common in all strains of E. coli, CFA I was limited only to ST producing ETEC and EAEC; although MRHA is a useful screening procedure, serologic tests seem to be necessary to comfirm CFA I production. CFA I was associated with a lower proportion of ETEC isolates in Korea than has been reported for other locations.

A Study on the Effectiveness of Herbal Medicine Treatment for 755 Traffic Accident Patients-A Retrospective Review According to the Type of Herbal Medicine Prescription

  • Objectives: This study conducted a retrospective review to compare the effectiveness of herbal medicine, while confirming the effectiveness of Korean medicine (KM) in traffic accidents (TA). Methods: We investigated the medical records of TA patients who were hospitalized at Muuido Korean medicine hospital from January 2016 to June 2018 retrospectively. Medical records included general characteristics (Gender, age), TA & treatment-related information (Collision type, chief complaint, hospitalization period, and herbal medicine prescription), and clinical scales (Five-scale improvement, numerical rating scale (NRS)). Statistical analysis was performed for data distribution and effectiveness comparison. Results: 755 TA patients showed 0.78:1 gender ratio. 20-50s years old, rear collision, pain in the cervical and lumbar spine, and hospitalization period within 14 days were the majority. KM improved 83.18% of 755 TA patients' symptoms and ROM over 'improvement' level, which indicated 30-70% improvement. With common KM treatments including acupuncture, cupping, heat therapy, and manipulation therapy, TA patients received various herbal medicines and herbal medicine for breaking severe blood stasis (Tongdo-san) showed improvement in both five-scale improvement and NRS change at the same time. Conclusions: We could confirm the effectiveness of KM, particularly herbal medicine in TA. It would be necessary to develop new herbal medicines such as Tongdo-san to treat TA-related symptoms.

Pathological Entity of Jueyin Disease and the Relationship between the Concept of Three-Yin-Three-Yang in 《Shanghanlun》 (《상한론(傷寒論)》 궐음병의 병리본질과 삼음삼양(三陰三陽) 개념과의 관계)

  • In order to research the pathological entity of Jueyin disease in ${\ll}Shanghanlun{\gg}$, some sharing concept of three-yin-three-yang used in ${\ll}Neijing{\gg}$ and ${\ll}Shanghanlun{\gg}$ were investigated first, and then the meaning of jueyin and jueyin disease were analyzed. In cold damage disease, time-space factor is important because the pathological change is rapid and the symptoms along path are similar, therefore three-yin-three-yang having complex meaning of time and space can be used as an appropriate pathological concept. So to speak, it is able to be interpreted as various modes like variations of yin-yang, qi-blood, change of pulse condition, theories of opening, closing, pivot or exuberance and debilitation of form and qi manifested in the six districts of the human body following disease process. Jueyin is between front taiyin and rear shaoyin, and it's attribution is inherent in qi stagnation and yin exuberance in relative to the location of flank and liver. Putting together above descriptions, pathological entity of jueyin disease is that the symptoms mingled with cold and stagnant heat competing each other when a subject having qi stagnation in flank with cold in extremities and lower abdomen in particular is seized with cold influenza.

Korean medicine treatment including oral administration of Gyejibongnyeong-hwan and acupuncture therapy for calf edema and pain due to deep vein thrombosis of lower leg: A Case Report (하지 심부정맥혈전증으로 인한 부종 및 통증에 계지복령환 투약과 침 요법을 포함한 한의 치료를 시행한 증례 1례 보고)

  • Objectives: This study was aimed to report changes in clinical symptoms and signs after treatment with Korean medicine in patients who suffered from calf edema due to deep vein thrombosis (DVT). Methods: A 46-year-old male patient, who discharged home after receiving standardized treatment for acute DVT in the right leg, suffered pain and heat sense due to edema of the affected calf. Oral administration of herbal medicine (Gyejibongnyeong-hwan 4 g, twice daily) and acupuncture therapy were given to him for 6 weeks. The change in chief complaints, Villalta score, the right and left side difference of the circumference and the skin surface temperature of both calves, and blood level of D-dimer were observed before and after treatment. Results: The patient's chief complaints began to significantly improve from 2 weeks after treatment, and disappeared completely within 4 weeks. At the end of the treatment, a substantial decrease in the difference between the right and left calves in the circumference and skin surface temperature was observed. This effect was maintained even at the time of follow-up 3 months after the end of treatment, and the patient completely recovered indoor and outdoor life to the level before onset. Conclusions: This case suggests that Korean medicine treatment, including Gyejibongnyeong-hwan administration and acupuncture therapy, can be a viable option to improve edema and related clinical problems in the affected limbs due to DVT.

Effect of Samhwangsashimtang Extract on Lipopolysaccharide-Stimulated Inflammatory Response and Macrophage Activity (삼황사심탕(三黃瀉心湯) 추출물의 lipopolysaccharide 유도에 의한 염증 조절과 대식세포 활성에 대한 연구)

  • Objectives : Samhwangsashim-tang(SHSST), a mixture of Rhei radix et rhizoma, Scutellariae radix, and Coptidis rhizoma, has been regarded as being able to treat bleeding, gastric discomfort, dry mouth, insomnia and purpura due to Blood Heat. Currently, this herbal formula is applied to gastritis, gastric ulcer, hypertension, atherosclerosis or other types of vascular inflammatory disorders. Methods : We extracted this herbal mixture with 30% ethanol and examined for its effects on systemic inflammatory responses and in vitro macrophage activity. Mice were orally given to SHSST for 7 days and then lipopolysaccharide(LPS) was intraperitoneally injected. Tumor necrosis factor-${\alpha}$(TNF-${\alpha}$) levels in serum were measured 1 h after LPS challenge. Peritoneal macrophages were isolated from thioglycollate-injected mice and used for in vitro cellular activity. Cell death was measured using the MTT method and annexin V/propidium iodide staining. LPS-stimulated signaling molecules necessary for TNF-${\alpha}$ expression were determined by Western blotting. Results : Oral administration of SHSST for 7 days resulted in a significant reduction in LPS-stimulated TNF-${\alpha}$ release into serum. In vitro treatment of SHSST was cytotoxic in a concentration-dependent manner. However, SHSST caused a concentration-dependent reduction in necrosis and increase in apoptosis in mouse peritoneal macrophages. SHSST inhibited the activation of NF-${\kappa}B$, p38 and JNK signaling molecules in response to LPS. Conclusion : Taken together, our results demonstrated that SHSST was effective in lowering LPS-stimulated TNF-${\alpha}$ serum levels, possibly through its modulation of NF-${\kappa}B$, p38 and JNK in macrophages.

A Review on the of External Ointment Treatment for Diabetic Foot Ulcer (당뇨병성 족부 궤양의 외용 연고 처치에 대한 고찰)

  • Objectives : Diabetic Foot Ulcer(DFU) is one of the common complications of diabetes. DFU is difficult to treat compared to other chronic wounds and clinically effective treatments are limited. The purpose of this study is to review the of external ointment treatment for DFU. Methods : 8 databases such as PubMed, EMBASE, CENTRAL, CNKI, NDSL, RISS, KISS, OASIS including English, Korean and Chinese were searched by structured search strategies that consist of terms as 'diabetic foot', 'diabetic foot ulcer(DFU)' and 'randomized' from January 2001 to January 2021. All randomized controlled trials(RCTs) involving treatment group as external ointment or in combination with conventional treatment were included. Results: A total of 20 RCTs was identified and analyzed. In treatment group, ointment was applied based on conventional treatment. A total of 16 different ointments were used, and the frequency of use was highest in the order of Heat-clearing drug, Blood-activating and stasis-dispelling drug, Tonifying and Replenishing drug and Orifice-opening drug. The most used herbs were in the order of Coptis chinensis Franch., Phellodendron chinense Schneid., Borneolum syntheticum., Angelica sinensis(Oliv.) Diels. In treatment group, a clinically effective effect was obtained compared to the contrast group. Conclusions : The result of this study suggest that external ointment treatment based on conventional treatment can be applied to DFU.

Change in plasma cortisol and glucose levels of Oncorhynchus keta according to water temperature

  • Chum salmon (Oncorhynchus keta) is a species of anadromous salmonid inhabiting coastal rivers in the North Pacific and the Arctic in the Bering and is the most widely distributed among Pacific salmon species. It is an important fish species in Korea as the salmon releasing project is being actively carried out. This study was conducted to investigate changes in the physiological activity of O. keta according to water temperature. Three experiments were conducted according to the water temperature and period, and the plasma concentrations of cortisol and glucose were analyzed from the blood samples of the experimental groups. Experiment I is a short-term water temperature experiment, in which water temperature stimulation was given for 4 hours at water temperatures of 12℃, 16℃ (control), 20℃, and 24℃, and a recovery period was given for 4 hours. Experiment II is an experiment in which water temperature stimulation was given for 24 hours, 48 hours, and 72 hours at a high temperature of 24℃, and a recovery period was given for 12 hours, respectively. Experiment III is a long-term water temperature experiment, in which the water temperature was 12℃, 16℃ (control), 20℃, and 24℃ for 8 weeks. As a result of the experiment, in Experiment I, there was no significant difference in the survival rate between the experimental groups, but the concentration of cortisol and glucose in the plasma according to the set water temperature showed a significant difference. In Experiment II, there was no significant trend according to the maintenance time of the high-temperature state, but as the temperature increased, the plasma cortisol and glucose levels significantly increased compared to the control group. In Experiment III, all of the experimental group C (24℃) died in the 1st week, and there was no significant difference in the plasma glucose at the 1st and 8th weeks among the remaining experimental groups.

Effects of Scutellariae Radix on Gene Expression of Human Cervical Cancer Cells(SNU-703) (황금추출물이 인간 유래 자궁경부암세포의 유전자발현에 미치는 영향)

  • Purpose: In the theory of traditional medicine, Scutellariae Radix (SR) can clear away heat and remove dampness, purge the sthenic fire and remove toxic materials, cool blood and stop bleeding to prevent miscarriage. Recently, SR is known to have anti-cancer activity. For this reason, the present author designed to investigate the effect of SR on proliferation rates of cervical cancer cell line, then effects on genetic profile by SR. Methods: The genetic profile for the effect of SR on human derived cervical cancer cell line, SNU-703, was measured using microarray technique, and the functional analysis on these genes was conducted. Results: Total 519 genes were up-regulated and 606 genes down-regulated in cells treated with SR. Genes induced or suppressed by SR were all mainly concerned with metabolic process, regulation of biological process and protein binding. The network of total protein interactions was measured using cytoscape program, and some key molecules, such as TNFRSF1A, AKT1, MAPK3, and STAT3 that can be used for elucidation of therapeutical mechanism of medicine in future were identified. Conclusion: These results suggest possibility of SR as anti-cancer drug and also suggest that related mechanisms are involved in TNFRSF1A, AKT1, MAPK3, and STAT3 related signalling pathways.
