심혈관 수술 신생아의 혈당수준에 따른 환아 결과 (Patient Outcomes according to Blood Glucose Level in Neonates with Cardiovascular Surgery)

  • Purpose: The purpose of this study was to provide basic data for suitable neonate blood glucose maintenance by investigating and analyzing the blood glucose level of post cardiovascular surgery neonates in the ICU for the effect of blood glucose levels on the following outcomes; ICU stay, hospital stay, mechanical ventilation time, morbidity, and mortality. Methods: The participants were 143 neonates in the ICU after having had cardiovascular surgery. The design for this study was an investigation of the blood glucose levels of the neonates and retrospective analysis of patient outcomes according to blood glucose level. Results: The results for the neonate groups showed that the factors of hospital stay, ICU stay, mechanical ventilation time and mortality, for the group with a blood glucose level over 140 mg/dL were longer and higher than for the group with blood glucose of less than 100 mg/dL or the group between 100-139 mg/dL. Conclusion: The results of this study indicate that when caring for neonates after cardiovascular surgery, it is important to recognize the influence of blood glucose levels on patient outcomes like hospital days, ICU stay, length of time on mechanical ventilation and mortality. Further, care guidelines for neonates' glucose level management need to be developed.

  • Purpose: The purpose of this study was to examine relationships among blood glucose, HbA1c, and self management comparing these with general and illness characteristics of subjects with Type II Diabetes living in the community. Methods: Using a comprehensive survey developed for the study, data were collected from 82 type II diabetes patients who were registered at five community health departments. The variables of self management, blood glucose and HbA1c were assessed by nurses. Data were analyzed with descriptive statistics including t-test, ANOVA and Pearson's correlation coefficient to compare self management and level of HbA1c by subject's general and illness characteristics and to examine the relationships among variables. Results: About 73.2% of the subjects' HbA1c were 7.0% or higher and 54.9% of subjects' blood glucose were 200 mg/dL or higher. The level of self management was moderate. Most frequently perceived reasons for failure of blood glucose control were dietary failure (32.9%). There was significant relationship between self management and HbA1c(r=-.223, p=.040). The mean score of self management were higher among female (t=-2.37, p=.021), who are not on diabetes medication (t=6.70, p=.011). Conclusion: Comprehensive intervention is needed to improve dietary self management, especially for male and those who is on diabetes medication.

PPG와 기계학습을 활용한 혈당수치 예측 연구 (The study of blood glucose level prediction using photoplethysmography and machine learning)

  • 논문은 광용적맥파(photoplethysmography, PPG) 센서에서 획득한 생체 신호, ICT 기술 및 데이터 기반의 혈당수치 예측 모델을 개발하고 검증하는 연구이다. 혈당 예측은 기계학습의 MLP 아키텍처를 이용하였다. 기계학습 모델의 입력층은 심박수, 심박변이도, 나이, 성별, VLF, LF, HF, SDNN, RMSSD, PNN50의 10개의 입력노드와 은닉층은 5개로 구성된다. 예측모델의 결과는 MSE=0.0724, MAE=1.1022 및 RMSE=1.0285이며, 결정계수(R2)는 0.9985이다. 비채혈방식으로 디지털기기에서 수집한 생체신호 데이터와 기계학습을 활용한 혈당 예측 모델을 수립하고 검증하였다. 일상에 적용하기 위해 다양한 디지털 기기의 기계학습 데이터셋 표준화와 정확성을 높이는 연구가 이어진다면 개인의 혈당 관리에 대안적 방법이 될 수 있을 것이다.

Effects of soybean supplementation on blood glucose, plasma lipid levels, and erythrocyte antioxidant enzyme activity in type 2 diabetes mellitus patients

  • The purpose of this study was to investigate the effect of soybean on blood glucose and lipid concentrations, and antioxidant enzyme activity in type 2 diabetes mellitus(DM) patients. We divided patients into two groups and fed them, respectively, a basal diet(control group) and a basal diet with 69 g/d of soybean(soybean group) for 4 weeks. Pills with roasted soybean powder were provided to the soybean supplementation group three times a day. Macronutrients intake except dietary fiber was similar between the two groups. No significant differences were observed in dietary intakes or body weight before and after the supplementation. Energy composition ratio of C:F:P was 65:19:16 in the control group, 64:20:16 in the soybean group. The blood parameters of subjects before supplementation, such as fasting blood glucose, postprandial glucose level, total cholesterol, triglyceride, LDL-cholesterol and $HbA_1C$ were not different between the two groups. After supplementation, fasting blood glucose(p<0.001), postprandial glucose level(p<0.001) and serum triglyceride level(p<0.05) were significantly reduced in the soybean group in comparison with the control group. The total cholesterol level was not significantly different between the control and the supplemented group after 4 weeks of treatment. TBARS levels of the soybean group were not significantly different from those of the control group. The activities of catalase(p<0.01) and glutathione peroxidase(p<0.05) were significantly higher in the soybean group compared to the control group. The results of this study suggest that soybean supplementation would be helpful to control blood glucose and serum lipid in diabetic patients. Also, soybean showed an antioxidant activity that may contribute to enhance the effect of antioxidant defense. This activity contributes to protection against oxidative damage in type 2 DM patients. Soybean may have potential use in the disease management of patients with DM.

Management of Diabetes from HbA1c Test

  • HbA1c test measures the amount of glycated hemoglobin in blood. HbA1c shows the average of blood glucose levels for the past three months, this is a better indicator of how overall diabetes is doing. HbA1c gives a much better idea of how the body is breaking down the glucose. Therefore, this HbA1c is very important tool for maintaining normal glucose levels for pre-and diabetic patients. Total 408 participants were tested HbA1c voluntarily from Chosunilbo Health Expo (8th~11th, July 2010). Through this small-scaled direct HbA1c, about 54.7% (207 out of 408) was shown glucose tolerance and diabetes. However, 61 from 157 participants who were shown under 6.9% HbA1c (normal and pre-diabetic stage) are taking only antidiabetic drugs to maintain a normal blood glucose. Regular HbA1c test can bring an important management and awareness about controlling blood sugar level and prevention of diabetic complications.

당뇨병 환자의 우울상태에 따른 자가 관리 이행 (Depression and Self Care Behavior in Patients with Diabetes Mellitus)

  • Purpose: To investigate the level of depression and self management behavior in patients with diabetes. Method: From May to August 2004, 124 diabetic patients 18 years or older were surveyed using the Korean version of the BDI, the self-management scale by Toobert(2000) and hospital lab data for HbAl, FBS and Blood Sugar-2hr. Results: 1. Mean score on the BDI for the patients was 10.85 points. The score on the BDI was significantly higher in hyperglycemic patients than in normoglycemic patients. 2. The level of self management for the past week showed care on an average of 4.2 days. According to level of depression, patients in the normal group and patients in the mild group had significantly higher levels of self management than those in the moderate group. 3. According to level of depression, patients in the normal group had significantly lower levels of blood glucose than those in the moderate group, and for HbAlc and FBS those in the mild group had lower levels compared to those in the moderate group. 4. The factors significantly associated with level of self management were depression (25.66%) and duration of illness. Conclusion: Programs should be developed to combat depression in patients with diabetes in order to increase compliance through self care management and thus increase of blood glucose levels.

심탄도와 인공지능을 이용한 혈당수치 예측모델 연구 (The study of blood glucose level prediction model using ballistocardiogram and artificial intelligence)

  • 논문은 심탄도(BCG, Ballistocardiogram) 센서를 이용하여 생체신호 데이터를 비침습, 무구속적인 방식으로 수집하고, ICT 기술과 고성능 컴퓨팅 환경에서 인공지능 기계학습 알고리즘을 활용하여 데이터 기반 혈당 예측 알고리즘 모델 개발 및 검증하는 방법을 제시하고 연구하는 것이다. 혈당수치 예측모델은 MLP 아키텍처에 입력노드는 심박수, 호흡수, 심박출량, 심박변이도, SDNN, RMSSD, PNN50, 나이, 성별이며, 은닉층 7개를 사용하였다. 실험 결과는 5회 실험한 학습데이터의 평균 MSE, MAE 및 RMSE 값은 각각 0.5226, 0.6328 및 0.7692이며 검증데이터 평균 값은 각각 0.5408, 0.6776, 0.7968이었으며, 결정계수(R2) 수치는 0.9997의 결과를 보였다. 데이터를 기반으로 한 혈당수치를 예측하는 모델을 표준화하고 데이터셋 수집과 예측 정확성을 검증하는 연구가 계속적으로 진행된다면 비침습 방식의 혈당 수준 관리에 활용될 수 있을 것으로 사료된다.

PPG 혈당 모니터링 시스템의 분석적 평가 - 연구자 임상 (Analytical Evaluation of PPG Blood Glucose Monitoring System - researcher clinical trial)

  • 본 연구는 모세관 혈당의 혈당값을 대조군으로 연구 참가자의 혈액 포도당을 모니티링할 때 PPG 센서와 DNN 예측알고리즘이 융합된 혈당모니터링 시스템(PPG-BGMS)의 성능을 평가하는 것이다. 연구는 2023년 9월부터 2023년 11월까지 참가자를 대상으로 실시된 연구자 임상시험이다. PPG-BGMS는 1분간의 심박수, 심박변이도 정보와 DNN 예측알고리즘을 활용한 예측된 혈당수치와 개인용혈당관리시스템의 혈당측정기로 측정한 모세관혈당 수치와 비교했다. 총 100명의 참가자 중 제2형 당뇨(T2DM) 유병인은 50명이며, 평균연령은 67세(28세~89세)이다. PPG-BGMS의 예측혈당의 100%가 Clarke 오류그리드 및 Parker(Consensus) 오류그리드의 A+B 영역에 분포하는 것으로 나타났다. PPG-BGMS 예측 혈당의 MARD 값은 5.3 ± 4.0 %이다. 결과에 의하면 비채혈식 PPG-BGMS는 임상표준의 채혈식 개인용 혈당측정시스템의 순간 혈당수치와 비교하여 열등하지 않는 것으로 분석되었다.

영양상담이 제2형 당뇨병 환자의 혈당과 식사요법에 대한 지식과 실천에 미치는 영향 (Effects of Nutrition Counseling on Diabetes Management in Type 2 Diabetes Mellitus Patients)

  • The purpose of this study was to investigate the effects of nutrition counseling on diabetes management by determining changes in anthropometry and blood components as well as knowledge and practice of diet therapy and nutrient intake in 34 (male 11, female 23) type 2 diabetes patients. The knowledge and the practice of diet therapy, drinking, smoking and exercise were analyzed by questionnaires. Dietary nutrient intake were obtained from the patients by the 1 day 24-hr recall. Blood glucose level and blood pressure were measured before and 3 months after the treatment. The results are summarized as follows: Average weight (p<0.05) and body mass index (p<0.05) were significantly lower post-counseling. Fasting blood glucose levels (p<0.01) and postprandial-2hour blood glucose levels (p<0.01) were also significantly lower post-counseling. In lifestyle changes for self-management the patients showed significantly higher exercise habits post-counseling (p<0.01). Regarding their level of diet knowledge, they showed significantly higher levels post-counseling in six items such as importance of diet therapy for diabetes (p<0.001), principles of diet therapy (p<0.001), nutrient composition of foods (p<0.01), carbohydrate composition of foods (p<0.001), the prescribed calories (p<0.001) understanding food item and exchange units of cereals, grains (p<0.001) and fruits, juices (p<0.001). Regarding their diet practices, the patients showed significantly higher levels of practice post-counseling in keeping within permitted meal size (p<0.001), using food exchange lists (p<0.001), keeping exact meal times (p<0.01), and controling sweet foods (p<0.001). Protein (p<0.05), animal lipid (p<0.05), and vitamin C (p<0.05) intakes were significantly higher post-counseling.

Effect of the Dietary Protein Level on Plasma Glucose, Lipids and Hormones in Streptozotocin-Diabetic Rats

  • Atherosclerotic vascular disease is a major cause of the increased morbidity and mortality assciated with diabetes mellitus. The prominent role of nutrition in hypercholesteolemia and atherosclerosis is generally accepted. Diet is a key element in the management of diabetes (type I-IDDM), yet the appropriate diet for patient with diabetes mellitus is not well known. Dietary protein has been shown to have a significant effect on plasma cholesterol levels in both experimental animals and humans. The present experiment was designed to determine the effect of the dietary protein level(20% vs 60%) on plasma glucose concentration, lipids profile, insulin and glucagon levels from non-diabetic and streptozotocin-induced diabetic rats. Results showed that a high protein diet decreased triglyceride concentration in diabetic rats. Also diabetic rats fed a high protein diet were hypocholesterolemic than rats fed a control diet. There were no effects by level of protein on fasting blood glucose concentration and insulin/glucagon ratio. Results from the present study suggest that a high protein diet may be beneficial to control pasma lipids in chemically-induced diabetic rats.
