Successful Removal of Endobronchial Blood Clots Using Bronchoscopic Cryotherapy at Bedside in the Intensive Care Unit

  • Acute airway obstruction after hemoptysis occurs due to the presence of blood clots. These conditions may result in lifethreatening ventilation impairment. We report a case of obstruction of the large airway by endobronchial blood clots which were removed using bronchoscopic cryotherapy at the bedside of intensive care unit. A 66-year-old female with endometrial cancer who had undergone chemotherapy, was admitted to the intensive care unit due to neutropenic fever. During mechanical ventilation, the minute ventilation dropped to inadequately low levels and chest radiography showed complete opacification of the left hemithorax. Flexible bronchoscopy revealed large blood clots obstructing the proximal left main bronchus. After unsuccessful attempts to remove the clots with bronchial lavage and forceps extraction, blood clots were removed using bronchoscopic cryotherapy. This report shows that cryotherapy via flexible bronchoscopy at the bedside in the intensive of intensive care unit is a simple and effective alternative for the removal of endobronchial blood clots.

Atelectasis by Bronchial Secretion: Report of Four Cases (기관내 분비물로 인한 무기폐: 4례 보고)

  • This is a report on four cases of atelectasis which were implicated as one case of mucoid impaction and three cases of blood clots in main bronchus. The 1st case was found to be massive atelectasis on Lt. entire lung due to mucoid impaction with pulmonary tuberculosis. This case was performed the Lt. pneumonectomy. There are contain-ing impacted yellowish-gray jelly like thick materials on the Lt. whole bronchial trees and pathologic findings were consistent with tuberculosis including caseation necrosis and multinucleated giant cell on whole left lung thoroughly in microscopic findings. The 2nd and 3rd case [25 years old female and 30 years old female] were diagnosed as one sided entire lung ateletasis which were led by accumulation of old blood clots on bronchus following incomplete expectoration of sudden massive hemoptysis from pulmonary tuberculosis lesion. These two cases were recovered by removal of blood clots and bronchial irrigation under bronchoscopy. Follow up chest film revealed well aeration. The 4th case [45 years old] was taken the removal of old blood clots and tissue debris under the bronchoscopy as bronchial obstruction following massive hemoptysis. The cytologic findings revealed the class III in fresh sputum and class 1V in bronchial irrigation which may suggest of malignancy. However, we could not found the causative lesions suggestive malignancy by the bronchography and other diagnostic measurements.

The influence of sodium hypochlorite concentration on the fibrin structure of human blood clots and transforming growth factor-beta 1 release: an ex vivo study

  • Objective: This study investigated the effects of various concentrations of sodium hypochlorite (NaOCl) on human whole-blood clotting kinetics, the structure of the blood clots formed, and transforming growth factor (TGF)-β1 release. Materials and Methods: Human whole blood was collected from 5 healthy volunteers and divided into 4 groups: CG (control, 0.5 mL of blood), BN0.5 (0.5 mL of blood with 0.5 mL of 0.5% NaOCl), BN3 (0.5 mL of blood with 0.5 mL of 3% NaOCl), and BN5.25 (0.5 mL of blood with 0.5 mL of 5.25% NaOCl). The effects of NaOCl on clotting kinetics, structure of fibrin and cells, and release of TGF-β1 were assessed using thromboelastography (TEG), scanning electron microscopy (SEM), and enzyme-linked immunosobent assay, respectively. Statistical analysis was conducted using the Kruskal Wallis and Mann-Whitney U tests, followed by the post hoc Dunn test. A p value < 0.05 indicated statistical significance. Results: The blood samples in BN0.5 and BN3 did not clot, whereas the TEG of BN5.25 showed altered clot formation. Samples from the CG and BN3 groups could only be processed with SEM, which showed that the latter lacked fibrin formation and branching of fibers, as well as clumping of red blood cells with surface roughening and distortion. TGF-β1 release was significantly highest in BN3 when all groups were compared to CG (p < 0.05). Conclusions: Each concentration of NaOCl affected the release of TGF-β1 from blood clots and altered the clotting mechanism of blood by affecting clotting kinetics and cell structure.

  • We describe unusual manifestations of congenital cystic adenomatoid malformation (C.C.A.M.) of the lung, such as movable fungal ball-like intracystic blood clots and hemoptysis, which were detected in previously healthy 23 years-old woman. We identified blood clots only after left upper lobectomy and could not distinguish from fungal ball with radiographic methods. CCAM of the lung, rare and lethal form of congenital pulmonary cystic disease, was initially introduced by Ch, in and Tang in 1949. The histogenesis of this lesion is characterized by polypoid glandular tissue proliferation and overgrowth of mesenchymal elements due to cessation of bronchiolar maturation which occurred in after 16weeks intrauterine period. In 80-95% of reported cases, the lesion was confined to a single lobe and there was no lobe and right and left lung predilection The clinical presentation may be widely variable, ranging from intrauterine fetal death to late discovery in childhood with recurrent pulmonary infection. But there,s no reports which were misdiagnosed with intracystic fungal ball. The treatment choice is lobectomy of affected lobe. There,s a few case reports with rhabdomyosarcoma, bronchiolar cell carcinoma and myxosarcoma arising in CCAM patients. Therefore, early resection is recommended even if asymtomatic cases. We experienced a rare case of CCAM of the lung in 23 years old female, and there were intracystic fungal ball-like movable blood clots in lower portion of left lung. After left upper lobectomy was performed, now she is discharged and followed up without any complications.

The Effect of Recombinant Tissue Plasminogen Activator on the Intracerebral Hematomas in Experimental Cat Models

  • Objective: Recent clinical studies have demonstrated that intracisternal administration of recombinant tissue plasminogen activator(rt-PA) can facilitate the normal clearing of blood from the subarachnoid space. Urokinase, a first generation fibrinolytic agent, has been used to liquify such clots with some success. Therefore, recombinant tissue plasminogen activator, a second generation fibrinolytic drug that may be safer and more effective, is studied to evaluate its dosage to lyse clots in vitro and reactivity in the brain parenchyme. Methods: Intracerebral hematomas were created by stereotactically injecting 2ml of clotted autogenous blood into the brain parenchyme of total 28 anesthetized adult cats (weighting 3.8 to 4.1 kg). The control animals (group A) received 1 ml of normal saline injected into the clots and the experimental animals received each 0.1 mg of rt-PA (group B), 0.5mg of rt-PA (group C) and 1 mg of rt-PA (group D) at 6 hours after the clot injection. Results: 1. The amount of remained clots after lysing the hematomas were as follows: $1.80{\pm}0.17ml$ in group A, $1.65{\pm}0.23ml$ in group B, $0.61{\pm}0.37ml$ in group C and $0.52{\pm}0.34$ in group D. The result indicated that hematomas in rt-PA treated groups (C & D) were lysed better than the control group. 2. At least 0.5mg of rt-PA should be required for the lysis of 2ml of hematomas. 3. Light microscopic examination revealed no histological evidence of hemorrhage in tissue sections from each brain. Conclusion: Recombinant tissue plasminogen activator may be safely and effectively employed for the lysis of intracerebral hematomas in animal model.

Predicting Factors of Chronic Subdural Hematoma Following Surgical Clipping in Unruptured and Ruptured Intracranial Aneurysm

  • Objective : The aim of this study is to analyze the differences in the incidence, predicting factors, and clinical course of chronic subdural hematoma (CSDH) following surgical clipping between unruptured (UIA) and ruptured intracranial aneurysm (RIA). Methods : We conducted a retrospective analysis of 752 patients (UIA : 368 and RIA : 384) who underwent surgical clipping during 8 years. The incidence and predicting factors of CSDH development in the UIA and RIA were compared according to medical records and radiological data. Results : The incidence of postoperative CSDH was higher in the UIA (10.9%) than in the RIA (3.1%) (p=0.000). In multivariate analysis, a high Hounsfield (HF) unit (blood clots) for subdural fluid collection (SFC), persistence of SFC ${\geq}5mm$ and male sex in the UIA and A high HF unit for SFC and SFC ${\geq}5mm$ without progression to hydrocephalus in the RIA were identified as the independent predicting factors for CSDH development (p<0.05). Conclusion : There were differences in the incidence and predicting factors for CSDH following surgical clipping between UIA and RIA. Blood clots in the subdural space and persistence of SFC ${\geq}5mm$ were predicting factors in both UIA and RIA. However, progression to hydrocephalus may have in part contributed to low CSDH development in the RIA. We suggest that cleaning of blood clots in the subdural space and efforts to minimize SFC ${\geq}5mm$ at the end of surgery is helpful to prevent CSDH following aneurysmal clipping.

An outbreak of bovine coccidiosis in Korean native cattle (한우 콕시듐병의 발생)

  • Authors observed an outbreak of bovine coccidiosis at a farm stock in Gyeongnam, Korea, which were diagnosed on the ground of clinical and fecal examination with the following results. Twenty one of thirty four Korean native cattles evacuate feces containing blood and mucus or evacuate entirely of clots of fresh and red blood. On fecal examination showed eimeria zuernii. The patients were recovered with sulfamethazine and amprolium for 5 days. From these results the outbreak was considered to be typical bovine coccidiosis.

5 Cases of Primary Infertility treated by Jokyungjongok-tang gamibang (조경종옥탕가미방(調經種玉湯加味方)을 병용한 원발성 불임환자 치험 5례(例))

  • Objectives : According to 2013 statistics, the primary infertility rate is 13.5%. The current situation is much worse than that reported, so it is even emerging as a social problem. The purpose of this study is to analyze the 5 cases of successful infertility treatment with herbal treatment and to give hope to the infertile couple to become pregnant and to suggest the possibility of herbal treatment in infertility treatment. Methods : We administered Jokyungjongok-tang gamibang as a herbal medicine and treated with acupuncture, moxibustion. Treatment was done once or twice a week. The herbal medicines were taken 30 minutes after meals and 3 times a day. While taking the herbal medicine, they were prohibited from eating flour, pork, liquor, tobacco and coffee. Patient status was assessed by consultation through pulse, tongue and abdominal diagnosis. The diagnosis of pregnancy was confirmed by ultrasonography at hospitals and the success of the treatment was judged based on healthy birth. Results : Patients had common features, such as lower abdominal pain, menstrual cramps, and blood clots during menstruation. Herbal medicine combined with acupuncture, moxibustion improved patients' overall fatigue and reduced the body's coldness. Since then, the dysmenorrhea and blood clots have decreased, and the digestion condition has improved. Therefore they became pregnant and gave birth to a healthy baby. Conclusion : Jokyungjongok-tang gamibang was effective in improving the body's coldness and uterine condition and was successful in pregnancy and helped to give birth to a healthy baby.

Blood Compatibility of Hollow Fiber Membranes Treated with Plasma Polymerization (플라즈마 중합 처리된 중공사 막의 혈액 적합성)

  • Surface modification of polypropylene hollow fiber membranes was peformed in order to develop blood-compatible biomaterials for use in the blood contacting and oxygenation membranes of a lung-assist device(LAD). Blood compatibility was determined by using anticoagulation blood and evaluating formation of blood clots on their surfaces as well as activation of plasma coagulation cascade, platelet adhesion, and aggregation. It was verified that the number of platelets on the silicone coated fibers was significantly lower than those on polypropylene. It was also found that the polypropylene hollow fiber membranes using plasma treatment exhibited suppression of complement activation in blood compatibility test.
