인영혈(人迎穴) 자침(刺鍼)이 혈압에 미치는 영향 (Effect of Acupuncture on Inyǒng(ST9) on the Blood Pressure)

  • Objective : This study was conducted with the purpose to evaluate the decreasing effect of the blood pressure on the hypertensive patients who were given to acupuncture on Iny$\check{o}$ng(ST9). Methods : We investigated the patients who had essential or secondary high blood pressure in the kwangdong oriental hospital from October 2000 to September 2001 as an experimental group and the non-hypertensive patients as a control group. Result : While the acupuncture was done in the experimental group, there was statistically significant decrease of the blood pressure, There was no statistically significant changes of blood pressure in the control group. Conclusion : We found that acupuncture on Iny$\check{o}$ng(ST9) has statistically significance in decreasing blood pressure to the experimental group while it hasn't to the control group. We also found out the decreasing effect of blood pressure lasted for two hours at least.

광학 센서를 이용한 비관혈적 혈압 측정의 오차 보정 (Compensation of Error in Noninvasive Blood Pressure Measurement System Using Optical Sensor)

  • This study is attempted to correct an error of electronic blood pressure meter with an optical sensor. In general, for a hospitalized patient, ECG, blood pressure, oxygen saturation, and respiration are basically measured to monitor the patient's condition. Opening of a blood vessel after it is occluded by pressurizing the cuff influences the blood flow of peripheral blood vessels as well as oscillation changes in the cuff. Blood vessels are occluded and peripheral blood flow disappears at cuff pressure above the examinee's blood pressure, while blood vessels are opened and peripheral blood flow appears again at cuff pressure under the examinee's blood pressure. Then Disappear-Appear Point Length(DAPL) of peripheral blood flow can be judged with the signal of peripheral blood flow, thus is available as a factor of error correction for electronic blood pressure meter. Also, systolic or diastolic blood pressure can be corrected with Appear-Point-Pressure(APP) of cuff pressure at a point where blood flow occurs and Appear-Maximum Pressure(AMP) of cuff pressure at the maximum amplitude point of peripheral blood flow after peripheral blood flow appears again. For verification, 27 examinees were selected, and their blood value was obtained through experimental procedure of 4 stages including induction of blood pressure change. The examinees were divided into two groups of experimental group and control group, regression analysis was conducted for experimental group, and correction of a blood pressure error was verified with optical signal by applying the regression equation calculated in experimental group to control group. As an experimental result, mean of the whole measurement errors was 5mmHg or more, which did not meet the standard fur blood pressure meter. As a result of correcting blood pressure measurements with data of DAPL, APP, and AMP as drawn out of PPG signal, systolic blood pressure, mean blood pressure, and diastolic blood pressure were $-0.6{\pm}4.4mmHg,\;-1.0{\pm}3.9mmHg$ and $-1.3{\pm}5.4mmHg$, respectively, indicating that mean of the whole measurement errors was greatly improved, and standard deviation was decreased.

자기조절프로그램이 고혈압환자의 혈압 및 자가간호행위에 미치는 효과 (Effects of a Self-regulation Program on Blood Pressure Control and Self-Care in Patient with Essential Hypertension)

  • Purpose: This study was done to verify the effects of a self-regulation program for management of hypertension. Method: Thirty patients with hypertension registered in a community health center were selected as the experiment group, and control group were patients in another community health center, matched for age and gender. The self-regulation program included daily blood pressure checks, periodic counseling, and health education. A self-check digital device with instructions was provided for self-monitoring of blood pressure, and the participants were interviewed before they took part in the program. Results: The first hypothesis was supported: There will be a greater reduction in both systolic and diastolic blood pressure for patients with hypertension who participate in the self-regulation program compared to patients in the control group. The second hypothesis was also supported: Patients with hypertension who participate in the self-regulation program will perform self-care activities better than those in the control group. Conclusion: The findings indicate that a self-regulation program reduces systolic and diastolic blood pressure and improves self-care in patients with hypertension. It is recommended that this self-regulation program be used in community health clinics for management of hypertension and prevention of complications.

Role of peripheral vestibular receptors in the control of blood pressure following hypotension

  • Hypotension is one of the potential causes of dizziness. In this review, we summarize the studies published in recent years about the electrophysiological and pharmacological mechanisms of hypotension-induced dizziness and the role of the vestibular system in the control of blood pressure in response to hypotension. It is postulated that ischemic excitation of the peripheral vestibular hair cells as a result of a reduction in blood flow to the inner ear following hypotension leads to excitation of the central vestibular nuclei, which in turn may produce dizziness after hypotension. In addition, excitation of the vestibular nuclei following hypotension elicits the vestibulosympathetic reflex, and the reflex then regulates blood pressure by a dualcontrol (neurogenic and humoral control) mechanism. In fact, recent studies have shown that peripheral vestibular receptors play a role in the control of blood pressure through neural reflex pathways. This review illustrates the dual-control mechanism of peripheral vestibular receptors in the regulation of blood pressure following hypotension.

지역사회 성인의 혈압측정횟수 및 연령에 따른 혈압의 차이 (Differences in Blood Pressure among Adults in the Community according to Blood Pressure Measurement Time and Age)

  • Purpose: This study aimed to identify the differences in blood pressure among adults in the community according to age and time of the blood pressure measurement. Methods: This was a secondary analysis study, using data from a 2015 community health survey, conducted by the Korea Centers for Disease Control and Prevention. The data of three-time-measured blood pressure were collected from 337 subjects, 25, 35, 45, 55, 65, 75 years old, which are median ages by each age group. Results: The primary systolic pressure was significantly higher than the secondary systolic pressure (t= 3.46, p= .001) and the tertiary systolic pressure (t= 4.83, p= .001). The secondary systolic pressure was higher than the tertiary measurement (t= 2.05, p= .041). There was no significant difference between the three-time-measured values for diastolic pressure. There was a significant interaction between measurement times and age in the systoic blood pressure readings (F= 1.95, p= .036). However, there was no significant interaction between measurement times and age in the diastolic blood pressure readings (F= 1.03, p= .418). Conclusion: The findings suggest that attention must be paid to the use of blood pressure values in studies or one-time-measured clinical blood pressure values. In particular, the differences in systolic pressure readings taken at different times in the older age groups were significant. Therefore, it is more important to carefully assess blood pressure in adults over the age of 45 compared to other age groups.

규칙적인 필라테스 운동이 심혈관 및 호흡 변인에 미치는 영향 (The Effects of Regular Pilates Exercise on Blood pressure and Pulmonary Variables)

  • 본 연구는 건강한 정상인의 필라테스 운동에 참여가 심혈관 및 호흡 변인에 미치는 영향을 분석하여, 필라테스 운동이 심폐계에 미치는 영향을 규명하고자 하였다. 건강한 정상인 36명을 무작위로 필라테스 운동에 참여하는 실험군과 대조군으로 나누었다. 필라테스 운동은 8주간 주 3회, 1회당 60분 동안 실시되었고 운동 전과 후에 혈압, 최대산소섭취량, 산소이용률, 그리고 무산소성 역치 등 심혈관 및 호흡계와 관련된 변인들을 측정하였다. 8주의 필라테스 운동을 시행한 실험군의 이완기 혈압을 제외한 나머지 심혈관 및 호흡 변인들이 필라테스 운동전에 비해 통계적으로 유의하게 상승되었으며, 대조군과의 비교에서도 수축기 혈압, 최대산소섭취량, 산소이용률, 그리고 무산소성역치가 통계적으로 유의한 차이가 나타났다. 본 연구의 결과에서 필라테스 운동은 심혈관 및 호흡 변인에 효과가 있으며 필라테스 운동이 유연성, 통증조절 및 재활 치료의 효과 이외에도 유산소 운동의 효과를 나타내는 것을 알 수 있다.

The Effect of Public Health Center-Based Hypertension School on Hypertension-related Knowledge, Self-efficacy, Anthropometric Value and Blood Pressure

  • This study is to identify the effects of hypertension management program at a community health center on the disease-related knowledge, self-efficacy, anthropometric measurements and blood pressure of a hypertensive patient in local community. This study is a quasi-experimental study using nonequivalence control group no-synchronized design in order to verify the effects of the hypertension management program at a community health center on the hypertension-related knowledge, self-efficacy, anthropometric measurements and blood pressure of a hypertensive patient in local community. The result indicated a significant difference between the experimental group and control group in the scores of hypertension-related knowledge (t=-4.25, p<.001), self-efficacy (t=-4.20, p<.001), systolic blood pressure (t=7.70, p<.001) and diastolic blood(t=5.91, p<.001), body weight(t=2.32, p=.026) and abdominal circumference(t=2.17, p=.036). The hypertensive patients' knowledge and self-efficacy were improved, and their weight and abdominal circumference as well as systolic blood pressure and diastolic blood pressure were reduced. Therefore, it was confirmed that multilateral approaches in terms of physical and psychosocial aspects only targeting hypertensive patients were required for managing hypertensive patients in local community.

중년 남성에서 고정상혈압에 의한 고혈압발생 위험 규명을 위한 코호트내 환자-대조군 연구 (A Nested Case-Control Study on the High Normal Blood Pressure as a Risk Factor of Hypertension in Korean Middle-aged Men)

  • Objectives : High-normal blood pressure' is a factor influencing decision to initiate targeted intensive intervention strategy in westernized populations. JNC-VI offered the vigorous lifestyle modification for persons with 'high-normal blood pressure', who could be early detected. As a hypertension seems to be the result of multiple genetic factors operating in concert with associated environmental factors, it will be necessary to identify the high-normal blood pressure as a risk factor of hypertension for applying primary prevention strategy in Korean people. Methods : Although cohort study design might be adequate to recruit incidence cases, to keep time sequence of events, and to prevent information bias, nested case-control study was chosen for avoiding measurement errors because hypertension is a benign disease. Source population was the 'Seoul Cohort' participants and follow-up was done by using Korea Medical Insurance Corporation's database on the utilization of health services from 1 Jan93 to 30Jun97. Incidence cases were ascertained through the chart review, telephone contacts, and direct blood pressure measurements. Controls included the pairing of 4 individuals to each case on the basis of age. Results : As 75% of 247 incident cases had high-normal blood pressure, the crude odds ratio for hypertension was 2.04 (95% CI 1.47-2.83). Another statistically significant risk factors of hypertension were body mass index, dietary fiber, alcohol consumption, weekly activity and history of quitting smoking. The multivariate odds ratio of high-normal blood pressure adjusted for all risk factors was 1.84 (95% CI 1.31-2.56). Among high-normal blood pressure group, body mass index, weekly ethanol amounts, weekly physical activity, and dietary fiber except history of quitting smoking were still risk factors of hypertension. Conclusion : 'High-normal blood pressure' is a risk factor for hypertension in Korean middle-aged men, which represents that the vigorous lifestyle modification for persons with 'high-normal blood pressure' is need.

복식호흡이 심장 수술후 36시간 이내 환자의 통증, 상태불안, 혈압 및 심박동수에 미치는 효과 (Effect of Abdominal Breathing on Postoperative Pain, State Anxiety, Blood Pressure, and Heart Rate of Patients Undergoing Cardiac Surgery)

  • Purpose: This study is to evaluate the effect of abdominal breathing on postoperative pain, state anxiety, blood pressure, and heart rate of cardiac surgery patients treated in intensive care unit. Methods: A non-synchronized design of nonequivalent control group pre-post test was used. Participants were divided into an experimental group(n=22) and a control group(n=21). Just usual postoperative treatments were provided to the control group, while abdominal breathing interventions were provided to the experimental group, in addition to usual treatments. The intervention was consisted of 4 stages-introductions, breathing perception, breathing training, and closing-and it lasted twelve minutes in total. Pain, state anxiety, blood pressure and heart rate were the dependent variables of the abdominal breathing. Results: Those who carried out abdominal breathing showed a significant reduction in pain, as well as decrease of systolic blood pressure. However, it turned out to have no effect on state anxiety, diastolic blood pressure and heart rate. Conclusion: Abdominal breathing is simple and harmless and is effective for reducing postoperative pain.

아로마 향기요법이 자궁근종 여성의 수술전 불안, 혈압 및 맥박에 미치는 효과 (Effect of Aroma Therapy on the Anxiety, Blood Pressure and Pulse of Uterine Leiomyoma Patients before Surgery)

  • Purpose: The study was performed to evaluate effects of aroma therapy on the anxiety, blood pressure and pulse of uterine leiomyoma patients before surgery and to apply it as the actual nursing intervention. Method: From Aug1st to Nov. 30th, 2007, the experimental group (n=30) and the control group (n=30) were selected in 1 hospital, Busan. Lavender and Bergamot oil mixed together at a 3:2 ratio was applied to the experimental group two times at 8-9 pm the day before surgery and twice 30 minutes before the surgery. Anxiety, VAS, blood pressure, and pulse were measured to evaluate the effects of aroma therapy. The data analysis was done using the SPSS/PC 12.0 program using chi-square test and t-test. Results: Hypothesis 1 was supported. The VAS of the experimental group was lower than control group after aroma therapy. Hypothesis 2 was partly supported. The systolic blood pressure and pulse of experimental group was significantly decreased, but diastolic blood pressure wasn't difference. The VAS, systolic blood pressure and pulse of control group were increased. Conclusion: The study verified that the aroma therapy is a very effective nursing intervention that can reduce anxiety before surgery. It is recommended to apply aroma therapy actively to patients experiencing anxiety before surgery procedure.
