Total Spinal Block for Treatment of Post-traumatic Cervical Syndrome (외상성 경부 증후군의 치료를 위한 전척수신경차단의 효과)

  • In March of l992, We performed two intentional total spinal blocks for the relief of pain. This was for 2 cases of post-traumatic cervical syndrome whose various symptoms were chronically unresponsive to the usual conservative treatments. We regularly checked the blood pressure, pulse rate, oxygen saturation and observed clinically the changes of respiration, consciousness, lid and light reflexes during the total spinal block. Pain relief was evaluated by using the Visual Analog Scale which is designed to measure the subjective intensity of pain. The results were as follows; 1) The effectiveness of total spinal block was 60% in case 1, 40% in case 2. 2) We observed two complications from the procedure. Firstly during the block, these were transient periods of hypotension. Following the block, accidental procedures related direct neural trauma resulted in anterior chest wall pain. In conclusion we believe that total spinal block is a satisfactory and reliable method for the treatment of post-traumatic cervical syndrome.

  • In this paper, experimental and numerical attempts are made fur application of powder metallurgy forming technology to making the cylinder block of a hydraulic pump of which height reaches nearly 70 mm and is ten times larger than the wall thickness. Leak tests with several compositions are carried out in order to find allowable powder composition to prevent leak under high pressure in service and CAE techniques are applied to finding proper process conditions. Through the research, the possibility of the powder formed cylinder block that is very competitive from the point of both cost and mass production has been shown, even though its thickness exceeds the recommended limit considering heterogeneous density distribution caused by the friction between a powder compact and dies.

  • In the ordinary reinforced earth wall, which was constructed by incremental construction method, the horizontal deformation of the facing due to the compaction induced horizontal earth pressure was unavoidable. Thus the KOESWall system which are adopted the isolated construction method was developed by I&S Eng. Co., Ltd. in 1999. Due to its systematical feature, KOESWall system is able to minimizes the horizontal deformation of reinforced wall effectively and it can be used as temporary structures more economically without the lacing block. In this report, it is shown that the concept and case histories of KOESWall system as a retaining structures.

  • In this study, the simulated cyclic train loading test was carried out in order to investigate the dynamic behavior in/at the block type reinforced earth retaining wall. The results in this test were compared with unreinforced and reinforced case, respectively. It was shown that we confirmed the correlation between earth pressure and displacement, the confining effect of wall displacement by the effect of geogrid.

  • Disaster robots have accepted tele-operation in order to share the intelligence of human operators and robot systems. Virtual wall is one of the tele-operation technology to support recognition of human operator. If the virtual wall can block the robot from dangers, the operator will feel comfortable and can concentrate on fundamental missions. In this paper, we proposes and compares three methods for virtual wall visualization in tele-operation using 3D reconstruction. First is a virtual wall visualized only with edges. A wall filled with transparent color is the second method. Finally, third method is a texture-mapped virtual wall. In the experiments, we discuss their merits and demerits in view of robot tele-operation.

  • In this study, the performance of the cut-off wall with the specific functional adsorption layers(containing SAC), which are formed in order to block harmful materials such as heavy metal ions contained in leachate (or outflow water) from either waste landfills or exhausted mines, was Investigated by determining experimental data such as hydraulic conductivities, unconfined compression strengths, adsorption capacities. The performance was compare to those of the present cut-off wall materials such as clay, bentonite-mixed soil, and soil-cement.

  • The use of geogrid for SRW systems and bridge abutment has increased rapidly over the past 10 years in Korea. The concept of segmental retaining walls and reinforced soil is very old and for example The Ziggurats of Babylonia(i.e. Tower of Babel) were built some 2,500 to 3,000 years ago using soil reinforcing methods very similar to those described in current design. Modern SRW(Semental Retaining Wall) units were introduced in 1960's as concrete crib retaining wall systems. In this paper, the friction properties between segmental concrete units and geogrid are investigated by performing various tests.

  • Allowable level of blasting vibration for earth retaining wall was examined in this study. Blasting vibration was measured at near field blasting to evaluate the influence of the blasting work to earth retaining wall and rear ground. Although small scale blasting with $0.5{\sim}2.0kg$ explosives per round merely influenced to the structure and ground, but it was suggested to blast at the distance of twice the least burden considering the block movement.

  • Epidural block under general anesthesia has been widely used to control postoperative pain. In this anesthetic state many hemodynamic parameters are changed. Moreover pulse transit time is influenced by this memodynamic change. m change in the finger and the toe due to relaxation of arterial wall muscle after general anesthesia and epidural block under general anesthesia. This study, in the both general anesthesia and epidural block under general anesthesia, ${\Delta}PTT$ of the toe and of the finger are measured. In addition, ${\Delta}PTT$(toe-finger) of the epidural block under general anesthesia and of the general anesthesia were compared.

Cryoanalgesia vs Bupivacain Intercostal Nerve Block for the Post -thoracotomy Pain (개흉술 후 흉부동통억제를 위한 늑간신경냉동 요법과 약물주입요법에 관한 비교연구)

  • Post-thoracotomy pain is so severe that lead to postoperative pulmonary complications due to the patient`s inability to cough, deep breathing and chest wall motion. Many authors have been trying to reduce the post-thoracotomy pain, but there is no method of complete satisfaction. In 1990, we reported the result that the cryoanalgesia reduce the immediate postoperative pain significantly. We try to compare the effect of cyroanalgesia with the effect of Bupivacain intercostal nerve block for the post-thoracotomy pain control. Ninety patients, who undergoing thoracotomy in Korea University Medical Center Guro Hospital between the January 1993 and September 1993, were evaluated. The patients were divided into three groups : Group A, control, the patients without pain control procedure [N=30], Group B, the patients with Bupivacain intercostal nerve block [N=30], Group C, the patients with cryoanalgesia [N=30]. Postoperative analgesic effects were evaluated by the scoring system which made arbiturary by author. The results were that the Bupivacain intercostal nerve block markedly reduced the immediate postoperative pain compare with two other groups, the cryoanalgesia reduced the immediate postoperative pain significantly compare with control group, the pain reduction effect of the two groups- cryoanalgesia and Bupivacain intercostal nerve block- were not different at postoperative seventh day, probably due to the action time of Bupivacain and the result that there were no significant complications of the procedures.
