바로 선 자세에서 발목과 무릎관절의 고정이 자세안정성에 미치는 영향 (Effects of Immobilization of the Ankle and Knee Joints on Postural Stability in Standing)

  • This study was designed to examine the effects of temporary immobilization of the ankle and knee joints on standing in healthy young adults with the use of a postural control mechanism. The subjects were twenty-four college students (12 males and 12 females, aged between 20 and 28). A Biodex balance system SD 950-302 and its software were used to measure indirect balance parameters in standing. Each subject underwent postural stability tests in 4-different joint conditions: free joints, ankle immobilization only, knee immobilization only, and ankle and knee immobilization. In addition, the postural stability test was conducted once with the subject's eyes open and once with the eyes closed conditions. For data analysis of the postural stability tests, the overall stability index, antero-posterior stability index, and medio-lateral stability index were recorded. The overall stability index (p=.000) and medial-lateral index (p=.003) were significantly greater different conditions with eyes closed in postural stability. Therefore, the eyes closed condition is expected to be used as an effective postural stability training for treatment planning in patients with unstable postures. In addition, training based on the dynamic multi-segment model can improve postural stability and is available to therapeutic programs, helping people with unstable balance to reduce their risk of falling.

한쪽 상지의 D2 패턴 운동동안 반대측 상지, 하지 및 체간 근육의 활성도에 미치는 영향 (The Effects of Contralateral Upper and Lower Limb and Trunk Muscle Activation During Ipsilateral Upper Limb D2 Pattern Exercise)

  • Purpose: The aim of this study was to examine the activation of the contralateral upper and lower extremities and trunk muscle during ipsilateral upper extremity diagonal isokinetic exercise. Methods: Twenty-one healthy male subjects with no history of shoulder injury undertook ipsilateral diagonal isokinetic exercise at 60, 120, and $180^{\circ}/sec$, utilizing a standard Biodex protocol. Muscle activation amplitudes were measured in the upper trapezius, pectoralis major, biceps brachii, rectus abdominis, external oblique, rectus femoris, adductor longus, and biceps femoris muscles using electromyography. A one-way analysis of variance and paired t-tests were conducted, and the data were analyzed using SPSS, version 21.0. Results: The results revealed no statistically significant interaction between motion and angular velocity and no statistically significant contralateral muscle activation according to angular velocity (p>0.05). However, they revealed statistically significant contralateral muscle activation according to motion (p<0.05). Conclusion: These results suggest that the movements involved in contralateral upper extremity diagonal isokinetic exercise can enhance muscle strength in patients affected by stroke, fracture, burns, or arthritis.

기구운동과 복합운동이 발목염좌 경험이 있는 태권도학과 학생의 발목 안정성에 미치는 효과 (The Effects of Board Training and Complex Training on Ankle Stability in Taekwondo Students with a History of Ankle Sprain)

  • Purpose: The purpose of this study is to compare the effects of board training and complex training on ankle stability in taekwondo college students with a history of ankle sprain. Methods: Twenty-seven taekwondo college students were randomly assigned into a board training (BTG, n=9), complex training (CTG, n=9), or control groups (CG, n=9). BTG carried out disk and trampoline training 3 times a week for 8 weeks. CTG carried out resistive and plyometric training 3 times a week for 8 weeks. All subjects completed ankle stability test for static and dynamic balance in anterior/posterior and medial/lateral stability with New Balance System (BIODEX Medical System, USA). Results: There were no significant differences between three groups in ankle stability. Those who participated in BTG significantly improved static balance of left foot in anterior/posterior stability, and dynamic balance of both feet in mediolateral stability, but there were no change in CTG. Conclusion: Board training is more improved ankle stability in taekwondo college students with a history of ankle sprain than complex training.

치료면의 질에 따른 복합균형운동이 정상성인의 균형능력에 미치는 영향 (The Effects of Complex Balance Exercise Based on Altered Surface Conditions on Balance Ability in Healthy Adults)

  • 본 연구의 목적은 치료면의 질에 따른 복합균형운동프로그램이 균형능력에 어떠한 영향을 미치는지 알아보기 위해 본 연구를 실시하였다. 정상 성인 21명을 대상으로 연구를 진행하였으며 복합균형 운동을 실시한 Aero-step군(ASG)과 평지면군(Flat floor group;FFG) 그리고 처치 없이 실시한 대조군에게 Biodex balance system을 이용하여 전체안정성지수(Overall stability index;OSI), 전후안정성지수(Anteroposterior index;APSI), 좌우안정성지수(Mediolateral stability index;MLSI)를 측정하였다. 본 연구 결과에서 전체안정성지수와 전후안정성지수는 Aero-step군의 그룹 내 비교에서 유의한 차이가 있었으며 좌우안정성지수는 평지면군의 그룹 내 비교에서 유의한 차이가 있었다. 전후안정성지수의 그룹간 비교에서 유의한 차이가 있었다. 본 연구의 결과에 따라 치료면의 질에 따른 복합균형운동이 안정된 지지면 보다는 불안정한 지지면의 균형능력 향상에 더 효과적이라 사료된다.

테이핑과 신장운동이 슬관절 굴곡근과 신전근의 근력 및 근지구력에 미치는 영향 (The Effect of Taping and Stretching Exercise on Muscle Strength and Endurance of Knee Flexors and Extensors)

  • Purpose : to investigate change of isokinetic muscle strength and isokinetic muscle endurance of quadriceps femoris and hamstring after application of taping and static stretching. Methods : For this study, 14 male members of college soccer team participated in this research. Participants were divided into two groups: seven was assigned to a taping group(n=7) and the others were assigned to a static stretching exercise group(n=7). Main outcome was measured by using biodex. The isokinetic muscle strength test was processed at the speed of the $60^{\circ}$/sec five times, and isokinetic muscle endurance was processed at the speed of $180^{\circ}$/sec ten times. Results : From the outcomes of the measurement, the results show that there is a more significant difference in the taping group than in a static stretching exercise group. Conclusion : These results will be utilized for the preliminaries in the future.

전동식 운동기기의 부하방식에 따른 근력증진 효과 (Effect on Improvement of Muscle Strength for Loading Pattern using Electric Exercise Instrument)

  • This study is to compare muscle strength between isotonic exercise and isotonic & isokinetic exercise. Participants are 12-man and 10-woman whom they are healthy without medical history in shoulder, elbow and lumbar joint. We performed experiment total 4-weeks that exercise 3-days a week each exercise pattern. We measured shoulder, elbow and lumbar joint torque with BIODEX and circumference of muscle in upper arms once a week. The result showed that isotonic & isokinetic exercise pattern significantly more improved joint torque in shoulder, elbow, lumbar than isotonic exercise pattern. Because that isotonic & isokinetic exercise pattern supplied muscle strengthen and caused muscle contraction. This exercise pattern can be used new exercise training method for major athlete and normal people. Also this pattern can be used rehabilitation treatment.

  • The purpose of this study was to suppose basis data the influence of different chair type and pelvic control on quadriceps muscle activity and strength during knee joint extension isometric exercise in hemiplegic patients. This research were investigated in ten healthy adults and tens hemipelgic patients. Surface electromyography (EMG) and Biodex system were used to collect kinematic data and muscle activity, respectively. Independent t-test, paired t-test and one-way repeated ANOVA were used to determine a statistical significance. The results showed as follows: (1) Posterior pelvic angles in healthy group and hemiplegic group were significantly different on isokinetic equipment (p<.05). (2) Different chair type and pelvic control on quadriceps muscle activity and strength were significantly different in hemiplegic patients (p<.05). From the result of this research, posterior pelvic angle control during knee joint extension isometric exercise in hemiplegic patients on isokinetic equipment is necessary to increase quadriceps muscle strength in hemiplegic patients.

The Effects of Vibration Exercise after Modified Bröstrom Operation in Soccer Players with Ankle Instability

  • Background: Vibration exercise after ankle surgery improves proprioception and ankle muscle strength through vibration stimulation. Objective: To examine the effects of vibration exercise on the ankle stability. Design: Randomized controlled clinical trial (single blind) Methods: Twenty soccer players were randomly divided into experimental group and control group. The Vibration exercise program was conducted 12 weeks and 3 times a week. Ankle joint proprioceptive sensory test and Isokinetic muscle strength test were performed using Biodex system pro III to measure plantar flexion / dorsiflexion and eversion / inversion motion. Results: The result of isokinetic test of ankle joint is showed significant improvement in all measurement items, such as leg flexion, lateral flexion, external and internal muscle forces, compared to previous ones by performing vibration movements for 12 weeks. However, in the comparison group, plantar flexor ($30^{\circ}$), eversion muscle ($120^{\circ}$), inversion ($30^{\circ}$) of limb muscle strength were significantly improved compared with the previous phase; was no significant difference in dorsi-flexion. There was no significant difference between groups in all the items. Conclusions: In this study, we analyzed the effects of rehabilitation exercise on soccer players who had reconstructed with an ankle joint ligament injury through vibration exercise device. As a result, we could propose an effective exercise method to improve the ability, and confirmed the applicability as an appropriate exercise program to prevent ankle injuries and help quick return.

The Effect of Lower Extremity Strengthening Exercise Using Sliding Stander on Balance and Spasticity in Chronic Stroke: A Randomized Clinical Trial

  • Purpose: Generally, patients with stroke present with decreased balance and increased spasticity following weakness of the paralyzed muscles. Muscle weakness caused by stroke has two causes. This is caused by a decrease in motor output and an adaptive muscle change, resulting in muscle weakness and muscle paralysis. The purpose of this study was to investigate the effect of strengthening exercise on balance and spasticity in chronic stroke patients and to suggest the basis of clinical treatment. Methods: Twenty subjects were divided into two groups: a lower-extremity strengthening group (experimental group) and a general physical therapy group (control group). The sliding stander equipment was used for the experimental group and a regimen of warm-up exercise, the main exercise routine, and cool-down exercise were used for the muscle strengthening exercise program. Balance and spasticity were measured before and after the training period. Balance ability was measured by the Berg balance scale, the Timed up and Go test and the weight distribution of the paralyzed muscles by the Spacebalance 3D. Spasticity was measured by the Biodex system. Results: After the training periods, the experimental group showed a significant improvement in BBS, weight distribution of the paralyzed muscles, and decreased spasticity when compared to the control group (p<0.05). Conclusion: This study supported the hypothesis that lower-extremity strengthening exercise improves the balance and decreases the spasticity of stroke patients. If it is combined with conventional neurologic physiotherapy, it would be effective rehabilitation for stroke patients.

압력 바이오피드백 제공에 따른 고유수용성신경근촉진법 하지패턴 적용이 뇌졸중 환자의 근력과 보행능력에 미치는 영향 (The Effects of Pressure Biofeedback Units in Lower-Limb PNF Pattern Training on the Strength and Walking Ability of Stroke Patients)

  • Purpose: The purpose of this study was to compare the strength and walking ability of chronic stroke patients following either proprioceptive neuromuscular facilitation (PNF) pattern training with pressure biofeedback units (feedback group) or PNF pattern training without pressure biofeedback units (control group). Methods: Eighteen participants with chronic stroke were recruited from a rehabilitation hospital. They were divided into two groups: a feedback group (n = 8) and a control group (n = 10). They all received 30 minutes of neurodevelopmental therapy and PNF training for 15 minutes five times a week for three weeks. Muscle strength and spatiotemporal gait parameters were measured. Muscle strength was measured by hand-held dynamometer; gait parameters were measured by the Biodex Gait trainer treadmill system. Results: After the training periods, the feedback group showed a significant improvement in hip abductor muscle strength, hip extensor muscle strength, step length of the unaffected limb, and step time of the affected limb (p<0.05). Conclusion: The results of this study showed that proprioceptive neuromuscular facilitation pattern training with pressure biofeedback units was more effective in improving hip muscle strength and walking ability than the proprioceptive neuromuscular facilitation pattern training without pressure biofeedback units. Therefore, to strengthen hip muscles and improve the walking ability of stroke patients, using pressure biofeedback units to improve trunk stability should be considered.
