A Study on Gak-Gi(脚氣) in Comparison with Beriberi (각기(脚氣)에 대한 고찰(考察) -Beriberi와의 비교를 중심으로-)

  • Objectives :The Korean Medical concept Gak-Gi(脚氣) is understood as being identical to Beriberi and Vitamin B1 deficiency, without the necessary investigation that is required in such identification. Therefore this study aims to systematically compare these two concepts for thorough investigation. Methods : Research was carried out in two directions. First, the cause, symptoms, treatment principles of Gak-Gi(脚氣) in the medical texts were analyzed. Second, the process of the Beriberi theory development and its identification with Gak-Gi(脚氣), medical information on Beriberi, and the basis of various diseases that have been linked to Gak-Gi(脚氣) were studied. Results of the two directions were used to compare Gak-Gi(脚氣) and Beriberi. Results : Gak-Gi(脚氣) and Beriberi overlap in many aspects. However, unlike Beriberi which is understood as nutrient deficiency essentially, Gak-Gi(脚氣) can be caused by exterior pathogens such as wind toxin, while its different manifestations and treatment principles cannot be fully explained by nutrient deficiency. Conclusions : Gak-Gi(脚氣) could be understood as a type of syndrome in which causes and symptoms are connected, where symptoms start in the legs, moving upwards. It cannot be identified with Beriberi, which has been interpreted as a single disease.

  • After this study: I report the following result from it. 1. Beriberi is defined as sensory or motor disorder of leg with a painful-swelling and pyrexia. If it progress severely, the disease transmits to heart and cause numbness of the lower abdomen, vomiting, anorexia, palpitation, chest distress and confusion. 2. The external etiologic factors of Beriberi are wind-toxic pathogen, dampness, improper diet and climate sickness, the internal etiologic factors are deficiencies of the kidney, both Gi and blood and original vital energy. 3. The treatment of Beriberi is free-going of blockage, because it is a blocked disease. 4. Danggwiy-juntongtang and Ganghwar-dochetang is the most in the order of frequency of use. Mahwang-juagyongtang, Banha-juagyongtang, Daehwang-juagyongtang, Shinbi-juagyongtang are used in sequence. 5. The prescriptions of Beriberi are made up of the herbs that clear heat, for example, Ganghwar, Changchul, Mahwang and Bangpung, and the herbs that reinforces Gi, for example, Insam and Bakchul.

  • Beriberi, which is caused by thiamin deficiency, is a rare disease in recent years. But it has been described in the Eastern literature as far back as the 17th century. Early symptoms are fatigue, apathy, irritability, drowsiness, anorexia, nausea, vomiting and abdominal pain. Signs and symptoms of progression are peripheral neuritis, paresthesia, decreased tendon reflex and congestive heart failure. Thiamin deficiency remains as an important health care issue in many world population, specially in AIDS, pregnancy women and TPN associated patients. The best diagnostic test is assessing clinical response to administration of thiamin. We have experienced a case of Beriberi caused by continuous rice-soup feeding during one and half years in a 7-year-old boy.

  • After We treated Beriberi disease belonging to the argument of interior-overheating-symptom of the Tae-Eum-In caused by liver's receiving heat due to Seven emotional excess with Constitutional prescription. we had a Significant effect on patient's Character and Beriberi disease. Therefore, if we add some Herbs to Constitutional prescription according to patient's Symptoms. we think we can maximize The Effect and Application of Constitutional prescription.Through this Clinical analysis. We think we need a Systematic study on applying This Principle to more Various diseases.

Cardiovascular beriberi: rare cause of reversible pulmonary hypertension

  • Cardiovascular beriberi is caused by thiamine deficiency and usually presents as high cardiac output failure associated with predominantly right-sided heart failure and rapid recovery after treatment with thiamine. Because of its rarity in developed countries, the diagnosis can often be delayed and missed. We recently experienced a case of cardiovascular beriberi with pulmonary hypertension which successfully treated with thiamine infusion. A 50-year-old man with chronic heavy alcoholics was refered to our department for dyspnea with mental change. Echocardiography showed marked right ventricular (RV) dilatation and flattening of the interventricular septum with a D-shaped deformation of the left ventricle. Moderate tricuspid valve regurgitation was found and estimated RV systolic pressure was 52 mm Hg. Because of his confused mentality and history of chronic alcohol intake, neurological disorder due to thiamine deficiency was suspected and intravenous thiamine was administered and he continuously received a daily dose of 100 mg of thiamine. Follow up echocardiography showed marked reduction of RV dilatation and improvement of a D-shaped deformation of the left ventricle. He finally diagnosed as cardiovascular beriberi on the basis of dramatic response to intravenous thiamine. Thiamine deficiency can cause reversible pulmonary hypertension, and can still be encountered in the clinical setting. Thus high index of suspicion is critically needed for diagnosis.

  • Gak-gi-byung in Korean Medicine have many things in common with Beriberi disease, Guillain-Barre syndrome, Transverse myelitis and Cauda equina syndrome. Dong-Ui-Bo-Gam define Gak-gi-byung as syndrome that includes sudden lower limbs weakness, pain and edema. Gak-gi-byung start from the foregoing symptoms and could progress to general digestive, neurological or respiratory symptoms is found on many medical practitioners. In this case, we described a 60-years old man diagnosed as Gak-gi-byung in Korean Medicine. He complained Rt lower limb weakness, Rt Knee Clumsiness and limitation of his Rt knee & ankle motor. And his condition was improved through Korean Medical treatment such as acupuncture and herbal medicine like 'Chung-yul-sa-seup-tang' about 15 days.

  • Prunus serrulata var. spontanea(Rosaceae) is a large sized tree widely distributed throughout Korea. The red fruits are edible and are used in traditional folk medicine against heart failure from beriberi. dropsy. mastitis, an emmenagogue, and toothache. Also. the bark of P. serrulata var. spontanea has been used for detoxification. and relaxation and as an antitussive in traditional Korea medicine. (omitted)

  • Saururus Chinensis Baill (Saururaceae) has been used as folk medicine for analgesics, beriberi, edema, hepatitis, and icterus, etc. Hepatoprotective effects of Saururus chinensis Baill (SCB) administration on function of the biochemical parameters in liver of 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) treated rats were investigated. After 7 days from TCDD(1$\mu\textrm{g}$/kg) injection, SCB(200mg/kg) was administered into rats intraperitoneally for 4 week.s We examined the antioxidative enzymatic activity by measuring the level of AST and ALT in serum and SOD, Catalase, GPx, GSH and GSSG in liver tissue of rats. Pharmacognostical Studies on the "Nok Je Cho" ("녹제초"의 생약학적 연구)

  • Chinese crude drug "Lu-ti-cao"(鹿蹄草) has been used as a remedy for beriberi, cut bleeding, snake bite, etc. In Korea, the drug is called "Nok Je Cho" and has been used as a folk medicine of the same effect as that of Chinese equivalent. The original plant of this drug has not yet been as certained. To clarify the botanical origin of "Nok Je Cho", the morphological and anatomical characteristics of the leaves of Pyrola species growing in Korea, i.e. P. incarnata, P. japonica, P. minor, P. renifolia, P. secunda were studied. As a result, it was clarified that "Nok Je Cho" was the leaf of Pyrola japonica and Pyrola minor.

Pharmacognostical Studies on "Ga Si O Gal Pi" (가시오갈피의 생약학적 연구)

  • "Ga Si O Gal Pi" is one of the Korean crude drugs used mainly to cure arthritis, acute gastritis, and beriberi. With regard to the botanical origin of "Ga Si O Gal Pi", it has been considered to be Acanthopanax species of Araliaceae, but there was no pharmacognostical confirmation on it. To clarify the botanical origin of "Ga Si O Gal Pi", the anatomical characteristics of the branch of Acanthopanax species growing wild in Korea and Japan, A. chiisanensis, A. divaricatus, A. koreanum, A. rufinerve, A. senticosus, A. seoulense, A. sessiliflorus, A. sieboldiaum were studied. As a result, it was clarified that "Ga Si O Gal Pi" was the branch and rarely the bark of branch of Acanthopanax senticosus.
