Bell's palsy is not a serious disease and recovery is spontaneous in more half of the patients, but this recovery is long delayed in many instances and the cosmetic effect of an altered facial appearance is psychologically distressing. The etiology is unknown, but Kettle's ischemic hypothesis has been widely accepted. The aim of treatment is to reduce edema and improve circulation to the facial nerve. Stellate ganglion block (SGB) resulted in abolishing cerebral vascular spasm and in increasing cerebral blood flow. Thus, stellate ganglion block is effective in treatment of Bell's palsy. From 1978 to Oct. 1988, we have treated 222 patients (20 patients were recurred cases) with facial palsy by SGB and analysed the effect of SGB. We noticed that SGB was very effective in early repetitive treatment (90% recovery). This study is reported and the literature is reviewed.

Objectives : The purpose of this study is to find out proper method for clinical application of acupoints in Bell's palsy patients by using of digital infrared thermographic imaging(DITI). Methods : We investigated 44 cases of patients with Bell's palsy and checked thermal differences between abnormal and normal acupoint sites by using of DITI. Then we drew a comparison between acupoints of DITI and HBGS, ENoG, NET. We used Pearson's correlation for statistical analysis. Results : The HBGS was significantly correlated with ENoG, NET. The thermal difference of ST3 was significantly correlated with HBGS, Lip and Buccal abnormal site of NET. And the thermal differences between ST3 and GB21 were significantly correlated with ENoG, Forehead abnormal site of NET. Conclusions : Severity of Bell's palsy can be evaluated through DITI.

Objective: The aim of this research was to investigate the effect of acupuncture and the effect of Dong's acupuncture on Bell's palsy. Methods: We measured the facial palsy changes of the patients who were admitted for Bell's palsy in the Oriental Medical hospital of Dongeui medical center from 07-01-2003 to 09-30-2003. Bell's palsy patients were divided into two groups. One group(A group) was treated by acupuncture. The other group(B group) was treated by Dong's acupuncture. The effects of these treatment was evaluated by Yanagihara's unweighted grading system. Results : B group was marked more higher than A group in treatment outcome. we discovered that it is significant differences between two groups after 2 week. Conclusion : These results provided that A group is more effective than B group on. For clearly comparing the Effect of Acupuncture and Dong's Acupuncture on Bell's palsy, it is need more sample's number and more treatment's duration.

Background: Bell's palsy produces a complex problem that involves not only facial motor weakness, but also psychiatric issues. However, the relationship between facial neuromotor system impairment and psychological adjustment has not been well understood. Methods: We have performed psychological evaluations in patients with acute unilateral Bell's palsy within 2 weeks after onset. Thirty patients with Bell's palsy (10 men, 20 women) were included, who were diagnosed by neurologic examination, electrophysiologic study and/or brain MRI. We measured facial motor scale of impairment (House-Brackmann, HB scale) and psychosocial adjustment [Beck Anxiety Inventory (BAI), Beck Depression Inventory (BDI)] at the time of initial presentation and 1 month after diagnosis. Results: The age of the enrolled patients ranged from 16 to 80 years. The mean grade of initial and follow up HB scale were 3.87 (SD: 0.63, range 2~5) and 1.77 (SD: 1.10, range 1~5). The mean score of initial and follow up BAI, BDI were 11.93 (range; 0 to 47, SD: 9.65, very low anxiety), 14.73 (range; 0 to 41, SD: 9.21 minimal depression) and 7.5 (range; 0 to 36, SD: 8.58, very low anxiety), 9.33 (range; 0 to 30, SD: 8.19 minimal depression). There was positive correlation between improvement of HB scale and improvement of BAI and BDI score. Conclusions: Bell's palsy is associated with the psychological problems such as depression and anxiety, and the improvement of motor symptom is associated with the improvement of these psychological problems.

Objectives : The aim of this study was to report Bell's palsy patients with postauricular pain treated by Calculus Bovis Fel Ursi Moschus(BUM) Pharmacopuncture. Methods : The 6 patients were treated by the Korean medica treatment including herbal medication, dry needle acupuncture and physical therapy. We performed BUM Pharmacopuncture in Yepung for the treatment of postauricular pain once or twice. We have evaluated the effect of BUM Pharmacopuncture by Visual Analog Scale(VAS). Results : After BUM Pharmacopuncture for the treatment of postauricular pain, VAS were decreased in all cases. Conclusions : It was suggested that BUM Pharmacopuncture might have effect on control of auricular pain in Bell's palsy. Further, systemic studies will be needed to evaluate obvious effect of BUM pharmacopuncture on postauricular pain of Bell's palsy.

Objectives : The aim of this study was to investigate the effect of oriental medical treatment on Bell's palsy patients receiving chemotherapy. Methods : A clinical study was done on 3 patients who were diagnosed and treated as Bell's palsy at the Dep. of Acupuncture and Moxibustion College of Oriental Medicine Daejeon University. The effects of combined treatment of acupuncture, herbal medicine and physiotherapy were evaluated with Yanagihara's unweighted grading tool. Results : 1. There was significant improvement on Yanagihara score after treating with Gunbiikqi(健脾益氣) method, which was based on pattern differentiation as Hur(虛證). 2. Three patients commonly complained heaviness, dyspepsia, anorexia after receiving chemotherapy and oriental medical treatment showed significant improvement on the symptoms. 3. All three cases received herbal medicine treatment and liver function test. Three cases showed normal range of liver function during and after the treatment. Conclusions : Oriental-medicine treatment did not damage in liver function and showed significant improvement on Bell's palsy in patients receiving chemotherapy. But it is considerd that we need to keep limits in getting enough patients.

Purpose: The purpose of this study is to report the effect of oriental treatment to Bell's palsy occurred during pregnancy. Methods: We treated 28 year old woman and 31 year old woman who had the Bell's palsy occurred during pregnancy. 2 patients were treated with oriental treatment such as herb medicine and acupuncture therapy. The effects of these treatment was evaluated by Yanagihara's unweighted grading system. Results: As a result, symptoms were remarkably alleviated and Yanagihara score was improved. Conclusion: According to this result, we concluded the oriental treatment could improve pregnant Bell's palsy patient. After this paper, further study and clinical approach based on oriental treatment will be needed about pregnant Bell's palsy patient.

Objective : The purpose of this study is to analyze research trends on facial synkinesis after bell's palsy in order to build stepping stone in the hope of further related study in Korean medicine society. Methods : We searched Pubmed with the title "facial synkinesis" only developed after bell's palsy for the last 10 years and analyzed their impact factors, interventions, types of study, published countries, journals and years. Results : We found 12 papers and interventions included in each studies turned out to be effective and showed meaningful outcomes, but, the number of papers is still inadequate though, constantly published annually. And we were unable to find any studies with oriental treatments. Conclusions : Further attention and studies are needed on this topic in Korean medicine society in order to search appropriate oriental clinical care for the bell's palsy patients suffering facial synkinesis.

The effect of combined Korean medicine treatment including facial chuna manual therapy was retrospectively investigated in patients with Bell's palsy. There were 16 patients (14 males) who received 2 months outpatient treatment following > 14 days of hospitalization. The Yanagihara score and House-Brackmann grade were used to measure the severity of symptoms and recovery. The patients (mean age 58.5 ± 24.74 years) were categorized based on their medical history of hypertension (n = 10), diabetes millitus (n = 10), or oral intake of steroids early in the diagnosis of Bell's palsy (n = 10). There was no difference between patients who had or did not have a previous history of diabetes mellitus or oral steroid. Both the Yanagihara-score and House-Brackmann grade improved significantly in the total patient sample after treatment (p < 0.001). Combined Korean medicine including facial chuna manual therapy may have a beneficial effect on patient symptoms and recovery.
