비디오 디브리핑을 이용한 기본소생술 시뮬레이션 교육이 간호학생의 비기술적 술기와 기술적 술기 수행능력에 미치는 효과 (Effects of Simulation-based Training for Basic Life Support Utilizing Video-assisted Debriefing on Non-Technical and Technical Skills of Nursing Students)

  • Purpose: The purpose of this study was to investigate the effects of simulation-based training (SBT) for basic life support (BLS) utilizing video-assisted debriefing (VAD) about non-technical skills (NTSs) and technical skills (TSs). The goal of the proposed study is the evaluation of a teaching method about the correct application of cardiopulmonary resuscitation (CPR). Methods: The study design was a control group pre- and post-test non-synchronized experimental design. The sample included twelve teams of 36 nursing students. Both the experimental and the control groups received the SBT for BLS. Only the experimental groups received VAD where as the control groups had a verbal debriefing. Raters who used checklists for TSs and NTSs evaluated both groups. Data were analyzed by the SPSS 20.0 using Cronbach's ${\alpha}$, Intraclass Correlation Coefficient (ICC), Mann-Whitney U test and Willcoxon signed rank test. Results: The experimental groups scored higher than the control groups in both TSs (p=.004) and the NTSs (p=.008). Conclusion: The findings of this study suggest that NTSs are an important factor that lead CPR successfully, so VAD can be used as an efficient teaching-learning strategy in the SBT for BLS for nursing students and nurses.

기본심폐소생술 교육프로그램이 중학생의 기본심폐소생술 관련 지식, 태도 및 수행능력에 미치는 효과 (Effects of Basic Life Support Education Program in Middle School Students' Basic Cardiac Life Support Knowledge, Attitude and Performance)

  • 본 연구의 목적은 기본심폐소생술 교육프로그램이 중학생의 심폐소생술 관련 지식, 태도 및 수행능력에 미치는 효과와 그 지속성을 확인하는 것이다. 연구대상자는 D시에 소재하는 2학년 2개 학교를 대상으로 실험군과 대조군 각각 35명, 총 70명으로, 자료수집기간은 2012년 7월부터 2011년 11월까지였다. 자료 분석은 SPSS WIN 21.0을 이용하여 빈도와 백분율, $x^2$-test, t-test, repeated measures ANOVA로 분석하였다. 기본심폐소생술 교육프로그램을 이용한 실험군은 지식, 태도, 수행능력이 교육 전보다 교육 후 대조군보다 유의하게 증가하였으며, 기본심폐소생술에 대한 태도와 지식은 교육 후 2개월까지 지속되었고, 기본심폐소생술에 대한 수행능력은 교육 직후 유의한 수준으로 감소하였다. 따라서 기본심폐소생술에 대한 지식, 태도, 수행능력을 유지하기 위해 학교 기본교육과정 속에서 기본 심폐소생술 교육은 물론 일정기간을 둔 재교육이 실시되어야 한다.

Pediatric advanced life support and sedation of pediatric dental patients

  • Programs provided by the Korea Association of Cardiopulmonary Resuscitation include Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), and Korean Advanced Life Support (KALS). However, programs pertinent to dental care are lacking. Since 2015, related organizations have been attempting to develop a Dental Advanced Life Support (DALS) program, which can meet the needs of the dental environment. Generally, for initial management of emergency situations, basic life support is most important. However, emergencies in young children mostly involve breathing. Therefore, physicians who treat pediatric dental patients should learn PALS. It is necessary for the physician to regularly renew training every two years to be able to immediately implement professional skills in emergency situations. In order to manage emergency situations in the pediatric dental clinic, respiratory support is most important. Therefore, mastering professional PALS, which includes respiratory care and core cases, particularly upper airway obstruction and respiratory depression caused by a respiratory control problem, would be highly desirable for a physician who treats pediatric dental patients. Regular training and renewal training every two years is absolutely necessary to be able to immediately implement professional skills in emergency situations.

A Study on the Effect of Basic Life Support Training on the First Responsive Police Officers

  • 이 연구는 경찰관에게 실시한 기본인명소생술 훈련의 효과를 증명하기 위해 실시되었고, 연구대상자는 K 경찰관 중 10명의 실험군과 10명의 대조군으로, 교육프로그램은 60분 이론과 30분 실습을 포함하였다. 기본인명소생술 술기 측정은 미국심장협회에서 제시한 응급 심혈관 치료의 가이드라인을 따랐다. 결과는 다음과 같다. 실험군에서 현장확인기술, 1차 평가수행기술 및 기본인명소생술 능력(심장압박, 인공호흡, 의학적 평가)이 대조군에 비하여 높게 나타났다. 결론적으로, 이 연구결과는 시뮬레이션 교육프로그램이 전통적인 이론실습강의보다 경찰관의 임상술기능력을 향상시키는데 효과적이라는 것을 증명하였다. 그러므로 시뮬레이션 교육프로그램을 심정지 응급치료에 대한 교육프로그램으로 적용하는 것을 제안한다.

간호학생 대상 시뮬레이션기반 기본소생술 반복교육의 효과와 지속성 (Effectiveness and Retention of Repeated Simulation-based Basic Life Support Training for Nursing Students)

  • Propose: This study was to investigate the educational effect and retention of repeated simulation-based basic life support (BLS) training for nursing students. Methods: A comparison group design with pretest and posttest was used. A total of 35 nursing students (18 for the experimental group, 17 for the control group) participated in the study. A repeated simulationbased BLS training program which include a lecture, skills training, and two repeated sessions of simulation practice and debriefing was provided twice for experimental group. Knowledge, self-efficacy, and skill performance of cardiopulmonary resuscitation (CPR) were measured three times: at baseline, week 2, and week 6. Descriptive analysis, repeated measures ANOVA, and t-test were used for data analyses. Results: Knowledge, self-efficacy and skill performance of CPR were not significantly changed by group assignment, by the time, and interaction of group by time. Effectiveness of intervention was not maintained until Week 6. Conclusion: The results suggest that the timing of repeat education, total training time, and students' mastery of CPR performance should be considered when developing simulation-based programs to improve and maintain students' CPR knowledge, self-efficacy, and skill performance.

치과진료 시 소아고급생명구조술의 적용 (Application of a Pediatric Advanced Life Support in the Situation of a Dental Treatment)

  • In a dental treatment, a dentist has to know the possibility to happen all kinds of the emergency and to prepare for managing that situation. Especially, the cardiac arrest is the most serious emergent problem. If the accident were happened, most dentists got embarrassed. The American Heart Association (AHA) is offering the Basic Life Support (BLS), Advanced Cardiopulmonary Life Support (ACLS) and Pediatric Advanced Life Support (PALS) programs for the healthcare who need to prepare the life threatening situation. The PALS is specialized to someone who participate in pediatric health-care field. This program is composed of three major emergency problems, such as respiratory emergencies, shock and cardiac arrests. The main concepts of the PALS are early recognition and systemic team approach. The purpose of this study was to introduce about PALS and to prepare response system for emergencies in the dental environment.

  • Purpose: This study was done to identify the retention effects of a basic life support training program for nursing students on their attitude, knowledge, and skillfulness. Methods: A one-group repeated posttest design was employed for this study. The subjects included 44 junior nursing students from Gachon University in Incheon. Data were collected by a structured questionnaire test immediately after, 3 months after, 6 months after, and 9 months after the training respectively from May 2014 through March 2015. Results: The respective attitude, knowledge and skillfulness score on BLS of the nursing students had reduced significantly at 3 months after (p<.001), 6 months after (p<.001), and 9 months after (p<.001, p=.011 for Attitudes) the training compared to immediately after the training. Conclusion: Re-training on basic life support for nursing students should be implemented within 3 months for retention of educational effects. Thus, it is recommended that basic life support training programs, including a curriculum for nursing students, is developed to support re-training. And it is expected that the improved skills of nursing students on basic life support would contribute to the successive first aid nursing for patients at risk of cardiac arrest.

일반인 및 의료종사자들의 자동제세동기를 이용한 제세동에서 전극부착 위치에 관한 연구: 마네킨을 이용한 연구 (Automated external defibrillator electrode pad placement by the general public and health-care providers : A manikin study)

  • Purpose: This study aimed to assess defibrillator pad positioning by the general public and healthcare providers during basic life support (BLS) renewal education (RE). Methods: A total 130 subjects performed defibrillation using an automated external defibrillator (AED) equipped with electrode pads developed by the researchers and male resuscitation manikins, in order to assess electrode pad placement (EPP) before BLS RE. Subjects included 54 Healthcare providers (HP) and 76 members of the general public (GP). Results: The apex EPP was positioned in the risk area (RA), more than 5 cm distant from the area recommended, by 63.2% of the GP (n = 48). The apex EPP was positioned in the RA by 44.4% of the HP (n = 23). The distance from the apex (GP $5.27{\pm}2.21cm$, HP $4.18{\pm}1.71cm$) was statistically significant, using Student's t-test. Conclusion: For both GP and HP, apex EPP was significantly in error. Thus, to prevent defibrillation failures, EPP education and practice for GP and HP in all BLS education programs should be improved, and incorrect Korean cardiopulmonary resuscitation guidelines and related literature should be revised and corrected.

일부지역 치과위생사의 심폐소생술관련 지식과 태도에 대한 연구 (Knowledge and attitude toward cardiopulmonary resuscitation in dental hygienists)

  • Objectives : The purpose of this study is to investigate the knowledge and attitude toward cardiopulmonary resuscitation(CPR) in some dental hygienists. Methods : A self-reported questionnaire was filled out by 207 dental hygienists in Gwangju from February to March, 2014. The questionnaire consisted of knowledge and attitude toward CPR dental hygienists. The data were analyzed using IBM SPSS 19.0 program for chi-square test, independent t-test, one-way ANOVA and multiple regression analysis. Results : Dental hygienists having basic life support(BLS) provider accounted for 16.9%. CPR knowledge was 8.40 and attitude was 3.59 points. There were positive correlations between CPR knowledge and attitude including education, performance, and confidence(r=.231, r=.207, r=.377). In the regression analysis, attitude toward CPR confidence were positively associated with BLS provider certificate within two years. Conclusions : Dental hygienists had low level of knowledge and confidence toward CPR. It is important to educate BLS provider course to the dental hygienists.

한국형 이동식 심폐소생기 개발 보고 I. 실험견을 이용한 개흉식과 폐쇄식 심폐소생술 비교 (Report for Development of Korean Portable Cardiopulmonary Bypass Machine)

  • 배경: 고려대학교 흉부외과학교실에서는 심폐소생술에서 인공심 사용이 기존의 표준 심폐소생술에 비해 나은 결과를 보인다는 점에 착안하여 한국형 이동식 심폐소생기를 개발하고자 하였다. 1997년 1월부터 8월까지 한국형 이동식 심폐소생기 개발의 전단계로 심폐정지 모델 결정 및 표준 폐쇄식/ 개흉식 심폐소생술의 비교와 관찰지표 설정을 위한 준비실험을 실시하였다. 대상 및 방법: 실험은 한국산 잡견 9마리(28-35kg)를 대상으로 폐쇄식 심폐소생술군 4마리와 개흉식 심폐소생술군 5마리로 나누어, 4분 간의 심정지 및 15분간의 일차 심폐소생술(basic life support; BLS)과 30분간의 이차 심폐소생술(advanced life support; ALS)을 실시하였다. 심장압박은 폐쇄식군의 경우 흉부에 압박을 가하였고 개흉식군에서는 직접 심장을 맛사지하였다. 소생술기간에 양군 모두 동일한 조건의 폐환기 상태를 유지하였으며, 자발성 순환회복은 이차심폐소생술 기간 초기부터 재세동과 에피네프린 및 탄산수소 나트륨을 투여하여 유도하였다. 결과: 심폐소생술 기간안에 평균 체동맥압은 BLS 동안 폐쇄식군이 33$\pm$11 mmHg인데 비해 개흉식군은 45$\pm$15 mmHg로 높게 유지되었으며, ALS 동안에도 폐쇄식군 44$\pm$15 mmHg에 비해 개흉식군이 83$\pm$36 mmHg로 높게 유지 되었으나 통계상의 유의성은 없었다. 한편 평균 폐동맥압은 BLS 동안 폐쇄식군에서 32$\pm$10 mmHg로 평균 체동 맥압과 비슷한 정도로 증가하였으나 개흉식군은 22$\pm$4 mmHg로 평균 체동맥압의 약 50%정도까지만 증가하였고, ALS 동안에도 폐쇄식군은 32$\pm$15 mmHg로 개흉식군의 24$\pm$10 mmHg보다 높게 유지되었으나 통계처리상 유의성 은 없었다. 자발성 순환회복(restoration of spontaneous circulation; ROSC) 및 심폐소생 성공 여부에서 폐 쇄식군은 4마리 모두 사망하였으나 개흉식군은 5마리중 4마리가 생존하였고 생존기간은 384$\pm$705시간이였다 (p<.05). 결론: 본 연구 결과 개흉식 심폐소생술은 폐쇄식 소생술에 비해 비록 통계학상의 차이는 없었으나 소생술 기간 동안 비교해서 안정된 혈역학 상태를 유지하여서 자발성 순환회복 및 장단기 생존율을 향상시킬 수 있었다고 판단된다.
