Butein Disrupts Hsp90's Molecular Chaperoning Function and Exhibits Anti-proliferative Effects Against Drug-resistant Cancer Cells

  • Hsp90 shows great promise as a therapeutic target due to its potential to disable multiple signaling pathways simultaneously. In this study, we discovered that a natural product, butein moderately inhibited the growth of drug-resistant cancer cells (A2780cis and H1975), and brought about the degradation of oncogenic Hsp90 client proteins. The study demonstrated that butein would be a therapeutic lead to circumvent drug-resistance in cancer chemotherapy. The structure-based screening, synthesis, and biological evaluation of butein are described herein.

  • In cancer chemotherapy. it is becoming increasingly clear that the DNA topoisomerases play an active role in the expression of the cytotoxic action of drugs. The amino substituted azaanthraquinones have attracted much interest due to their possible role as topoisomerase inhibitors. In connection with our interests in the design and synthesis of potent topoisomerase inhibitor. we herein described the preparation of a series of benzoquinoxalinedione derivatives. These were designed based on the SAR of azaanthraquinones and structural analysis of products which are fitted with doxorubicin.

고형암에 대한 항암화학요법 치료 중 병발한 결핵의 특성 (Characteristics of Tuberculosis Detected during Chemotherapy for a Solid Tumor)

  • 연구 배경 : 면역 기능 저하를 초래하여 결핵의 재활성화에 관여할 것으로 생각되는 주기적인 항암화학요법치료 중인 고형암 환자에서 발생한 결핵의 임상적 특징과 치료 반응을 파악하고자 하였다. 대상 및 방법 : 2000년 1월부터 2004년 7월까지 국내 일개 3차 병원에서 항암화학요법 치료 중에 세균학적으로, 병리학적으로 또는 임상적으로 진단된 결핵 환자 중 기존에 알려진 결핵 재활성화의 위험 요인이 없는 22명을 대상으로 후향적 연구를 시행하였다.. 결 과: 22명의 평균 연령은 56.5세(21세-78세)였고, 남녀비는 3.4:1였다. 폐결핵이 가장 흔한 양상이었으며 (20례, 90.9%), 기저 질환으로 위암 (10례, 45.4%)과 림프종(4례, 18.2%)이 흔하였으며, 폐암, 두경부암, 유방암, 자궁경부암 및 난소암 순이었다. 15례 (68.2%)에서 환자가 흉부 방사선 사진 상 결핵 반흔이 있었다. 간기능이 저하된 1명을 제외한 21명이 isoniazid와 rifampicin을 기본으로 하는 초치료를 시작하였으며, 평균 치료기간은 $9.9{\pm}2.4$개월이었다. 기저 질환으로 사망한 5명을 제외하면 70.6%의 환자가 치료를 완료하였다. 결 론 : 항암화학요법 치료 중에 발생한 결핵의 임상적인 특징 및 치료에 대한 반응은 기존에 보고된 일반 인구에서 발생한 폐결핵의 특성과 크게 다르지 않았다.

소세포폐암에 대한 Cisplatin과 Etoposide(PVP) 복합화학요법의 효과 (A Phase II Trial of Combination Chemotherapy with Cisplatin & Etoposide in Small Cell Lung Cancer)

  • 연구배경: 소세포폐암은 빠른 증식속도와 조기에 전신전이를 나타내지만 화학요법에 비교적 반응을 잘한다고 알려져 있다. 최근 10년간 CAV에 실패한 환자의 이차 치료로서 사용되던 cisplatin과 etoposide 복합 화학요법(PVP)을 소세포 폐암 환자에서 일차 치료로서 시행하여 높은 반응률을 보고하고 있다. 이에 저자등은 PVP요법의 유효성 및 안정성을 평가하고자 하였다. 방법: 1989년 11월부터 1991년 12월까지 원자력 병원에서 소세포 폐암으로 진단받은 61명의 환자들을 대상으로 cisplatin $30mg/m^2$와 etoposide $100mg/m^2$을 제1일부터 제3일까지 정주하고 매 3주 간격으로 반복 치료하여 그 결과를 판정 하였다. 결과: 총 61명의 환자중 평가가 가능한 55명에서 완전반응이 13예(24%), 부분반응이 29예(53%), 불변이 9예(16%), 진행이 4예(7%)로 총반응률은 77% 였다. 제한기에서는 완전반응이 8예(26%), 부분반응이 21예(68%)였고 확대기에서는 완전반응이 5예(21%), 부분반응이 8예(33%)였으며, 전체 생존기간의 중앙치가 55.8주, 제한기가 61.1주, 확대기가 51.3주였다. 그리고 전체 반응군의 반응유지기간은 29.1주였고 심각한 부작용은 없었다. 결론: 소세포 폐암에 있어서 PVP요법은 일차치료로 사용시 심각한 부작용이 없으면서 비교적 효과적인 복합 화학요법으로 생각된다.

간전이 위암환자의 치료 결과 및 예후 (Prognosis and Treatment Outcomes of Gastric Cancer Patients with Hepatic Metastasis)

  • 목적: 간전이 위암환자들을 대상으로 치료 결과를 알아보고 생존 분석을 통해 예후를 확인하고자 하였다. 대상 및 방법: 1990년 3월부터 2006년 3월까지 영남대학교 영남의료원 내과 및 외과에 내원한 위암환자 중 간전이가 발견된 85예(동시성 62예, 이시성 23예)를 대상으로 후향적 연구를 시행하였다. 임상병리학적 인자와 위절제술, 간전이병소 절제술 및 항암화학요법 등 치료 관련 인자들에 대해 생존분석을 실시하였다. 결과: 간전이 위암환자들의 중앙생존기간은 11개월(동시성 11개월, 이시성 17개월)이었다. 동시성 간전이군의 경우 위절제율은 24.1%, 간전이병소 절제율은 16.1%였다. 간외 전이 동반율은 23.5%였고 생존기간에 차이가 있었다(P<0.05). 간전이 병소 절제 및 위절제군, 위절제군, 비절제군의 중앙생존기간은 각각 60개월 이상, 10개월 및 9개월이었다(Fig. 1, P<0.05). 이시성 간전이군의 경우 위암절제 후 무병생존기간의 중앙치는 8개월이었으며 초기병기와 간전이 빈도 사이에는 차이가 없었다. 동시성 간전이군에서는 단변량 분석 결과 간외 전이, 위절제술, 간전이 병소 절제술, 외과적 절제 형태 및 항암화학요법에 대한 반응이 유의하였고 다변량 분석 결과 간전이병소 절제술, 항암화학요법에 대한 반응 및 간외 전이가 독립적 예후인자였고, 이시성 간전이군에서는 간외 전이, 항암화학요법에 대한 반응 및 원발암 세포의 분화도가 유의하였고 간외 전이가 독립적 예후인자였다. 결론: 간전이 위암환자에서 예후는 간전이 병소의 절제, 원발 병소의 절제, 간외 전이 유무 및 항암화학요법에 대한 반응여부에 의해 결정되므로 치료에 있어 간외 전이가 없는 경우 보다 적극적인 절제 노력과 항암화학요법이 생존율 향상에 기여할 것으로 판단된다.

폐암환자를 위한 웹기반 교육프로그램 개발 및 평가 (The Development and Evaluation of Web-based Education Program for Lung Cancer Patient)

  • The purpose of this study were to develop an web-based education program for Lung cancer patients and to test its effects on patients' self-care knowledge, compliance to medical regimen, nutrition status and pain. The program was developed by the following process: first, Lung cancer patients demand on the web-based program was investigated. and second, the program was developed with the help of various reference books and then validation of experts group. last, educations effects on the patients is evaluated and compared the differences in self-care knowledge, compliance to medical regimen, nutrition status and pain between on experimental group and a control group on before discharge 1day and 3weeks after. SPSS/Win 11.0 program was used for data analysis. It was proven with $x^2$ test and t-test, and Pearson Correlation coefficient, and Chronbach's alpha coefficient were done for the reliability of measuring instruments. 1. The summary of the Program development is as follows. The program is based on patients' questionnaire and reference material and is made for users friendly. Not only Bigger font size and bright colors but also illustrations or pictures were adopted to help enhance patients' understanding. 2. The summary of the study results is as follows. 1) Compared with control group, the web-based educated experimental group showed a statistical significant difference on self-care knowledge, Especially disease, radiation treatment, medication & analgesics, chemotherapy side effect, but there was no significant difference in the field of chemotherapy, in the fields of operation, diet & general knowledge. 2) Compared with control group, the web-based educated experimental group showed a statistical significant difference on compliance to medical regimen, especially in the field of follow up care, everyday life, diet, but there was no significant difference in the field of medication, exercise. 3) Compared with control group, web-based educated experimental group showed no significant difference in nutrition status, but partially significant difference in body weight. 4) Compared with control group, the web-based educated experimental group showed no significant difference in pain level. 5) The significantly positive correalation self-care knowledge with the compliance to medical regimen. 6) Users satisfaction with the web-based education program of the contents quality, the level of recommendation to others, content layout, medical information quality, but interesting got a low mark.

Lung Cancer in Women, a Different Disease: Survival Differences by Sex in Turkey

  • Purpose: In this study, we aimed to evaluate the effects of sex-based non-small cell lung cancer (NSCLC) varieties on survival rates. Materials and Methods: A retrospective study was performed in patients with NSCLC who were diagnosed by histological methods between the years 2000 and 2010. A chi-square test was used to compare variables. Overall survival (OS) was estimated by the Kaplan-Meier method. Results: Of the 844 patients, 117 (13.9%) were women and 727 (86.1%) were men. Adenocarcinoma was more common in women than in men (p<0.0001). There were more women non-smokers than men (p<0.0001). There was no statistically significant difference in ECOG PS, weight loss>10%, stage, LDH, albumin and treatment between women and men. Women younger than 65 years (17.0 vs 12.0 months; p=0.03), who had adenocarcinoma histology (15.0 vs 10.0 months; p=0.006) and who had a hemoglobin level ${\geq}12g/dL$ (18.0 vs 12.0 months; p=0.01) were found to have a better median OS rate than men. Median OS rates were found to be 13.0 months in females and 12.0 months in males (p=0.14). Among metastatic patients, the median OS was 11.0 months in females and 8.0 months in males (p=0.005). Among stage IIIB and stage IV patients who had first line platinum-based chemotherapy, the median OS was 17.0 months in women and 11.0 months in men (p=0.002). The response rate of chemotherapy was higher in women than in men (p=0.03). Conclusions: In our study, we found that survival duration is longer and chemotherapy response is better in women with NSCLC who do not have anemia or comorbidities and who are mostly non-smokers with adenocarcinomas. Further studies regarding the causes of these differences may provide clarity on this subject.

Role of Tumor Necrosis Factor-Producing Mesenchymal Stem Cells on Apoptosis of Chronic B-lymphocytic Tumor Cells Resistant to Fludarabine-based Chemotherapy

  • Background: B-cell chronic lymphocytic leukemia B (B-CLL), the most common type of leukemia, may be caused by apoptosis deficiency in the body. Adipose tissue-derived mesenchymal stem cells (AD-MSCs) as providers of pro-apoptotic molecules such as tumor necrosis factor-related apoptosis-inducing ligand (TRAIL), can be considered as an effective anti-cancer therapy candidate. Therefore, in this study we assessed the role of tumor necrosis factor-producing mesenchymal stem cells oin apoptosis of B-CLL cells resistant to fludarabine-based chemotherapy. Materials and Methods: In this study, after isolation and culture of AD-MSCs, a lentiviral LeGO-iG2-TRAIL-GFP vector containing a gene producing the ligand pro-apoptotic with plasmid PsPAX2 and PMDG2 virus were transfected into cell-lines to generate T293HEK. Then, T293HEK cell supernatant containing the virus produced after 48 and 72 hours was collected, and these viruses were transduced to reprogram AD-MSCs. Apoptosis rates were separately studied in four groups: group 1, AD-MSCs-TRAIL; group 2, AD-MSCs-GFP; group 3, AD-MSCs; and group 4, CLL. Results: Observed apoptosis rates were: group 1, $42{\pm}1.04%$; group 2, $21{\pm}0.57%$; group 3, $19{\pm}2.6%$; and group 4, % $0.01{\pm}0.01$. The highest rate of apoptosis thus occurred ingroup 1 (transduced TRAIL encoding vector). In this group, the average medium-soluble TRAIL was 72.7pg/m and flow cytometry analysis showed a pro-apoptosis rate of $63{\pm}1.6%$, which was again higher than in other groups. Conclusions: In this study we have shown that tumor necrosis factor (TNF) secreted by AD-MSCs may play an effective role in inducing B-CLL cell apoptosis.

End Stage Palliative Care of Head and Neck Cancer: a Case Study

  • Background: Locally advanced head and neck cancer is generally incurable and has a short survival rate. This study aimed to evaluate symptom relief, disease response, and acute toxicity after palliative hypo-fractionated radiotherapy and long-term survival in affected patients. Materials and Methods: Between January 2011 to December 2011, 80 patients who were histopathologically diagnosed as having stage III or stage IV head and neck squamous cell carcinoma based on Eastern Cooperative Oncology Group (ECOG) performance status 1-3, were offered palliative radiotherapy (20 Gy/5Fr/5 Days). Later these patients were evaluated on 30th day after completion of treatment for disease response based on World Health Organisation (WHO) criteria and palliation of symptoms using symptomatic response grading and acute toxicities by the Radiation Therapy Oncology Group (RTOG). Many patients were given post radiation therapy (RT) palliative chemotherapy for appropriate palliative care and a few patients were selected for further curative RT. The overall survival was also evaluated among this group of patients with last follow up date of 1st May, 2014. Results: The most common presenting complaint was pain followed by dysphagia. Most patients (60-70%) had appreciable relief in their presenting symptoms. A good response was observed in the majority following palliative RT; a few patients had progressive disease and some had stable and regressed disease. None of the patients experienced radiation toxicity that required hospital admission. Almost all showed grade one and two acute skin and mucosal toxicity one month after completion of treatment. The mean survival days for patients given only hypofractionated palliative RT was 307 days, those with post palliative RT and palliative chemotherapy was 390 days and patients who went on to receive further palliative RT and curative RT dose had significantly overall survival of 582 days. Conclusions: Advanced head and neck cancer should be identified for suitable palliative hypofractionated radiotherapy to achieve acceptable symptom relief in a great proportion of patients and should be followed by palliative chemotherapy or curative RT in suitable cases for long-term symptom-free survival.
