Three Case Reports of Patients with Secondary Dementia Treated with Korean Traditional Medicine, Especially Boshiniknoe-tang-gami (속발성 치매 환자에 대한 보신익뇌탕가미방의 효과 3례)

  • Purpose: To describe the effectiveness of traditional Korean medicine (TKM) on the patients with secondary dementia (case 1: Rt. MCA infarction; case 2: traumatic subacute subdural hemorrhage and traumatic intracranial hemorrhage; case 3: delayed encephalopathy after carbon monoxide poisoning). Methods: We used a complete TKM treatment. The acupoints used were as follows: acupoints frequently used in hemiparesis (GV20, GV24, ST4, LI11, LI14, ST36, GB34, SP9, and LR3), and we used herbal medicine (補腎益腦湯加味, Boshiniknoe-tang-gami). The efficacy was evaluated using the Mini-Mental State Examination for Dementia Screening (MMSE-DS) and the Modified Barthel Index (MBI). Results: After treatment, the total MMSE-DS score was increased from 14 to 24 (case 1), 14 to 19 (case 2), and 0 to 17 (case 3). The total K-MBI was increased from 30 to 44 (case 1), 82 to 86 (case 2), and 30 to 86 (case 3) Conclusions: We suggest that TKM could positively influence cognition and improve activity of daily living (ADL). This study has limitations, so further studies are needed.

The Efficacy of Adjusting Leg Length Inequality by Chuna Manual Treatment for Post-Stroke Hemiplegia (뇌졸중 편마비 환자에서 추나요법을 통한 하지길이차이 교정의 효과)

  • Objectives : The purpose of this study is to investigate the efficacy of adjusting leg length inequality(LLI) by chuna manual treatment for post-stroke hemiplegia. Methods : 39 patients with stroke-originated hemiplegia who had leg length inequality were included in this study. Chuna manual treatment at pelvic girdle was applied to 20 patients and they had been treated by general oriental rehabilitation therapy(test group). The other 19 patients had been treated by general oriental rehabilitation therapy only(control group). Outcomes were assessed by Modified Barthel Index(MBI), Berg Balance Scale(BBS), lower extremity Fugl-Meyer Assessment(FMA) at the point of beginning and end of the study. Results : 1. In terms of activity of daily living(ADL), test group showed statistically meaningful differences compared to control group(p<0.05). In subacute group(onset had been past under 6 months), test group didn't showed statistically meaningful differences compared to control group. In chronic group(onset had been past over 6 months), test group didn't showed statistically meaningful differences compared to control group. 2. In terms of lower extremity function and balance, test group showed statistically meaningful differences compared to control group(p<0.05). In subacute group, test group didn't showed statistically meaningful differences compared to control group. In chronic group, test group showed statistically meaningful differences compared to control group(p<0.01). 3. In terms of lower extremity function of hemiplegic side, test group showed statistically meaningful differences compared to control group(p<0.05). In subacute group, test group didn't showed statistically meaningful differences compared to control group. In chronic group, test group showed statistically meaningful differences compared to control group(p<0.05). Coclusions : Adjusting LLI by chuna manual treatment is efficacious for rehabilitation of stroke-originated hemiplegia, in terms of ADL, balance and lower extremity function, and is especially efficacious for chronic patients.

Relationship Among Parents' Quality of Life, Functional Level, Performance in the Activities of Daily Living, and the Quality of Life in School-aged Children With Spastic Cerebral Palsy (학령기 경직성 뇌성마비 아동 부모의 삶의 질과 아동의 삶의 질 및 기능과 일상생활활동 수행 수준의 관련성)

  • Background: In cerebral palsy (CP), parents' quality of life and rearing attitude are considered possible factors that influence patients' quality of life, function level, and performance in daily activities. Despite these facts, little attention has been given to demonstrate a relation between these factors. Objects: This study aimed to demonstrate the relationship between parents' quality of life, functional level, and performance in daily activities, the quality of life of school-aged children with spastic CP. Methods: This study included 24 parents of school-aged children with spastic CP. The KIDSCREEN-52 and World Health Organization quality of life (WHOQOL)-BREF questionnaires (including physical, psychological, social, and environmental domains) were used as research tools to assess the quality-of-life profiles of the children with spastic CP and those of their parents, respectively. In addition, the function levels and performance in daily activities of the children with CP were assessed by using the gross motor function measure (GMFM) and modified Barthel index (MBI). Correlation and multiple regression analyses were performed to clarify the determinants of the quality of life of the children with CP. Results: The KIDSCREEN-52 score correlated with the total score (r=.735, p<.01) and all domains of the WHOQOL-BREF questionnaire (physical: r=.542, p<.01; psychological: r=.690, p<.01; social: r=.568, p<.01; and environmental: r=.783, p<.01). In addition, significant correlation was found between the KIDSCREEN-52 and MBI scores (r=.411, p<.05), and between the MBI and GMFM scores (r=.427, p<.05). After controlling for age, gender, paralytic type, GMFM, and MBI, the WHOQOL-BREF score ($R^2=.621$), particularly in the environmental domain ($R^2=.699$), remained independently related to the quality of life of the children. Conclusion: These findings suggest that the quality of life of school-aged children with spastic CP can be influenced by the quality of life of their parents. This study provides useful information for future studies to investigate the quality of life of children with CP.

  • BACKGROUND The purpose of this study was to find out predictors influencing on the clinical course of stroke during the first 4 weeks after the onset through statistical research, especially whether the blood glucose level after stroke and the stroke with Diabetes Mellitus correlate with functional impairments and neurological outcome or not. METHOD During 7 months period(from 1-1-1997 to 7-31-1997), 32 selected patients prognosed as acute cerebral thrombosis were studied by using Modified Barthel Index, PULSES profile in an attempt to correlate Diabetes Mellitus and hyperglycemia(more than $120mg/d{\ell}$, $150mg/d{\ell}$) with functional impairment and neurological outcome and to evaluate the influence of sex, the side of hemiparesis and age at admission, 1 week and 4 weeks after admision(admitted within 2 days after the onset). RESULT 1. The sex, side of hemiparesis and age had no significant effect upon functional impairment during first 4 weeks after the onset, but recurrent-stroke resulted in significantly higher degree of functional impairment than first-stroke during first 4 weeks after the onset. 2. The patients with Diabetes Mellitusin in acute cerebral thrombosis resulted in significantly higher degree of functional impairment than the patients without Diabetes Mellitus in acute cerebral thrombosis during first 4 weeks after the onset. 3. The patients with hyperglycemia in acute cerebral thrombosis resulted in significantly more severe neurological outcome than the patients without hyperglycemia in acute cerebral thrombosis within 2 days after the onset. CONCLUSION The study suggested that recurrent-stroke and Diabetes Mellitus were the poorer prognosis factors of functional impairment in acute cerebral thrombosis patients during first 4 weeks after the onset. and the poorer prognosis factor of neurological outcome in acute cerebral thrombosis patients was hyperglycemia within 2 days after the onset.

The Effect of Action Observation Training on Dexterity of Affected Upper Extremity Motor Function and ADL in Stroke Patients (과제지향적 동작관찰훈련이 뇌졸중 환자의 환측 상지기민성과 일상생활활동에 미치는 영향)

  • Objective : The purpose of present study was to determine effects of task oriented action observation training on dexterity of upper limb function after stroke. Training was progressed to imitation and intensive training after observation to required action in ADL. Method : 15 stroke patients were selected and one group pretest-posttest design was applied. Targets observed performance actions of task orientated task through a video and limitated action repeatedly, Training was performed 3 times a week and 20~30 minutes for each round for 3 weeks. Manual function test and 10-second test and Box and block test were performed for a dexterity of upper extremity motor function, Modified Barthel index and Motor activity log were performed for ADL. Result : After the training, a dexterity of affected upper extremity motor function of the target showed significant difference between before and after intervention, ADL showed significant improvement between before and after intervention. Conclusion : Task orientated Action observation lead a positive effect of recovering a dexterity of upper extremity motor function and ADL. Therefore, this study suggest a action observation training as one strategy of an intervention type of th e occupational therapy.

Effects of Virtual Reality-Based Activities of Daily Living Training on Activities of Daily Living and Rehabilitative Motivation in Patients With Traumatic Brain Injury: A Pilot Study (가상현실 기반의 일상생활활동 훈련이 외상성 뇌손상 환자의 일상생활활동 및 재활동기에 미치는 효과 : 예비연구)

  • Objective : The purpose of this study was to investigate the effects of virtual reality-based activities of daily living (ADL) training on ADL and rehabilitative motivation in patients with traumatic brain injury. Methods : This study was performed using a pre-post design with seven traumatically brain injured patients. Subjects were subjected to virtual reality-based ADL training for 30 minutes a day, 2 to 3 times a week for 4 weeks. Evaluation was conducted before and after the intervention using the Korean Modified Barthel Index (K-MBI), Cognitive Functional Independence Measure (C-FIM), and Volitional Questionnaire (VQ). Changes before and after intervention were analyzed by Wilcoxon signed-rank test, and correlations were analyzed using Spearman's coefficient. Results : After intervention, patients with traumatic brain injury showed significant improvements in K-MBI (p<.05). There was no significant change in total C-FIM score and VQ score (p>.05). Total C-FIM score correlated significantly with VQ score (p<.05, r=.755). The social cognition domain of C-FIM had a significant correlation with VQ score (p<.05, r=826). Conclusions : Virtual reality-based ADL training can improve ADL performance, but further research is needed to determine whether improvements in social cognition and rehabilitative motivation are possible.

A Comparative Study of the Cognition, ADL and Quality of Life According to General and Pathological Characteristics of the Elderly with Dementia (치매노인의 일반적 및 병리학적 특성에 따른 인지기능, 일상생활동작, 삶의 질의 비교연구)

  • This study was conducted to compare the effects of general and pathological characteristics of elderly with dementia on their cognitive ability, performance of activities of daily living (ADL), and quality of life. Data were collected between July 15 and August 30, 2016 from 136 elderly with dementia who used day care centers. The Korean version of Mini-Mental Status Examination, Korean version of Modified Barthel Index, and Korean version of the WHO Quality of Life Scale Abbreviated Version were used for data collection and values obtained were analyzed accordingly. Significant correlations were found between cognitive ability and performance of ADL, between quality of life and cognitive ability, and between cognitive ability and performance of ADL (p<.001). Moreover, quality of life of elderly with dementia was greatly affected by cognitive ability and performance of ADL (p<.01). Therefore, it is believed that more active therapeutic interventions and studies are needed from sociophysical aspect of elderly with dementia.   • This study was to investigate the correlation among swallowing function, dietary level, cognitive function, daily living, and comparison for general characteristics in stroke patients with dysphagia. Subjects of this study participated to 56 stroke patients with dysphagia. Outcome measures is evaluated by Functional Dysphagia Scale(FDS), and Amerian Speech-Language-Hearing Association National Outcomes Measurements System(ASHA NOMS), and Korean Mini-Mental State Examination(K-MMSE), and Korean Modified Barthel Index(K-MBI). Collected all data analyezed to independent t test for four assessments, and general characteristics of study subjects analyzed by pearson correlation coefficient for four assessments. Results of study, swallowing function according to lesion location differed significantly(p<.05). Cognitive function according to onset duration differed significantly(p<.05). Age of subjects and dietary level, cognitive function showed a significant correlation(p<.05). Swallowing function and dietary level, cognitive function showed a significant correlation(p<.05). Cognitive function and dietary level, daily living showed a significant correlation(p<.05). Based on current results, we suggest that swallowing rehabilitation for stroke patients with dysphagia performed with consideration for cognitive function and characteristic of patients.

The Effects of Tabata Exercise using Health Care Application on Balance, Gait and Activities of Daily Living in Patients with Stroke (건강관리 애플리케이션을 통한 타바타 운동이 뇌졸중 환자의 균형, 보행 및 일상생활 활동에 미치는 영향)

  • The purpose of this study was examined the effect of Tabata exercise using health care application on balance, gait and activities of daily living in patients with stroke. Twenty patients with stroke were participated patients with stroke in this study. Nineteen participants were randomly allocated to ten in experimental and nine in control groups. The both groups received the rehabilitation therapy during for 4 weeks. The experimental group performed Tabata exercise based on health care application, supervised under caregivers, during 30 min/day. The control group performed self-exercise supervised under caregivers, during 30 min/day. Both groups exercised for 4 weeks, 3 times a week, 30 min/day. Tabata exercise was trained by an physical and occupational therapists. The outcome measures were the BBS(: Berg Balance Scale), TUG(: Timed Up and Go, K-MBI(: Korean Modified Barthel Index). In results, the experimental groups showed a significant improvements after intervention in all assessments(p<.05), The control groups showed a significant improvements after intervention in BBS(p<.05) The experimental group showed a greater significant improvements than control group in TUG, BBS, K-MBI(p<.05). We suggest that Tabata exercise using health care application can have a effects of the improvements of balance, gait and activities of daily living than only self-exercise in patients with stroke.

Effects of Game-based Postural Vertical Training on Pusher Behavior, Postural Control, and Activity of Daily Living in Patients With Acute Stroke: A Pilot Study (게임 기반의 자세수직 훈련이 급성 뇌졸중 환자의 밀기행동, 자세조절, 그리고 일상생활동작에 미치는 영향: 사전연구)

  • Background: Visual and somatosensory integration processing is needed to reduce pusher behavior (PB) and improve postural control in hemiplegic patients with acute stroke. Objects: This study aimed to investigate the effects of game-based postural vertical training (GPVT) on PB, postural control, and activity daily living (ADL) in acute stroke patients. Methods: Fourteen participants with acute stroke (<2 months post-stroke) who had PB according to the Burke lateropulsion scale (BLS) (score>2) were randomly divided into the GPVT group ($n_1=7$) and conventional postural vertical training (CPVT) group ($n_2=7$). The GPVT group performed game-based postural vertical training using a whole-body tilt apparatus. while the CPVT group performed conventional postural vertical training to reduce PB (30 minutes/session, 2 times/day, 5 days/week for 3 consecutive weeks). The BLS was evaluated to assess the severity of PB. And each subject's postural control ability and ADL level were assessed using the postural assessment scale for stroke (PASS), balance posture ratio (BPR), and Korean-modified Barthel index (K-MBI). Outcomes were measured pre- and post-intervention. Results: Comparison of the pre- and post-intervention assessment results showed that both interventions led to the following significant changes: decreased severity of PB scores and increased PASS, BPR, and K-MBI scores (p<.05). In particular, statistical analysis between the two groups, the BLS score was significantly decreased in the GPVT group (p<.05). And PASS, BPR, and K-MBI scores were significantly improved in the GPVT group than in the CPVT group (p<.01, respectively). Conclusion: This study demonstrated that GPVT lessened PB severity and improved postural control ability and ADL levels in acute stroke patients.
