Low Back Pain and Related Factors in Bus Drivers (일부 버스 운전기사의 요통경첩 및 관련요인)

  • Objectives: The objective of this study was to investigate the experience and point prevalence rate and factors related with Low Back Pain (LBP) in bin drivers. Methods: Questionnaires were completed by 200 drivers at 2 bus companies in Andong city in October.2000. The information was used to estimate odds ratio (OR) and $99\%$ confidence intervals (CI) for factors relation to LBP. A cross-section study design was used. Results: The experience rate for LBP was $53.5\%$, point prevalence rate was $31.5\%$. Variables significantly associated with LBP experience were employ duration and stress related occupation. The higher subjects had experienced stress. the higher experience for LBP was (OR=2.9, $95\%$ Cl 1.2-7.2). The longer subjects had employ duration in bus company, the higher experience far LBP was (OR=1.3, $95\%$ Cl 1.2-7.2). Significant factors relation to a week point prevalence far LBP were obesity and employ duration. LBP increased as weight increased (OR=4.1, Cl 1.6-10.7). The longer subjects has employ duration at bus company, the higher LBP is (OR=1.4. $95\%$ Cl 1.6-2.9). Conclusions: Results from this study indicated that a statistically significant factor relation to LBP was stress related occupation. employ duration, and obesity.

Stress Corrosion Cracking of High Strength Al-Zn-Mg-Cu Aluminum Alloy with Different Compositions (고강도 Al-Zn-Mg-Cu 합금에서 조성에 따른 응력부식균열 특성)

  • High strength 7xxx series Al-Zn-Mg alloy have been investigated for using light weight automotive parts especially for bump back beam. The composition of commercial 7xxx aluminum has the Zn/Mg ratio about 3 and Cu over 2 wt%, but this composition isn't adequate for appling to automotive bump back beam due to its high resistance to extrusion and bad weldability. In this study the Zn/Mg ratio was increased for better extrusion and Cu content was reduced for better welding. With this new composition we investigated the effect of composition on the resistivity against stress corrosion cracking. As the Zn/Mg ratio is increased fracture energy obtained by slow strain rate test was decreased, which means degradation of SCC resistance. While the fracture energy was increased with Cu contents although it is below 1%, which means improvement of SCC resistance. These effects of composition change on the SCC resistivity were identified by observing the fracture surface and crack propagation.

  • In the present work, the evolution rules for the internal variables including continuum damage factors are obtained using the thermodynamic framework, which are in turn facilitated to derive the elastic-plastic constitutive relation for the particulate composites. Using the Mori-Tanaka scheme, the homogenization on state and internal variables such as back-stress and damage factors is carried out to procure the rate independent plasticity relations. Moreover, the degradation of mechanical properties of constituents is depicted by the distinctive damages such that the phase and interfacial damages are treated individually accordingly, whereas the kinematic hardening is depicted by combining the Armstrong-Frederick and Phillips' back-stress evolutions. On the other hand, the present constitutive relation for each phase is expressed in terms of the respective damage-free effective quantities, then, followed by transformation into the damage affected overall nominal relations using the aforementioned homogenization concentration factors. An emphasis is placed on the qualitative analyses for strain localization by observing the perturbation growth instead of the conventional bifurcation analyses. It turns out that the proposed constitutive model offers a wide range of strain localization behavior depending on the evolution of various internal variable descriptions.

THREE-DIMENSIONAL PHOTOELATIC STRESS ANALYSIS OF CLASP RETAINERS INFLUENCED BY VARIOUS DESIGNS ON UNILATERAL FREE-END REMOVABLE PARTIAL DENTURES (하악 편측 유리단 국소의치의 직접유지장치 형태에 따른 3차원적 광탄성 응력분석 연구)

  • The extent and direction of movement of removable partial dentures during function are influenced by the nature of the supporting structures and and the design of the prosthesis. Since forces are transmitted to the abutment teeth through occlusal rests, guide planes and direct retainers during functional movements, proper design based on the avaialble research data will maintain the health of abutment teeth and their supporting structures. The purpose of this in vitro study is evaluating stress distribution clinically around the abutment teeth prepared following 4-type clasping systems for unilateral free-end removable partial dentures. Three-Dimensional Photoelastic Stress Analysis method was used because it shows a visual display of stresses of the simulated abutment teeth and residual ridges and reveals stress concentration that can be read at any given points in terms of direction and magnitude. For this study, the author fabricated 4 mandibular photoelastic epoxy models missing left 1st and End molar. Epoxy models were duplicated and 4 unilateral removable partial dentures were construe- ted in accordance with 4-type direct retainers. Unilateral free-end removable partial dentures were positioned on their own models. 6kg force was loaded on the every removable partial dentures of the epoxy model on the central fossa of mandibular left 1st molar vertically by the loading device. After the stress was frozen in a stress freezing furnace, 6 specimens of 6-mm thickness were made from every epoxy model and examined with the circular polariscope. The results were as follows : 1. Generally I-bar clasp revealed the most favorable stress distribution around the abutment teeth. 2. At the end portion of the free-end ridge, Back action clasp showed the highest stress concentration at the bucco-lingual and top portions of the residual alveolar ridge. 3. At the distal area of the abutment teeth, Akers clasp and Roach clasp showed higher stress concentration bucco-lingually and apically than the others. 4. To the abutment tooth, I-bar clasp showed the least stress distribution bucco-lingually but the others showed irregular stress distribution. 5. At the mesial area of the abutment teeth, the order of effective stress distribution was I-bar clasp, Back-action clasp, Akers clasp and Roach clasp. There was big difference of stress distribution between them. 6. At the right 2nd premolar and 1st molar, the stress concentration of Akers clasp was a little high but that of I-bar clasp was low.

Outcome of Low-Back Pain and Sciatica : Relationship among Self-reported Pain Intensity, Disability, Sleep Disturbance and Fatigue (요통 및 좌골신경통 환자의 치료결과 : 수면장애 및 피로감과 자각적 통증 및 장애정도의 관계)

  • Objective : Psychological factors may have a major influence on the outcome of treatment for back pain. We investigated the relationship between the outcome and some psychological factors, such as self-reported pain intensity, disability, sleep disturbance and fatigue. Method : The study was conducted as a survey using a questionnaire and telephone interviews. The survey included consecutive 294 patients who visited the neurosurgical out-patient department complaining of low back pain and contacted by telephone on average seven months after the first visit. Pain intensity was measured by visual analog scale, and disability was assessed by Waddell's chronic disability index. Results : The outcome of treatment for back pain was recovered in 36.7%, improved in 30.6%, almost same in 28.6%, and aggravated in 4.1%. Overall rate of improvement was 67.3%. The rate of improvement was related to the duration, patterns and intensity of the symptom, and Waddell index. It was not influenced by the doctors, special studies, and methods of treatment. When the duration was more than 6 months, there were the symptoms of both back and legs, and the self-reported Waddell index was 1-3, the rate of improvement was relatively low. Although the intensity of the pain and disability was closely related to the degree of sleep disturbance, fatigue, appetite, or indigestion, the outcome of treatment for back pain was not always bad in patients with high psychological stress. Actually the outcome of the patients who complained severe pain and disability was better than the outcome of the others. Conclusion : The outcome of the back pain can be predicted by the duration, patterns and intensity of the symptom, and Waddell index. The multidisciplinary treatment will be necessary for the patients whose expected outcome is not good to reduce not only the physical symptoms but also the psychological stress.

The Effects of 12 Weeks Pilates Mat Exercise on Body Composition, Lumbar Muscle Strength, Low Back Pain and Pregnancy Stress in Pregnant Women (12주간 필라테스 매트 운동이 임산부의 신체구성, 요부근력, 요통 및 임신 스트레스에 미치는 영향)

  • This study is to identify the effects of pilates mat exercise on the body composition, lumbar muscle strength, low back pain and pregnancy stress in pregnant women. In order to carry out this study, nineteen pregnant women were divided into two groups: pilates mat exercise group (n=11) and control group (n=8). EXE group performed pilates mat exercise for 60 min per day, 2days/week for 12 weeks. Our results showed that there was no significant changes in total body water, body fat mass, BMI, body fat percent, and BMR, but the EXE group showed significant increase in skeletal muscle mass. Furthermore, EXE group was significantly increased in hip extension and lumbar extension ROM. Analysis of low back pain and pregnancy stress level showed no significant difference between group. In conclusion, our study suggest that pilates mat exercise can be safely applied to pregnant women by increasing the muscle mass of pregnant women and by showing a positive effect on the lumbar muscle strength.

Health Risk Factors of Nurses in the Operating Room (수술실간호사의 건강위험요인)

  • In order to investigate and compare the health risk factors of nurses in the operating room(OR nurse) and ward (WARD nurse), the questionnaire survey for subjective symptoms was carried out on 553 nurses(132 OR nurses and 421 WARD nurses) who were employed at seven hospital. The self-administered questionnaries were composed of low back pain, subjective fatigue symptoms, musculo-skeletal symptoms, psychological stress and reproductive function. The results were as follows : 1. In the type of working posture and working environment, there were significant difference between two groups for working posture, waist form, height of working table, satisfaction of chair, lifting & carring. 2. Job satisfaction, duration of work, height of working table, satisfaction of chair, lifting & carring were significantly associated the low back pain. 3. In the complaints of subjective fatigue symptoms, the total mean score was higher in OR nurse than WARD nurse, but there was not significant. The items that the mean score of OR nurse was significantly higher than WARD nurse were 'head feels muddled', 'apt to forget', 'feel choky'. 4. In the complaints of musculo-skeletal syrrptoms, the total mean score was higher in OR nurse than WARD nurse, but there was not significant. The item that the mean score of OR nurse was significantly higher WARD nurse was 'wrist discomfort or pain'. 5. The comparison of spontatenous abortion in married nurses who had the experience of pregnancy were significantly associated the stress risk group. 6. In all of OR and Ward nurses, the job satisfaction is associated with subjective fatigue symptoms, musculo-skeletal symptoms, and stress. In conclusion, it suggested that working posture, working environment, stress, and job satisfaction were health risk factors of nurses working in the operating room. Further prospective intervention studies should be conducted to educate right working posture, improve of working environment, decrease of stress, and increase of job satisfaction.

  • Back pains include pain in one or both side costa portion and lateral abdomen. There are different kinds of flank-related disease such as intercostal nerve pain, chronic gastritis, duodenitis, herpes zoster, pepticulcer, cholecystitis, cholelithiasis and so on. Back pain arises from pathological abnormal activities. On the other hand psychogenic pains are caused by psychogenic factors without pathological abnormal activities. This is similar to KiWol back pain, which resulted from psychogenic factors like stress, anxiety, anger and etc. In oriental medicine, KiWol is the state of depression of Ki, and Shihosogansan used to treat the back pain diagnosed as stagnation of the liver Ki. So, we decided to apply Shihosogansan to a patient who suffered from back pain diagnosed as stagnation of the liver Ki. Therefore the patient treated with Shihosogansan and improved in consciousness symptoms, so we report it for the better treatment.

The Effect of Low Back Pain on the EMG of Professional Golfer's Drive Swing (요통에 따른 프로 골퍼의 드라이버 스윙에 관한 근전도 분석)

  • The purpose of this study is to compare and analyze the muscle activations between the professional golfers without low back pain symptom and the professional golfers with low back pain symptom, and so identify the stress related to golf swings, and provide the basic data to minimize the low back pain and the injury risk. Using surface electrode electromyography, we evaluated muscle activity in 6 male professional golfers during the golf drive swing. Surface electrodes were used to record the level of muscle activity in the Abdominal Oblique, Elector Spinae, Rectus Abdominis, Gluteus Maximus muscles during the golfer's swing. These signals were compared with %RVC(Reference voluntary contraction) which was normalized by IEMG(Integrated EMG). The golf swing was divided into five phases: take away, forward swing, acceleration, early follow through, late follow through. we observed patterns of trunk muscle activity throughout five phases of the golf swing. The results can be summarized as follows: RES(Right Elector Spinae) had statistically significant difference in take away phase, LGM(Left Gluteus Maximus), LRA(Left Rectus Abdominis), LOA(Left Oblique Abdominal) had statistically significant difference in forward swing phase, RES(Right Elector Spinae), RGM(Right Gluteus Maximus), ROA(Right Oblique Abdominal) had statistically significant difference in acceleration phase, RES(Right Elector Spinae), RGM(Right Gluteus Maximus) had statistically significant difference in early follow-through phase, LES(Left Elector Spinae), RGM(Right Gluteus Maximus) had statistically significant difference in late follow through phase.

Effects of Local Cooling on Heat Strain in the Hot Environment -On the Trunk Region-

  • This study was to determine the effect of cooling part of the trunk without harm for the health. The results provide basic data for the development of clothing which could increase work efficiency and reduce body strain in hot environment. Eight males took part in the study. The experiment was conducted in a climate-chamber controlled with $37{\pm}1^{\circ}C,\;50{\pm}5%R.H$. The trunk was divided into six areas to be cooled: head, neck, chest, abdomen, the upper back, the lower back. According to preceding studies, permissible safety cooling limits of skin temperature, of each part of the trunk for four hours cooling were $25^{\circ}C$ on the head, $20^{\circ}C$ on the neck, $27^{\circ}C$ on the chest, $25^{\circ}C$ on the abdomen, $20^{\circ}C$ on the upper back, $20^{\circ}C$ on the lower back. So cooling temperatures of each region set up temperatures above mentioned. In conclusion, the head, the neck and the upper back cooling could reduce sweating amount, rectal temperature and heart rates and reduce the heat stress of workers exposing in the hot environment by decreased subjective sensations of heat and comfort. Thus, it was concluded that effectiveness of cooling among the trunk was best on the head and the neck.
