Survival Effect of Complete Multimodal Therapy in Malignant Pleural Mesothelioma

  • Background: Malignant pleural mesothelioma (MPM) is an aggressive pleural malignancy, and despite all multimodal treatment modalities, the 5-year overall survival rate of patients with MPM is less than 20%. In the present study, we aimed to analyze the surgical and prognostic outcomes of patients with MPM who received multimodal treatment. Methods: In this retrospective, single-center study, the records of patients who underwent surgery for MPM between January 2010 and December 2020 at our department were reviewed retrospectively. Results: Sixty-four patients were included in the study, of whom 23 (35.9%) were women and 41 (64.1%) were men. Extrapleural pneumonectomy, pleurectomy/decortication, and extended pleurectomy/decortication procedures were performed in 34.4%, 45.3%, and 20.3% of patients, respectively. The median survival of patients was 21 months, and the 5-year survival rate was 20.2%. Advanced tumor stage (hazard ratio [HR], 1.8; p=0.04), right-sided extrapleural pneumonectomy (HR, 3.1; p=0.02), lymph node metastasis (HR, 1.8; p=0.04), and incomplete multimodal therapy (HR, 1.9; p=0.03) were poor prognostic factors. There was no significant survival difference according to surgical type or histopathological subtype. Conclusion: Multimodal therapy can offer an acceptable survival rate in patients with MPM. Despite its poor reputation in the literature, the survival rate after extrapleural pneumonectomy, especially left-sided, was not as poor as might be expected.

Validation of the Dutch Eating Behaviour Questionnaire Children (DEBQ-C) version in Turkish preadolescence children

  • BACKGROUND/OBJECTİVES: It is important to determine Dysfunctional eating behaviors such as dietary restraint and overeating tendencies in order to provide weight management and acquire the right habits in children. The purpose of this study was to test the reliability and validity of Dutch Eating Behaviour Questionnaire Children (DEBQ-C) with Turkish preadolescent children. MATERIALS/METHODS: This research included 440 preadolescents (9.3 ± 6.9 years and 235 girls, 205 boys). The instrument is divided into three subscales, each with 20 items. Emotional eating, restrained eating, and external eating are the three subscales. Confirmatory factor analysis (CFA) was used to assess the construct validity of the Turkish version of the DEBQ-C, and Cronbach α values were computed to evaluate the subscale reliabilities. There were 20 observable variables and three latent variables in the hypothesized model. RESULTS: Fit indices for the hypothesized model were good (×2/degree of freedom = 1.96; root mean square error of approximation = 0.05; comparative fit index = 0.95; goodness of fit index = 0.93). These findings revealed that the Turkish version of the DEBQ-C has a factor structure that was identical to the three-factor structure of the original scale. The Turkish version of the DEBQ-C subscales has internal consistency coefficients ranging from 0.72 (external eating) to 0.86. (emotional eating). CONCLUSIONS: The DEBQ-C Turkish version is a viable and reliable tool for measuring overeating tendencies in Turkish preadolescents, according to the findings.

Biodistribution and pharmacokinetic evaluation of Korean Red Ginseng components using radioisotopes in a rat model

  • Background: Although many studies have evaluated the efficacy and pharmacokinetics of Korean Red Ginseng (KRG) components (Rg1, Rb1, Rg3, Rd, etc.), few have examined the in vivo pharmacokinetics of the radiolabeled components. This study investigated the pharmacokinetics of ginsenosides and their metabolite compound K (CK), 20(s)-protopanaxadiol (PPD), and 20(s)-protopanaxatriol (PPT) using radioisotopes in rat oral administration. Methods: Sprague-Dawley rats were dosed orally once with 10 mg/kg of the tritium(3H) radiolabeled samples, and then the blood was collected from the tail vein after 0.25, 0.5, 1, 1.5, 2, 4, 6, 8, 12, 24, 48, 96, and 168 h. Radioactivity in the organs, feces, urine, and carcass was determined using a liquid scintillation counter (LSC) and a bio-imaging analyzer system (BAS). Results and conclusion: After oral administration, as the 3H-labeled ginsenosides were converted to metabolites, Cmax and half-life increased, and Tmax decreased. Interestingly, Rb1 and CK showed similar values, and after a single oral administration of components, the cumulative excretion ratio of urine and feces was 88.9%-92.4%. Although most KRG components were excreted within 96-168 h of administration, small amounts of components were detected in almost all tissues and mainly distributed to the liver except for the digestive tract when observed through autoradiography. This study demonstrated that KRG components were distributed to various organs in the rats. Further studies could be conducted to prove the bioavailability and transmission of KRG components to confirm the mechanism of KRG efficacy.

  • Bronchial adenoma, firstly described by Muller[1882] had been reported on the subject stressed their benign nature prior to 1940`s, but these tumors including carcinoid tumor, mucoepidermoid carcinoma, adenoid cystic carcinoma, pleomorphic adenoma are now known to possess the various degree of malignant natures from benign course, low grade malignant potential to distant lymphatic or hematogenous metastasis or combination. Although histologically diffeerent, four varieties except carcinoid tumor which is a spectrum of neu-roendocrine tumor originating Kulchitsky cell of the bronchial epithelium and form the part of the APUD tumor spectrum, are morphologically and in many respects clinically similar to the corresponding tumor of the salivary gland is a specific varient of adenocarcinoma that occurs most commonly in the major and minor salivary gland and less frequently tra-cheobronchial tree, esophagus etc. To better understand the clinical characteristics and assess more precisely the malignat nature of bronchial adenoma, we studied 17 cases of bronchial adenoma, which had been experienced at the Department of Thoracic and Cardiovascular surgery of Catholic University Medical College from April 1977 to september 1991. Seventeen cases of bronchial adenoma consist of 2 carcinoid tumors, 6 adenoid cystic carcinomas, 8 mucoepidermoid carcinomas and one pleomorphic adenoma. There is a slight predominace of male patients[10/17] and the age of pt studied varied with a higher incidence occurring between the ages of 40 years and 60 years[mean age, 46.5 years]; the youngest being 15 years and oldest 69 years. Their leading complaints were hemoptysis[4], exertional dyspnea[8], fever & chilness [4], and symptoms mimicking the bronchial asthma[4]. Diagnosis was aided by the radiologic studies such as chest X-ray, polytomography, CT scan, brochography and bronchoscopy. The preferred locations of fumor were in the trachea[4], main stem bronchus[3], bronchus intermedius[3], bronchus of RUL[2], LUL[1], RLL[1], LLL[3] with no peripheral location. Modalities of treatments were single or combination of surgical resection, radiation therapy, chemotherapy. Complete resections were permitted in 12 cases with late recurrences of 4 cases ranging from 6 months to 10 years: pneumonectomy[4], lobectomy[4], bil-obectomy[2], sleeve resection[2]. Gross findings of resected specimens in 14 cases showed that 4 cases were polyp-like pedunculated mass[entirely intraluminal mass] with intact mucosa, 8 cases were broad-bas-ed sessile mass[predominatly intraluminal] and the main portions were located below the mucosa similar to tip of iceburg[predominantly extraluminal] in 2 cases. Follow-up information was availble in all 17 cases ; eight were alive without evidence of disese ranging from 1 month to 13 years. But seven cases died of the causes related to tumor[6 cases within 12 months, one case 10 years after pneumonectomy]. We concluded that 8 cases[47%] of 17 cases were metastasizing bronchial adenoma and precise survival rate cannot be answered by the scanty materials available for study.

Determination of Biogenic Amines using an Amperometric Biosensor with a Carbon Nanotube Electrode and Enzyme Reactor (Carbon Nanotube 전극과 효소반응기로 구성된 Amperometric Biosensor를 이용한 Biogenic Amines 검출)

  • Biogenic amines are synthesized by microbial decarboxylation for the putrefaction or fermentation of foods containing protein. Although biogenic amines such as histamine, tyramine, and putrescine are required for many physiological functions in humans and animals, consumption of high amounts of biogenic amines can cause toxicological effects, including serious gastrointestinal, cutaneous, hemodynamic, and neurological symptoms. In this study, a novel amperometric biosensor wasdeveloped to detect biogenic amines. The biosensor consisted of a working electrode, a reference electrode, a counter electrode, an enzyme reactor with immobilized diamine oxidase, an injector, a peristaltic pump and a potentiostat. A working electrode was fabricated with a glassy carbon electrode (GCE) by coating functionalized multi-walled carbon nanotubes (MWCNT-$NH_2$) and by electrodepositing Prussian blue (PB) to enhance electrical conductivity. A sensor system with PB/MWCNT-$NH_2$/GCE showed linearity in the range of $0.5 {\mu}M{\sim}100 {\mu}M$ hydrogen peroxide with a detection limit of $0.5 {\mu}M$. The responses for tyramine, 2-phenylethylamine, and tryptamine were 95%, 75%, and 70% compared to that of histamine, respectively. These results imply that the biosensor system can be applied to the quantitative measurement of biogenic amines.

A Commissioning of 3D RTP System for Photon Beams

  • The aim is to urge the need of elaborate commissioning of 3D RTP system from the firsthand experience. A 3D RTP system requires so much data such as beam data and patient data. Most data of radiation beam are directly transferred from a 3D dose scanning system, and some other data are input by editing. In the process inputting parameters and/or data, no error should occur. For RTP system using algorithm-bas ed-on beam-modeling, careless beam-data processing could also cause the treatment error. Beam data of 3 different qualities of photon from two linear accelerators, patient data and calculated results were commissioned. For PDD, the doses by Clarkson, convolution, superposition and fast superposition methods at 10 cm for 10${\times}$10 cm field, 100 cm SSD were compared with the measured. An error in the SCD for one quality was input by the service engineer. Whole SCD defined by a physicist is SAD plus d$\sub$max/, the value was just SAD. That resulted in increase of MU by 100${\times}$((1_d$\sub$max//SAD)$^2$-1)%. For 10${\times}$10 cm open field, 1 m SSD and at 10 cm depth in uniform medium of relative electron density (RED) 1, PDDs for 4 algorithms of dose calculation, Clarkson, convolution, superposition and fast-superposition, were compared with the measured. The calculated PDD were similar to the measured. For 10${\times}$10 cm open field, 1 m SSD and at 10 cm depth with 5 cm thick inhomogeneity of RED 0.2 under 2 cm thick RED 1 medium, PDDs for 4 algorithms were compared. PDDs ranged from 72.2% to 77.0% for 4 MV X-ray and from 90.9% to 95.6% for 6 MV X-ray. PDDs were of maximum for convolution and of minimum for superposition. For 15${\times}$15 cm symmetric wedged field, wedge factor was not constant for calculation mode, even though same geometry. The reason is that their wedge factor is considering beam hardness and ray path. Their definition requires their users to change the concept of wedge factor. RTP user should elaborately review beam data and calculation algorithm in commissioning.

  • The CL network fer CBDS(Connectionless Broadband Data Service) defined by ITU-(International Telecommunication Union-Telecommunication) consists of CL(ConnectionLess) servers which route frames and links which connect between CL user and CL server or between CL servers[1][2]. In this configuration, two kinds of congestion, CL link congestion and CL server congestion may occur. Therefore this paper has proposed the scheme that can deal with the above congestion state of LAN traffic efficiently in interworking LANs in ATM network. Proposed schemes ale link-by-link scheme and hybrid scheme. Link-by-link scheme uses the ABR feedback loops of AIM layer between CL user and CL server or between CL servers, and the header translation table carrying out the routing function of frame. And hybrid scheme is the scheme that adds end-to-end loop to link-by-link scheme and then uses RM message using CLNAP-PDU(ConnectionLess Network Access Protocol - Protocol Data Units) of CL layer. These schemes can be implemented easily because of using the ABR loop of ATM layer, end-to-end loop of CL layer, and the routing function of CL server. In this paper the characteristic and effects of two schemes in CL server bas been compared and analyzed.

The Effects of Supplementation of BSA or Fatty Acid Free BAS on the Motility of Fresh or Cryopreserved Rooster Spermatozoa (BSA 또는 Fatty Acid Free BSA 첨가가 닭 희석 정자와 동결 정자의 생존성에 미치는 영향)

  • This study examined factors affecting the analysis of motility of chicken semen. The viability of spermatozoa was estimated using varying dilution ratios and supplementation with BSA or fatty acid free (FAF)-BSA as protein sources in semen diluent. Fresh semen was examined after preparing dilutions in beltsvile poultry semen extender (BPSE) of 1/8, 1/16 and 1/32 at $25^{\circ}C$. The motility of incubated semen at each dilution was observed at 3 min (89.9%, 69.9% and 53.2%), 30 min (86.7%, 71.4% and 51.7%), 1 h (89.5%, 74.0% and 53.5%) and 3 h (78.5%, 66.5% and 45.7%), respectively. The addition of BSA or FAF-BSA to BPSE diluent significantly increased the viability of semen in 1/32 dilution with results of 53.2% (control), 84.8% (BSA) and 92.9% (FAF-BSA) (p<0.05). This phenomenon was also observed in the dilution of frozen semen, where FAF-BSA treatment increased the viability of thawed semen from 17.6% to 34.0% in a 1/8 dilution (p<0.05). When the protein sources were used in the dilution, the survival rates of diluted chicken semen were also increased with time lapse. These results show that FAF-BSA may act to protect chicken semen and is suitable as a basic component of chicken semen diluent for the method of analyzing rooster semen after freezing.

Effect of DanGuiBoHyulTangGami-Bang on the Alopecia and Hair Growth Stimulation (當歸補血湯加味方이 모발탈락 및 성장에 미치는 영향)

  • We made an experiment if the extracts of DanGuiBoHyulTangGami-Bang(DBTG) and 15 kinds of the medical herbs used the materials of DBTG were effective on the hair formation palpation and the falling out of hair, and came to the following conclusions. 1. The extracts of Paeonia lactiflora, Cuscuta chinensis and Angelica tenuissima of DBTG consisted of the 15 kinds of the medical herbs kept the activity of 5${\alpha}$-reductase type Ⅱ from being active 75.3$\%$, 63.8$\%$. 75.5$\%$. 2. The hair growth index, 1.6(control group 0.8) of the extracts of DBTG bas a little effect on the hair growth palpation and that of Rubus coreanus 1.8(control group 0.4) was the most effective one of the medical herbs, and Paeonia lactiflora 2.3(control group 1.7) and Vitex rotundifolia 2.3(control group 1.5) showed the effect on hair formation palpation. 3. The hair growth period couldn't be extended by DBTG in this experimental stage. 4. The 15 kinds of constitution medicines of DBTG didn't have effects in dermal papilla cells DNA increase, IGF- I, KGF, HGF the revelation of a gene heredity, the protein synthesis of the hair follicle tissues. 5. All of the 15 kinds of constitution medicines of DBTG didn't have the antibacterial activity in Paper disc rule. 6. The results from the test of a radical scavenging activity of the 15 kinds of constitution medicines of DBTG showed that the extracts of Paeoria lactiflora, Scutellaria baicalensis, Rubus coreanus have the superior antioxidant activity in the concentration of 0.01$\%$ and 0.001$\%$ 7. In the formation controlled experiment, Vitex rotundifolia (70.6$\%$), Scutellaria baicalensis (47.1$\%$, Saposhnikovia (44.8$\%$) of the 15 kinds of constitution medicines of DBTG in the 50㎍/㎖ concentration controled NO forming and Vitex rotundifolia (12.7$\%$) controled NO forming in the 5㎍/㎖ concentration in order. 8. MTT(lC/50) of the extracts of Rehmannia glutinosa, Paeonia lactiflora, Scutellaria baicalensis, Lycium chinense, Rubus coreanus of the 15 kinds of constitution medicines of DBIG was more than 500㎍/㎖ and had the least cell virulence.

Effect of Molasses or Rice Gruel Inclusion to Urea Supplemented Rice Straw on Its Intake, Nutrient Digestibilities, Microbial N Yield, N Balance and Growth Rate of Native (Bas indicus) Growing Bulls

  • The possibility of using rice gruel compared to that of the cane molasses as a source of readily fermentable energy for a urea supplemented straw diet has been studied. Twelve native growing bulls of $237{\pm}8.7kg $ live weight and months old were randomly allocated to three treatments fed solely rice straw enriched with : (1) 3% urea (US), (2) 3% urea + 15% molasses (UMS) and (3) 3% urea + 30% rice gruel (UGS). The feeding trial continued for sixty days. Organic matter (OM) intake was significantly (p < 0.05) higher in the UMS ( $64g/kg\;W^{0.75}/d$) followed by UGS ($53g/kg\;W^{0.75}/d$) and US ($49g/kg\;W^{0.75}/d$). Estimated (from digestible OM intake) metabolizable energy (ME) intake were 396, 348 and $301kJ/kg\;W^{0.75}/d$ for UMS, UGS and US respectively. The maintenance (i.e., no change in live weight) ME intake calculated to be $308{\pm}7.4kJ/kg\;W^{0.75}/d$. Urinary purine derivatives excretion was nonsignificantly higher in the UMS (51.73 mmol/d), followed by UGS (42.53 mmol/d) and US (35.26 mmol/d). The estimated microbial N (MN) yield were 21.10, 14.00 and 11.60 g/d for UMS, UGS and US respectively. For each MJ increase in ME intade, MN yield increased by $1.29{\pm}0.134g$. Observed live weight changes during the experimental period were 292, 125 and -19 g/d respectively for UMS, UGS and US. It was concluded that supplementation of readily fermentable N (urea) alone was not enough to optimize the rumen function and a source of readily fermentable energy was required. Rice gruel was less effective than molasses as fermentable energy source to remove a restriction on voluntary intake and provide less amino acids of microbial origin for absorption from the small intestine, Thus more substrate for protein synthesis and gluconeogenesis were available for growth in the molasses than the rice gruel supplemented animals. However, in situation where molasses is not available or costly, rice gruel does appear to have a place as readily fermentable energy source on a urea supplemented straw diet.
