Changes of Temporal Processing and Hearing in Noise after Use of a Monoaural Hearing Aid in Patients with Sensorineural Hearing Loss: A Preliminary Study

  • Background and Objectives: The relationship between hearing aid (HA) use and improvement in cognitive function is not fully known. This study aimed to determine whether HAs could recover temporal resolution or hearing in noise functions. Materials and Methods: We designed a prospective study with two groups: HA users and controls. Patients older than 45 years, with a pure tone average threshold of worse than 40 dB and a speech discrimination score better than 60% in both ears were eligible. Central auditory processing tests and hearing in noise tests (HINTs) were evaluated at the beginning of the study and 1, 3, 6, and 12 months after the use of a monaural HA in the HA group compared to the control group. The changes in the evaluation parameters were statistically analyzed using the linear mixed model. Results: A total of 26 participants (13 in the HA and 13 in the control group) were included in this study. The frequency (p<0.01) and duration test (p=0.02) scores showed significant improvements in the HA group after 1 year, while the HINT scores showed no significant change. Conclusions: After using an HA for one year, patients performed better on temporal resolution tests. No improvement was documented with regard to hearing in noise.

고해상도 전산화 단층촬영에서 확인된 골절 유형에 따른 측두골 골절의 진단에서 뇌전산화 단층촬영의 유용성 (Utility of Brain Computed Tomography in Detecting Fractures of the Temporal Bones Correlated with Patterns of Fracture on High-Resolution Computed Tomography)

  • Purpose: The clinical utility of brain computed tomography (CT) in detecting temporal bone fracture is not well established. We performed this study to determine the utility of brain computed tomography (CT) in detecting fractures of the temporal bones in correlation with fracture patterns. We used high resolution computed tomography (HRCT) as the gold standard for diagnosing temporal bone fracture and its pattern. Methods: From January 2007 to December 2009, patients who underwent both brain CT and HRCT within 10 days of head trauma were investigated. Among them, 58 cases of temporal bone fracture confirmed by HRCT were finally included. Fracture patterns (transverse or non-transverse, otic capsule sparing or otic capsule violating) were determined by HRCT. Brain CT findings in correlation with fracture patterns were analyzed. Results: Among 58 confirmed cases of temporal bone fracture by HRCT, 14 cases (24.1%) were not detected by brain CT. Brain CT showed a significantly lower ability to detect temporal bone fracture with transverse component than without transverse component (p=0.020). Moreover, brain CT showed lower ability to detect otic capsule violating pattern than otic capsule sparing pattern (p=0.015). Among the 14 cases of temporal bone fracture that were not detected by brain CT, 4 cases lacked any objective physical findings (facial palsy, hemotympanum, external auditory canal bleeding) suggesting fractures of the temporal bones. Conclusion: Brain CT showed poor ability to detect temporal bone fracture with transverse component and otic capsule violating pattern, which is associated with a poorer clinical outcome than otic capsule sparing pattern. Routine use of HRCT to identify temporal bone fracture is warranted, even in cases without evidence of temporal bone fracture on brain CT scans or any objective physical findings suggestive of temporal bone fracture.

조현형 인격 성향을 가진 대학생의 P300 국소화 분석 (Analysis of source localization of P300 in college students with schizotypal traits)

  • 본 연구는 청각 oddball 방안, 사건관련전위와 sLORETA를 사용하여 조현형 인격 성향을 가지는 대학생의 P300 신호원을 조사하였다. 또한 P300 신호원의 전류밀도와 조현병 증상간의 관계를 조사하였다. Schizotypal personality questionnaire(SPQ)의 점수에 근거하여 조현형 인격성향군(n=37)과 정상통제군(n=42)을 선정하였다. P300은 자주 제시되는 표준 자극(1000Hz)과 드물게 제시되는 목표 자극(1500Hz)으로 구성되는 청각 oddball 과제를 사용하여 측정하였으며, 참여자들은 목표 자극이 몇 번 제시되었는지 횟수를 세어 실험 후 보고하는 것이 요구되었다. 행동분석 결과, Oddball 과제의 정확률에서는 두 집단 간 유의한 차이가 관찰되지 않았다. 사건관련전위를 분석한 결과, 조현형 인격성향군이 정상통제군에 비해 유의하게 감소된 P300 진폭을 보였다. P300의 신호원을 추정하기 위하여 sLORETA를 사용하여 분석한 결과, 두 집단 모두에서 좌우반구 전두엽, 두정엽, 측두엽, 후두엽에 분포되어 관찰되었다. 두 집단의 P300 신호원의 전류밀도를 비교한 결과, 조현형 인격성향군이 정상통제군에 비해 좌반구 상측두회와 우반구 중측두회에서 감소된 활성화를 보인 반면 좌반구 하전두회와 우반구 상전두회에서는 활성화 증가를 보였다. 이에 덧붙여, 조현형 인격성향군에서 우반구 상전두회의 전류밀도와 SPQ의 와해 점수 사이에 부적 상관이 관찰되었다. 이러한 결과는 P300 신호 원인 전두 및 측두 영역의 이상을 조현병 환자뿐만 아니라 경미한 증상을 가지고 있는 조현형 인격성향군도 가지고 있음을 시사한다. 이에 덧붙여 본 연구 결과는 조현병 고위험군의 선별에 사용되는 SPQ의 경우 총점보다는 와해 점수가 고위험군의 선별에 더 유용하게 사용될 수 있음을 시사한다.
