청력손실과 보청기 조절 오류의 발견 지연 1례 보고 (A Case Report of Delayed Identification of Hearing Loss and Hearing Aid Fitting Failure)

  • 청각학적 재활은 청력손실 배경 정보 획득, 청각학적 평가, 보청기 전기음향특성 분석, 음향 이득 결정만으로도 충분히 만족스러운 서비스를 제공할 수 있다. 그러나 청력은 나빠질 수 있고, 보청기는 출력음압이 낮아지거나 고장이 발생할 수도 있다. 따라서 청력손실의 조기 발견과 정기적인 추적 관찰은 매우 중요하다. 이 연구는 정상적인 추적에도 불구하고 보청기 조절 실수가 확인된 증례를 통해 효과적인 청각언어재활 방향을 고민하는데 목적이 있다.

  • 학령 전기 아동은 옹알이 단계부터 8세까지 모국어의 음운 체계를 습득하고 정상적으로 산출한다. 이 시기 청력손실은 외부 소리 자극을 수용하는데 어려움이 따른다. 청각적 수용의 문제는 언어 습득 및 발달을 지체시키고, 의사소통 장애를 일으킬 수 있다. 이외에도 학습, 사회성, 정서 발달 등에도 영향을 준다. 청력 손실은 조기 선별과 진단 및 중재가 중요하다. 그러나 그 정도가 낮거나 특정 음역에 국한된 경우 발견이 어려울 수 있고, 발견하더라도 재활 중재로 연결되기 어려운 경우가 많다. 이 연구는 학령 전기 아동에서 나타난 경도 및 중등고도 대칭성 고음급추형 감각신경성 난청의 사례를 청각학적 평가 결과 해석과 함께 의사소통 관련 문제를 분석하고, 청각 언어병리학적 재활 방향 등을 고찰하고자 한다.

가청범위압축방식 보청기의 청각학적 이득에 관한 연구 (Audiological Benefits in Dynamic Range Compression Hearing Aids)

  • Hearing aids play an very important role in aural rehabilitation for hearing impaired person who could not be medically treated. Especially, hearing aids with dynamic range compression (level dependent frequency response: LDFR) method concentrated dynamic output range of receiver into the narrowing dynamic range of sensorineural hearing impaired person. Thus, if aided hearing threshold level is improved and uncomfortable loudness level rises, then the users of hearing aids are favored with extended dynamic range. This study aims to evaluate audiological benefits in dynamic range compression hearing aids. To achieve this, pure tone and speech audiometry were examined to 15 children and 3 adults with narrow dynamic range moderate-to-severe binaural sensorineural hearing loss.

Evaluation of Saccule Function in Patients with Vitamin D Deficiency

  • Background and Objectives: Considering important role of vitamin D in many physiological processes including vestibular system in the ear, aim of present study is to evaluate saccule function via cervical vestibular evoked myogenic potential (cVEMP), in patients with vitamin D deficiency. Subjects and Methods: After routine audiological tests, cVEMP were recorded in 15 patients with vitamin D deficiency and 16 normal subjects. The short tone burst (95 dB nHL, 500 Hz) was presented to ears. cVEMP was recorded with surface electromyography over the contracted ipsilateral sternocleidomastoid muscle. Results: Mean of p13, n13, interpeak latencies and amplitude ratios were measured in both groups. Statistical analysis did not show differences between two groups. Conclusions: Maybe serum 25-hydroxyvitamin D concentration was not low enough to have effect on saccule in the patients in present study or saccule have had low susceptibility to effects of vitamin D deficiency. For better judgment about effect of vitamin D deficiency on saccular function planning studies with high sample size is recommended.

정상 청력 아동의 음절 간 쉼 간격에 따른 어음이해도 변화 (Changes of Speech Discrimination Score Depending on Inter-syllable Pause Duration in Normal Hearing Children)

  • 어음 이해는 발화속도에 영향을 받는다. 발화속도는 쉼 간격으로 조절할 수 있는데, 쉼 간격은 정보 처리과정에서 여유 시간을 가질 수 있어서 정보의 과부하를 피할 수 있다. 이 연구는 쉼 간격이 노화에 따른 청력손실과 청각재활, 청각처리 과정에 미치는 영향을 알아보기 위한 기초 연구로서 그 정상치를 알아보고자 한다. 연구 대상은 청각학 및 언어병리학적 문제가 없는 일반 초등학생 남자 7명, 여자 8명으로 하였다. 검사 도구는 3음절 20개를 1 set으로, 모두 4 set를 제작하였다. 이들 모든 낱말은 각각 보통(250 ms), 느린(500 ms), 아주 느린(1000 ms) 속도로 쉼 간격을 조절하였다. 선택용 보기는 올바르게 표기한 3음절 낱말 하나와 음소 하나씩을 오류 표기한 세 개의 낱말을 포함한 4개의 낱말로 하였다. 대상자에게는 3음절 낱말을 들려 준 후, 하나를 선택하게 하였다. 연구 결과 쉼 간격에 따른 평균 어음이해도는 250, 500, 1,000 ms의 순서로 $73{\pm}19.4%$, $84{\pm}12.2%$, $88{\pm}8.8%$로 각각 나타났다.

A Review of Contemporary Teleaudiology: Literature Review, Technology, and Considerations for Practicing

  • The scope of teleaudiology has been noted with telehealth due to Coronavirus disease (COVID-19) recently. As the notion has been around us for more than 20 years ever since 1999, it is necessary to perceive the knowledge accurately and prepare for the successful implementation of it. Therefore, the literature review including screening and diagnostic audiometry, cochlear implants and hearing aids, and aural rehabilitation, telecommunications technology regarding several fields of teleaudiology, and considerations for practicing were identified. Although overall internet-based audiological services showed benefits in terms of outcome and accessibility, uncertainties of cost-effectiveness, the optimal level of support, and a need for further studies of many aspects for teleaudiology has arisen. In the view of technology, the store-and-forward (asynchronous/hybrid) and a real-time (synchronous) methods were introduced with one applied and nine registered patents recorded from 2004 to 2020 for the invention of teleaudiology in the United States. Also, 10 checklists were suggested for planning teleaudiology practice from prior experience in hosting the teleaudiology program. Conclusively, it is hoped that this review sheds light on recognizing and improving the existing teleaudiology services and helps overcome the challenges faced in the era of pandemic and untact world to come.

외이도용적에 따른 외이도공명의 변화 (Resonance Changes in the External Auditory Canal Associated with the Ear Canal Volume)

  • The external ear generates resonance gain because of anatomical characteristics. The ear canal resonance is influenced by the length and volume of the ear canal, the pinna, the concha cavity, the body trunk, and the speed of sound wave. This study is focus on the influence of the volume of ear canal. 17-healthy-adult (32 ears) were participated. They did not have any medical and ear disease history. The maximum resonance frequency of the ear canal was 2675 (${\pm}265$) Hz at azimuth $0^{\circ}$ and 2784 (${\pm}268$) Hz at azimuth $45^{\circ}$. The resonance gain was 18.1 (${\pm}3.9$) dB at azimuth $0^{\circ}$ and 17.9 (${\pm}3.8$) dB at azimuth $45^{\circ}$, respectively. The ear canal volume was 0.78 (${\pm}0.2$) cc and 1.32 (${\pm}0.8$) cc including static compliance. The ear canal resonance was changed depending on the ear canal volume. It was also statistically correlated at azimuth $0^{\circ}$ (p=0.038) and $45^{\circ}$ (p=0.013), respectively. The resonance gain was not correlated with the ear canal volume. The change of resonance frequency according to the ear canal volume will be useful information in the field of audiological rehabilitation especially for hearing aids fitting. In addition, we expected this study can provide the basic information for the study of the external ear resonance characteristics.

Masking Level Difference: Performance of School Children Aged 7-12 Years

  • Background and Objectives: In masking level difference (MLD), the masked detection threshold for a signal is determined as a function of the relative interaural differences between the signal and the masker. Study 1 analyzed the results of school-aged children with good school performance in the MLD test, and study 2 compared their results with those of a group of children with poor academic performance. Subjects and Methods: Study 1 was conducted with 47 school-aged children with good academic performance (GI) and study 2 was carried out with 32 school-aged children with poor academic performance (GII). The inclusion criteria adopted for both studies were hearing thresholds within normal limits in basic audiological evaluation. Study 1 also considered normal performance in the central auditory processing test battery and absence of auditory complaints and/or of attention, language or speech issues. The MLD test was administered with a pure pulsatile tone of 500 Hz, in a binaural mode and intensity of 50 dBSL, using a CD player and audiometer. Results: In study 1, no significant correlation was observed, considering the influence of the variables age and sex in relation to the results obtained in homophase (SoNo), antiphase (SπNo) and MLD threshold conditions. The final mean MLD threshold was 13.66 dB. In study 2, the variables did not influence the test performance either. There was a significant difference between test results in SπNo conditions of the two groups, while no differences were found both in SoNo conditions and the final result of MLD. Conclusions: In study 1, the cut-off criterion of school-aged children in the MLD test was 9.3 dB. The variables (sex and age) did not interfere with the MLD results. In study 2, school performance did not differ in the MLD results. GII group showed inferior results than GI group, only in SπNo condition.
