한국형 환자참여 간호사 침상인계 프로토콜 개발 및 평가 (Development and Evaluation of a Protocol for Bedside Nursing Handoff with Patient Engagement in a Tertiary Hospital in South Korea)

  • Purpose: This study aimed to develop a bedside nursing shift report protocol and evaluate the effect of the protocol in a tertiary hospital in South Korea. Methods: The bedside nursing handoff protocol with patient engagement was developed based on the literature review and the validation of an expert group. The effect of the protocol on clinical implication was tested in three medical-surgical units in a tertiary hospital. Outcomes were assessed by patient perception, nurse perception, and reporting time. Data collected from June to August in 2018 and analyzed with descriptive statistics and One-way ANOVA using SPSS version 25.0. Results: The bedside nursing shift report protocol with patient engagement consisted of two steps: nurse to nurse report and bedside report with patients. Nurse's perception with patient engagement was significantly increased after applying protocol (F=17.85, p<.001). Patient's perception was significantly improved in the areas of discharge plan (F=7.86, p<.001), health information privacy (F=4.46, p=.012) and identify attending nurse (F=3.19, p=.042). There were no differences in reporting time between the bedside nursing shift report and a traditional shift report (F=0.61, p=.054). Conclusion: Patient perception was significantly increased, while nurse perception was not different after applying this protocol. For the change in the perception of nurses, education may be preceded to improve nurses' competence for the bedside shift report. Furthermore, the support in enough nurse staffing should be needed for encouraging the bedside shift report. The bedside shift report may enhance patient engagement. Therefore it may improve patient safety and health outcome in clinics.

Incidental findings in a consecutive series of digital panoramic radiographs

  • Purpose: The aim of this study was to determine the prevalence of incidental findings(IFs) on digital dental panoramic radiographs(DPRs) of asymptomatic patients attending a general dental practice. Materials and Methods: This was a retrospective study of 6,252 consecutive digital (photostimulatable phosphor) DPRs of patients who visited a Canadian general dental practice for a complete new patient examination. The IFs were grouped into dental-related anomalies, radiopacities and radiopacities in the jaws, changes in the shape of the condyles, and other findings in the jaws, such as tonsilloliths and mucosal antral pseudocysts. Their prevalence was determined. Results: Thirty-two percent of the DPRs showed at least 1 IF. The highest prevalence was found for dental-related anomalies(29% of all DPRs), of which impacted teeth were the most prevalent finding (24% of all DPRs), followed by idiopathic osteosclerosis(6% of all DPRs). A lower prevalence was noted for tonsilloliths(3%), and the prevalence of root tips, mucosal antral pseudocysts, and anomalies in condylar shape was approximately 1% each. Conclusion: The observed prevalence of 32.1% for IFs of any type underscores the need for a dental practitioner to review the entire DPR when a patient presents for an initial dental examination (or check-up) or for dental hygiene. Only a single IF (a central giant cell granuloma) provoked alarm, as it was initially considered malignant. Similarly, impacted teeth and suspected cysts need careful evaluation upon discovery to determine how they may be optimally managed.

Utilization of a Scored Patient-Generated Subjective Global Assessment in Detecting a Malnourished Status in Gynecologic Cancer Patients

  • Purpose: To assess the prevalence of malnutrition in gynecologic cancer patients using the Scored Patient-Generated Subjective Global Assessment (PG-SGA) questionnaire. Materials and Methods: A total of 97 gynecologic cancer patients who never had any treatment but were planned for surgery were enrolled. The patients were asked to complete the scored PG-SGA form before the treatment was started. Attending physicians were also asked to complete other information in the PG-SGA form. Total scores were calculated and the patients were classified into 3 nutritional status levels. Results: Mean age was 54 years. Postoperative diagnoses were endometrial cancer in 42 cases (43.2%), ovarian cancer in 29 cases (29.9%), and cervical cancer in 26 cases (26.8%). Mean PG-SGA score was 5.2+4.7. Malnutrition (PG-SGA B and C) was found in 52 patients (53.6%, 95% CI 43.7% - 63.2%). Preoperative BMI, hemoglobin, serum albumin, and cancer stage were not significantly associated with nutritional status. Malnutrition was significantly more common among patients diagnosed with ovarian cancer, compared to other types of cancer (79.3% vs. 42.6%, p 0.004). Conclusions: Prevalence of malnutrition among gynecologic cancer patients was 53.5%, according to the scored PG-SGA. Malnutrition was significantly more common among patients with ovarian cancer.

A Pilot Study on Factors Associated with Presentation Delay in Patients Affected with Head and Neck Cancers

  • Background: Patient delay can contribute to a poor outcome in the management of head and neck cancers (HNC). The main objective of the present study was to investigate the factors associated with patient delay in our population. Materials and Methods: Patients with cancers of the head and neck attending a regional cancer center of North East India were consecutively interviewed during the period from June 2014 to November 2014. The participation of patients was voluntary. The questionnaire included information on age, gender, residential status, educational qualification, monthly family income, any family history of cancer, and history of prior awareness on cancer from television (TV) program and awareness program. Results: Of 311 (n) patients, with an age range of 14-88 years (mean 55.4 years), 81.7% were males and 18.3% females (M:F=4.4). The overall median delay was 90 days (range=7 days-365 days), in illiterate patients the median delay was 90 days and 60 days in literate patients (P=0.002), the median delay in patients who had watched cancer awareness program on TV was 60 days and in patients who were unaware about cancer information from TV program had a median delay of 90 days (p=0.00021) and delay of <10 weeks was seen in 139 (44.6%) patients, a delay of 10-20 weeks in 98 (31.5%) patients, and a delay of 20-30 weeks in 63 (20.2%) patients. Conclusions: Education and awareness had a significant impact in reduction of median patient delay in our HNC cases.

An unusual abscess formation in the masticator space after acupressure massage: a case report

  • Clinical features of masticator-space abscess (MSA) are very similar to those of parotitis or temporomandibular disorder (TMD), making early differential diagnosis difficult. Local causes of MSA include nerve block anesthesia, infection after tooth extraction, and trauma to the temporomandibular joint (TMJ); the systemic cause is immunodeficiency. Odontogenic causes account for most etiologies, but there are also unusual causes of MSA. A 66-year-old male patient visited the emergency room (ER) presenting with left-side TMJ pain three days after receiving an acupressure massage. He was tentatively diagnosed with conventional post-trauma TMD and discharged with medication. However, the patient returned to the ER with increased pain. At this time, his TMD diagnosis was confirmed. He made a third visit to the ER during which facial computed tomographic (CT) images were taken. CT readings identified an abscess or hematoma in the left masticator space. After hospitalizing the patient, needle aspiration confirmed pus in the infratemporal and temporal fossa. Antibiotics were administered, and the abscess was drained through an incision made by the attending physician. The patient's symptoms decreased, and he was discharged.

소아 방사선 검사 시 보호자 참여에 대한 인식도 조사 (A Study on the Guardian's Perception of Attending Patient in Pediatric Radiography)

  • 소아 방사선 검사 시 참여 여부와 방사선에 대한 인식도를 파악하여 검사의 질을 향상시키기 위해 부산 P대학병원에 내원한, 소아 아동이 있는 보호자 210명을 대상으로 2013년 8월 20일부터 2013년 9월 15일까지 설문조사하였으며 "과거에 소아 방사선 검사에 참여 해본 적이 있는가?" 라는 질문에 "그렇다"(66.2%)로 나타났고 참여한 이유는 "방사선사의 요청"(66.0%)로 가장 높게 나타났으며 "소아 방사선 검사 시 보호자의 참여가 필요하다고 생각하는가?"라는 질문에 "그렇다"(84.3%)로 나타났으며 그 이유는 "아이의 안정을 위해"(80.8%)였으며 "참여할 생각이 없다면 그 이유는 무엇인가?" 라는 질문에는 "방사선사의 업무이기 때문에"(54.8%)로 나타났다. 보호자의 심리상태 인식도 분석에서는 성별 분석을 보면 여성(3.40)이 남성(3.23)보다 평균이 높게 나타났으며, 소아 연령 분석을 보면 평균이 영아기(3.62)가 다른 소아 연령대보다 높은 것으로 나타났고 학령기(3.34), 유아기(3.25)순으로 나타났으며 통계적으로 유의한 차이가 있었다.(p<0.05) 보호자의 방사선 인식도 분석에서는 성별 분석을 보면 평균이 남성(3.09)이 여성 (2.78)보다 높게 나타났으며, 학력별 분석을 보면 평균은 대학원 (3.36)이 가장 높게 나타났고 전문대졸(2.90), 대졸(2.80), 고졸(2.77) 순서로 나타났으며 통계적으로 유의한 차이가 나타났다.(p<0.05) 보호자의 방사선 피폭 방지에 대한 인식도 분석에서는 연령별로 살펴보면 평균이 20대(3.60)가 가장 높게 나타났으며 50대(3.33), 30대(3.04)의 순으로 나타났고 40대(2.94)가 가장 낮게 나타났으며 성별 분석을 보면 평균이 남성(3.12)은 여성(2.96)보다 높게 나타났고, 소아 연령별 분석을 보면 평균이 영아기(3.27)가 가장 높게 나타났으며 유아기(2.98), 학령기(2.98)로 나타났고, 학력별 분석을 해보면 평균이 대학원(3.31)이 가장 높게 나타났으며 고졸(2.99), 대졸(2.99) 순으로 나타났고 전문대졸(2.94)이 가장 낮게 나타났으며 통계적으로 유의한 차이가 있었다.(p<0.05) 보호자들은 우선적으로 아이의 안정과 정확한 검사를 위해 방사선사와 동반검사가 필요하다고 인식하고 있었으며, 동반검사 전에 검사방법에 대해 숙지를 시키고 보호자에게 보호 장구를 필히 착용시키며 소아의 심리적 불안감을 없애기 위해 검사실의 환경 등을 개선하고 시각적, 청각적으로 편안하게 다가갈 수 있는 방안이 필요할 것으로 사료된다.

동일 의료기관 내에서의 분업과 협진에 대한 법적 고찰 (A Legal Study on Division of Labor and Collaboration within the Same Medical Institution)

  • 우리나라에서 통상 사용하는 협진의 의미는 동일한 의료기관 내에서 서로 다른 진료과목의 의사가 환자를 함께 치료하는 경우로 이해된다. 그렇기 때문에 협진은 다양한 의료관여자들이 각자의 전문성에 의거하여 역할을 분담하여 환자를 치료하는 의료팀의 양상을 띠게 된다. 또한 의료팀 내 다른 진료과목의 의사는 동등한 지위에서 각각 전문성에 의거하여 수평적 분업을 하게 되므로 협진은 분업의 원칙에 따라 법적 책임이 분배된다. 대법원도 "여러 명의 의사가 분업이나 협업을 통하여 의료행위를 담당하는 경우 먼저 환자를 담당했던 의사는 이후 환자를 담당할 의사에게 환자의 상태를 정확하게 알려 적절한 조치를 할 수 있도록 해야 한다."고 하여 의료팀을 이루어 환자를 함께 치료하는 경우를 인정하고, 의사의 협진의무에 대하여 판단하고 있다. 다수의 진료과목이 있는 의료기관 내에서 서로 다른 진료과목의 의사가 분업이나 협업을 통하여 의료행위를 담당하게 되는 협진의 경우, 환자를 담당했던 의사는 환자의 상태에 따라 협진 여부를 결정하여야 하며, 이후 환자를 담당할 진료과목의 의사에게 환자의 상태를 정확하게 알려 적절한 조치를 취하게 하여야 한다. 협진을 하게 된 후임 의사 또한 환자에 대한 치료 종료 시까지 협진을 요청했던 전임 의사에 대하여 환자의 상태와 관련된 치료사항을 적극적으로 고지하고 서로 소견을 교환하여야 할 것이다. 다만 협진의 필요성에 대한 결정은 당시 환자의 상태에 따라 판단이 이루어져야 하는 것으로, 모든 경우에 협진의무가 강제되는 것이라고 단언할 수는 없다. 그리고 협진의 필요성에 대한 결정에 있어서 과실이 존재하는지 여부는 의사의 주의의무 판단에 대한 법리가 적용될 것이다.

두개척추접합부 결핵 : 이비인후과적 관점에서의 증례 보고 (Craniovertebral Junction Tuberculosis : A Case Report in Otolaryngologic Aspect)

  • Craniovertebral junction (CVJ) tuberculosis is a rare disease, and potentially fatal due to the risk of atlantoaxial dislocation. The disease usually accompanies a retropharyngeal cold abscess, which can cause subsequential otolaryngologic symptoms such asdysphagia, odynophagia, or airway obstruction. Such a patient must be handled with great care to avoid a disaster. The disease is diagnosed with microbiological or other laboratory tests on the pus collected through puncture and aspiration, which will need otolaryngologic skills. For treatment, otolaryngologists play an important role by doing incision and drainage of the retropharyngeal abscess, or by attending the transoral vertebra surgery as partners with spine surgeons who will manage the CVJ lesions and ensure the craniocervical stability.

한방병원 야뇨 환아의 후향적 연구 (A follow-up study of enuretic children)

  • Objective: The aim of this study was to investigate enuretic children attending oriental hospital and classify subtypes and evaluate possible factors that enhance or hamper of continence. Methods: Children attending kyeongju dongguk oriental hospital pediatrics between January 2000 and December 2003 with chief complaint of nocturnal enuresis were enrolled. The data of present symptoms and improvement progress was collected via telephone interviews with their parents. Results: The total number of children was 61. 32 of them were male and 29 female. Sex ratio was 1.1:1. Patient under 6 years of age was 68.8%. Using acupuncture, acupressure or chimsband with herb medicine, treatment frequency was increased. Patients with nocturnal enuresis(NE) was 48, and primary nocturnal enuresis(PNE) 39, secondary (SNE) 9. 13 was impossible to diagnose. The ratio of male to female was 1:1.09. PNE was 4 times as many as SNE. Male was predominent in PNE, and female in SNE. In the progress of PNE, male and female were similiar. but SNE, male was all cured, female remained 50%. And elapsed time of male to improve was longer than that of female. In the progress of improvement, many parents mentioned that their child was improved spontaneously regardless of subtype. Conclusion: We consider age, sex distinction, subtype, bladder symptoms, frequency, time of progress as influencing factor of prognosis, but failed to verify significance. To develop guideline of NE and find influencing factor of prognosis, more prospective study through taking history carefully and using questionnaire is needed.

Barriers to Low Vision Services and Challenges Faced by The Providers in Pakistan

There were two objectives of the study, first was to identify the barriers as perceived by the patients and providers to access the low vision services and second was to identify the challenges faced by the main providers. Study design. Structured questionnaire based interviews of patients and providers Methodology. To find out the barriers to access of low vision services, the interviews based on structured questionnaire were conducted for two patient groups. The first group consisted of 97 visually impaired individuals attending the department of low vision services at Al-Shifa Trust Eye Hospital Rawalpindi while the second group included 56 visually impaired individuals attending the four rehabilitation centers/schools for the blind in Rawalpindi/Islamabad. To identify the barriers as perceived by the main providers of low vision services and challenges faced by them the interviews based on structured questionnaire were conducted for 19 low vision service providers. Results. From patients point of view, major barrier to low vision services identified was inability to visit hospital /rehabilitation center alone - 29.8% in hospital group and 33.9% in rehabilitation centers group, while the lack of social support, lack of family support, cost of travelling, long distance, afford ability, hesitation in using devices and lack of satisfaction were other important barriers identified. From providers' point of view, major barrier to uptake of services was the need for repeated follow-ups. Optometrists were the main provider of low vision services contributing to 47.4% of the providers. The major challenge faced by the providers was motivation of patients to use low vision devices. Conclusion. The major barrier to low vision services according to the patients is inability to visit the hospital alone, while according to providers, it is the need for repeated follow up which proves major barrier towards uptake of services. The motivation is the major challenge faced by providers, majority of which are optometrists.
