도침을 이용한 안면부 위축성 흉터의 치험례 (A Clinical Report on Facial Atrophic Scars treatment with Acupotomy)

  • Objectives: The purpose of this study is to evaluate the clinical effectiveness of acupotomy in the treatment of various facial atrophic scars except acne scars. Methods: Four patients with facial atrophic scars were treated by superficial dermal undermining with acupotomy. They were treated six times, and evaluated by comparing before and after photos, PGA(Physician's Global assessment), SESES(The Stony Brook Scar Evaluation Score) and patient satisfaction. Results and Conclusions: After observing case photos, PGA, SBSES, patient's satisfaction, it can be considered that acupotomy is an effective treatment modality for facial atrophic scars and it has no significant side effects.

  • Background and Objective : Atrophic scars are the most common complication of acne. Many modalities are proposed but each does not yield satisfactory clinical outcomes. Among therapeutic modalities of acne scars, subcision is a simple, safe procedure with a different and basic mechanism for correcting atrophic and depressed scars. However, there are few reports about such procedure. This study performed to evaluate the effect of subcision treatment on atrophic acne scar. Methods : Five patients with atrophic acne scars of various types(rolling, superficial and deep boxcar, pitted and icepick) were treated by superficial dermal undermining with 25~27 gauge needles. We used GASC(Global Acne Scarring Classification). Satisfaction degree was asked from the patients. Results and Conclusions : After observing patients satisfaction, comparing case photos and GASC, it can be considered that subcision treatment brings great results when performed on atrophic post acne scars.

새싹테라피 3단계를 이용한 위축성 여드름 흉터 치료 20례 (Clinical Reports on Atrophic Acne Scar Treatment with Sae-ssack Therapy)

  • Objectives : The purpose of this study was to evaluate the therapeutic efficacy of Sae-ssack(sprout) therapy on atrophic acne scars. Methods : Twenty patients had atrophic acne scars on the face were treated with Sae-ssack therapy. Sae-ssack therapy consists of a total of three treatments, a first-stage Sae-ssack acupuncture, a second-stage texture treatment, and a third-stage anti-pigmentation therapy. The evaluation of the treatment effect was made by the Korean medical doctor who did not participate in the procedure, and the PGA(Physician's Global Assessment), QGASC(Qualitative Global Acne Scarring Grade System) and patient satisfaction were used as the evaluation index. Results and Conclusions : According to the above indicates, the Sae-ssack therapy was evaluated to be effective in the repair of atrophic acne scars. There were no visible or consistent side effects and patient satisfaction was also very high.

한의 피부 재생 치료로 호전된 대상포진과 수두로 인한 위축성 흉터 치료 10례 (Korean Medicine Skin Regeneration Therapy for Atrophic Scars Caused by Herpes Zoster and Varicella: A Report of Ten Cases)

  • Objectives : This study aims to evaluate the effectiveness of incisional needle therapy, a type of Korean medicine treatment, on atrophic scars caused by herpes zoster and varicella. Methods : The patients with atrophic scars resulting from herpes zoster and varicella were treated with incisional needle therapy and the effectiveness of the treatment was evaluated with Physician's Global Assessment (PGA) and The Stony Brook Scar Evaluation Score (SBSES). Results : After treatment, all patients demonstrated improvements in PGA and SBSES. The PGA score improved to 3.73±0.40, with SBSES changing from 1.60±0.60 to 4.80±0.80. Conclusions : We suggest that incisional needle therapy demonstrates effectiveness in treating scars resulting from herpes zoster and varicella, indicating its potential as a viable therapeutic option for individuals affected by these conditions.

  • 본 논문은 수두로 인한 위축성 흉터 환자 16명에 대한 새싹테라피(절개침) 시술 후 개선도를 평가하여 연구한 바를 기록하였다. 그 결과 새싹테라피는 수두 흉터에 대해 시술효과를 판단하는 척도인 PGA, SBSES에서 좋은 결과를 보였으며 환자 만족도도 3가지 항목으로 세분화하여 평가했을 때 높은 개선 점수를 보였다. 이런 임상 결과로 미루어 볼 때 새싹테라피는 수두로 인한 위축성 흉터에게 좋은 치료적 방법이 될 것으로 예상되며, 시술 중 통증 및 시술 후 붉은 기의 개선이 과제로 남기에 이에 대한 한의학적 치료법 연구가 추가로 요구되는 바이다.

하니매화레이저를 이용한 여드름 흉터 치료 1례 (Case Study on Treating Acne Scar Using Hani-maehwa Laser)

  • Objective : The aim of this study is to report the effect of Hani-maehwa laser(fractional mode) treatment on acne scar. Methods : The patient with atrophic facial acne scars was treated with 5 sessions of laser therapy at 4-week intervals. The therapeutic response to treatment was assessed at 3 months after last laser session. The treatment effect was evaluated by standardized photography, anti aging, skin brightness and flatness. That was measured by micro fluorescence measurement camera(ECO SKIN, Cuvitz Inc.) in 16(left zygoma) and 26(right zygoma) spot. And side effects were also checked. Results : Atrophic facial acne scars have improved markedly. Anti aging, skin brightness and flatness were improved. Especially anti aging and flatness were noticeably improved. Adverse effects were not reported.Conclusions : Korean medicine cautery method applies to high level laser(CO2 Hani-maehwa laser). These are different designs but same principle. It can be considered that laser therapy is effective method for treating acne scars.

  • Scars can cause great psychological stress among patients. Currently, there are numerous topical agents, laser and surgical treatments available for skin rejuvenation and scar minimization. Laser-assisted drug delivery (LADD) is a treatment method that increases drug delivery by stimulating the skin physically and chemically to enhance the penetration of topical agents. This is one of the areas of great interest in the treatment of various skin diseases in addition to its use for cosmetic purposes. In particular, LADD is relatively non-invasive and has advantages in terms of accessibility and stability. Poly-L-lactic acid (PLLA) is a collagen stimulator known to gradually restore skin volume by inducing inflammation and fibroplasia. Herein, we report a case of treatment of an atrophic scar with fractional carbon dioxide laser-assisted PLLA delivery.

  • Objectives : The purpose of this study is to present the effect of subcision, microneedle therapy system and herbal peeling therapy which is treatment of Korean medicine to atrophic acne scar patients. Methods : We administered subcision, microneedle therapy system and herbal peeling therapy to atrophic acne scar patients and tested the results by Qualitative Global Acne Scarring Grading System and Korean Acne Grading System as well. Results : After the treatment, all of the patients showed improvements in Qualitative Global Acne Scarring Grading System and Korean Acne Grading System as well. Qualitative Global Acne Scarring Grading System of patients changed from grade 2.71±0.71 to grade 1.14±0.86 along with the change of Korean Acne Grading System from grade 1.57±0.57 to grade 1.00±0.00. Conclusions : Subcision, microneedle therapy system and herbal peeling therapy may be effective in the treatment of moderate to severe acne scars, and could be applied in clinical practice.

  • Objective: Acne scarring is the most common sequela after dermal inflammation caused by acne. Many methods are currently used to treat acne scarring; doctors using traditional Korean medicine have also attempted to treat acne scarring in a new way that was developed from traditional methods. This study examines the effectiveness of re-cell therapy.Method: Eleven patients with various types of acne scars were treated by re-cell therapy. We used the global acne scarring classification (GASC) and ECCA (from the French “echelle d’evaluation clinique des cicatrices d’acne”), both of which are generally used to measure and grade the degree of acne scarring.Results: As a result of the treatment, the conditions of all patients were conspicuously improved in both scales. The GASC scores of the patients changed from grade 19.6±10.4 to grade 4.9±3.3 (p=0.001), and the ECCA scores changed from grade 120.9±28.7 to grade 41.8±18.3.Conclusion: Re-cell therapy showed effectiveness in treating acne scarring and could be applied in clinical cases.

Childhood cutaneous leishmaniasis: report of 117 cases from Iran

  • Cutaneous leishmaniasis (CL), due to Leishmania major, is endemic in different parts of Iran and has long been recognized in most provinces of Iran. This study was conducted to determine the prevalence of childhood leishmaniasis in 3 areas at the southeast of Kashan, A descriptive study was carried out on all children referred to central laboratories during a 3-year period. Initial information including age, sex, sites of ulcer on the body, number of lesions, address, and the place of the disease was obtained. The study gathered 117 children, and the results showed a prevalence of 7.2% in patients with lesions among the population and 4.2% of people displayed lesion and scar. The ages of subjects were from 6 to 15 years (average 9.75 years). The boy: girl ratio was 1.2. All of our patients lived in an endemic area. The face was affected in 47.0% of cases. The encountered forms of leishmaniasis are as follows: papulonodular 27.4%, ulcer 60.7%, sporotrichoid 6%, impetiginous 2.5%, and erysipeloid 3.4%. Treatment with intramuscular meglumine antimoniate 20-30 mg/kg/day was done for 93 patients. Meglumine antimoniate treatment was tolerated with no side effects. All leishmaniasis lesions healed within an average period of 2-14 months. Hyperpigmented scars were formed in 25.6% of the patients, atrophic scars in 4.3%, and hypopigmented scars were in 3.4%, respectively The findings of this study indicate increased prevalence of CL in the villages at the area of Kashan and Aran-Bidgol. The clinical finding patterns belonged to different endemic strains of L. major in Isfahan, which indicates the possible transmission of infection from Isfahan to this area.
