아토피 피부염 환자의 지방산 섭취와 혈중 지방산 조성 및 면역 지표에 관한 연구 (Study of Dietary Fatty acids, Blood Fatty Acid Composition, and Immune Parameters in Atopic Dermatitis Patients)

  • The prevalence of atopic dermatitis (AD) continues to rise in industrialized countries related to Western lifestyle, including dietary habits, especially imbalance of intake of dietary fatty acids. The purpose of this study was to evaluate the dietary fatty acids and the assess the blood fatty acid composition and immune parameters in AD patients. AD (n = 50) patients and gender ${\cdot}$ age matched healthy controls (HC) were studied in case-control clinical trail. Current fatty acids intake status was determined by 3-day food record method. Blood sample were collected from 30 subjects in each group and blood fatty acid composition and immune parameters were analysed. AD patients consumed less PUFA and their n-6/n-3 PUFA ratio was higher than that of HC. Both the ratios of PUFA and MUFA were positively correlated with SCORAD in AD patients (p < 0.05). In the AD patients, there were abnormalities in the fatty acid composition of the RBC and WBC, SFA being significantly high and most n-3 PUFA being significantly low. Moreover, both the ratios of EPA and DHA in WBC were negatively correlated with dietary n-6/n-3 PUFA ratio in AD patients (p < 0.05). Serum total IgE and IL-4 levels of AD patients increased significantly compared with the levels of HC (p < 0.01). Ratios of monocyte and eosinophil in WBC of AD patients increased significantly compared with the levels of HC including total WBC count (p < 0.01), and ratios of Iymphocyte and basophil in WBC of AD patients decreased significantly compared with the levels of HC (p < 0.05). Moreover, the ratios of eosinophil in WBC were positively correlated with dietary P/M ratio (p < 0.05), and the ratios of monocyte in WBC were positively correlated with n-6/n-3 PUFA ratio (p < 0.05) in AD patients. This results indicated that AD patients had significantly high intake of dietary n-6/n-3 PUFA compared with HC. Imbalance of intake of dietary fatty acids affected fatty acid compositions in the RBC and WBC, and these lead to immune imbalance and grow worse of AD.

Materials and Methods : Atopic dermatitis (AD) usually develops in patients with an individual or family history of allergic diseases, and is characterized by chronic relapsing inflammation seen specially in childhood, association with IgE hyperproduction and precipitation by environmental factors. However, the exact etiology of AD has been unclear. To further explore the pathogenesis and treatment of AD, a suitable animal model is required. We found that skin lesions, which were chnically and histologically very simlar to human AD, mite antigen-induced dermatitis on the face, neck, ears and dorsal skin of inbred NC/Nga mice. Results and Conclusion : Kami-KangHwalSan medicines controlled CD3+/CD69+, CD4+/CD25+, B220+/IgE+, and B220+/CD23+ revelation that an experiment that motive allergy immune reponse because an in vitro experiment stimulates splenocytes of a NC/Nga mouse same t1me by PWM, and interleukin-4, eotaxin 2, CCR3, TARC mRNA outturn that bear in splenocytes decreased remarkably by Kami-KangHwalSan medicines. Th1 cell and Th2 cell observe to be shifted by secretion amount of IL-4 and IFN-$\gamma$ by Kami-KangHwalSan medicines could know that Kami-KangHwalSan medicines can use usefully in allergy autoimmnune diease.

아토피피부염 동물 병태 모델에서 청화탕(淸華湯)의 면역조절작용에 관한 연구 (A Study on the Immune Modulation of Chunghwatang(CHT) in Atopic Dermatitis Animal Models)

  • Objectives : The aim of this study was to investigate the effect of Chunghwatang (CHT) for atopic dermatitis. Methods : CHT, a verified anti-oxidant and anti-inflammatory effect, was treated in atopic dermatitis animal model to investigate cytokine levels and immunoglobulin production. Results : Clinical skin index was 47.1%, suggesting significant efficacy of CHT in atopic dermatitis treatment. Serum IL-4, IL-6, IL-13, TNF-${\alpha}$ and histamine productions were significantly decreased to 52.3%, 61.8%, 68.0%, 37.4%, and 46.6%, respectively. The production of IL-5 was decreased to 33.3%. The increase of Immunoglobulin IgG1 production along with IgE through the interaction of IgE and IL-4 induced by IL-4 and IL-13 were measured as 20.7% and 23.4%, respectively. Conclusions : The results above, along with the in vitro test results, strongly supports the CHT samples as effective immunomodulator in AD treatments, suggesting its use in clinical practice and basic information for EBM.

금은화, 연교 및 황련해독탕 약침이 아토피 피부염 유발 NC/Nga 생쥐에 미치는 영향 (The Effects of Lonicerae Flos, Forsythiae Fluctus and Hwangryunhaedok Decoction Pharmacopuncture on Atopic Dermatitis in NC/Nga Mice)

  • Objectives : Atopic dermatitis is a chronic inflammatory skin disease characterized by pruritic and erythematous skin lesions. The purpose of this study was to investigate the suppressive effects of Lonicerae Flos, Forsythiae Fluctus and Hwangryunhaedok Decoction Pharmacopuncture on the development of atopic dermatitis-like skin lesions in NC/Nga Mice. Methods : The Atopic Dermatitis was induced by biostir AD on the mice's back skin. Experimental groups were divided into three including LFP(Lonicerae Flos Pharmacopuncture, EtOH extract), FFP(Forsythiae Fluctus Pharmacopuncture, EtOH extract) and HHP(Hwangryunhaedok Decoction Pharmacopuncture, Hydrodistillation extract). Every second day, the mice of three groups were treated with $0.1m{\ell}$ of pharmacopuncture using a syringe at right and left acupoints ($BL_{13}$), alternatively. On the control group, normal saline was used instead of pharmacopuncture. Subsequently optical observation with a handscope, a clinical skin score, Tissue(general/immune) mast cell, Serum IgE level, Serum histamine level, and Serum lymphokine(IL-2, IL-4, $IFN-{\gamma}{\gamma}$) were measured. Results : FFP and HHP decreased the clinical skin score, the total cell number of mast cells, and the Serum total IgE level and Serum histamine level. In Serum lymphokine levels, all groups were decreased to the IL-4 level, LFP and FFP were increased to the IL-2 level, and LFP was increased to the $IFN-{\gamma}$ level. Conclusions : From the above results, Forsythiae Fluctus Pharmacopuncture (EtOH extract) and Hwangryunhaedok Decoction Pharmacopuncture (Hydrodistillation extract) exerted anti-allergic and anti-inflammatory effects, suggesting a promising agent for improving atopic dermatitis related symptoms.

울산 일 지역 영유아 및 학령기 아동의 아토피 피부염의 유병관련특성 및 관리실태 (Prevalence Related Characteristics and Management Status in Children with Atopic Dermatitis in Ulsan)

  • Purpose: This study was performed to investigate prevalence related characteristics and management status in children with atopic dermatitis(AD) for developing health promotion strategies led by community. Methods: Data were collected from 6,562 children with AD in preschool and elementary schools in a district of Ulsan city. Results: The majority of children(44.4%) were onset in less than 2 years after the birth. Seventy six point seven percent of subjects had skin rash and the pathologic lesion of them were flexion of upper extremities and extension of lower extremities. The prevalence rate of the respondents was 30.3%. In bathing, the children in AD group had used soap more frequently than children in normal group. The most preference type of complementary therapy in frequency order were aloe, aroma oil and Pasture. Conclusion: More effective management strategy for alleviating symptom and preventing advanced disease such as asthma and allergic rhinitis should be developed. And also scientific evidence for using complementary therapy is necessary to be provided.

아토피피부염 동물 병태 모델에서 석자해기탕(石紫解肌湯)의 면역조절작용에 관한 연구 (Study on the Immune Modulatory Activity of Seokjahaeki-tang Using Atopic Dermatitis Animal Models)

  • In order to evaluate the efficacy of SHT against atopic dermatitis (AD), various immune related cytokines as well as histological comparison were performed in animal models, and the results are described. Clinical skin index of the SHT treated group decreased significantly in weeks 11 and 13, compared to the control group. Also, CD4+ immune cell ratio in the dorsal skin was significantly decreased to 69%, and both epidermal and dermal skin thickness was decreased. Serum IL-4, IL-5, IL-6, IL-13, and TNF-${\alpha}$, which are all important markers of inflammation, were decreased to 64%, 44%, 87%, 48%, and 45%, respectively. The expression of histamine, a chemical transmitter increasingly released during the progression of inflammation, was significantly decreased to 47%. The production of IgE immunoglobulin was significantly decreased to 16% compared to the control group. In conclusion, SHT pacifies the activation of T cells, leading to suppression of both Th2 cytokine overexpression and infiltration of immune cells into skin. As a result, relative thinning of both epidermis and dermis were observed. With the results obtained from in vitro studies, the immune modulatory effect of SHT in AD animal models was experimentally demonstrated. This study should provide solid information to construct EBM and for clinical practice.

Treatment with phosphodiester CpG-ODN ameliorates atopic dermatitis by enhancing TGF-β signaling

  • Synthetic oligodeoxynucleotides (ODNs) containing unmethylated CpG phosphorothioate (PS CpG-ODN) are known to decrease IgE synthesis in Th2 allergy responses. Nonetheless, the therapeutic role of PS CpG-ODN is limited due to cytotoxicity. Therefore, we developed a phosphodiester (PO) form of CpG-ODN (46O) with reduced toxicity but effective against allergies. In this study, we first compared the toxicity of 46O with CpG-ODNs containing a PS backbone (1826S). We also investigated the therapeutic efficacy and mechanism of 46O injected intravenously in a mouse model of ovalbumin (OVA)-induced atopic dermatitis (AD). To elucidate the mechanism of 46O underlying the inhibition of IgE production, IgE- and TGF-β-associated molecules were evaluated in CD40/IL-4- or LPS/IL-4-stimulated B cells. Our data showed that the treatment with 46O was associated with a lower hematological toxicity compared with 1826S. In addition, injection with 46O reduced erythema, epidermal thickness, and suppressed IgE and IL-4 synthesis in mice with OVA-induced AD. Additionally, 46O induced TGF-β production in LPS/IL-4-stimulated B cells via inhibition of Smad7, which suppressed IgE synthesis via interaction between Id2 and E2A. These findings suggest that enhanced TGF-β signaling is an effective treatment for IgE-mediated allergic conditions, and 46O may be safe and effective for treating allergic diseases such as AD and asthma.

Bee Venom Within Liposomes Synergistically Inhibit Atopic Dermatitis in Mice

  • Background: This study was performed to determine the effects of liposome-encapsulated bee venom (BV) treatment of inflammatory factors in atopic dermatitis (AD) compared with BV treatment. Methods: AD was induced by phthalic anhydride in mice and the effects of BV liposomes were measured. Using Leica Application Suite, thickened epidermis and dermis were measured after BV liposome treatment (0.05 and 0.1 ㎍/mL). The number of stained mast cells and the concentration of immunoglobulin (Ig)E were measured. Serum IgE concentration was analyzed using an enzyme-linked immunosorbent assay. The serum concentrations of interleukin (IL)-1, IL-4, and IL-6 inflammatory cytokines were measured. The levels of messenger ribonucleic acid expression of proinflammatory cytokines and chemokines were measured using reverse transcription polymerase chain reaction. Inhibition of mitogen-activated protein kinase activation, was analyzed on western blot. To measure the transcriptional activity (NF-κB inhibition by BV liposomes), western blots (p65, p-IκB, p50, and IκB) were also performed. Results: The weight of lymph nodes, serum IgE concentrations, morphological changes in the skins from the backs of the mice, and mast cell numbers in inflamed tissues were noticeably lower in the BV liposome treatment group compared with the BV treatment group. The concentrations of pro-inflammatory cytokines (IL-1, IL-4, IL-6) and chemokines (TSLP, CCL22) were also reduced. Activation of mitogen-activated protein kinase (p-ERK and p-p38), and transcriptional activity (p65, p-IκB, p50, and IκB) was strongly suppressed in the BV liposome group. Conclusion: BV liposomes may have a better therapeutic effect than BV for the treatment of AD.

아토피 피부염에서 한약 외용제와 양약 병용 치료의 효과: 체계적 문헌고찰 및 메타분석 (Efficacy of Combination Treatment of Herbal Medicine for External Use and Western Medicine for Atopic Dermatitis: A Systematic Review and Meta Analysis)

  • Objectives: This study was conducted to evaluate the effect of combination treatment of herbal medicine for external use and western medicine for atopic dermatitis (AD). Methods: We searched randomized controlled trials (RCTs) which assess the effect of combination treatment of herbal and western medicine for AD through 8 electronic databases from the start to December 2022. The data synthesis was conducted by using Review Manager (RevMan, ver.5.4.1) and Cochrane risk-of-bias tool was used to evaluate the risk of bias. Results: 13 RCTs were included. The combination treatment group had significantly higher total efficacy rate(p<0.0001) and lower SCORAD score (p<0.00001) than the western medicine treatment group. The adverse event rate was also significantly lower in the combination treatment than the western medicine treatment group (p<0.0001). But there was no significant difference in recurrence rate (p=0.09). Conclusion: This study demonstrates that the combination treatment of herbal and western medicine could be safe and effective for AD. However, due to limits of included studies such as high heterogeneity between the literature and unclear risk of bias, further studies are warranted.
