폐쇄성 하지동맥 경화증에서 동맥 우회술의 임상적 고찰 (Clinical Analysis of Arterial Bypass on the Atherosclerotic Occlusive Disease in Lower Extremities)

  • 복부대동맥과 그 이하 주요분지들의 동맥폐쇄에 의한 하지 만성허혈은 동맥경화증이 주원인이며 이 질환의 임상적 중요성은 혈관이 진행적으로 폐쇄됨에 따라 혈류장애가 발생하여 간헐적 파행, 휴식시 동통이 일어나며 더 진행되면 조직의 괴사가 발생,결국괴사된 조직의 절단을 초래하게된다. 본한국보 훈병원 흉부외과에서는 1992년 12월부터 1995년 12월까지 폐쇄성 하지동맥 경화증으로 입원한 20명의 환자를 대상으로 동맥간 우회술을 시 행하였다. 임상증상은 괴사및 괴저 9례(45B4), 파행 7례(35%), 안정 시 동통 4례(20%)순이었다. 사용한 혈관은 복부대동맥및 총대퇴동맥의 경우 Y-형및 I-형 인조혈관을 사 용하였고 대퇴동맥이하의 혈관에서는 주로 자가 대복재정 맥을 사용하였다. 술후 합병증은 4례로 20% 를 보였고 이식된 혈관내 혈전증 2례, 창상감염 2례 였다. 술후 혈관개존율은 복부대동맥과 양측 대퇴동 맥간 우회술의 경우 100%였고 대퇴동맥 이하에서는 6개월이 92.6%, 1년 84.2%, 2년 75.4%, 3년 69.4% 였다.

Multimodal Nonlinear Optical Microscopy for Simultaneous 3-D Label-Free and Immunofluorescence Imaging of Biological Samples

  • In this study, we demonstrated multimodal nonlinear optical (NLO) microscopy integrated simultaneously with two-photon excitation fluorescence (TPEF), second-harmonic generation (SHG), and coherent anti-Stokes Raman scattering (CARS) in order to obtain targeted cellular and label-free images in an immunofluorescence assay of the atherosclerotic aorta from apolipoprotein E-deficient mice. The multimodal NLO microscope used two laser systems: picosecond (ps) and femtosecond (fs) pulsed lasers. A pair of ps-pulsed lights served for CARS (817 nm and 1064 nm) and SHG (817 nm) images; light from the fs-pulsed laser with the center wavelength of 720 nm was incident into the sample to obtain autofluorescence and targeted molecular TPEF images for high efficiency of fluorescence intensity without cross-talk. For multicolor-targeted TPEF imaging, we stained smooth-muscle cells and macrophages with fluorescent dyes (Alexa Fluor 350 and Alexa Fluor 594) for an immunofluorescence assay. Each depth-sectioned image consisted of $512{\times}512$ pixels with a field of view of $250{\times}250{\mu}m^2$, a lateral resolution of $0.4{\mu}m$, and an axial resolution of $1.3{\mu}m$. We obtained composite multicolor images with conventional label-free NLO images and targeted TPEF images in atherosclerotic-plaque samples. Multicolor 3-D imaging of atherosclerotic-plaque structural and functional composition will be helpful for understanding the pathogenesis of cardiovascular disease.

백하수오 추출액이 고지혈증 및 Streptozotocin 유발 당뇨성 흰쥐의 혈청 지질성분 및 효소활성에 미치는 영향 (Effects of Cynanchum wilfordii Extract on Serum Lipid Components and Enzyme Activities in Hyperlipidemic and Streptozotocin-Induced Diabetic Rats)

  • The purpose of this study was designed to observe the effects of the feeding Cynanchum wilfordii extract on the improvement of the blood glucose, lipid components in the serum of dietary hyperlipidemic and streptozotocin(STZ) -induced diabetic rats(S.D. strain, ♂) fed the experimental diets for 5 weeks. Concentrations of total cholesterol, atherosclerotic index, LDL, LDL-Cholesterol, free-cholesterol. cholesteryl ester, TG, PL and blood glucose in serum were significantly higher in the cholesterol administration groups((group 2(cholesterol+water), 4(cholesterol+Cynanchum WIlfordii 3.5g% extract)) than those in the control group (group1 , basal diet+water). But the concentrations of total cholesterol. atherosclerotic index, LDL, LDL- cholesterol. free-cholesterol, cholesteryl ester, TG, PL and blood glucose in serum were remakably lower in the group 4 than those in the group 2. In the STZ(55mg/kg B.W.)-induced diabetic groups((group 3(STZ, IP.)+water), 5(STZ(IP.)+Cynanchum WIlfordii 3.5g% extract? the serum total cholesterol, atherosclerotic index, LDL, LDL-cholesterol, free-cholesterol. cholesteryl ester, TG, PL and blood glucose concentrations actions were rather lower in the group 5 than those in the group 3. In the ratio of HDL -cholesterol concentration to total cholesterol and HDL-cholesterol concentration, Cynanchum wilfordii extract administration groups were higher percentage than III the groups 2 and 3. The activities of aspartate aminotransferase(AST), alanine aminotransferase(ALT), alkaline phosphatase (ALP) and lactate dehydrogenase(LDH) in serum were rather lower in the Cynanchum wllfordii extract administration groups (group 4,5) than in the cholesterol diet group(group 2) and STZ-induced diabetic group (group 3). From the above research, the physiological activity substances in Cynanchum wllfordii were effective on the improvement of the blood glucose, lipid compositions in serum of dietary hyperlipidemic and STZ-induced diabetic rats. And particularly, physiological activity substance in Cynanchum wilfordii was more effective therapeutic regimen for the control of metabolic derangements in adult disease.

Significance of Hemodynamic Effects on the Generation of Atherosclerosis

  • Atherosclerosis, which is a degenerative vascular disease, is believed to occur in the blood vessels due to deposition of cholesterol or low density lipoprotein (LDL). Atherosclerotic lumen narrowing causes reduction of blood flow due to hemodynamic features. Several hypothetical theories related to the hemodynamic effects have been reported : high shear stress theory, low shear stress theory, high shear stress gradient theory, flow separation and turbulence theory, and high pressure theory. However, no one theory clearly explains, the causes of atherosclerosis. The objective of the present study was to investigate the mechanism of the generation of atherosclerosis. In the study, the database of Korean carotid and coronary arteries for geometrical and hemodynamic clinical data was established. The atherosclerotic sites were predicted by the computer simulations. The results of the computer simulation were compared with the in vivo experimental results, and then the pathogenesis of atherosclerosis by using the clinical data and several hypothetical theories were investigated. From the investigation, it was concluded carefully that the mechanism of the generation of atherosclerosis was related to the hemodynamic effects such as flow separation and oscillatory wall shear stress on the vessel walls.

하행대동맥 수술의 기왕력을 가진 환자에서 발생한 대동맥궁 침투성 궤양의 파열 (Ruptured Penetrating Atherosclerotic Ulcer of the Aortic Arch in a Patient with a Previous History of Replacing the Descending Thoracic Aorta)

  • 대동맥수술 후 대동맥질환의 재발은 주로 하부에 대동맥류의 형태로 나타나며 상부에 침투성 동맥경화성 궤양의 형태로 나타나는 것은 매우 드물다. 대동맥류 수술의 기왕력을 가진 환자에서 대동맥궁과 하행대동맥의 근위부에 걸쳐 발생한 침투성 동맥경화성 궤양의 파열을 완전순환정지 하에 인조혈관을 사용하여 패취봉합을 성공적으로 시행하였기에 문헌고찰과 함께 보고하는 바이다.

Effects of the Zizyphus jujuba Seed Extract on the Lipid Components in Hyperlipidemic Rats

  • The purpose of this study was designed to observe the effects of Zizyphus jujuba seed extract on the concentrations of the lipids and blood glucose in the S.D. rats fed the experimental diets for 4 weeks. Concentrations of total cholesterol, atherosclerotic index, LDL, LDL-cholesterol, free-cholesterol, cholesteryl ester, triglyceride (TG), phospholipid (PL) and blood glucose in serum were significantly higher in the cholesterol administration groups (group 2 (cholesterol+water), group 3 (cholesterol+Zizyphus jujuba seed extract)) than those in the control group (group 1, basal diet+water). But the concentrations of total cholesterol, atherosclerotic index, LDL, LDL-cholesterol, free-cholesterol, cholesteryl ester, TG, PL and blood glucose in serum ware remarkably lower in the group 3 than those in the group 2. In the ratio of HIDL-cholesterol concentration to total cholesterol and HDL-cholesterol concentration, Zizyphus jujuba seed extract administration group was higher percentage than in the group 2. The activities of aspartate aminotransferase (AST), alanine amino-transferase (ALT), lactate dehydrogenase (LDH) and alkaline phosphatase (ALP) in serum were rather lower in the Zizyphus jujuba seed extract administration group (group 3) than in the cholesterol diet group (group 2). From the above research, Zizyphus jujuba seeds were effective on the improvement of the blood glucose, lipid compositions in serum of dietary hyperlipidemic rats. And particularly, Zizyphus jujuba seeds were more effective as a therapeutic regimen for the control of metabolic derangements in adult disease.

  • Atherosclerosis is the deposition of plaque in the main arteries. It is an inflammatory condition involving the accumulation of macrophages and various lipids (low-density lipoprotein [LDL] cholesterol, ceramide, S1P). Moreover, endothelial cells, macrophages, leukocytes, and smooth muscle cells are the major players in the atherogenic process. Sphingolipids are now emerging as important regulators in various pathophysiological processes, including the atherogenic process. Various sphingolipids exist, such as the ceramides, ceramide-1-phosphate, sphingosine, sphinganine, sphingosine-1-phosphate (S1P), sphingomyelin, and hundreds of glycosphingolipids. Among these, ceramides, glycosphingolipids, and S1P play important roles in the atherogenic processes. The atherosclerotic plaque consists of higher amounts of ceramide, glycosphingolipids, and sphingomyelin. The inhibition of the de novo ceramide biosynthesis reduces the development of atherosclerosis. S1P regulates atherogenesis via binding to the S1P receptor (S1PR). Among the five S1PRs (S1PR1-5), S1PR1 and S1PR3 mainly exert anti-atherosclerotic properties. This review mainly focuses on the effects of ceramide and S1P via the S1PR in the development of atherosclerosis. Moreover, it discusses the recent findings and potential therapeutic implications in atherosclerosis.

숙성 마늘 유래 S-allylmercaptocysteine의 human low density lipoprotein (LDL)에 대한 항산화 효과 (Antioxidative Effect of S-allylmercaptocysteine Derived from Aged Garlic on Oxidation of Human Low Density Lipoprotein)

  • Low density lipoprotein (LDL)의 산화는 동맥경화의 유발과 진행에 중요한 역할을 하는 것으로 알려져 있다. 본 연구는 숙성 마늘 유래 유황화합물인 S-allylmercaptocysteine의 항산화 효과를 실험하였다. S-allylmercaptocysteine의 항산화 효과는 $Cu^{2+}$ 유도 LDL에 대하여 thiobarturic acid substance (TBARS)와 전기영동 이동거리, 공액 이중결합으로 측정하였다. 사람 LDL을 $Cu^{2+}$ 유도로 산화시킬 때 S-allylmercaptocysteine은 용량 의존형으로 나타났으며, 농도가 $20{\mu}g/ml$ 일 때 거의 완전한 억제 효과를 나타내었다. S-allylmercaptocysteine에 대한 전기영동 이동거리로 $20{\mu}g/ml$ 일 때 가장 낮았으며 공액 2중결합 형성도 $20{\mu}g/ml$ 일 때 거의 억제되었다. 한편 S-allylmercaptocysteine을 항산화제인 $d{\ell}-{\alpha}$-tocopherol과 LDL에 대한 항산화력을 비교한 결과 같은 농도에서 S-allylmercaptocysteine이 약간 높았다. 따라서 S-allylmercaptocysteine이 동맥경화의 발병과 진행을 예방하는 역할을 할 것으로 생각된다.

가족성 고콜레스테롤혈증은 언제 의심해야 할까?: 관상동맥 질환이 없는 젊은 이상지질혈증 환자 증례 (When Should Familial Hypercholesterolemia Be Suspected?: A Case of Dyslipidemia in Young Patient without Coronary Artery Disease)

  • Familial hypercholesterolemia (FH) is a genetic disease that is not well known or diagnosed in Korea. This disease is associated with persistently high levels of low-density lipoprotein cholesterol (LDL-C), which increase the risk of coronary artery disease at a young age. Therefore, early diagnosis and treatment are important; however, there are no global consensus diagnostic criteria. In Korea, the Dutch Lipid Clinic Network diagnostic criteria, and the Simon Broome diagnostic criteria were used for diagnosis of FH according to the agreement announced at the Korean Society of Lipid and Atherosclerosis (KSoLA) in 2022. Recently, the absence of coronary artery calcification has been considered a good prognostic factor, even among patients with very high LDL-C levels who are considered to be at high risk for atherosclerotic cardiovascular disease. We describe throughout this paper the diagnosis and treatment of FH in a young male without coronary artery calcification.

원위 속목동맥과 중간대뇌동맥에 발생한 섬유근육형성이상 (Fibromuscular Dysplasia of the Distal Internal Carotid and Middle Cerebral Artery)

  • Fibromuscular dysplasiais an uncommon condition of idiopathic, non-inflammatory and non-atherosclerotic disease of the musculature of arterial walls. The disease is rare, but it commonly affects young and middle aged women. Isolated intracranial cerebral fibromuscular dysplasia is extremely rare because cerebral fibromuscular dysplasia usually affects extracranial vessels. A 20-year-old woman was admitted with light hemiplegia and global aphasia. Brain MRI and MRA demonstrated acute left middle cerebral artery territory infarction with a multifocal stenosis and dilatation of the left middle cerebra artery and left internal carotid. The characteristic conventional cerebral angiographic findings demonstrated a typical string-of-beads appearance in the left distal internal carotid artery and proxiaml portion of the left middle cerebral artery, which suggested a medial type fibromuscular dysplasia. We report a case of isolated intracranial fibromuscular dysplasia with left middle cerebral artery territory infarction. Fibromuscular dysplasia should he considered as a stroke risk factors in children and young adults, especially in patients with no known cardiovascular risk factors.
